Abstract
Description
The present invention relates to an input support method and an input support system that support image interpretation report creation in the medical field.
As a method of diagnosis support at the time of image interpretation input, a method of outputting a region having a high possibility of an abnormal part by CAD is known (Japanese Patent No. 3085724, Japanese Patent Laid-Open No. 5-12352, Japanese Patent Laid-Open No. 6-259486, JP 2000-311212, JP 2004-305551, JP 2004-305562). Among them, there are systems that support comparative interpretation by comparing CAD results of a plurality of images (Japanese Patent Laid-Open No. 2004-180932) and mechanically verifying output results of a plurality of images (Japanese Patent Laid-Open No. 2003-284690). ).
The above system is a system that supports diagnosis based on image information, but as a recent trend, when inputting an interpretation report, it is checked against diagnostic guidelines, textbooks, or diagnostic logic in the head of an experienced doctor. Therefore, there is a need for a function that can support the creation of high-quality reports with few oversights and omissions.
As one method, there is a method in which a standard template is created by, for example, transcription of a diagnostic guideline, a textbook, or an interview with a doctor, and an interpretation report is input based on the standard template (JP 2001-126007, JP 2002). -123605, JP2003-288332).
Another known method is to search past similar reports based on keywords being entered, CAD image features, inspection information, etc., and display them in a list with similarities. (JP 2003-122848, JP 2003-122850).
However, in the method of creating a standard template and inputting an interpretation report based on it, diagnostic guidelines and textbooks are usually written in free text format with a fixed format, so it is used for diagnosis support in interpretation scenes In this case, it takes a considerable amount of time to extract knowledge useful for diagnosis and make it usable in a system such as a template format. In the first place, it takes time for diagnostic guidelines to be prepared in a usable form (for example, guidelines for several fields such as cerebral hemorrhage, lung cancer, diabetes, etc. are currently published by the Japan Medical Function Evaluation Organization) Are published in the form of books before they can be searched on public pages), so they tend to be outdated. Furthermore, the guideline itself changes as medical technology advances, and each time revision is made, it takes time to create a new template.
In addition, in the system that displays a list of similar report search results, it is not possible to know whether there is a problem such as a shortage or contradiction when the input content is specifically compared to the tendency of past reports only by referring to the text. .
In this way, without taking the trouble of writing a template for input support from diagnostic guidelines and textbooks, depending on the input contents of interpretation, appropriate contents such as content that is likely to be missed or contradictions in diagnostic results are appropriate. Presenting input support information has become important.
Means for solving the problem will be described with reference to the process flow diagram of FIG. The approved past findings report is analyzed by text processing techniques such as
Next, a flow that uses the accumulated n-term pattern will be described. In the analysis of the finding input sentence, the
Next, the
The input support information extracted by the above procedure shows the correspondence between the position in the input sentence and the support information, thereby facilitating correction of input based on the support information. In addition, the validity of the information can be confirmed by making it possible to refer to the data from which the support information is extracted and its corresponding location.
Using the description pattern of the past interpretation report as an information source, it is possible to present input support information for recognizing omissions and contradictions when inputting the interpretation report. Compared to the case where a template is created from a diagnostic guideline or the like and input support is performed, not only is the effort for creating a template unnecessary, but the diagnosis support pattern can be customized only by changing past findings data. For example, if the facility A report is used, support information reflecting the standard diagnosis pattern in the facility A is presented, and if the facility B report is used, support information reflecting the standard diagnosis pattern in the facility B is presented. Presented. Similarly, even in the same medical facility, it is possible to reflect the standard diagnosis pattern in each data simply by classifying the usage data according to other attributes related to reports such as the time and doctors to approve Is possible. In addition, it is possible to provide timely input support information as compared to guidelines that are updated slowly.
FIG. 1 illustrates a standard system configuration that can be considered when implementing the present invention. Used as a component in “Interpretation Client” 10 for inputting interpretation reports and presenting support information, “Support Information Calculation Server” 13 for calculating input support information based on past interpretation reports, and Support Information Calculation Server And a “storage device” 14 which holds past finding data and language analysis result data. A storage device may be prepared for each hospital (11, 12). Each component is connected by a network, and data can be transmitted and received via the network. These components may run on a single machine rather than on a network.
The doctor who performs the interpretation corresponds to the
Next, the interpretation flow will be described. In the interpretation reporting system, reports are usually created according to the flow shown in Fig. 3. That is, while looking at the image to be interpreted, the findings are input at 301 and it is determined at 302 whether or not to confirm the input contents. If the input has not been completed up to the end, the data is temporarily saved at 305, read again as necessary, and the editing operation is resumed at 301. After confirming the input content, it is approved by another doctor at 304 and saved as an approved report at 31. According to the flow, in the present invention, as shown in FIG. 4, a finding is input at 401 and it is determined whether or not to confirm the input content at 402. As in FIG. 3, if the input is not completed until the end, or if the doctor requests support information, it is temporarily stored at 407. When the support information is not requested, re-reading is performed as necessary as in the flow in FIG. Alternatively, when the support information is requested at 406, the support information is presented at 405, and the input person performs editing operations such as addition and correction at 401 while referring to the support information at 408. The method of referring to the support information at 408 may clearly indicate the correspondence between the input shortage / error part and the support information. An example of the association method will be described later.
Next, calculation of input support information will be described. When the input support information is requested by the
Next, accumulation of past finding data will be described. As described above with reference to the example of FIG. 3, in the normal interpretation flow, the operation may be interrupted during input. In order to increase the reliability of the report, the report may be approved by another doctor after the input is confirmed. In the present invention, in order to improve the quality of the calculated input support information, it is desirable to use an approved report for past findings used as data. The
Next, extraction of a multinomial relationship will be described. The frequency is calculated for morpheme and phrase patterns that appear in the same sentence. As elements constituting the pattern, morphemes, phrases, representative morphemes of phrases, representative semantic categories of phrases, fixed character strings, and the like can be considered. Each pattern is expressed as an n-ary set and an n-ary string of these components. FIG. 5 shows an example of the phrase n-term pattern. For n-ary sequences, the order between items must be the same, but for n-ary sets, the order in the sentence between items is irrelevant. In addition, the items may be adjacent to each other or separated from each other. Also, there may be a case where some of the n-term relationships are n-term sets and the n-term sets have an x-term string relationship. An example is shown in FIG. 71 represents the result of the dependency analysis of the sentence “A remarkable irregularity is found in the anterior stomach wall”. An
If there is a computer with sufficient hard disk capacity and memory, all the n-term patterns are generated for each sentence, and a statistical measure based on the frequency and frequency is calculated. If sufficient computer resources do not exist, an n-term pattern is generated using a data mining method or the like while cutting off what is considered unnecessary in terms of frequency and statistical scale. In addition, when calculating the frequency of the n-term pattern, it is often performed that the identifiable language expression pairs are defined in a separate table and the frequency count is normalized.
Other importance calculation methods will be described next. Of the n-term patterns in the used past finding data, a pattern having a high frequency can be estimated as a standard description pattern in the findings report of the hospital, clinical department, time, and doctor of the data providing source. In addition, by using the data division described above, an object having a higher frequency than other data sets at a specific hospital, a specific department, or a specific time may have a higher importance in the data set.
In addition, simply using the frequency is a rare case in the past findings, but there is a possibility that important patterns that should not be overlooked cannot be extracted. Therefore, in addition to patterns that are dominant in frequency and other statistical measures, use as many n-term patterns generated from input as possible, and include in reports with high similarity (but there is a discrepancy between past reports and input) It seems that other devices such as increasing importance are necessary for the pattern to be displayed. It can be considered that a method combined with the user interface is also effective, such as discarding patterns not used by doctors and increasing the importance of the used patterns.
Next, the support information presentation method will be described. According to the interpretation flow shown in FIG. 4, input support information is presented when a request is made from a doctor before the finding input is confirmed. An example of the interpretation client shown in FIG. 6 is given as an example of the diagnosis support screen. First, it is assumed that the image sent from the modality is displayed on the interpretation report client as in the normal interpretation flow. Next, the observation input is performed in the
In addition, in order to provide accurate support, the past findings data that was the basis of diagnosis support can be referred to from the provided diagnosis support information as necessary. It becomes possible to confirm.
For example, it is used by being incorporated into a radiology interpretation report system.
