Description

The present invention relates to an endoscope capable of inserting and removing a treatment tool inserted into a body cavity via an endoscope.

In general, when performing a treatment of a required part in the body or an examination of a living tissue by an operation from outside the body using an endoscope, for example, a plurality of treatment tools such as a needle knife and a grasping forceps are continuously used. Sometimes. In such a case, conventionally, the selected treatment tool is inserted into the body through a forceps channel provided in the insertion portion of the endoscope, and after performing a predetermined treatment, the treatment tool is pulled out of the body, and the forceps is channeled again. And so on.

Therefore, as a treatment tool used in an endoscope for observing living tissue such as an affected part in a body cavity, a treatment tool such as a forceps inserted into the body cavity is automatically inserted through a forceps opening formed in the endoscope. , One having a device for removing and controlling the amount of insertion has been proposed. (For example, refer to Patent Document 1).
The endoscope described in Patent Document 1 is provided with two gears provided in an operation unit on the endoscope side and fixed to a rotating shaft of a micromotor, a drum attached to the gear, and a drive circuit. And a motor drive control unit for controlling the micromotor. When the micromotor is excited and rotated by the driving of the micromotor, the gear rotates, so that the treatment instrument is conveyed in the forceps channel by inserting the treatment instrument between the drums.
JP 57-117823 A (Fig. 1 etc.)

However, the endoscope described in Patent Document 1 requires a control unit such as a micromotor and a motor drive circuit, which increases the number of components and makes it difficult to have a simple configuration.
The present invention has been made in view of the above-described circumstances, and provides an endoscope that can perform a procedure easily by performing an operation for transporting a treatment instrument in a forceps channel in a short time with a simple configuration. The purpose is to do.

In order to achieve the above object, the present invention provides the following means.
The endoscope of the present invention has an insertion portion to be inserted into a body cavity, an operation portion for operating the insertion portion, a distal end opening portion with one end opening at the distal end of the insertion portion, and the other end is In an endoscope comprising a first channel connected to a forceps opening that opens in the vicinity of the operation section, and a treatment instrument that is inserted into the first channel, the insertion section of the insertion section is located near the forceps opening. A second channel provided over the distal end, an operation wire inserted from the proximal end of the second channel, folded back at the distal end of the insertion portion and inserted into the first channel, and provided at the distal end of the operation wire And an engagement member engaged in the vicinity of the distal end of the treatment instrument, and a treatment instrument advancing / retreating operation portion provided at the proximal end of the operation wire.

In this invention, after attaching the treatment tool to the engaging member provided at the distal end of the operation wire, the operation wire is moved from the first channel by pulling the treatment tool advancing / retreating operation portion toward the proximal end side of the second channel. It moves to a 2nd channel and a treatment tool is conveyed to a front-end | tip opening part. Therefore, when the treatment instrument is transported to the distal end opening, it is not necessary to provide a device such as a motor or a control unit as in the prior art, and the treatment instrument can be easily advanced and retracted with a simple configuration.

In the endoscope of the present invention, the operation wire is divided into a first wire portion having the engagement member and a second wire portion having the treatment instrument advancing / retreating operation portion, and the first wire portion. The first rotating member wound on the proximal end side of the wire and the distal end side of the second wire portion are wound, and urged in the rotation direction in which the second wire portion is wound into the second channel. A second rotating member; and when the second rotating member pulls the second wire portion from the proximal end side of the second channel, the first rotating member becomes the second rotating member. When the second rotating member rotates in the same direction in conjunction with and rotates in the direction in which the second wire portion is wound, the locking portion restricts the rotation of the first rotating member in the same direction. It is characterized by having.

In this invention, after attaching a treatment tool to the engaging member provided in the first wire part, the operator pulls the treatment tool advancing / retreating operation part toward the proximal end side of the second channel, so that the second rotation is performed. The member rotates. With this rotation, the first rotating member also rotates, the first wire portion is wound around the first rotating member, and the treatment instrument moves in the direction of the distal end opening. Next, when the operator loosens the pulling force of the treatment instrument advancing / retreating operation portion, the second rotating member is biased in the direction of winding the second wire portion, and the first rotating member is engaged with the second rotating member. Since rotation in the same direction is restricted, only the second rotating member rotates, and the second wire portion is wound around the second rotating member. Therefore, by repeating this operation, the treatment instrument can be transported to the distal end opening. That is, by providing the first and second rotating members, the second wire portion is returned into the second channel when the second wire portion is pulled toward the proximal end side of the second channel. It is wound around the rotating member. Thereby, the second wire portion exposed from the second channel is shortened, and the operability can be improved.

The endoscope treatment instrument insertion / extraction apparatus of the present invention is an endoscope treatment instrument insertion / extraction apparatus that advances and retracts a treatment instrument in a forceps channel formed in the endoscope, and is attachable to and detachable from the distal end portion of the endoscope. A hood, an operation wire inserted from the proximal end of the forceps channel, folded back by the hood and inserted into the forceps channel, and engaged in the vicinity of the distal end of the treatment instrument provided at the distal end of the operation wire. An engaging member, a treatment instrument advancing / retreating operation portion provided at the proximal end of the operation wire, and a forceps plug provided in a forceps opening inserted through the forceps channel or a fixing provided in the vicinity of the forceps plug. And an insertion tube having a distal end fixed to the hood and a proximal end fixed to the fixing member, and the operation wire on the treatment instrument advancing / retreating operation portion side being folded back at the distal end of the forceps channel. It is characterized in that it comprises a.

In this invention, first, the hood is attached to the distal end portion of the endoscope, and the fixing member is attached to the forceps plug or the vicinity of the forceps plug. Then, the treatment instrument is attached to the engaging member provided at the distal end of the operation wire, and the treatment instrument advance / retreat operation portion provided at the other end of the operation wire is pulled toward the proximal end side of the forceps channel. As a result, the engaging member is conveyed in the forceps channel and the treatment instrument can be conveyed to the distal end side of the insertion tube. In this case, by attaching the endoscope treatment tool insertion / extraction device to the endoscope, the endoscope can be easily transported.

The endoscope treatment instrument insertion / extraction apparatus according to the present invention is characterized in that when the treatment instrument is attached to the engaging member, a support base for supporting the treatment instrument is provided on the fixing member. .
In this invention, since the support member is provided on the fixing member, when the treatment tool is attached to or detached from the engagement member, the work can be performed while the support member supports the engagement member. Therefore, the treatment tool can be easily attached and detached, so that the attachment and detachment time can be shortened.

According to the endoscope and the endoscope treatment instrument insertion / extraction device according to the present invention, the treatment instrument can be advanced and retracted by pulling the treatment instrument advancing / retreating operation portion provided on the operation wire. Therefore, since the treatment tool can be inserted into and removed from the endoscope by a simple technique, the technique can be facilitated.

It is comprised from the flexible part 15 of a scale. The bending portion 13 can be bent in any direction in the vertical and horizontal directions by operating a bending operation knob 16 provided in the endoscope operation portion 14. A universal cord 17 extends from the endoscope operation unit 14 and is connected to an endoscope body (not shown). In addition, a treatment instrument transport mechanism 30 that transports the treatment instrument 20 for performing treatment in the body cavity is provided inside the endoscope insertion portion 11.

As shown in FIG. 2, the treatment instrument transport mechanism 30 advances and retracts the treatment instrument 20 from a forceps port 18 provided in the endoscope operation section 14 to a distal end opening portion 12 a that opens to the endoscope distal end section 12. It is. As shown in FIGS. 3 and 4, the treatment instrument transport mechanism 30 has a distal end opening portion 12 a that opens to the endoscope distal end portion 12, and the other end opens to the endoscope operation portion 14. A first channel 31 that is connected to the forceps port 18 and allows a treatment tool to be inserted therein, a second channel 32 provided from the vicinity of the forceps port 18 to the distal end of the endoscope insertion portion 11, and the first channel 31 The operation wire 34 is inserted from the proximal end 32 a of the second channel 32 and is folded back by a rotatable pulley 33 provided at the distal end portion 12 of the endoscope and inserted into the first channel 31. The operation part wire includes a first wire part 34a having an engagement member 35 engaged with the distal end of the treatment instrument 20, and a second wire having a ring-shaped handle part (treatment instrument advance / retreat operation part) 36. It is divided into a part 34b.

Further, as shown in FIGS. 1 and 4, the treatment instrument 20 is connected to a flexible insertion portion 21 inserted from the forceps port 18 into the first channel 31 and a distal end 21 a of the insertion portion 21. A forceps portion 22 and an operation portion 23 that is connected to the proximal end 21 b of the insertion portion 21 and operates the forceps portion 22.
As shown in FIG. 4, the engaging member 35 is formed with a notch 35b provided perpendicular to the support plate 35a. Further, since the insertion portion 21 has an outer diameter smaller than that of the forceps portion 22, the cutout portion 35 b can be locked at the distal end 21 a of the insertion portion 21 corresponding to the connection portion between the insertion portion 21 and the forceps portion 22. Yes.

Further, the treatment instrument transport mechanism 30 is provided with a ratchet mechanism 40 that winds the operation wire 34, and is provided on a support member 41 that has an opening 41 a and is provided at the base end 32 a of the second channel 32. ing. When the ratchet mechanism 40 is rotated in the A direction, the first rotating member 42 wound around one end of the first wire portion 34a and the one end side of the second wire portion 34b are rotated when rotated in the B direction. The second rotating member 44 to be wound, the supporting shaft 45 that rotatably supports the first rotating member 42 and the second rotating member 44, and is attached to the supporting member 41 so as not to rotate, and the supporting shaft The coil spring 46 disposed on the outer peripheral surface 45 and urged in the direction in which the first rotating member 42 and the second rotating member 44 approach each other, and the second rotating member 44 is urged to rotate in the B direction. The spiral spring 43 is accommodated, and a spring case 47 joined to the second rotating member 44 so as to rotate integrally with the second rotating member 44, and a recess 47a provided along the outer peripheral surface of the spring case 47 Mating and relative to the spring case 47 And a rolling-enabled mobile member 48. In addition, the spiral spring 43 has one end joined to the support shaft 45 and the other end joined to the inner peripheral surface of the spring case 47, so that the second wire portion 34 b is wound in the second channel 32. It is arranged to be energized.

As shown in FIG. 5, the first rotating member 42 has holes 42 b formed radially at equal intervals around the support shaft 45 on the side wall surface 42 a facing the second rotating member 44 (for example, 8). In addition, the second rotating member 44 is provided with a locking claw 44b disposed on the side wall surface 44a facing the first rotating member 42 so as to be inclined with respect to the side wall surface 44a. The locking claw 44b can be engaged. Further, the first rotating member 42 can be moved in the C direction along the support shaft 45 by a stopper (not shown), and the movement in the D direction is restricted. Therefore, the first rotating member 42 can be rotated at a fixed position. Yes. Accordingly, when the second rotating member 44 pulls the second wire portion 34 b from the base end 32 b side of the second channel 32, the first rotating member 42 is interlocked with the second rotating member 44. Rotate in the same direction. Further, when the second rotating member 44 rotates in the direction in which the second wire member 34 b is wound, the inclination angle of the locking claw 44 b and the elastic force of the coil spring 46 are relative to the second rotating member 44. The first rotation member 42 is formed so as to restrict rotation in the same direction. That is, the hole 42b and the locking claw 44b constitute an engaging portion, whereby when the second rotating member 44 rotates in the A direction, the first rotating member 42 rotates in the A direction following the rotation. . On the other hand, when the second rotating member 44 rotates in the B direction, only the second rotating member 44 rotates in the B direction, and the first rotating member 42 does not rotate.

A fixed lead screw 49 is formed on the inner peripheral surface of the opening 41 a of the support member 41. Further, a male screw 50 that is screwed into the female screw 49 is formed on the outer peripheral surface of the moving member 48. That is, by rotating the moving member 48 in the A direction, as shown in FIG. 6, the second rotating member 44 moves in the C direction, the hole 42b and the locking claw 44b are engaged, By rotating the moving member 48 in the B direction, the second rotating member 44 moves in the D direction, and the engagement between the hole 42b and the locking claw 44b is released.

Next, a method for using the endoscope of the present embodiment configured as described above will be described below.
First, after inserting the endoscope insertion portion 11 into the body cavity, as shown in FIG. 4A, the notch portion 35b of the engaging member 35 is attached to the distal end 21a of the insertion portion 21 of the treatment instrument 20, and FIG. As shown in FIG. 8A, the treatment instrument 20 is inserted into the first channel 31 from the forceps opening 18. At this time, as shown in FIG. 8B, the second wire member 34 b is wound around the second rotating member 44. 9A, when the operator pulls the handle portion 36 of the second wire portion 34b outward from the base end 32b side of the second channel 32, the forceps portion 22 of the treatment instrument 20 is moved. It protrudes from the tip opening 12a. That is, the second rotating member 44 rotates in the A direction, and following this rotation, the first rotating member 42 also rotates in the A direction. As a result, as shown in FIG. 9B, the first wire portion 34 a is wound around the first rotating member 42 via the pulley 33, and the second wire portion 34 b is connected to the second channel 32. It is exposed to the outside from the base end 32b side.

Next, when the operator releases his / her hand, the second rotating member 44 rotates in the direction of winding the second wire portion 34b by the elastic force of the spiral spring 43 as shown in FIG. As shown in FIG. 10A, the handle portion 36 is located in the vicinity of the second channel 32. In this state, the operation part 23 is operated, the forceps part 22 is opened and closed, and the affected part in the body cavity is observed and treated. When advancing and retracting, the insertion portion 21 may be operated as it is, and there is no need for operations such as releasing the treatment instrument transport mechanism 30. When the treatment of the affected area or the like is completed, the second rotating member 44 is moved to D as shown in FIG. 11B by rotating the moving member 48 in the B direction with the forceps 22 closed. The engagement between the hole 42b and the locking claw 44b is released. As described above, when the treatment tool 20 is pulled from the proximal end side of the first channel 31 after the second rotation member 44 is separated from the first rotation member 42, the engagement of the ratchet mechanism 40 is released. Therefore, only the first rotating member 42 rotates. As a result, as shown in FIG. 11A, the treatment instrument 20 moves to the proximal end side of the first channel 31 and removes the treatment instrument 20 from the engagement member 35.

That is, according to the endoscope 1 according to the present embodiment, when the treatment instrument 20 is conveyed to the distal end opening portion 12a by providing the treatment instrument conveyance mechanism 30, devices such as a motor and a control unit are conventionally used. The treatment instrument 20 can be easily inserted into and removed from the endoscope distal end 12 side of the first channel 31 with a simple configuration. Further, by providing the ratchet mechanism 40, when the second wire portion 34b is pulled toward the proximal end 32a side of the second channel 32, the second wire portion 34b is returned into the second channel 32 and is second The rotating member 44 is wound around. As a result, the second wire portion 34b exposed from the second channel 32 is shortened, and operability can be improved.

Next, an endoscope insertion / extraction device according to a second embodiment of the present invention will be described with reference to FIGS. In each embodiment described below, portions having the same configuration as those of the endoscope 1 according to the first embodiment described above are denoted by the same reference numerals and description thereof is omitted.
In the endoscope insertion / extraction apparatus 60 according to the present embodiment, the difference from the first embodiment is that the endoscope 1 has a first channel 31 and a second channel 32 inside the endoscope insertion portion 11. However, instead of this, it is detachable from the endoscope 70 having the forceps channel 61.

As shown in FIGS. 12 and 13, the endoscope insertion / extraction device 60 according to the present embodiment is attached to the endoscope 70 and advances and retracts the treatment instrument 20 in a forceps channel 61 formed in the endoscope 70. A hood 64 that is attachable to and detachable from the endoscope distal end portion 12, a pulley 62 that is rotatably provided on the hood 64, and is wound around the pulley 62 and folded back into the forceps channel 61. The distal end of the operation wire 63 is fixed to the forceps plug 65 provided in the forceps port 18 inserted through the forceps channel 61 and the hood 64 is fixed to the fixing member 66. The proximal end side is fixed, and an insertion tube 67 is provided through which the operation wire 63 on the handle portion 36 side folded back at the distal end of the forceps channel 61 is inserted. Further, the hood 64 is a pipe-like member, and the forceps 22 can protrude outward from the insertion hole 64a.

The operation wire 63 is provided at one end, can be attached to the distal end side of the treatment instrument 20 and can be inserted into the forceps channel 61, and is provided at the other end from the proximal end side of the forceps channel 10 to the outside. And a handle portion (treatment tool advancing / retreating operation portion) 36 is provided.
The fixing member 66 is provided with a support base 68 that supports the treatment instrument 20. As shown in FIG. 14, the support base 68 is formed with a recess 68a for placing the forceps 22 of the treatment instrument 20 and a rail 68b on which the insertion portion 21 can be slid.

Next, a method of using the endoscope insertion / extraction apparatus 60 of the present embodiment having the above configuration will be described below.
First, the hood 64 is attached to the endoscope distal end portion 12, the fixing member 66 is attached to the forceps plug 65, the engaging member 35 is installed on the support base 68, and the insertion tube 67 is attached to the endoscope distal end portion 12 with an adhesive tape or the like. Thus, it is fixed in some places and the insertion part 11 of the endoscope 70 is inserted into the body cavity. After the insertion, the insertion portion 21 is fitted into the rail 68b. In this state, the cutout portion 35b of the engagement member 35 is attached to the distal end 21a of the insertion portion 21 of the treatment instrument 20. Then, the handle portion 36 is pulled outside from the forceps opening 18 of the forceps channel 61. As a result, the operation wire 63 is wound around the pulley 62, and the operation wire 63 is moved from the insertion tube 67 toward the fixing member 66, and the treatment instrument 20 is moved along the distal end portion 12 of the endoscope as the operation wire 63 moves. The forceps 22 is moved from the endoscope distal end 12.
Next, when the treatment of the affected part or the like is completed, the operation wire 63 is moved to the forceps port 18 side via the pulley 62 by pulling the treatment tool 20 from the forceps port 18 side to the outside. As a result, the treatment instrument 20 is moved to the fixing member 66, the treatment instrument 20 is supported by the support base 68, and the treatment instrument is removed from the engagement member 35.

That is, according to the endoscope insertion / extraction device 60 according to the present embodiment, the treatment tool 20 can be conveyed to the distal end side of the forceps channel 61 via the pulley 62. In addition, by attaching the endoscope insertion / extraction device 60 to the endoscope 70, the endoscope can be easily transported. In addition, since the support member 68 is provided on the fixing member 66, when the treatment tool 20 is attached to and detached from the engagement member 35, the work can be performed while the engagement member 35 is supported on the support table 68. Therefore, since the attaching / detaching operation of the treatment instrument 30 becomes easy, the attaching / detaching time can be shortened.

In the present embodiment, the fixing member 35 can be attached to and detached from the forceps plug 65. However, instead of this, the fixing member 35 may be in the vicinity of the forceps plug 65, and may be attachable to and detachable from the operation unit 14, for example. Moreover, in each said embodiment, although the forceps part 22 was used as the treatment tool 20, it is not restricted to this, For example, things, such as a snare, may be sufficient.
The technical scope of the present invention is not limited to the above-described embodiments, and various modifications can be made without departing from the spirit of the present invention.
