Abstract
Description
The present invention relates to an improvement of a stent that can be embedded in blood vessels and body cavities and is used for treatment of a stenosis. The present invention relates to a stent that provides a surface such as suppression and prevents restenosis.
A stent is a tubular medical device that is placed at a site in order to expand a stenosis part or the like and secure a necessary lumen region when a blood vessel or other in-vivo lumen is narrowed or occluded. The stent is inserted into the body with a small diameter and expanded at a stenosis or the like to increase the diameter and expand and hold the lumen.
For example, in recent years, for the purpose of reconstructing a stenotic blood vessel, treatment by expansion using a PTCA balloon catheter and placement of a stent is performed by percutaneous coronary angioplasty (hereinafter referred to as PTCA: Percutaneous Transluminal Coronary Angioplasty). ing. The stent is intended to prevent or suppress restenosis by placing it in a blood vessel formed by inflating and expanding air by sending it to a balloon attached to a catheter.
In this way, stent placement was performed with the expectation of suppressing restenosis for angioplasty by balloon expansion alone, but in reality, the restenosis rate was as high as the initial expectation. The current situation is that no decline has been obtained.
This is considered to be one of the causes that induces thrombus formation and smooth muscle proliferation due to a vascular disorder caused by the stent itself when the stent is expanded and placed.
For this reason, conventionally, in catheters and the like, attempts have been made to contain or bind these anticoagulants and the like in a polymer constituting the catheter tube to locally release these agents. However, in the case of a stent, since the base material is usually a metal, it is difficult to apply a technique such as bonding or impregnating directly to the base material, as is mainly done with catheters. Therefore, for a stent, only the method of dissolving and mixing the drug in an organic solvent together with a polymer and applying it is practically applicable.
However, in the inventions described in
The problems to be solved by the present invention are that it is difficult to adjust the amount of water-soluble drug retained in the polymer material and the sustained release, and the polymer material is easily peeled off.

Hereinafter, the present invention will be described in detail.
The
The drug holding layer M is made of a water-absorbing polymer material that contains a bioactive agent, releases the bioactive agent in vivo, and absorbs at least 100% of its own weight of water.
The primer layer P is made of a material having high affinity for both the stent base material 11 and the water-absorbing polymer material constituting the drug holding layer M.
The coating layer 12 is made of a polymer material that can coat the drug holding layer M.
In the present invention, in forming the drug holding layer M on the stent base material 11, first, a primer having a high affinity for both the stent base material 11 and the water-absorbing polymer material forming the drug holding layer M, The primer layer P is formed by treating the surface of the stent substrate 11.
Next, the drug-retaining layer M is formed by preparing a solution in which the water-absorbing polymer material is dissolved and applying or immersing the solution on the stent substrate surface 11 on which the primer layer P is formed to volatilize the solvent. Can do.
In addition, in order to introduce a water-soluble drug into the drug holding layer M, the stent base material 11 on which the drug holding layer M is formed in a desired physiologically active drug aqueous solution is immersed for an arbitrary time and then dried. Can be introduced. Further, the drug holding layer M is covered with a coating layer 12 made of a polymer material. In this manner, three coating layers (primer layer P / drug holding layer M / coating layer 12) are formed on the inner surface and outer surface of the stent base material 11, respectively. The
FIG. 1 is a schematic view showing an example of the
The material constituting the base material 11 of the stent may be known per se, and is not particularly limited. For example, a shape memory alloy such as stainless steel such as SUS316L, Ti—Ni alloy, Cu—Al—Mn alloy, or the like. , Titanium, titanium alloy, tantalum, tantalum alloy, platinum, platinum alloy, tungsten, tungsten tube, etc. A stitch-like substantially tubular body formed by the above is used. In addition, about the shape of a substantially tubular body, if the target physical property is obtained, it will not specifically limit.
The drug holding layer M used in the present invention is formed of a water-absorbing polymer material that contains at least a physiologically active drug and absorbs at least 100% of its own weight of water that can release the physiologically active drug in vivo. The
In the present invention, examples of the water-absorbing polymer material include known starch / polyacrylonitrile hydrolysates, cross-linked starch / polyacrylate, cross-linked carboxymethyl cellulose, saponified vinyl acetate / methyl acrylate copolymer, polyacrylic Sodium acid cross-linked product, cross-linked polyvinyl alcohol, and polyacrylamide-based material, among which materials having a water absorption of 100 times or more of their own weight can be used. The water-soluble polymer material is preferably a water-absorbing polymer material that is soluble in an organic solvent and can be coated on a stent, and that can follow at least without exfoliation according to the expansion of the stent. Polypropylene oxide chain, polytetramethylene A polymer having a polyether chain such as an oxide chain in its structure. Urethane elastomers and gels or gel-like water absorbent polymer material based on them are preferred. Examples of the bioabsorbable polymer include polymers such as polylactic acid, polylactic acid-based copolymer, polyglycolic acid, and polyglycolic acid-based copolymer.
The water-absorbing polymer material that can be used in the present invention is not limited to those listed above, and does not dissolve in water, blood, or physiological saline, and efficiently contains a water-soluble bioactive agent. Therefore, it is possible to contain water of 100% or more of its own weight (the same amount as its own weight), and an effective amount of the physiologically active agent is gradually eluted from the surface through the coating layer 12 in the living body (sustained release). Anything having the above characteristics can be used.
Further, as the material of the coating layer 12, the release of the bioactive agent from the drug holding layer M as a stent is not hindered, and controlled release over a certain period by controlling the type (material), thickness, etc. of the coating layer 12 Polyether-based, polycarbonate-based polyurethane, polyester, polyether polyamide, and the like are preferable because they are easy to maintain. More specifically, an intravascular stent placed in a blood vessel is placed in the body semi-permanently (implant) unlike a medical device that performs temporary treatment or placement such as a catheter. Polycarbonate-based polyurethane having high hydrolyzability is most preferable as a material for the coating layer 12 of the endovascular stent of the present invention.
As will be described later, the water-absorbing polymer material constituting the drug retaining layer M is applied to the stent substrate 11 and dried. In the present invention, the stent substrate 11 and the stent substrate 11 are coated. It is important that the primer layer P is formed on the stent base material 11 in advance by pretreatment with a primer having high affinity for both of the water-absorbing polymer materials.
That is, the primer layer P is formed by first treating the surface of the stent substrate 11 with a primer having affinity for both the constituent material (metal) of the stent substrate 11 and the water-absorbing polymer material, Then, after applying with a water-absorbing polymer material to form the drug holding layer M, the stent substrate 11 is dipped in an aqueous solution of the bioactive drug for a predetermined time and then dried to dry the bioactive drug. It is introduced into M and further forms a coating layer 12 made of a polymer material.
In the present invention, various primers can be used. For example, parylene (polyparaxylylene, polychloroparaxylylene) can be used. Parylene can be formed as a primer layer P by forming a thin film on the stent by vacuum chemical vapor deposition (CVD).
A silane coupling agent having a hydrolyzable group and an organic functional group is also a preferred primer.
The hydrolyzable group (for example, alkoxy group) of the silane coupling agent forms a silanol group and is bonded to the metallic stent substrate by covalent bond or the like, while the organic functional group (for example, epoxy group, amino group, mercapto) Group, vinyl group, methacryloxy, etc.) are bonded to a water-absorbing polymer material containing a bioactive agent by chemical bonding, and the water-absorbing polymer material and the stent substrate 11 are bonded via a silane coupling agent. Compared to the case where this is not used, it is fixed much more closely. Here, an epoxy-based silane coupling agent having an epoxy group as an organic functional group is particularly preferable, but an amino-based or mercapto-based silane coupling agent having the same performance as this can also be suitably used. .
In addition, cyanoacrylate adhesives and polyurethane paste resins can be used as appropriate.
As the silane coupling agent, an appropriate one having an organic functional group corresponding to the resin of the water-absorbing polymer material to be used may be selected. For example, the following can be used preferably. That is, 3-glycidoxypropyltrimethoxysilane, 3-glycidoxypropyldimethoxysilane, 2- (3,4-epoxycyclohexyl) ethyltrimethoxysilane, vinyltriethoxysilane, 3-methacryloxypropyltrimethoxysilane, N-2 (aminoethyl) 3-aminopropylmethyldimethoxysilane, N-2 (aminoethyl) 3-aminopropyltrimethoxysilane, 3-aminopropyltriethoxysilane, 3-mercaptopropyltrimethoxysilane, N-phenyl- 3-Aminopropyltrimethoxysilane 3-chloropropyltrimethoxysilane and the like are preferably used.
In addition to the above silane coupling agent, the primer has an organotitanium cup such as tetrabutoxy titanate, tetrastearyl titanate, tetraisopropoxy titanate, isopropoxy titanium stearate, titanium lactate having the same function as this. Ring agent; Aluminum coupling agent such as acetoalkoxyaluminum diisopropylate: Chrome coupling agent; Organophosphoric acid coupling agent can also be used as appropriate.
A primer such as a silane coupling agent having affinity for both the constituent material (metal) and the water-absorbing polymer material of the stent base 11 is usually water, a suitable organic solvent, or water and an organic solvent. Pretreatment is carried out by dissolving in a mixed solvent and applying as a treatment solution to the substrate surface and drying. Although it does not specifically limit as an organic solvent, For example, methanol, ethanol, isopropanol, the mixed solvent of the said alcohol and water, benzene, toluene, xylene, etc. are used preferably. A relatively dilute solution is used as the primer concentration in the treatment solution, and it is 0.05 to 2% by mass, preferably 0.1 to 1.0% by mass.
In the present invention, such a primer treatment (formation of the primer layer P) is performed on the stent base material 11 before the water-absorbing polymer material is coated, so that the stability of the water-absorbing polymer material coating is improved. When the stent obtained is placed in the body and implanted in a blood vessel, for example, it is placed in the body for a period sufficient to form an intima, for example, for at least about one month. Even in this case, the coating layer (drug holding layer M, coating layer 12) can be stably maintained without any peeling or the like in a water (physiological saline solution, phosphate buffer solution) environment. An effect can be produced. In this regard, when the water-absorbing polymer material is directly applied to the metal constituting the stent substrate, depending on the conditions, the coating layer (drug holding layer M) can be formed in several days to several weeks in an aqueous environment. Since peeling may occur, it can be said that for a long period of time as described above, the stent is extremely difficult to place in the body.
In the present invention, the primer layer P is formed on the stent base material 11 as described above, and the stent base material 11 is coated with a water-absorbing polymer material that absorbs at least 100% of its own weight. After the drug holding layer M is formed on the base material 11, the stent base material 11 is immersed in an aqueous solution of the bioactive drug for a predetermined time and then dried to introduce the bioactive drug into the drug holding layer M. it can. Further, the medicine holding layer M of the stent base material 11 is covered with a polymer material to form a covering layer 12, thereby forming three coating layers (primer layer P / drug on the inner surface and outer surface of the stent base material 11). A holding layer M / covering layer 12) is formed, and the

Here, the bioactive agent defined in the present invention is, for example, an anticancer agent such as paclitaxel, a gene therapy agent such as HGF (hepatocyte grows factor), an antiplatelet agent such as ticlopidine hydrochloride, other antibiotics, and an immunosuppressive agent. In addition, it is not particularly limited as long as it is water-soluble and can be introduced into the drug holding layer M, such as a statin drug, and can be gradually released (released) from the coating layer 12 by diffusion or other mechanisms in the living body, These known physiologically active agents can be used, and other than these may be used.
Hereinafter, embodiments of the stent of the present invention will be described with reference to the drawings.
FIG. 1 is a schematic view showing an example of the stent of the present invention as described above, but the
As described above, the coating layer 12 and the drug holding layer M can be formed by applying the coating solution by spraying or dipping, as described above. Here, spraying and dipping may be performed once, but the thickness of the coating layer (coating layer 12, drug holding layer M) is adjusted by the concentration of the polymer material forming the coating layer 12 and drug holding layer M, etc. In order to do this, a method of coating in multiple times is usually used.
