Abstract
Description
The present invention relates to an insufflation apparatus and a trocar for supplying an insufflation fluid such as carbon dioxide gas for the purpose of ensuring a space for ensuring a visual field or a space for smoothly operating a treatment instrument in the abdominal cavity. And a pneumoperitoneum system.
In recent years, laparoscopic surgery (hereinafter, also referred to as “surgical operation”) for performing therapeutic treatment without performing laparotomy has been performed for the purpose of reducing invasion to patients. In this surgical operation, as shown in FIG. 28, for example, a
In this surgical operation, for the purpose of ensuring the field of view of the
The pressure in the abdominal cavity is detected by a pressure sensor (not shown) provided in the carbon dioxide gas supply path in the
In addition, during the surgical operation, the patient may experience tremors called shivering due to a decrease in body temperature due to factors such as inhibition of the central nervous system due to anesthesia, room temperature, inflow of carbon dioxide or water supply. For this reason, methods such as warming a warmer mat or shoulder blanket over the patient's body or heating carbon dioxide gas fed into the body cavity to near the body temperature have been used.
However, the humidifying and warming device shown in the surgical system of Japanese Patent Application Laid-Open No. 2003-153912 is separate from the insufflator and relatively large. For this reason, the space for installing this apparatus is required, and problems such as complicated preparations before surgery occur. In addition, there is a problem that it takes a cost to newly purchase this apparatus.
In the
Further, the flow path of carbon dioxide gas in a state where the
In the insufflator, the pressure in the abdominal cavity is measured by intermittently supplying carbon dioxide gas into the abdominal cavity, and the pressure is detected when the insufflation is stopped. Therefore, the supply of carbon dioxide gas into the abdominal cavity is intermittently performed. For this reason, the malfunction that it takes time until a pressure reaches | attains a predetermined pressure arises.
In addition, there is a case where it is desired to stop the insufflation of the temporary insufflation device during the operation and collapse the carbon dioxide in the abdominal cavity. At this time, in order to switch the operation state of the apparatus from the air supply state to the stop state, it is necessary to operate a switch provided in the insufflation apparatus main body. However, a doctor who performs an operation in a clean area cannot switch the switch of the insufflation apparatus main body installed in the unclean area. For this reason, instructions are given to nurses in the filthy area each time to switch the switch. Therefore, not only does the switching take time, but the doctor is mentally stressed.
The present invention has been made in view of the above circumstances, and an object of the present invention is to provide an insufflation system that can smoothly supply the insufflation fluid into the abdominal cavity and is excellent in usability.
The insufflation system of the present invention includes a trocar inserted into the abdominal wall, an insufflation device that performs supply control of a predetermined insufflation fluid, and the insufflation device and the trocar connected to the insufflation device. An insufflation system comprising an insufflation tube for feeding an insufflation fluid to be supplied,
The trocar includes a tube attachment portion to which the pneumoperitoneal tube is connected, a fluid supply path for guiding the pneumoconitory fluid sent through the pneumoperitoneal tube to the abdominal cavity, and pressure, temperature, or humidity in the abdominal cavity. At least one detection means of a pressure sensor, a temperature sensor, and a humidity sensor to detect directly and a state in which a pneumoperitoneal fluid is sent into the abdominal cavity or a pneumoperitoneal fluid that is fed into the abdominal cavity is sent. A selection means for selecting whether to stop breathing; and a signal cable through which a signal line for transmitting an electrical signal output from the selection means and the detection means is inserted. It has a function of setting the pressure of the insufflation fluid supplied into the abdominal cavity to a predetermined pressure, an insufflation base connected to the insufflation tube, an electrical connector to which the signal cable extending from the trocar is connected valve A knit, a sensor unit that detects the state of the pneumoperitoneum fluid, and a control unit that performs air supply control based on a detection signal output from the sensor unit and an electrical signal output from the trocar. .
According to this configuration, the insufflation fluid supplied from the insufflator to the insufflation tube is smoothly supplied into the abdominal cavity through the fluid supply path provided in the trocar. Also, by operating the selection means provided on the trocar by the surgeon in the sterilized area, the insufflation state of the insufflator can be maintained without bothering the hands of the nurse in the unclean area. Can be switched to. Further, the detection signal output from the sensor unit and the electrical signal directly detected and output by the detection means provided on the trocar are input to the control unit. Thus, the control unit performs optimal control based on the input detection signal and electrical signal.
Further, it is detected whether or not a signal line for transmitting an electrical signal output from a pressure sensor as one of the detection means is inserted in the signal cable connected to the electrical connector in the pneumoconitory device. The pressure sensor presence / absence detection unit outputs the detection result to the control unit.
According to this configuration, when a detection signal from the pressure sensor presence / absence detection unit is input to the control unit, the control mode is switched so as to correspond to the detection signal from the pressure sensor which is one of the detection means provided in the trocar. The valve unit is controlled based on the electrical signal related to the intra-abdominal pressure to be sent, and the insufflation fluid is continuously supplied from the insufflator to the trocar. On the other hand, when a detection signal from the pressure sensor presence / absence detection unit is not input to the control unit, the control mode is switched so as to correspond to the control signal, and the valve unit is controlled based on the detection signal from the sensor unit. The insufflation fluid is intermittently supplied from the vessel to the trocar.
Further, in the configuration in which the temperature sensor for directly detecting the temperature in the abdominal cavity is provided as a detecting means on the trocar,
The trocar includes heat generating means in the vicinity of the tube mounting portion, and controls the heat generation state of the heat generating means on the basis of an electrical signal output from the temperature sensor by the control unit of the insufflator.
According to this configuration, the temperature of the insufflation fluid flowing in the valve unit, the temperature in the operating room, and the like are detected by the sensor unit, while the temperature in the abdominal cavity is detected by the temperature sensor, and the temperature information detected by each is controlled. To input. Then, the control unit adjusts the heat generation state of the heat generating means to an optimum one based on the input temperature information. As a result, the ideal temperature of the pneumoperitoneum fluid is supplied into the body cavity.
Further, in the configuration in which the temperature sensor for directly detecting the temperature in the abdominal cavity is provided as a detecting means on the trocar,
The trocar includes a moisture absorber in addition to the heat generating means in the vicinity of the tube attachment portion.
According to this configuration, by supplying moisture to the hygroscopic body and supplying the insufflation fluid from the insufflator, the insufflation fluid containing moisture is supplied into the abdominal cavity.
Further, the trocar is provided with a humidity sensor for directly detecting the humidity in the abdominal cavity as a detecting means, and a display unit for displaying the detection result of the humidity sensor is provided on the abdominal cavity.
According to this configuration, the humidity detected by the humidity sensor located in the abdominal cavity is displayed on the display unit, so that the humidity in the abdominal cavity can be reliably grasped.
According to the present invention, the supply of the pneumoperitoneum fluid into the abdominal cavity can be smoothly performed through the fluid supply path provided in the pneumothorax trocar. In addition, the abdominal device can be switched to the insufflation state or the insufflation stop state by operating the selection means at hand. Control the valve unit based on electrical signals output from various sensors to the control unit or detection signals output from the sensor unit to the control unit, so that a predetermined pressure is applied from the insufflator to the trocar. It is possible to provide a pneumothorax system that can supply a fluid and that is excellent in ease of use, such as guiding a pneumoperitoneum fluid at an ideal temperature into the abdominal cavity or guiding a pneumoperitoneum fluid containing moisture in the abdominal cavity. it can.

The

As shown in FIGS. 5 and 6, the

As shown in FIGS. 2 and 8, a

In addition, the setting of the pressure in the abdominal cavity, the setting of the flow rate of the insufflation gas, the temperature setting of the temperature in the abdominal cavity, etc. can also be performed by a setting display panel (not shown) arranged in the sterilization area of the operating room (not shown).

In using the
Next, the
The
Next, the intraperitoneal
Thereafter, a predetermined amount of
Here, the air
In this air supply state, the
As a result, the remaining gas amount is displayed on the gas remaining
Further, the
Further, when the
Here, when the doctor temporarily stops the insufflation by the
On the other hand, when it is determined that the pneumoperitoneum desired by the operator has been reached before the intraabdominal pressure detected by the
If the
In this way, the pneumothorax supplied to the pneumoperitonee trocar by supplying the pneumoperitoneum trocar having a dedicated flow channel for air supply into the abdomen and supplying the pneumoperitoneum fluid from the pneumoperitoneum The insufflation fluid can be surely guided into the abdominal cavity by smoothly passing through the flow path. In this way, it is possible to supply a pneumoperitoneum fluid into the abdominal cavity as requested by the doctor.
In addition, by providing a pressure sensor for detecting the pressure in the abdominal cavity in the pneumothorax trocar, the pressure sensor located in the abdominal cavity enables continuous measurement of the intra-abdominal pressure with high accuracy. The supply time can be shortened by continuously supplying the fluid into the abdominal cavity.
In addition, when the second through hole in which the pressure sensor is disposed and the outside are communicated with each other by the second side hole having a plurality of measurement holes, when the pneumothorax trocar is inserted into the abdominal wall, Even if one or more of the side holes become clogged, the intraperitoneal cavity and the second through hole are in communication with each other through several side holes or one side hole, and the pressure by the pressure sensor Can be reliably measured.
Furthermore, by providing a temperature sensor for detecting the intraperitoneal temperature at the insertion portion of the pneumothorax trocar, the temperature in the abdominal cavity can be measured with high accuracy by the temperature sensor located in the abdominal cavity.
In addition, the pneumo-abdominal trocar is provided with a heating unit along with a temperature sensor, the pneumoconitory device is provided with a fluid temperature measurement sensor and a room temperature measurement sensor, and the heating unit is fed back based on the measurement values input from these sensors to the control unit. By controlling, the temperature of the insufflation fluid guided into the abdominal cavity is controlled to an optimum temperature, and it is possible to reliably prevent the body temperature from being lowered by the insufflation fluid introduced into the abdominal cavity. In addition, the intraperitoneal temperature can be maintained at a predetermined temperature.
In addition, by providing an air supply control switch on the body part of the pneumothorax trocar, the doctor in charge of the operation in the sterilization area directly operates the air supply control switch to determine the air supply state of the insufflation apparatus, etc. Control can be performed instantly by one's own action.
Further, by providing a hygroscopic body in the flow path of the insufflation trocar, when the insufflation fluid is a dry carbon dioxide gas having a humidity of approximately 0%, by preliminarily containing moisture in the hygroscopic body The carbon dioxide gas passing through the flow path contains moisture in the hygroscopic material, and the carbon dioxide containing moisture can be introduced into the abdominal cavity.
Note that the pneumothorax trocar of this embodiment is a disposable type.

As shown in the figure, in the insufflation trocar 9A of the present embodiment, the
In this way, by providing a dedicated tube connector portion having a through-hole having a large diameter, the insufflation fluid can be supplied to the insufflation trocar at a large flow rate. As a result, the insufflation fluid is more reliably supplied into the abdominal cavity.

Thus, by providing a first tube connector part that can connect a general-purpose insufflation tube to an insufflation trocar, and a second tube connector part that can supply an insufflation fluid at a large flow rate, A general-purpose pneumoperitoneal tube can be used, and by preparing a dedicated connector, a pneumoperitoneal fluid can be supplied to the pneumothorax trocar at a large flow rate.
In addition, by providing a check valve in each through hole of each tube connector part, the carbon dioxide gas supplied into the abdominal cavity from the tube connector part not connected to the pneumoperitoneal tube flows backward and is discharged to the outside. Can be prevented.

Here, since the contact area of the high-frequency treatment instrument 61 with the human body is small, the current density of the high-frequency current becomes extremely high, and the cell tissue in the contact portion is evaporated by the high-frequency energy. On the other hand, since the contact area on the

In the case where the
In this way, in addition to the first through hole and the second through hole in the insertion portion of the insufflation trocar, by further providing a through hole and providing a humidity sensor for detecting humidity in the abdominal cavity in the insufflation trocar, The humidity in the abdominal cavity can be continuously measured with high accuracy by the humidity sensor located in the abdominal cavity.
In addition, when the third through hole serving as the second flow path is communicated with the outside through the third side hole having a plurality of communication holes, the pneumoconiosis trocar is inserted into the abdominal wall in the unlikely event. Even if one or more of the holes become clogged, the abdominal cavity and the third through hole are surely communicated with each other through several side holes or one side hole, and exhaust, water supply, etc. It can be performed.

In the present embodiment, the
As described above, according to the pneumothorax trocar configured by arranging the solenoid valve for switching the pneumothorax fluid between the air supply state and the air supply stop state in the internal space portion of the main body, Since supply control into the abdominal cavity can be performed in the vicinity of the abdomen, measurement of the intraperitoneal pressure can be performed with high accuracy in the air supply stop state.
As shown in FIG. 19, the

The

In this way, according to the insufflation trocar configured by arranging the manual opening / closing mechanism for switching the insufflation fluid between the insufflation state and the insufflation stop state in the internal space of the main body, FIG. In addition to the effects of the pneumoperitoneum trocar shown in Fig. 1, supply of pneumoconitory fluid at the operator's discretion, such as by observing the intra-abdominal pressure displayed on the pneumoperitoneum and the intra-abdominal state displayed on the display device Control can be performed.
20 to 22 relate to a second embodiment of the present invention, FIG. 20 is a diagram for explaining the configuration of an insufflation trocar through which an endoscope is inserted, and FIG. 21 is a sectional view taken along the line D-D in FIG. FIG. 22 is a view showing a state where an endoscope is inserted through a pneumothorax trocar.
As shown in FIGS. 20 and 21, the
Specifically, the
For example, a
On the other hand, the
An
The
The closing
Moreover, the
On the other hand, the
The signal lines 24 a, 25 a, 26 a, 27 a and the
Other configurations are the same as those of the first embodiment, and the same members are denoted by the same reference numerals and description thereof is omitted.
As shown in FIG. 22, the

Thus, even when the central through hole for the purpose of inserting the insertion portion of the endoscope is formed in the pneumothorax trocar, by providing the air supply through hole serving as a dedicated flow channel for air supply, The insufflation fluid supplied from the insufflator to the insufflation trocar can be reliably guided into the abdominal cavity.
In this embodiment, the insertion portion of the endoscope is inserted through the central through hole provided in the pneumothorax trocar. However, the medical instrument inserted through the central through hole is limited to the endoscope. It may be a treatment tool or the like.
FIG. 23 to FIG. 25 are diagrams for explaining a configuration example of a reusable insufflation trocar according to the third embodiment of the present invention, and FIG. 23 explains an insufflation trocar in which a balloon is provided in the insertion portion. FIG. 24 is a cross-sectional view, FIG. 24 is a view for explaining the action of the pneumothorax trocar, and FIG. 25 is a view for explaining the relationship between the pressure applied to the balloon and the intraabdominal pressure.
In the
As shown in FIG. 23, the second through

The operation of the

Here, as shown in FIG. 23, the open / close valve 154a provided in the
Next, for example, the insufflation state is operated by operating the

When the
In this way, it is difficult to clean and disinfect by attaching a balloon to the insertion portion of the pneumothorax trocar and covering the communication hole communicating with the second through hole in which the pressure sensor is arranged with this balloon. The body fluid or the like can be prevented from entering the measurement hole or the second through hole, and the pneumothorax trocar can be reused.
When the air
At this time, the temperature and humidity in the abdominal cavity are measured by the
Smoke is exhausted by the
In this way, the pneumothorax trocar is composed of a trocar body and a measuring tool, and the measuring tool is provided with a through-hole that requires measurement that was difficult to clean and disinfect. In this way, the trocar body can be reused.
It should be noted that the present invention is not limited to the embodiments described above, and various modifications can be made without departing from the spirit of the invention.
