Abstract
Description

L=ra。
【0130】
a=(Et3/6Pr3)1/2・・・(式2)
【0131】

This application claims priority to and from U.S. Patent Application No. 60 / 257,171, "Rectifying Vent," U.S. Patent Application No. 60 / 257,171, filed December 22, 2000, which is incorporated herein by reference.
[0002]
The present invention relates to a vent valve device for use with a system for supplying humans with breathable gas pressurized above atmospheric pressure.
[0003]
The invention is primarily used to control the emission of exhaust in a continuous positive airway pressure (CPAP) gas supply system used, for example, in the treatment of obstructive sleep apnea (OSA) and similar sleep disordered breathing. Developed to be. The invention may also be used in connection with masks and gas supply systems suitable for assisted ventilation treatment applications.
[0004]
The term "mask" is intended herein to include facial masks, nasal masks, mouth masks, attachments proximate any of these masks, and the like.
[Background Art]
[0005]
Treatment of OSA with a CPAP gas delivery system involves the continuous delivery of air (ie, breathable gas) pressurized above atmospheric pressure through a tube and mask to the patient's airway. Typically 4cmH 2 O ~ 30cmH 2 CPAP pressure of O is used for treatment of sleep disordered breathing due to OSA and / or central apnea, depending on the requirements of the patient.
[0006]
Therapeutic pressure for assisted ventilation is 32 cmH, depending on patient requirements. 2 It can vary up to O as well as beyond.
For both OSA treatment and assisted ventilation applications, the pressure of the gas delivered to the patient can be constant, two levels (synchronized when the patient exhales and exhales), or automatically. Level can be adjusted. Throughout this specification, CPAP is intended to include reference to any one or combination of these forms of pressure supply. Prior art methods of providing CPAP treatment have vents for outgassing of the gas stream. The vent is typically located at or near the mask or in a gas supply line. Gas flow through the vent is essential to remove gas exhaled from the breathing circuit. Adequate gas emissions are typically around 4 cmH for adults 2 O, 2cmH for pediatric applications 2 Achieved by selecting the size and shape of the vent to allow minimal safe gas flow at the lowest operating CPAP pressure, which can be as low as O.
[0007]
Existing vent configurations include one or more holes, foam or other diffusers, slots and combinations thereof. Reference herein to a vent may be understood to include reference to one or more holes, foam or other diffusers, slots or any combination thereof.
DISCLOSURE OF THE INVENTION
[Problems to be solved by the invention]
[0008]
Obviously, in order for the standard configuration to properly provide the unique treatment required by a wide variety of users, it is desirable for the CPAP system to have a wide pressure range that can be implemented. Increasing the CPAP pressure causes more gas to pass through the vent, which creates more noise. Existing prior art vents have a CPAP pressure of about 4 cmH 2 Raising above O can generate excessive noise. This noise adversely affects the comfort of the patient and the person sleeping with the patient. At higher pressures, existing vents are not efficient. Because existing vents allow more gas to flow through the vent than is needed for proper exhaust gas flow, thereby providing more flow to the flow generator than is necessary to maintain the required therapeutic pressure. Must provide. Furthermore, if a therapeutic gas such as oxygen is being supplied, excess therapeutic gas is released and is therefore unnecessarily wasted. Similar waste occurs when the gas being supplied is wet.
[0009]
The flow of gas from the gas supply system through the vent to the atmosphere depends on when the delivered gas passes through the vent to the atmosphere, as well as CO2 2 When a patient exhales a gas that contains gas, a noise is generated. The CPAP system has a flow rate through the vent to the atmosphere that ensures that clinically undesirable levels of exhaled gas do not remain in the breathing circuit (ie, in the gas supply and mask chambers). Have to do it. This stagnation of gas is the result of the gas exhaled during the exhalation phase of breathing not being vented to the atmosphere, but instead moving down the gas line towards the flow generator or accumulating in the dead space of the mask chamber. Happens as. Appropriate flow of gas to the atmosphere requires the selection of the appropriate vent size to achieve the desired therapeutic pressure range for the therapeutic range of clinically desirable pressure and the volume of gas that the flow generator can provide. May be obtained by: Typically, this choice selects a vent size large enough to obtain adequate flow at the lower end of the pressure range, and does not cause noise above the acceptable noise level as pressure increases through the pressure range With the compromises that can be made between things. Also, the large vent, which allows for abundant effluent flow at the lower end of the pressure range, the flow generator has the appropriate capacity to provide the necessary flow to obtain the desired higher pressure in the pressure range. Determine what must be done. Simply put, if the vent size is selected to achieve a quiet gas outflow at the higher pressure level within the pressure range, it allows an acceptable outflow at the desired bottom end of the pressure range May not be appropriate for you. Also, vents that are large enough to obtain adequate effluent flow at low pressures within the pressure range tend to produce unacceptable noise at the desired higher end of the pressure range. Also, the choice of a larger vent allows the gas source to have the capacity to deliver the required flow rate for higher pressure levels, thus consuming more power, generating more noise, It then determines that additional noise attenuating features are needed to keep the total noise within acceptable limits.
[0010]
Due to the limitations of the CPAP system design resulting from venting, an option may be made to limit the lower or upper achievable pressure. That is, for any upper or lower pressure, the delta P between that pressure and the other end of the range may be inconsistently constrained. Delta P is selected to achieve the desired purpose of adequate evacuation gas exhalation at the lower end of the pressure range while limiting noise and power consumption at the higher end of the pressure range. The choice of upper or lower pressure and such limitations on Delta P significantly limit the usefulness of CPAP. This is because the standard configuration preferably has the capacity to provide the widest pressure range so that it can meet the clinical requirements of as many users as possible. An implementation of this objective is the operation of a control algorithm where the CPAP treatment changes the pressure delivered to the user during the treatment period (eg, between two or more pressures per breath or in a more complex manner during the treatment period). This is particularly important when accompanied by Similarly, computer-controlled CPAP systems that vary pressure during a treatment period in response to a control algorithm have operating parameters that reflect the ventilation characteristics of the breathing circuit. For this reason, it is undesirable to change from the mask specified for the control algorithm with respect to the concern that the new mask must introduce ventilation characteristics that are not within the operating parameters of the control algorithm. The inability to change masks due to the introduction of unknown or incompatible ventilation features adversely affects the patient's suitability for CPAP treatment. This is because if the CPAP treatment is implemented through a particular mask, and the mask is incompatible with the prescribed CPAP system control algorithm, the patient may have to endure the CPAP treatment.
[0011]
It is, therefore, another object of the present invention to provide a method of constructing and fabricating a vent that can change the ventilation characteristics of a mask so that the mask is more compatible with the operating parameters of a CPAP system control algorithm.
[0012]
A further object of the present invention is a method and apparatus for a ventilation system that produces a vent having a flow region that changes in response to pressure changes that occur over part or all of the pressure operating range of a CPAP system.
[0013]
It is known in the art that the breathing circuit of a CPAP system has a valve that limits or blocks ventilation to the atmosphere under any circumstances.

[0016]
Although each of these documents discloses embodiments that limit gas flow as gas supply pressure increases, it is not limited to achieving a constant flow rate but provides the opportunity to change the flow through the vent as the pressure changes. However, there is a need to provide a rectifying vent that is simpler and easier to manufacture.
[0017]
These valves are commonly known as anti-rebreathing or anti-asphyxia valves. One example of an anti-rebreathing valve is U.S. Pat. No. 5,438,981 to Star et al. For an automatic safety valve and diffuser for nasal and / or oral gas supply masks. It has a

Other examples of safety valves include U.S. Patent Nos. 5,896,857 and 6,189,532 (Helly / Risgow: Assigned to ResMed Limited) and WO 00/38722 (Walker et al .: (Transferred to ResMed Limited).
Embodiments of the vents of the present invention may also serve as anti-rebreathing or anti-asphyxia valves.
[0019]
Therefore, an object of the present invention is to provide a rectifying vent that can solve the above-mentioned problems. This object is achieved by a combination of features described in the independent claims. The dependent claims define further advantageous embodiments of the present invention.
[Means for Solving the Problems]
[0020]
The present invention is a rectifying vent that regulates flow from a pressurized gas supply. The vent is a fixed part adapted to engage the gas supply pipe and a movable biased by a spring force connected to the fixed part by a hinge and connected to the pressurized gas supply. Part. The fixed part has a gas flow opening. The movable part is pivotable between a relaxed position and a fully pressurized position. At a certain minimum operating pressure, the movable part is pivoted away from the fixed part by a spring force to a relaxed position, establishing a first gas flow area between the movable part and the gas flow opening. At certain higher operating pressures, the pressurized gas cancels the spring force and pivots the movable part to a fully pressurized position adjacent to the fixed part, thereby moving the movable part and the gas flow opening. To establish a minimum gas flow area. In a preferred embodiment, the fixed and movable parts are formed as one piece from one piece of material, for example a sheet of stainless steel or a plastic sheet.
[0021]
By adjusting the operating characteristics of this rectifying vent (ie, the size of the gas flow openings at any pressure), the flow (which is the flow through the vent) curve is relatively constant over a specified operating pressure range, or A flow curve that is not constant above a specific operating pressure range can be fitted.
[0022]
In a further embodiment, the rectifying vent comprises as a flow meter by including a strain gauge for measuring the position of a movable part mounted between a fixed part and a movable part and providing an indicator of flow through the vent. Can work. The signal generated by the strain gauge transducer, together with the pressure, is used to determine gas flow through the vent.
[0023]
In another embodiment of the present invention, the rectifying vent has a flexible portion having a portion that engages or is attached to the fixed housing such that the free portion of the flap can move within any range relative to the fixed housing. It has a flap part which has the property. One side of the flap is exposed to the inside of the mask shell or gas flow tube that is pressurized when the CPAP system mask is used, and the other side is oriented toward the atmospheric side of the vent.
[0024]
The housing has a vent opening located below the free portion of the flap, and a portion of the housing surrounding the vent opening is curved. Although flexible, the flap has some inherent stiffness, thereby providing a spring resilience to bending of the flap. In the relaxed state, the free portion of the flap leaves the vent opening uncovered and the gas flow area between the vent opening and the flap is minimal. When the CPAP system is used, the force on the flap's spring resilience acts so that the free portion of the flap tends to move toward the vent opening. As the free portion of the flap moves closer to the vent opening as the mask pressure increases, it gradually closes the vent opening along the curved surface of the housing and the gas flow between the vent opening and the flap. Reduce the area. The interaction of the increasing mask pressure with the decreasing gas flow region serves to reduce the gas flow through the vent as compared to the flow achieved with a constant gas flow region vent.
[0025]
Another embodiment of the rectifying vent of the present invention opens the auxiliary expiratory vent opening while exhaling to allow more exhaled gas flow to the atmosphere. Certain embodiments may have an anti-rebreathing or anti-asphyxia valve function that reduces or eliminates exhaled gas retained in the gas circuit at the end of exhalation. These embodiments serve the desired purpose of eliminating, or at least reducing, the opportunity for a user to rebreath expiratory gas.
[0026]
In yet another embodiment, the vent of the present invention comprises a
[0027]
The rectifying vent of the present invention can be manufactured simply and inexpensively, but provides an effective, easily tailored rectifying vent. The rectifying vent reduces the operating noise of the CPAP system by reducing the required gas flow volume from the flow generator at high pressure, and thus also reducing the operating output of the flow generator. Vent is CO 2 And reduces rebreathing of other exhaled gases, providing faster pressure rise times, increasing the effectiveness of the CPAP system and the patient's suitability for CPAP treatment.

In a preferred embodiment, the vent is cut from one sheet of 0.1 mm thick polyester film of the type conventionally used for overhead projector transparency. Such films are available from the Orbit Company of Australia and other suppliers such as 3M and Xerox. Also, the vent need not be essentially flat as in the present embodiment, but can have any suitable different profile. For example, the vent may have a convex or concave shape. In addition, the thickness of the movable and / or fixed part can be increased, and the opening and / or the edge of the movable part can be provided with a smoother gas flow through the vent while allowing for a lower noise. The preferred embodiment described above uses a movable part located inside the fixed part, but in the opposite configuration where the outer part of the vent is movable and the inner part is fixed to the base or cover. It is also conceivable to use. It is also conceivable to use different combinations of alternative configurations discussed herein to create different vent embodiments. FIG. 10 shows a comparison of the performance of a preferred embodiment with a conventional vented CPAP mask “A” and “B”. As can be seen, the conventional masks "A" and "B" have a steeply rising flow curve, whereas the mask using the vent of the present invention has a less steep flow curve. ing. Therefore, 16 cmH 2 O, the mask using the rectifying vent of the present invention is 16 cmH 2 Has a flow rate that is about half the lower limit of the flow rate of the conventional mask “A” at O, and is 16 cmH 2 It has a flow rate less than half the average flow rate of the conventional mask "B" at O.
[0120]
In a bi-level CPAP, such as VPAPII by ResMed Limited, using the vent of the present invention, compared to a mask using a conventional fixed-area vent, the target mask pressure is obtained from the point at which the patient begins to inhale. Further tests show that the rise time is shorter. If the pressure rise time is too long, the patient will feel that they are not getting enough air when inhaling. Therefore, the rising time is preferably shorter. In one test, the rise time for the vent of the present invention was about 250 ms, compared to 300 ms for a conventional mask. Achieving improved rise time characteristics by incorporating the vents of the present invention into a CPAP system is a less expensive alternative to achieving the same effect by increasing the performance of the flow generator.
[0121]
In the CPAP mode, where it is desired that the pressure in the mask be kept relatively constant, the venting of the present invention is as effective as doing so, as a conventional fixed channel area vent. Thus, the vents of the present invention are compatible with constant pressure CPAP, and the vents of the present invention allow the CPAP system to exhale over a pressure range, even though the pressure remains fixed for any patient during the treatment period. May be used to ensure that the gas to be drained out properly. Further, in a mode where the flow generator is turned off, from testing, the vent of the present invention operates as an effective anti-asphyxia valve, opening the mask to the atmosphere by removing the
[0122]
The rectifying vent of the present invention operates to reduce the flow area of the vent as the pressure in the mask increases, thereby reducing the flow rate of the vent as compared to conventional fixed area vents. This is achieved by gradually moving the movable part of the vent with increasing pressure to gradually reduce the flow area between the movable part of the vent and the fixed part of the vent. Gradually moving the movable part can be achieved by exerting a spring force on the movable part so as to gradually resist the movement of the movable part with the increase in pressure.

One particular advantage of this embodiment over known vents is the ability and simplicity that the flow characteristics of the vent can be changed at a specific pressure within the expected operating pressure range. By changing the characteristics of the housing and flap as described above, the flow rate through the vent can be changed depending on the pressure level.

Generally, the vent of this embodiment operates under the following conditions. A large deflection angle results in a larger initial airflow through the vent, but slows the closing of the vent. A large radius of curvature causes the flap to close at a lower pressure. Large vents create a greater initial airflow, and the size of the vent opening is limited by the ability of the flap to seal the vent opening without deformation. In a mask using a fixed area bleed vent, the end of the

The rectifying vent of the present invention can be simple and inexpensive to manufacture, especially when cut from a single flat disk as described above, but provides an effective, easily adjustable rectifying vent. . With such an effective rectifying vent, the flow generator would supply high pressure and have the additional capacity to handle the increasing flow at higher pressures as in conventional CPAP systems. It does not have to be the size. Noise from the motor of the flow generator can also be reduced. This is because the motor can operate at lower RPM to achieve a reduced volume of high pressure airflow. The vent also acts as a noise barrier, reducing the level of noise from inside the mask, including noise generated from the flow generator and escaping to the atmosphere. In addition, the reduced flow at high pressure also reduces the noise generated from the airflow itself.
