【発明の属する技術分野】
本発明は、痛み緩和用光照射装置に関し、詳しくは皮膚上から光を皮下の目的部位に集中的に照射する痛み緩和用光照射装置に関する。

【従来の技術】
近年、ペインクリニックや整形外科領域では、肩こりや腰痛など皮下深部にある病巣の治療手段として低反応レベルレーザー治療機や直線偏光近赤外線治療機などの光線療法機器が利用されている。このような光線療法機器の例としては、レーザー光を治療目的部位に照射する装置(特許文献1)、また、単色光を治療目的部位に照射する装置(特許文献2)などがある。

一般にこれらの光線療法機器は、皮膚上から様々な波長の光を照射することで、こりや痛みを緩和するものとして知られている。

このような光線療法機器を用いた光線治療の作用としては、神経伝達遮断効果が知られていた。たとえば、小動物において、830nm半導体レーザーは運動神経や触覚を伝える感覚神経には影響を与えず、Aδ線維やC線維の興奮伝達を選択的に抑制することが報告されている(非特許文献1および2)。

さらに、循環改善による局所からの痛み関連物質(ブラジキニン、ヒスタミン、プロスタグランジンなど)や疲労関連物質(乳酸など)の拡散除去も重要視されている。また、このような循環改善効果の主たるメカニズムとして血管平滑筋に対する直接弛緩効果も知られるようになっている。

また、光の効果を高めるために、短波長側の光に効果があることが報告されている(非特許文献3〜5)。

これは、従来の光線療法器の光波長はレーザーでは810〜830nm、直線偏光近赤外線では600nm〜1600nm(ピーク波長1000nm)であるが、鎮痛機序の一つである循環改善(血管拡張など)効果はより短波長側で大きいことがわかってきたからである。特に、非特許文献3〜5によれば、非常に弱い出力の紫外線照射(300〜350nm Ultraviolet Irradiation)が血管を強く弛緩させるとしている。

【発明が解決しようとする課題】
上記した従来の光線療法機器は副作用が少ないということでは評価されているものの効果がまだ不十分であることや、治療が長期化するなどの問題点が指摘されている。

さらに、皮下深部にある病巣(筋・筋膜性の腰痛なら皮下脂肪組織の下にある筋膜や筋肉内の血管)に皮膚上から十分な光を到達させるには、比較的高い出力のエネルギーを照射する必要があり、皮膚表層部を損傷させてしまうこともある(臨床で使用されているもの中には出力が1000mWを越えるものもある)。

さらには、300〜350nmの紫外領域は皮膚へ有害な刺激作用があり、また組織深達性も低いことから皮膚からの光線治療には不向きと考えられる。また、それ以上の紫外領域および可視領域は有害な皮膚作用はないものの血液中のへモグロビンの吸収が大きく組織深達性は良くないため、事実上治療効果が得られない可能性が高い。

本発明は、上記のような問題点に鑑みてなされたもので、その目的としては、痛み緩和効果の高い波長の光を、病巣部に効率的に照射することのできる痛み緩和用光照射装置を提供することにある。

【課題を解決するための手段】
上記の目的は、下記の構成により達成される。

(1)痛み緩和作用を有する第1の波長の光を皮膚上から照射する第1の光照射手段と、痛み緩和作用を有する第2の波長の光を前記第1の光照射手段からの光とは異なる方向の皮膚上から照射する第2の光照射手段と、前記第1の光照射手段および前記第2の光照射手段から照射された光を皮下の目的部位で集光させる集光手段と、を有することを特徴とする痛み緩和用光照射装置。

(2)前記集光手段は、前記第1の光照射手段および前記第2の光照射手段の光照射方向を前記目的部位方向に保持する保持手段であることを特徴とする。

【発明の実施の形態】
以下、図面を参照して本発明の実施の形態を説明する。なお、各図において同一の機能を有する部材には同一の符号を付した。

まず、本願発明の痛み緩和用光照射装置について説明する前に、痛み緩和用光照射装置で用いる波長の光を照射することによる血管拡張作用について説明する。

これまで、紫外線領域の波長の光が血管を強く拡張させることは知られていたが、可視領域(400〜600nm)の波長の光はほとんど検討されていなかった。

そこで、本願発明者らは、ラット摘出血管を用いて、光照射による血管拡張作用について、可視領域の光として波長532nmのレーザー光と、これよりも長波長側の波長810nmのレーザー光とを用いて比較検討した。

その結果、波長532nmのレーザー光でも十分な血管拡張作用の生じることがわかった。以下にこの実験例の方法と結果を示す。

(血管標本実験)
被試験動物はラットを用い、撲殺潟血後、胸部大動脈(Descendingthoracic aorta)を摘出し、血管標本となる長さ3mmのリング標本(径1.5mm)を作製した。

この血管標本1を、図1に示すように、50mlのクレブス炭酸液(Krebs−bicarbonate溶液)2を入れたオーガンバス(Organ bath)3に懸垂し、張力変化を等尺性に記録する。オーガンバス3は、厚さ1〜2mmのガラス製で、外周部に水を通すことができる2重構造となっている。

クレブス炭酸液の温度はオーガンバス外周部に一定温度の水を流すことで、内部の液温度が33℃となるように調整した。また、内部の液には95%酸素と5%二酸化炭素の混合ガスを通気する。

クレブス炭酸液の組成は、NaCl 118mM、KCl 4.8mM、CaCl2 2.5mM、MgSO4 1.0mM、KH2PO4 1.2mM、NaHCO3 24mM、グルコース(Glucose)11mMである。

摘出血管には張力がないので、弛緩を見るため、あらかじめノルアドレナリン(交感神経の伝達物質)で収縮させた。

すなわち、ノルアドレナリン0.03μMで収縮させ、収縮が一定になった後、レーザー照射実験を開始した。

波長532nmのレーザー照射装置は、株式会社高知豊中技研製のKTGグリーンレーザー照射装置(出力を20mWまで可変可能)を使用した。

一方、波長810nmのレーザー照射装置は、100mWまで可変的に出力させることができる歯科用の株式会社ユニタク製半導体レーザー装置を使用した。

レーザー光4は、それぞれの波長を、直径1.0mmの光ファイバー5により導いて上記血管標本1に直接照射した。

1回当たりの照射時間は1分である。

血管標本1と光ファイバー5先端の距離は1〜2mmとして接触はさせないようにした。

なお、光ファイバー5先端からのレーザー照射強度はFieldMasterFM(COHERENT:米国)を用いて照射直前に測定した。

血管標本の弛緩の程度はノルアドレナリン収縮のパーセントで表示し、結果は平均値±SDで表現した。

血管標本1である、ノルアドレナリンでマイルドにトーヌスをもたせたラット摘出血管(大動脈)に対して、波長532nmレーザー光は1mWで比較的強い弛緩(ノルアドレナリン収縮を40%程度抑制する弛緩反応)を発生させたのに、波長810nmの光では4〜10mWではほとんど反応せず50mWの強度でわずかな弛緩が認められたにすぎなかった。

すなわち、この実験結果から波長532nmの可視波長レーザーの有用性が確認された。また、波長810nmの光でもそのエネルギーが50mW以上の強度で弛緩することがわかった。

クレブス炭酸液の温度を33℃から36℃に変えて、同じ実験を行ったところ、血管拡張作用は33℃の方が大きかった。

次に,丸ごと動物を用いた実験結果を示す。実験方法とその結果は以下の通りである。

実験動物はラットを用い、ペントバルビタールで麻酔した後、ラットの耳介部の内側に血流測定装置(アドバンスレーザーフローメーター ALF21R(株式会社アドバンス製)のプローブを密着させた。

耳介部の外側から、レーザー照射装置の照射口が内側のプローブの真上に位置するよう、耳介をはさみ込んだ。

耳介部の血流量はペンレコーダーで記録した。

レーザーを1分および5分照射直後の血流量を照射直前の血流量に対する増加率(平均値±SD)で示した。

温度は血流測定装置のプローブの変わりに、温度測定プローブを耳介内側に密着させ、同様にレーザーを照射し、照射1分、5分、10分後の温度を記録した。

波長532nm、5mW、および波長810nm、20mWのレーザー照射ではほとんど血流に影響を及ばさなかったが、波長532nm、20mWのレーザー照射では時間に依存して耳介血流を増大させた。

表3の結果からわかるように、波長532nmと波長810nmで、温度変化による差が認められなかったことから、上記表2の結果はレーザー照射による血流増加作用は光そのものによる影響と考えられる。

以上の実験結果から、波長532nmのレーザー光の方が、波長810nmのレーザー光よりも血管拡張作用が強いことが明らかとなった。

従来から低反応性レベルレーザーは810〜830nmの波長が使用されていたが、本発明はこれに波長532nmの光を加えることにより循環改善効果の増強をねらったものである。

また、542nmの光源を複数にして皮膚表面上の障害がでないように工夫した。

(第1の実施の形態)
図2は、本発明を適用した第1の実施の形態における痛み緩和用光照射装置を示す概略図である。

この痛み緩和用光照射装置10は、第1のレーザー照射手段として波長810nmのレーザー光を照射する第1半導体レーザー素子11と、第2のレーザー照射手段として波長530nmのレーザー光を照射する2つの第2半導体レーザー素子12を保持部材13に取り付け、この保持部材13全体を回転させるためのモーター14を有する装置である。

半導体レーザー素子の取り付け位置は、各半導体レーザー素子から照射されたレーザー光15および16が、皮膚50上から入り皮下の目的部位51で集光するように固定している。半導体レーザー素子の固定には、目的部位51に合わせて集光位置が変えられるように、保持部材13に対して半導体レーザー素子をネジ(不図示)などによって取り付けている。したがって、ここでは保持手段である保持部材12が集光手段として機能することになる。

また、出力エネルギーは、一つの半導体レーザー素子当たり、5〜1000mWであることが好ましい。これは、5mW以下であると、いくら複数の半導体レーザー素子を設けたとしても、一つのレーザーエネルギー自体があまりにも弱く、一つひとつのレーザー光15が皮膚組織から下に到達せず、治療効果が期待できないためである。一方、一つの半導体レーザー素子当たりその出力エネルギーが1000mWを超えると、皮膚組織への影響が心配されるためである。

また、取り付ける半導体データ素子の数は、1本当たりの半導体データ素子からの出力エネルギーによって異なるが、治療目的や期待される効果に合わせて、適宜決定すればよく限定されるものではない。ただし、上記の実験結果から波長530nmの方が血管拡張作用が大きいことから、本第1の実施の形態のように、波長530nmの半導体レーザー素子を波長810nmの半導体レーザー素子よりも多くすることが望ましい。また、取り付ける素子の数があまり多くなると、それら複数の半導体データ素子からのレーザー光15を目的部位51で集光させたときに、集光させた総エネルギー量によっては生体組織に悪影響がでることもあるため、総エネルギー量として50mW以下となるように注意する必要がある。なお、このようなレーザー光15の出力エネルギーの上限値については対象となる病巣部の状態や患者によって十分な注意を行う必要があるため、一概にこれらの値であればよいこと示すものではなく、適宜、十分な注意を払い決定させるべきことは言うまでもない。このような観点からは、半導体レーザー素子は、その出力を適宜統制できるように、可変であることが好ましい。

モーター14は、移動手段であり、目的部位51が中心軸となるように、保持部材13全体を回転する。なお、モーター14は直接その回転により保持部材13を回転させるようにしてもよいし、または、別途モーターを設置してそこからベルトプーリーやギア機構などにより動力を伝達して保持部材13を回転するようにしてもよい。

このように、本第1の実施の形態では、波長530nmと810nmの半導体レーザー素子からレーザー光15を照射して、それらのレーザー光15を目的部位51で集光させるようにすることで、一つひとつのレーザー光15としては弱い出力でよいため、一つひとつのレーザー光15が照射される皮膚組織に対してはレーザー光15による影響を防止して、なおかつ、目的部位51(すなわち病巣部)に対してはレーザー光15の集中により治療効果を高めることができる。

しかも、照射されるレーザー光15は、保持部材13ごと回転させているため、特に皮膚50に対する影響の大きな波長530nmのレーザー光15は、目的部位51を中心軸として照射位置が円周状に移動するため、一カ所にレーザー光15が当たる時間が短くなり、その影響もいっそう少ないものとなる。

これにより皮下深部にある病巣、たとえば、筋・筋膜性の腰痛なら皮下脂肪組織の下にある筋膜や筋肉内の血管などに対して、効果的な治療を行うことができる。

すなわち、表面照射、星状神経節照射、腰部交感神経節照射、末梢神経節照射、深部局所照射、関節照射、経穴(つば)照射が可能となる。

星状神経節に照射すると、顎関節症、進行性全身性強皮症などによる指や手の寒痛、レーノー病、脳血管後遺症、顔面神経麻痺、舌咽神経痛、三叉神経痛、類肩腕症候群、帯状庖疹痛、帯状庖疹後神経痛、自律神経失調症、膠原病、筋・筋膜性の落痛、突発性難聴、鼻アレルギー、脱毛症、頭痛、非定型顔面痛、術後痛に有用である可能性がある。

深部局所照射としては、筋・筋膜性の腰痛、肩こり、狭心症、裾創、閉塞性動脈硬化症(ASO)、閉塞性動脈炎(バージャー病TAO)、糖尿病性動脈閉塞など循環不全を伴う幅広い疾患に有用である可能性がある。

(第2の実施の形態)
図3および図4は、本発明を適用した第2の実施の形態における痛み緩和用光照射装置を示す概略図である。

また、本第3の実施の形態では、レーザー光15をビームスプリッタ31によって分割して複数のレーザー光15とすることで、皮膚50に直接当たる一つひとつのレーザー光15としては弱いエネルギーとなるので、皮膚組織に対する影響を防止して、なおかつ、目的部位51(すなわち病巣部)に対してはレーザー光15の集中により治療効果を高めることができる。また、集光反射鏡33を含めて全体を回転させているためレーザー光の照射位置が常に移動し、皮膚組織に対する影響をより少ないものとしている。

なお、本第3の実施の形態では、一つの半導体レーザー素子からのレーザー光15を2分割としたが、これに限らずさらに多数に分割してもよい。

以上本発明の実施の形態を説明したが、本発明は、これらの実施の形態に限定されるものではない。たとえば、上述した実施の形態では、いずれもレーザー光15を用いる例を示したが、レーザー光15に変えて、400〜600nmの吸収ピークをもつ単色光ランプと、波長810nm付近の好ましくは810〜830nmの吸収ピークをもつ単色光ランプを組み合わせて、光源として用いてもよい。

また、このようなランプを用いる場合は、ランプからの発熱により、反射鏡部分が熱くなるので、これを抑制するために反射鏡32を冷却するために冷却手段を設けてもよい。冷却手段としては、たとえば、反射鏡32の外側水冷パイプを配置するなどの方法をとることできる。

さらに、このようなランプを用いる場合は、たとえばランプから単色光を、反射鏡32を用いて複数の方向から目的部位51に照射するようにするとよい。これにより単色光ランプを用いた場合でも皮膚50に与える影響を少なくして目的部位51のみに単色光を集中させることができるようになる。

【発明の効果】
以上述べたように、本発明の痛み緩和用光照射装置によれば、照射表面組織を損傷することなく、深部の目的部位に対し神経伝達抑制性と血管拡張性の光線を十分に供給することができる。このため、生体深達性に優れ、痛み緩和作用を与えるために十分な光を皮下深部にまで供給することができる。
