本発明は、患部を治療するに際し、生体内に挿入されるアプリケータの最適位置を判断し提示することができるアプリケータの最適位置提示システムに関する。

【従来の技術】
生体内の腫瘍や肥大症等の疾患の治療方法として、患部にエネルギーを加えて細胞を凝固壊死等させる方法が知られている。エネルギーを加える手段としては、たとえば、レーザ加熱により、患部を加熱するアプリケータがある。

ここで、患部以外をレーザ加熱してしまうと、健全な部位まで損傷されてしまう。また、患部に適当にレーザ加熱されない場合には、充分な治療効果を得られないことがある。したがって、治療において、アプリケータの位置決めは、きわめて重要であり、慎重に行われなくてはならない。

アプリケータの位置決めのために、従来から種々の技術が提案されている。

従来技術(1)
アプリケータと共に超音波送受信部を設けたプローブを、体腔に挿入する技術がある。このプローブでは、体腔内で超音波を送受信することにより、体内の断面像を形成できる。したがって、医師等の使用者は、体内の断面像を観察しながら、アプリケータを位置決めできる(たとえば、特許文献1参照)。

位置決めのために、アプリケータと共に内視鏡を内蔵したプローブを、体腔に挿入する技術がある。使用者は、プローブに設けられた開口から内視鏡により生体内表面を観察しながら、アプリケータを位置決めできる(たとえば、特許文献2参照)。

しかし、上記従来技術(1)〜(3)では、次のような問題点がある。

上記プローブでは、超音波送受信部で送受信した超音波を解析して、エコー像などの画像を作成できる画像作成装置が必要となる。画像作成装置の導入は、治療システムを高コストなものとしてしまう。

アプリケータの他に、内視鏡が内蔵されているため、ケース内に内視鏡用の通路を設ける必要等があり、構造が複雑化し、プローブ外径が大きくなる。プローブ外径の増大は、該プローブが体内に挿入される患者の負担の増大にもつながり、好ましくない。

本発明は、上記問題点に鑑みてなされたものであり、画像作成装置を必要とせずに治療システムを簡易化でき、患者の負担を低減し、アプリケータの最適位置への容易な位置決めを可能とするアプリケータの最適位置提示システムの提供を目的とする。

【課題を解決するための手段】
本発明の上記目的は、下記の手段によって達成される。

(1)患部の診断に使用する診断用具と、前記診断用具に設けられる目印物体と、生体内に挿入されるアプリケータと、前記アプリケータに設けられ、生体にエネルギーを供給するエネルギー供給部と、前記アプリケータに設けられ、前記目印物体との距離を検出可能なセンサと、前記診断用具を用いた診断により前記目印物体が前記患部付近に配置された状態で、前記センサと前記目印物体との距離に基づいて、アプリケータの最適位置を判断する判断手段と、前記判断手段による判断結果を提示する提示手段と、を有するアプリケータの最適位置提示システム。

【発明の実施の形態】
以下、図面を参照して、本発明の実施の形態を説明する。

図1は、アプリケータの最適位置提示システムの概略構成図である。なお、図1に示す各構成は、説明の容易のために、形状を簡略化し、大きさ等も見やすいように変更して表現されている。

図1に示すように、アプリケータの最適位置提示システム1(以下、単にシステム1と称する)は、患者の患部の診断に使用するグローブ2(診断用具)と、グローブ2に設けられる目印物体3と、治療のために生体内に挿入されるアプリケータ4と、アプリケータ4に設けられ、生体にエネルギーを供給するエネルギー供給部5と、エネルギー供給部5に略隣接してアプリケータ4に設けられ、目印物体3との距離を検出可能なセンサ6と、グローブ2を用いた診断により目印物体3が患部付近に配置された状態で、センサ6と目印物体3との距離に基づいて、アプリケータ4の最適位置を判断する制御装置7(判断手段)と、制御装置7による判断結果を表示する表示装置8(提示手段)と、を有する。

グローブ2は、手先の微妙な感覚を損なわないように薄く、かつ治療中に破損しないように丈夫に形成されている。材料としては、たとえば、天然ゴムラテックスが用いられる。グローブ2は、感染症の防止のために、滅菌処理されている。

目印物体3も、同様に滅菌処理されている。目印物体3は、センサ6により検出可能なものであればよい。たとえば、センサ6が、超音波センサ6である場合には、目印物体3は、金属片のような超音波反射体により形成される。超音波反射体とは、生体とは異なる音響インピーダンスを有する物体であり、金属片の他にも、空気等でもよい。目印物体3に空気を用いる場合には、空気を小さな風船に詰めて、グローブ2に取り付ける。

目印物体3の取り付け位置は、グローブ2の指先が好ましい。目印物体3を指先に設けることにより、使用者が触診により患部を確認した姿勢のままで、その略中心に目印物体3を配置することができる。目印物体3は、触診の際に使用者の指先の感覚を損なわせないように、小型に薄く形成されている。角がない略丸型に形成されることが好ましい。

アプリケータ4は先端側に、細く長尺状に形成されて、基端側が保持しやすい形状の挿入部43を有する。挿入部43は、エネルギー供給部5が内蔵され、エネルギーを生体に向けて放射するための窓部41が設けられている。エネルギー供給部5は、挿入部43内部に沿って延びるエネルギー経路42を介して、エネルギー発生装置9に接続されている。エネルギー発生装置9は、使用者による治療開始の指示に従って、エネルギーを発生する。ここで、エネルギー発生装置9により発生されるエネルギーは、レーザ光やマイクロ波、高周波等の電磁波であり、エネルギー供給部5から患部に供給されて、加熱治療される。

エネルギー供給部5近傍には、2つのセンサ6が設けられている。センサ6は、挿入部43の長さ方向に並んで、エネルギー供給部5の中心から等間隔の位置に配置されている。センサ6は、グローブ2に設けられた目印物体3の検出のために、挿入部43外面に設けられることが望ましい。センサ6は、挿入部43内部に延びる図示しない信号線を介して、制御装置7に接続されている。制御装置7にはさらに表示装置8が接続されている。制御装置7には、所定の情報を記憶する記憶部70が設けられている。

次に、図2〜図4を参照して、上記システム1が使用者によりどのように治療に使用されるかについて説明する。以下では、前立腺肥大症の治療を行う場合について説明する。

図2は使用者が患部を特定する動作の流れを示すフローチャート、図3は使用者がアプリケータ4の最適位置を確認する動作の流れを示すフローチャート、図4は使用者が患部を治療する様子を示す図である。

まず、使用者は、治療に備えて、上記目印物体3が設けられたグローブ2を着用する(ステップS11)。そして、目印物体3が設けられた指(たとえば、中指)を、生理食塩水や水で充填した直腸に挿入し、患部の触診を開始する(ステップS12)。ここで、前立腺肥大の場合、球状の前立腺の最も膨らんでいる部分が患部の中心である。

使用者は、触診によって前立腺の最も膨らんでいる部分を患部として特定し(ステップS13)、特定した患部の位置に、目印物体3を配置する(ステップS14)。ここで、使用者は、目印物体3を患部側の直腸壁に密着させる。超音波の伝搬を容易ならしめるためである。使用者の触診によるので、前立腺の最も膨らんでいる部分は、容易に特定できる。

使用者は、直腸に中指を挿入し、患部に目印物体3を配置したままの状態で、生体内にアプリケータ4の挿入部43を挿入する(ステップS21)。ここでは、前立腺肥大症の治療であるので、尿道にアプリケータ4を挿入する。

使用者は、表示装置8の表示に従って、アプリケータ4を操作し、位置合わせする(ステップS22)。使用者は、最適位置と表示されたかどうかを判断する(ステップS23)。最適位置と判断しない場合(ステップS23:NO)には、位置合わせを続行する。最適位置と表示されたら(ステップS23:YES)、その位置でアプリケータ4を固定し、患部の治療を開始する(ステップS24)。ここで、治療の開始とは、具体的には、エネルギー発生装置9のスイッチをONにし、システム1にエネルギー供給を指示することである。図4に示すように、前立腺は、尿道を囲んでいる。前立腺肥大部分を治療するためには、アプリケータ4を任意の角度ずつ回転させて、エネルギーを前立腺肥大部分に供給する。

図5はシステム1がアプリケータの最適位置を判断する流れを示すフローチャート、図6はシステム1が位置ズレを検出する流れを示すフローチャート、図7は2つのセンサ6により目印物体3を検出する様子を示す図、図8はアプリケータが最適位置かどうかを判断する様子を示す図、図9は表示装置8に表示される画面を示す図である。図5に示す手順は、図2および図3に示す、ステップS11〜ステップS21までの動作が完了した後のものである。また、以下では、センサ6は、超音波センサ6であり、目印物体3は、金属片である場合について説明する。

システム1は、図7に示すように、センサ6により指向性を持った超音波の発振を開始する(ステップS31)。超音波があたる位置に目印物体3があれば、超音波が反射されるので、センサ6は反射された超音波を受信する(ステップS32)。制御装置7は、センサ6が超音波を発振してから受信するまでの時間を計測し、計測時間に基づいて目印物体3との距離を算出する(ステップS33)。

一方、アプリケータ4の最適位置からの位置ズレが許容範囲以上である場合(ステップS48:NO)、表示装置8は、警告音を発し、図9に示すように、画面に警告表示を表示する(ステップS52)。警告を受けて、使用者は位置ズレを修正することができる。制御装置7は、位置ズレが生じてから3秒以上経過したか否かを判断し(ステップS53)、3秒間の間にステップS44〜S48の位置ズレの検出を繰り返す(ステップS53:NO)。この間に位置ズレが修正されれば、ステップS48よりステップS49へ進むこととなる。

以上説明してきたように、本実施形態のシステム1によれば、グローブ2に目印物体3が設けられているので、使用者がグローブ2を用いて触診しながら患部の位置を示す目印として目印物体3を配置することができる。この目印物体3との距離をセンサ6により検出できるので、検出した距離に基づいて、アプリケータ4の最適位置を判断できる。判断結果は表示装置8により表示されるので、該表示に従って、使用者は、容易にアプリケータ4を患部に位置合わせできる。

上記実施形態では、センサ6を2個設けている。しかし、センサ6の数は、2個より多くてもよく、また、1個であってもよい。センサ6が多くなる程、精度を高めることができる。センサ6が2個より多い場合でも、2個の場合と同様に目印物体3との距離を算出することにより、アプリケータ4の最適位置を求めることができるので、この場合の説明は省略する。
