【発明の属する技術分野】
本発明は、人の呼吸サイクルに応じて人が必要とする間だけ酸素ガス又は濃縮酸素ガスを間歇的に供給するための呼吸用気体供給方法とそのための装置に関する。

【従来の技術】
従来、呼吸器疾患患者に対して酸素ボンベから酸素を吸入させることによる酸素療法が行われており、最近では、空気中の酸素を濃縮して酸素濃縮気体を得るための酸素濃縮装置が実用に供せられ、これを用いた酸素療法が盛んに行われるようになってきた。このような酸素濃縮装置としては、ゼオライトなどの窒素をより吸着しやすい吸着剤を充填した吸着筒を用意し、この吸着筒へ供給する空気に対して加圧と減圧を繰り返し行うことによって、空気中の窒素ガスの吸着と脱着を繰り返えす圧力変動式の吸着型酸素濃縮装置が好ましく使用されている。

ところで、このようにして酸素ボンベから供給される酸素、あるいは酸素濃縮装置から得られる濃縮酸素は、延長チューブを介してその先端に取り付けられた鼻カニューラから患者の鼻腔内へ連続的に導入される。しかしながら、このようにして連続的に酸素あるいは濃縮酸素を患者の鼻腔内へ供給するようにすると、患者がこれらの気体を吸入するとき以外は、これらの気体が外部に排出されて有効に利用されないという問題が生じる。

そこで、特開昭59―8972号公報に提案されているように、患者の呼吸サイクルに合わせて、吸入時においてのみ酸素濃縮気体を供給することによって、無駄な酸素あるいは濃縮酸素(以下、「酸素あるいは濃縮酸素」を総称して「濃縮酸素」と称する)の放出を防止し、無駄にこれらの気体を排出しない技術が必要とされている。なお、当然のことながら、このような技術では、患者が吸気を行っている期間だけ濃縮酸素を供給するという必要上から、患者が吸気を開始する吸気開始時点と、患者が濃縮酸素の吸入を終わる吸気終了時点を検出することが必須となる。しかも、呼吸器疾患を患う患者に対して、このような技術を適用するようになると、誤動作は許されない。したがって、前記吸気開始時点と吸気終了時点とを正確かつ精度良く検出することが要求される。

ところが、特開昭59―8972号公報に提案されている技術では、このような吸気開始時点と吸気終了時点とを熱電対形式のセンサーを用いて、測定しているため、その測定精度に問題がある。しかも、患者が体内から吐き出す呼気の温度と、患者が吸入する吸気の温度は、環境や使用する患者さんの個人差によっても変化するため、このようなセンサーで前記吸気開始時点と吸気終了時点とを正確に判断するのはきわめて難しい。また、判断の基準となる温度の設定も難しく、この基準が移動したりする。このため、この従来技術では実用的とはいえなかった。

このような従来技術が有する問題を解消するために、患者が呼吸する際に発生する気圧の変化を検出し、これによって、前記吸気開始時点と吸気終了時点とを正確に判断することが行われている。なお、上記の患者が呼吸する際に発生する気圧の変化は、微圧であるためこれを検出するための微圧センサーが必要とされるが、このような微圧センサーの例としては、特開昭64―21330号公報において提案されているダイヤフラム式の微圧センサーが好適に使用される。

ここで、この微圧センサーを簡単に説明すると、測定対象である圧力の変化に対応して変位する導電性高分子フィルムからなる移動電極と、これに対向して設けられた固定電極との間に発生する静電容量の変化を検出するダイヤフラム式微圧センサーである。なお、このような微圧センサーを使用すると、数mmAq程度の微圧あるいは微圧差が生じても、これを検出することができる。

しかしながら、このようなタイプの微圧センサーでは、前述のような数mmAq程度の微圧あるいは微圧差が生じても、これを検出することができるという利点を有しているものの、その検出感度が極めて高いために、使用する環境の影響、例えば温度や湿度の変化などによって、その特性が微妙に変化してしまうという問題がある。このように温度や湿度の変化などによって、検出した静電容量値などが変化してしまうと、その判断基準値がドリフトして、前述の吸気開始時点と吸気終了時点とを正確に判断できないという問題を有する。また、長期間に渡って使用していると、その判断基準値が変化してしまうという問題も生じる。

【発明が解決しようとする課題】
本発明は、以上に述べた従来技術が有する諸問題に鑑みなされたものである。すなわち、本発明が解決しようとする課題は、人が呼吸する呼吸サイクル中で変動する微圧又は微差圧を精度良く検出でき、これによって人の吸気開始時点を検出する基準値が変化しても、その基準値の変化を良好に検出して、正常な機能を維持できる新しい基準値に更新でき、これによって正常な呼吸用気体供給機能を喪失させることなく、所定量の濃縮酸素を正確且つ安定して供給することができる呼吸用気体供給方法とそのための装置を提供することである。

【課題を解決するための手段】
ここに、先ず前記課題を解決するための請求項1に係る本発明の方法として、「大気に開放した状態で酸素又は濃縮酸素気体からなる呼吸用気体を人の鼻孔及び/又は口内へ供給する呼吸用気体供給方法であって、前記方法は、人が呼吸する際の呼吸サイクルを呼吸用気体の微圧又は微差圧の変化で検出し、検出した微圧又は微差圧を電気信号に変換し、変換した電気信号に対して人が吸気中であることを判断するための判断基準となる基準値を設定し、設定した前記基準値を記憶し、記憶した基準値が変化した場合に変化後の新しい基準値を検出して変化してしまった基準値を前記新しい基準値に更新し、そして、更新した基準値によって吸気開始時点を検出して呼吸用気体の供給期間を算出することを特徴とする呼吸用気体供給方法」が提供される。

また、請求項2に係る本発明のように、請求項1記載の呼吸用気体供給方法に対して、前記基準値を表示自在であることが好ましく、このようにすることによって設備の補修者あるいは点検者が容易に基準値の変化を監視することができ、基準値を更新する必要があるかどうかを容易に判断できる。

また、請求項3に係る本発明のように、請求項1又は2記載の呼吸用気体供給方法に対して、特定符号の入力、あるいは特定の操作手順よって前記基準値を表示及び/又は自動更新させることが好ましい。何故ならば、このようにすることによって、特定符号の入力あるいは特定の操作手順を知らない呼吸器系疾患を持つ患者などは、基準値を表示及び/又は自動更新させることができないため、誤って基準値を更新してしまうような事態を回避することができるからである。まそして、特定符号の入力、あるいは特定の操作手順を知っている専門知識を有するか、あるいは設備の保守点検の教育を受けた者のみが前記基準値を表示させて、これを更新することができるため、装置を安全に取扱ったり、維持したりできるからである。

また、請求項4に係る本発明のように、請求項1〜3の何れか一項に記載の呼吸用気体供給方法に対して、前記基準値が正常機能を維持可能な範囲を合否判断の許容範囲として記憶させ、前記合否判断の許容範囲に基準値が収まっている場合は基準値の更新を許可しないことが好ましい。何故ならば、このようにすることによって、正常な機能を発揮できる基準値であるにもかかわらず、誤ってこれを正常な機能を発揮できない基準値へ更新してしまうことを回避できるからである。

また、請求項5に係る本発明のように、請求項1〜4の何れか一項に記載の呼吸用気体供給方法のように、前記基準値を更新することが可能な更新許容範囲を設定し、前記更新許容範囲内の基準値に対して基準値の更新を許可するようにすることが好ましい。何故ならば、このようにすることによって、基準値が更新許容範囲を逸脱してしまうことによって基準値がその役割を果たし得る限界を越えて変更されてしまうのを回避することができるからである。またもし、基準値が前記更新許容範囲を越えるような事態が生じたならば、その場合はより重大な事態、例えば設備自体に何らかの不具合が生じていると判断できるからである。

次に、前記課題を解決するための本発明の装置として、請求項6に記載のように「呼吸用気体を発生させるための呼吸用気体発生手段と、人が呼吸する際の呼吸用気体の微圧又は微差圧をオンラインで検出するための微圧検出手段と、検出された微圧又は微差圧を電気信号へ変換するための検出信号変換手段と、人が吸気及び呼気中でなく且つ呼吸用気体が供給されない無負荷状態における前記微圧又は微差圧に相当する電気信号値を基準値として読み書き自在に記憶する記憶手段と、オンラインで検出された前記電気信号値と前記基準値とから人の吸気開始時点を検出して呼吸用気体の供給期間を算出する供給期間判断手段と、前記基準値が変化した際に正常な基準値に設定する基準値設定手段と、前記呼吸用気体を大気開放状態で人の鼻孔又は口内へ供給する呼吸用気体供給手段とを少なくとも含む呼吸用気体供給装置」が提供される。

また、請求項7に係る本発明のように、請求項6記載の呼吸用気体供給装置に対して、前記微圧検出手段が測定対象である人の呼吸気体の圧力変化に対応して変位する導電性高分子フィルムを移動電極と、前記導電性高分子フィルムに対向して設けられた固定電極との間の静電容量を検出する方式のダイヤフラム式微圧検出器であることが好ましい。何故ならば、このようなダイヤフラム式微圧検出器を使用することによって、微小な気圧や気圧変化を安定且つ確実に検出することができるからである。

また、請求項8に係る本発明のように、前記請求項6又は7記載の呼吸用気体供給装置に対して、前記基準値が変化した場合に、変化した基準値を自動補正するための基準値補正手段を備えることが、装置の内部を開いて基準値を人手によって設定し直すという手間が省け、しかも、専門家がわざわざ出向いて装置点検する必要もなくなるため好ましい。

そして、請求項12に係る本発明のように、前記請求項6〜11の何れか一項に記載の呼吸用気体供給装置に対して、一端が前記呼吸用気体発生手段、他端が前記呼吸用気体供給手段にそれぞれ接続する導管と、呼吸器用気体の流通を制御して吸気期間中のみ呼吸用気体を供給自在にする前記導管の途中に設けられた流路開閉手段と、人の吸気開始時点を検出して算出した呼吸用気体の供給期間に対応させて前記流路開閉手段の開閉を制御する制御手段と、前記呼吸用気体を大気開放状態で人の鼻腔内へ供給する呼吸用気体供給手段とを設けることが好ましい。何故ならば、このようにすることによって、前記流路開閉手段を自在に制御して必要とする吸気期間だけ呼吸用気体を供給でき、無駄に呼吸用気体が大気に放出されるのを防止できるからである。

【発明の実施の形態】
以下、本発明の実施の形態について、図面を参照しながら詳細に説明する。

先ず、図1は、本発明に係る呼吸用気体供給装置の一実施形態を具体的に例示した概略装置構成図である。この図1において、1は呼吸用気体である酸素を充填した酸素ボンベ(本発明の「呼吸用気体発生手段」である)、2は酸素ボンベ1から供給される酸素の流量を所定量に設定するための流量設定器、3は導管、4は流路開閉手段、5はマイクロコンピューター、6は微圧検出手段、7は検出された微圧又は微差圧を電気信号へ変換するための検出信号変換手段、そして、8は患者に呼吸用気体を供給する鼻カニューラ(本発明の「呼吸用気体供給手段」である)をそれぞれ示す。なお、本発明においては、この実施形態例のように「呼吸用気体発生手段」として酸素ボンベ1を使用することもできるが、空気中の酸素ガスを濃縮して濃縮酸素ガスを発生させる公知の圧力変動吸着型の酸素濃縮装置なども好適に使用できることは言うまでもない。

このとき、前記流路開閉手段4としては、直流励磁型電磁弁、交流励磁型電磁弁、空気作動式自動弁、パイロット作動式電磁弁などが好ましく使用でき、直流励磁型電磁弁、空気作動式自動弁、及びパイロット作動式電磁弁が鉄芯などの動きが緩やかで寿命が長く、しかも、作動時に発生する音が静かなため、装置の低騒音化を進める上でより好ましく、その中でも、特に、直流励磁型電磁弁が実用的である。

また、前記マイクロコンピューター5は、図示省略したインターフェース装置を介して、流路開閉手段7を開閉するための制御を行う機能を有している。また、このマイクロコンピューター5には、微圧検出手段6によってオンラインで検出された微圧又は微差圧は、検出信号変換手段7によって電気信号へ変換され、前記インターフェース装置(図示せず)を介して、変換された電気信号が入力されるようになっている。しかも、このマイクロコンピューター5には、人が吸気及び呼気中でなく且つ呼吸用気体が供給されない無負荷状態における前記微圧又は微差圧に相当する電気信号値を基準値として読み書き自在に記憶する記憶手段が付設されている。したがって、マイクロコンピューター5によって、オンラインで検出された微圧又は微差圧の情報を含む前記電気信号値と前記基準値とから人の吸気開始時点を検出して呼吸用気体の供給期間を算出することができる。このように、前記マイクロコンピューター5は呼吸用気体の供給期間を適切かつ正確に判断する供給期間判断手段をも兼ねている。

また、前記微圧検出手段6としては、特開昭64―21330号公報に開示されているようなダイヤフラム式微圧検出器を好適に使用することができる。なお、このタイプの微圧検出手段は、特開昭64―21330号公報に詳細に記載されているので、その詳細説明を省略するが、その検出原理は、測定対象である人の呼吸気体の圧力変化に対応して変位する導電性高分子フィルムを移動電極と、前記導電性高分子フィルムに対向して設けられた固定電極との間の静電容量を検出しようとするものである。ただし、本発明の微圧検出手段としてはこのようなタイプのものに限定されることなく、本発明の主旨を満足するものであれば良いことは言うまでもない。

更に、本発明に使用する呼吸用気体供給手段は、人の鼻孔や口に対して密閉されない状態、すなわち大気に開放された状態で呼吸用気体を供給するものであって、前記図1に示した鼻カニューラ8やマスクなどがこれに相当する。

以上のように構成される呼吸用気体供給装置の実施形態例において、10はデマンドレギュレータと称されるものであって、このデマンドレギュレータ10は、使用者の呼吸サイクルを検知して使用者が必要とする前記呼吸用気体を吸気時と同調させて供給するために設けられ、前述の流路開閉手段4、マイクロコンピューター5、微圧検出手段6、及び検出信号変換手段7を含んで構成されている。なお、このデマンドレギュレータ10によって、呼吸用気体発生手段である酸素ボンベ1で発生した呼吸用気体である酸素ガスを患者の吸気期間のみ呼吸用気体供給手段である鼻カニューラ8へ供給することを可能とすることは言うまでもない。

次に、図2は、呼吸器疾患を持つ患者のように、本発明の呼吸用気体供給装置の使用者が呼吸するサイクルにおいて、前記微圧検出手段6によって検出された呼吸用気体の圧力変化の様子を示したグラフである。ただし、この図2において、図(a)は人が呼吸する際に得られる圧力変化の様子を示したグラフ、図(b)は人が呼吸する期間において人の吸気期間中にのみ酸素ガスを供給した場合のグラフ、そして、図(c)は人の呼吸や酸素ガスの供給を止めるなどして外部からの圧力変動を引き起こす要因を取り除いて得た無負荷時のグラフである。

なお、これら図(a)、(b)、及び(c)のグラフにおいて、その縦軸は微圧検出手段6によって検出された圧力値の電気信号を相対的に示したものであり、図中に示した「0」が大気圧であって、この「0」が基準値となる。そして、この基準値から上方が大気圧を基準として減圧サイドとなり、その下方が大気圧を基準として加圧サイドとなる。また、これらグラフの横軸は、時間経過をそれぞれ表している。

前記グラフ(a)〜(c)からも分るように、微圧検出手段6によって検出された微圧又は微圧差は、検出信号変換手段7によって電気信号(図中には「センサー信号」と記載)に変換されるが、このとき人が吸気を開始する時点を検出することが必要である。そのためには、人が呼吸しない無負荷状態にある場合には、微圧検出手段6が大気圧を検出し、この大気圧が電気信号に変換された際に「零値」となるように「基準値」を設定し、この「基準値」を前記コンピューター5に設けられたハードディスクや半導体メモリーなどの読み書き自在の記憶手段に記憶させておく必要がある。

なお、図2において、グラフ(c)はこのような無負荷状態を現出させ、「基準値」を補正する時の状態を示したグラブであって、このグラフ(c)のように、「基準値」が一定値(「零値」)を正確かつ安定して維持するように設定する必要がある。

なお、前記基準値については、人が吸気を開始する開始時点を検出するために極めて重要な役割を果たす。すなわち、グラフ(b)に示すように、「基準値」である「零値」から減圧サイド(吸気サイド)になったことが、「基準値」によって検出し、これによって、人の吸気開始時点を判断するという重要な役割を果たしている。なお、人の吸気開始時点の判断はマイクロコンピューター5を使用することによって好適に行うことができるが、ここで、この重要な判断を行うマイクロコンピューター5での判断処理についてその処理例を以下に具体的に述べる。先ず、マイクロコンピューター5にオンラインで取り込まれた微圧又は微差圧情報を含む変換された電気信号から、前記基準値との間に閾値として設けられたある一定の値を超えた時点を吸気開始時点と判断したり、大気圧から減圧サイド(吸気サイド)に変化する際の立ち上がり勾配を検出したりすることによって吸気開始時点を判断することができる。なお、このような処理はマイクロコンピューター5の記憶手段に記憶させたプログラムによってソフトウエア処理することもできるが、比較回路や微分回路などによるハードウエア処理によっても可能である。

以上に述べたようにして、呼吸用気体の供給開始時点が決定されると、次いで、その供給期間が決定される。なお、このような呼吸用気体の供給期間の決定は、減圧サイドに振れた電気信号が前記基準値に復帰した時点を検出することによっても可能である。しかしながら、呼吸用気体は人の吸気期間に正確に合わせる必要は無く、このような理由から予め設定された一定の閾値まで回復する時点までを呼吸用気体の供給期間と設定することもできる。なお、図2に示したグラフ(b)は、このようにして、人が必要とする期間だけ呼吸用気体を供給している様子を示したものであって、このグラフ(b)では、呼吸用気体が供給されると、圧力が急激に上昇し、微圧検出手段6が検出できる最大圧力に相当する電気信号まで上昇して、この状態が呼吸用気体の供給が停止されるまで続く。

ところで、既に述べたように前記「基準値」は極めて重要であって、この「基準値」が微圧検出手段6を使用する際の温度、湿度などの環境によって変化したり、クーラーなとのエアコンによる風の影響を受けたり、ドアの開閉による影響を受けたり、あるいは長期間使用したりしている間にその測定値がドリフトしてしまうようなことが生じれば、当然のことながらに人が吸気を開始した時点を判断するための基準が変わってしまう。しかしながら、人が呼吸する際の微圧又は微差圧の変化を検出するためには、極めて敏感な圧力検出能を有するセンサーが必要であって、このようなセンサーでは、圧力検出能が極めて鋭敏であるが故に、その基準値が変化し易いという問題が存在することは既に述べた通りである。

したがって、このような事態が惹起すれば、人が吸気を開始した時点を正確に検出できないことは勿論のこと、その信頼性にも問題が生じる。このような理由から、極めて重要な役割を果たす「基準値」を常に正常な値に維持しておくことが肝要である。このため、「基準値」を更新する際には、外乱を極力排除した図2に示したグラフ(c)に示したような状態で行う必要がある。

その際、「基準値」が変化しているかどうかを確認する必要が生じるが、このような確認を行うためには、記憶手段に記憶させた基準値と、その時点における基準値とが一致するかどうかを確認する必要がある。このとき行う確認は、点検者の肉眼で直接確認できるように、例えば図3に例示したような表示手段9に表示できるようにしておくことが重要であって、本発明の好ましい実施態様ではこのような表示手段9を備えている。なお、図3は、本発明の装置が備える操作パネルの一実施形態を例示したものであって、参照符号11は電源スイッチ、そして、参照符号12は操作キーをそれぞれ示す。ここで、前記表示手段9は、図示したように、図形表示部90と具体的な数値を表示する数値表示部91とで基本的に構成されている。

本発明の好ましい実施形態においては、呼吸用供給装置に設けられた前記表示手段9に対して、特定符号の入力、あるいは特定の操作手順よって前記基準値を表示させることができる。また、基準値の自動更新を行う場合にも同様な方法によって行うことができる。本発明において、このようにすることの理由としては、前述のような特定符号の入力、あるいは特定の操作手順による「基準値」の表示及び/又は自動更新は、通常の状態では必要ではないため、保守点検用に特化して行う性質のものだからである。逆に、このような操作が誰にでも簡単に行えるような状態にしておくと、装置の故障要因になったり、あるいは操作を誤って「基準値」を間違った値に設定してしまったりするおそれが生じる。また、使用者にとって必要がない表示が行われると、使用者は装置が故障したと勘違いするおそれがある。このため、パスワードなどの特定符号を入力して認証を行うようにしておいたり、特別な操作手順を要求するようにしたりして、認定された者だけがこのような操作を行うようにすることは、本発明においては好ましい態様である。

ここで、前記「特別な操作手順」の具体例としては、「2つの操作キー12を同時に押したままの状態で、電源スイッチ10を押す。」といった操作手順を挙げることができる。このような「特別な操作手順」は、通常の操作ではほとんど行われることがない操作手順であるため、「特別な操作手順」を知っている者だけが実施し得るものである。

したがって、前記のように、特定符号を入力したり、特別な操作手順を行ったりすることによって、表示手段9の数値表示部91に基準値を表示させることができる。その際、前記図形表示部90は、点検者、保守者、あるいは補修者に対して、数値表示部91に表示された基準値がどのような状態にあるかを視覚に訴える役割を果たす。そこで、この役割について、図4を参照しながら以下に具体的に説明する。なお、この図4は、前記図3に示した表示手段9からその一部を構成する図形表示部90のみを取り出して示した説明図である。
