【発明の属する技術分野】
本発明は、血液等の体液の臨床検査等を行う場合に、必要量の体液を採取するのと同時に、その採取した体液中の血糖値等の成分を測定する体液成分測定装置と制御方法の技術分野に属するものである。

【従来の技術】
従来、血液中の種々の成分を測定するために、血液中の特定成分と反応する特異酸素の反応生成物を測定する方法が検討されてきている。特に、血糖値の測定は、患者の状態をモニタするために重要であり、日常の血糖値の変動を患者自身がモニタする自己血糖測定が推奨されてきている。また、近年、糖尿病患者が増加してきており、簡便で苦痛のない測定方法、測定手段が求められてきている。この血糖値の測定は、グルコースオキシダーゼやグルコースデヒドロゲナーゼ等の酸素がブドウ糖を酸化する反応を用いて行われる場合が多い。現在、血液中のブドウ糖量に応じて呈色する試験紙を装着し、該試験紙に血液を供給、展開して呈色させ、その呈色の度合いを光学的に測定(測色)して血糖値を定量化する比色式の方法、上記の酸素反応の生成物を電気的に測定する電極式の方法等による血糖測定装置を用いて行われている(例えば、特許文献1、特許文献2参照)。
これらの測定方法においては、測定用の血液を採取することが必須である。これらの方法は、穿刺専用器具を用いて、ランセットと称される針状や小刀状の穿刺手段によって患者の指先等の皮膚に傷を形成して血液等の体液を滲出させた後、その体液を別に準備した試験紙あるいはセンサが印刷された電極片等に移して血糖測定装置で測定する。
また、近年、皮膚から血液を得るための穿刺針を有する穿刺具と上記の試験紙とを一体とした成分採取具が開発されている。このような穿刺具と試験紙を一体化することは、血糖測定装置に穿刺具とセンサを別々に装着することがなく、操作を簡略化できるので、非常に使い勝手を向上させることができる(例えば、特許文献3、特許文献4参照)。

【発明が解決しようとする課題】
しかしながら、穿刺針を穿刺する穿刺機構と、試験紙や電極センサ等により成分濃度を測定する測定機構を一体化した体液採取具とすることにより、構造が複雑となるため、不具合を有する体液採取具を血糖測定装置に装着してしまったり、一旦使用した穿刺針を有する体液採取具を装着してしまう可能性がある。また、誤って穿刺針を発射してしまった場合、試験紙や電極センサが未使用で穿刺針も清潔であっても、体液採取具を廃棄しなければならない等の課題も有している。

本発明は、上記の問題を解決するためになされたものであって、体液採取具の装着検出、穿刺部材の装着検出、穿刺部材が未使用品であるか否かの判別等を総合的に行うことができるように構成された体液成分測定装置と制御方法を提供することを目的としている。

【課題を解決するための手段】
上記の目的を達成するための本発明の請求項1の体液成分測定装置は、皮膚に傷を形成し体液を滲出させる穿刺部材と、滲出した該体液の成分を検出する成分検出部とを備えた体液採取具が脱着可能に装着される体液採取具装着部と、該体液採取具が該体液採取具装着部に装着された時に、該穿刺部材が先端部に脱着可能に装着される穿刺軸とを備えた体液成分測定装置において、該体液採取具が該体液採取具装着部に装着されていることを検出する体液採取具装着検出手段と、該穿刺部材が該穿刺軸に装着されたことを検出する穿刺部材装着検出手段とを備えたものである。
また、本発明の請求項1に従属する請求項2の体液成分測定装置は、前記穿刺軸は穿刺位置と待機位置とを有し、前記穿刺部材が未使用状態である前記体液採取具が前記体液採取具装着部に装着されるに伴い、該穿刺部材が該穿刺軸を待機位置へ押して該待機位置に係止し、該待機位置での該穿刺軸の係止を解除することによって、該穿刺軸で該穿刺部材を該待機位置から該穿刺位置へ発射する駆動機構とを備え、前記穿刺部材装着検出手段は、前記体液採取具の装着により該穿刺軸が該待機位置へ押し戻されて係止されたことを検出することにより構成されたものである。
また、本発明の請求項2に従属する請求項3の体液成分測定装置は、前記体液採取具が前記体液採取具装着部に装着された状態で、前記駆動機構により発射された前記穿刺部材を前記待機位置へ戻す再チャージ機構を有するものである。
また、本発明の請求項1ないし3に従属する請求項4の体液成分測定装置は、前記体液採取具が、前記穿刺部材の未使用の状態では、該穿刺部材を保持して前記穿刺軸の先端部への装着を可能にし、前記穿刺部材の使用後には、該穿刺部材の保持が解除されて、該体液採取具の前記体液採取具装着部への装着に伴う該穿刺部材の該穿刺軸への装着を不能とする穿刺部材保持部を有するものである。
また、本発明の請求項1ないし請求項4に従属する請求項5の体液成分測定装置は、前記体液採取具装着検出手段と前記穿刺部材装着検出手段のうちの何れか一方が検出不可となった時に、そのことを報知する報知手段を備えたものである。
また、本発明の請求項1ないし請求項5に従属する請求項6の体液成分測定装置は、前記体液採取具装着検出手段が出力する検出信号と、前記穿刺部材装着検出手段が出力する検出信号とを時系列に記憶する記憶手段を備えたものである。

上記のように構成された本発明の請求項1の体液成分測定装置は、体液採取具装着部に体液採取具が装着された時に、その体液採取具の装着が正しかったか否かを体液採取具装着検出手段によって検出すると共に、体液採取具内の穿刺部材が穿刺軸の先端部に正しく装着されたか否かも、穿刺部材装着検出手段によって検出するものである。
本発明の請求項1に従属する請求項2の体液成分測定装置は、穿刺部材が未使用である体液採取具が体液採取具装着部に装着された時に、穿刺部材で穿刺軸を待機位置へ押して係止し、駆動手段は待機位置での穿刺部材の係止を解除することにより、その穿刺部材を穿刺軸で待機位置から穿刺位置へ発射するようにし、体液採取具の装着時に、穿刺軸に装着された穿刺部材によって、その穿刺軸が待機位置まで押し戻されて係止されたか否かを穿刺部材装着検出手段で検出するものである。
また、本発明の請求項2に従属する請求項3の体液成分測定装置は、体液採取具が体液採取具装着部に装着されている状態で、待機位置から穿刺位置へ駆動機構によって発射された穿刺部材を、再チャージ機構によって穿刺位置から待機位置へ押し戻すことができるものである。
また、本発明の請求項1ないし3に従属する請求項4の体液成分測定装置は、体液採取具の穿刺部材が未使用の時には、体液採取具装着部への体液採取具の装着時に、穿刺部材を保持して穿刺軸の先端部へ装着することにより、その穿刺部材の穿刺軸への装着を可能にし、体液採取具の穿刺部材が使用済みの時には、穿刺部材の保持が解除されて、体液採取具装着部への体液採取具の装着時に、穿刺部材の穿刺軸への装着を不能にするものである。
また、本発明の請求項1ないし4に従属する請求項5の体液成分測定装置は、体液採取具装着検出手段と穿刺部材装着検出手段のうちの何れか一方でも検出不可となった時に報知手段が報知するものである。
また、本発明の請求項1ないし5に従属する請求項6の体液成分測定装置は、体液採取具装着検出手段から出力される検出信号と、穿刺部材装着検出手段から出力される検出信号とを時系列に記憶するものである。

また、本発明の請求項7の体液成分測定装置の制御方法は、体液採取具装着時に、体液採取具装着手段から体液採取具装着状態である旨の信号を受け、穿刺部材装着検出手段からは穿刺軸が待機位置まで押し戻されていない旨の信号を受けた場合に、第1警報を報知することにより、穿刺針が体液採取具ないで正規の位置にない異常な状態であることを使用者に認識させることができるものであり、また、体液採取具装着時に、体液採取具装着検出手段から体液採取具装着状態である旨の信号を受けている状態で、一度、穿刺部材装着検出手段から穿刺軸が待機位置にある旨の信号を受けた後に、穿刺部材装着検出手段から穿刺軸が待機位置にない旨の信号を受けた場合に、第2警報を報知するようにしたものであることにより、使用者に再チャージできることを認識させることができる。第1警報と第2警報の報知により、使用済みの体液採取具を使用を防止し、安全を図ると同時に、体液採取具の有効利用を図り、使用者の経済的負担を軽減させるものである。

【発明の実施の形態】
以下、本発明を適用した体液成分測定装置と制御方法の実施の形態を図を参照して説明する。
まず、図1〜図7によって、この体液成分測定装置1に使用される使い捨て要素である体液採取具(以下、チップと記載する)2について説明すると、このチップ2はモールド部品(合成樹脂成形品であって、以下、単にモールド部品と記載する)等で構成されたチップケース3を有している。このチップケース3の外形は扁平な長方体に形成されていて、内部の中心には円筒状の中空部4が形成されている。この中空部4は先端側が小径に形成され、後端側が大径に形成された段付き形状に形成されている。そして、穿刺部材である円筒状のランセット5がモールド部品等で構成されていて、このランセット5の中空部4内に収納されている。このランセット5の先端5aの中心に穿刺手段である穿刺針6が同心状に埋設され、このランセット5の後端5bの近傍位置の外周には円筒状の大径部5cが同心状に一体成形されている。そして、チップケース3には中空部4の後端近傍位置の内周に一体成形等にて形成されたリング状配列の複数の凸部やリング形状の1本の凸部等で形成されているランセット保持部8が形成されている。そして、ランセット5が中空部4内で軸方向に一定範囲内で移動自在に構成されていて、ランセット5の大径部5cがランセット保持部8内に抜き差し可能に軽圧入されるように構成されている。

そして、このチップ2は、ランセット5が未使用品であれば、図5及び図6に示すようにランセット5の後端5bの大径部5cがチップケース3のランセット保持部8内に軽圧入されている。そして、そのランセット保持部8とランセット5の大径部5cとの摩擦力によってランセット5がチップケース3内に保持されて、後述するチップ装着時に、ランセット5の後端5bが後述する穿刺軸23の先端部23aに脱着可能に装着することができるように構成されている。
一方、ランセット5が一度でも使用されて、使用済み品となると、図7に示されているように、ランセット5の後端5bの大径部5cがチップケース3のランセット保持部8内から穿刺針6の先端6a側に離脱(抜け落ちた状態)されている。この結果、使用済みランセット5は後述するチップ装着時に、穿刺軸23の先端部23aに装着することができなくなるように構成されている。
そして、チップ2の先端2aの内部には、血液等の体液を検出する成分検出手段である一対の電極からなる成分検出部9が収容されていて、その成分検出部9への通電用の一対の電極端子10が平行状に埋設されている。そして、この一対の電極端子10の後端がチップケース3の上面に形成された小窓である一対の端子穴11内に露出されている。

次に、図1〜図4に示すように、体液成分測定装置1のモールド部品等で構成されているハウジング12の上部に長方形状の表示部13が形成されていて、その表示部13内には液晶表示パネル14と回路基板15が上下に重ねられて収容されている。そして、このハウジング12の下部の中央部を貫通するモールド部品等で構成された円筒部16が設けられている。なお、この円筒部16は前側円筒部16aと後側円筒部16bとに2分割されていて、これら前側円筒部16aと後側円筒部16bは共にハウジング12内に固定されている。そして、前側円筒部16aの先端部に前述したチップ2の後端2b側が脱着可能に装着される体液採取具装着部(以下、チップ装着部と記載する)17が一体成形等されている。なお、このチップ装着部17はハウジング12に一体成形等にて形成することもできる。
そして、このチップ装着部17は扁平な角筒形等に形成されていて、その内部にチップ2の後端2b側で、そのチップ2の全長のほぼ2/3に相当する長さが脱着可能で、同心状に挿入されるように構成されているが、挿入されるチップ2の上下方向の向きを常に正しい状態に確定させるために、例えばチップ2の後端2b側及びチップ装着部17の剪断面形状を台形状等の異形に形成することが好ましい。
そして、このチップ装着部17の後端側の左右両側部には左右一対のチップロックピン18が左右対称状に配置されている。そして、このチップ装着部17内に脱着可能に挿入されたチップ2の後端の両側部に形成されている左右一対の凹部2c内に左右一対のロックピン18が左右一対のロック用ばね19の押圧力によってクリック的に嵌合されて、チップ2をロックするように構成されている。また、このチップ装着部17の上側に左右一対の接続用電極20が平行状に埋設されている。そして、このチップ装着部17内にチップ2の後端2b側が脱着可能に挿入されて、左右一対のロックピン18によってロックされた時に、この左右一対の接続用電極20の先端である圧着端子20aがチップ2の上面の左右一対の端子穴10内に露出されている左右一対の電極端子10に接続(圧着)される。これにより、成分検出部9が左右一対の電極端子10、接続用電極20を介して回路基板15にマウントされていて、後述する制御回路71に電気的に接続されるように構成されている。

次に、前側円筒部16a内には円柱状の穿刺軸23が同心状に配置されていて、この穿刺軸23の先端部23aが、例えば剪断面形状がほぼC型に形成されている。そして、この先端部23a内に前述したランセット5の後端5bが軽圧入方式で脱着可能に挿入(装着)されるランセット装着部に構成されている。
そして、円筒部16内に駆動機構24が収容されていて、この駆動機構24は穿刺軸23を待機位置P1と穿刺位置P2との間で矢印a、b方向に駆動するように構成されている。この駆動機構24には、穿刺軸23を図2及び図3に示す待機位置P1から図9に示す穿刺位置P2へ矢印a方向へ発射する圧縮コイルスプリング等からなる発射用ばね25と、その発射直後の穿刺軸23を図9に示す穿刺位置P2から図10に示す中間位置P3へ即座に引き戻す圧縮コイルスプリング等からなるリバウンド用ばね26とを備えている。この際、発射用ばね25は前側円筒部16aの後端部の内側に同心状に一体成形等された小円筒部27の外周に挿入されていて、穿刺軸23の外周に一体成形等にて形成されたばね当接用ばね当接用フランジ部28を矢印a方向に押圧するように構成されている。また、リバウンド用ばね26は小円筒部27より矢印b方向側で穿刺軸23の後端部23bの外周に挿入されていて、その後端部23bにねじ止め等にて同心状に固着された係止用フランジ部29を矢印b方向に押圧するように構成されている。

また、この駆動機構24は後述するチップ装着時等において、穿刺軸23を待機位置P1にに係止する係止機構32を備えている。この係止機構32は前側円筒部16aの後端の180°対向位置に一体成形等されて矢印b方向に延出された対称形状の一対の弾性係止アーム33を有していて、これら一対の弾性係止アーム33の先端33a側は矢印b方向へ進むに従って相互に近づくように対称状に傾斜されている。なお、これら一対の弾性係止アーム33には内側方向である矢印c方向への弾性付勢力が予め付与されている。また、これら一対の弾性係止アーム33の長さ方向のほぼ中間部の内側対向位置には対称形状の一対の係止爪34が一体成形等にて形成されている。そして、穿刺軸23が中間位置P3から待機位置P1まで発射用ばね25に抗して矢印b方向へ押し戻された時に、係止用フランジ部29が一対の弾性係止アーム33を外側方向である矢印d方向へ弾性に抗して押し広げながら、一対の係止爪34を矢印b方向へ乗り越える。そして、係止用フランジ部29が一対の係止爪34を矢印b方向へ乗り越えた瞬間に、一対の弾性係止アーム33が矢印c方向へ自動的に閉じて、係止用フランジ部29の両側が一対の係止爪34に係止されるように構成されている。
そして、後側円筒部16内には発射用操作軸37が同心状で、矢印a、b方向に移動自在に収容されていて、その発射用操作軸37の後端にねじ止め等にて同心状に固着された発射ボタン38が後側円筒部16bの後方に突出されている。そして、発射用操作軸37の前端には円錐形状のカム部39が形成されていて、発射用操作軸37と発射ボタン38は圧縮コイルスプリング等からなる戻しばね40によって後側円筒部16bに対して矢印b方向へ移動付勢され、発射ボタン38の前端の外周等に一体成形等された一対の突起41がストッパー部42に矢印b方向から当接されることによって発射用操作軸37及び発射ボタン38が定位置で停止されるように構成されている。

次に、この体液成分測定装置1には穿刺深さ調整機構44が備えられていて、この穿刺深さ調整機構44は前側円筒部16aと後側円筒部16bとの間に配置されている。この穿刺深さ調整機構44は、後述する穿刺時に、チップ2の先端2aからの穿刺針6の先端6aの突出量を調整するためのものであり、その突出量の調整は、穿刺軸23の後端部23bに固着されている係止用フランジ部29の停止位置の調整によって行われるものである。
そこで、まず、前側円筒部16aと後側円筒部16bとの間に回転調整筒45が同心状に配置されていて、この回転調整筒45は軸心の周りに回転調整自在に構成されている。そして、穿刺軸23の後端部23bに同心状に固着されている係止用フランジ部29の外周で、一対の弾性係止アーム23に対して直交する2箇所に一対の小突起46が一体成形等にて形成されている。そして、これら一対の小突起46が矢印a方向から衝突する円筒状の一対のストッパー部47が回転調整筒45の前端の内側に一体成形等にて形成されていて、この一対のストッパー部47が矢印a、b方向に対して階段形状で、対称状に形成されている。
そして、回転調整軸45の回転調整により、穿刺軸23と一体の一対の小突起46に対する階段形状の一対のストッパー部47の位相が調整されて、穿刺針6の先端6aの穿刺深さが調整されるように構成されている。

なお、回転調整筒45の外周には軸心と平行な多数の凹溝48が形成されていて、前側円筒部16aの後端の外周と後側円筒部16bの前端の外周とに一体成形等にて形成された前後一対のフランジ部49、50間に1本の位置決めピン51が回転調整軸45と平行状に架設されている。そして、この位置決めピン51の両端が一対のフランジ部49、50に形成されている放射方向に伸びる一対の長孔52内に摺動自在に保持されていて、板ばね等からなる押圧ばね53によって、この位置決めピン51が回転調整筒45外周の1つの凹溝48内に矢印e方向から選択的に係合されている。
また、ハウジング12の底部で、回転調整筒45の下部相当位置に窓穴54が開口されていて、その窓穴54内に手指を挿入して、回転調整筒45を回転調整することにより、位置決めピン51が押圧ばね53に抗して矢印e、f方向に平行移動しながら、その回転調整筒45の外周の凹溝48を1つづつ乗り越えるようにして、その回転調整筒45を一定角度づつ、クリック的に位置決めするように構成されている。

次に、この体液成分測定装置1には、再チャージ機構57が備えられている。この再チャージ機構57は再チャージ操作部58を有していて、この再チャージ機構57はハウジング12の底部に開口された窓穴59外に突出されて、この窓穴59内で矢印a、b方向にスライド自在に構成されている。そして、この再チャージ操作部58の内側に一体成形等にて形成されたアーム部60が穿刺軸23の先端部23aの近傍位置の外周に円形穴61によって相互に矢印a、b方向に摺動自在に挿入されていて、このアーム部60は穿刺軸23のばね当接用フランジ部28に矢印b方向から当接可能に構成されている。そして、この再チャージ操作部58は前側円筒部16aの外周に挿入されている圧縮コイルスプリング等の戻しばね62によって窓穴59内で矢印a方向に押し戻されるように構成されている。そして、この再チャージ操作部58を戻しばね62に抗して矢印b方向へスライド操作することによって、アーム部60で穿刺軸23のばね当接用フランジ部28を矢印b方向へ押し戻すようにして、後述するように、穿刺軸23を中間位置P3から待機位置P1へ再チャージ操作することができるように構成されている。

ここで、以上述べた構成に基づいて、この体液成分測定装置1の取扱操作について説明する。
(1) 未使用のランセット5が収納されているチップ2の装着操作
図5に示すように、チップ2内に収納されているランセット5が未使用品(以下、未使用のチップと記載する)であれば、このランセット5の大径部5cがチップ2のランセット保持部8内に軽圧入されている。一方、このとき体液成分測定装置1の穿刺軸23は図10に示す中間位置P3にある。
そこで、図6に示すように、この未使用のチップ2の後端2b側を体液成分測定装置1のチップ装着部17内に矢印b方向から挿入すると、ランセット5の後端5bが穿刺軸23の先端部23a内に同心状に軽圧入された後に、そのランセット5によって穿刺軸23が発射用ばね25に抗して図10に示す中間位置P3から図3に示す待機位置P1まで矢印b方向に押し戻される。そして、発射用ばね25にばね力がチャージされ、係止用フランジ部29の両側が係止機構32の一対の弾性係止アーム33の一対の係止爪34に係止されて、穿刺軸23が待機位置P1にロックされる。
そして、この後に、図6に示すように、チップ2の後端面2dがチップ装着部17の後端面17aに矢印b方向から突き当てられるが、その直前に、チップ2のランセット保持部8からランセット5の大径部5cが矢印a方向に相対的に抜け落ちる。そして、チップ2の後端面2dがチップ装着部17の後端面17aに矢印b方向から突き当てられるのと同時に、一対のロックピン18が一対のロック用ばね19の押圧力によってチップ2の一対の凹部2c内にクリック的に嵌合されて、チップ2の装着が完了し、チップ2は一対のロックピン18によってそのままロックされることになる。そして、このチップ2の装着により一対の接続用電極20の圧着端子20aがチップ2の一対の電極端子10に一対の端子穴11を通して接続(圧着)されることになる。

(2) 使用済みランセット5が収納されているランセット5の装着操作
