【発明の属する技術分野】
本発明は、スキャノグラム撮影とヘリカルスキャンとを実行可能なX線コンピュータ断層撮影装置に関する。

【従来の技術】
ヘリカルスキャンは、X線管及びX線検出器を連続回転させてデータ収集を繰り返しながら、被検体を載置した寝台の天板又は天板を固定した状態でX線管及びX線検出器を装備したガントリを連続的に移動させることによって実現される。このヘリカルスキャンは広範囲のデータを短時間に取得することができる利点を有している。この利点を生かして様々な応用が試みられている。

その一つに、X線撮影と同様に、ヘリカルスキャンを血管造影検査に使う試みがある。この試みでは、造影血管を断層像上で又は立体的に把握することができる。

しかし、X線撮影で慣用されているDSA(ディジタル・サブトラクション・アンジオグラフィ)のような造影剤の平面的な流れを映し、しかも造影前後の画像を差分することで背景を消し、造影血管だけを強調することで、濃度の薄い部分、例えば血管狭窄部分まで良好に映し出した画像を得ることができない。

【発明が解決しようとする課題】
本発明の目的は、X線コンピュータ断層撮影装置において、造影剤の平面的な流れを映し、しかも濃度の薄い部分まで良好に映し出した画像を得ることにある。

【課題を解決するための手段】
本発明の第1局面によるX線コンピュータ断層撮影装置は、X線管と、X線検出器と、前記X線管と前記X線検出器との間に被検体を載置する寝台と、前記被検体の周囲を回転可能に前記X線管及び前記X線検出器を支持する回転機構と、スキャノグラム撮影とヘリカルスキャンとを実行するために前記寝台と前記回転機構を制御するコントローラと、前記ヘリカルスキャンにより収集された投影データに基づいて断層像データを再構成する再構成処理部と、前記スキャノグラム撮影により収集された投影データと前記ヘリカルスキャンにより収集された投影データに基づいて特定部位が強調されたスキャノグラムデータを生成するスキャノグラム生成処理部とを具備する。
本発明の第2局面によるX線コンピュータ断層撮影装置は、X線管と、X線検出器と、前記X線管と前記X線検出器との間に被検体を載置する寝台と、前記被検体の周囲を回転可能に前記X線管及び前記X線検出器を支持する回転機構と、スキャノグラム撮影と第1のヘリカルスキャンと第1のヘリカルスキャンの後に行われる第2のヘリカルスキャンとを実行するために前記寝台と前記回転機構を制御するコントローラと、前記第1のヘリカルスキャンにより収集された投影データと前記第2のヘリカルスキャンにより収集された投影データとに基づいて特定部位が強調された断層像データを再構成する再構成処理部と、前記スキャノグラム撮影により収集された投影データと前記第2ヘリカルスキャンにより収集された投影データとに基づいて前記特定部位が強調されたスキャノグラムデータを生成するスキャノグラム生成処理部とを具備する。
本発明の第3局面によるX線コンピュータ断層撮影装置は、X線管と、X線検出器と、前記X線管と前記X線検出器との間に被検体を載置する寝台と、前記被検体の周囲を回転可能に前記X線管及び前記X線検出器を支持する回転機構と、スキャノグラム撮影と第1のヘリカルスキャンと第2のヘリカルスキャンとを実行するために前記寝台と前記回転機構を制御するコントローラと、前記第1のヘリカルスキャンにより収集された投影データと前記第2のヘリカルスキャンにより収集された投影データとに基づいて特定部位が強調された断層像データを再構成する再構成処理部と、前記第1のヘリカルスキャンにより収集された投影データと前記第2ヘリカルスキャンにより収集された投影データとに基づいて前記特定部位が強調されたスキャノグラムデータを生成するスキャノグラム生成処理部とを具備する。

【発明の実施の形態】
以下、図面を参照して本発明によるX線コンピュータ断層撮影装置(X線CT装置)の実施形態を説明する。なお、X線CT装置には、X線管と放射線検出器とが1体として被検体の周囲を回転する回転/回転(ROTATE/ROTATE)タイプと、リング状に多数の検出素子がアレイされ、X線管のみが被検体の周囲を回転する固定/回転(STATIONARY/ROTATE)タイプ等様々なタイプがあり、いずれのタイプでも本発明を適用可能である。ここでは、現在、主流を占めている回転/回転タイプとして説明する。また、1スライスの断層像データを再構成するには、被検体の周囲1周、約360°分の投影データが、またハーフスキャン法でも180°+ビュー角分の投影データが必要とされる。いずれの再構成方式にも本発明を適用可能である。ここでは、約360°分の投影データから断層像を再構成する場合で説明する。また、入射X線を電荷に変換するメカニズムは、シンチレータ等の蛍光体でX線を光に変換し更にその光をフォトダイオード等の光電変換素子で電荷に変換する間接変換形と、X線による半導体内の電子正孔対の生成及びその電極への移動すなわち光導電現象を利用した直接変換形とが主流である。X線検出器としては、それらのいずれの方式を採用してもよいが、ここでは、前者の間接変換形として説明する。また、近年では、X線管とX線検出器との複数のペアを回転リングに搭載したいわゆる多管球型のX線CT装置の製品化が進み、その周辺技術の開発が進んでいる。本発明では、従来からの一管球型のX線CT装置であっても、多管球型のX線CT装置であってもいずれにも適用可能である。ここでは、一管球型として説明する。

図1に、本実施形態に係るコンピュータ断層撮影装置の構成を示している。ガントリ100は、回転リング102と、回転リング102を回転するための電動機を備えたガントリ駆動部107とを含む回転機構を有している。回転リング102には、X線管101とX線検出器103とが対向して取り付けられている。X線管101には、高電圧発生装置111からスリップリング108を経由して高電圧信号が印加される。それによりX線管101からX線が発生する。回転リング102の中央部分は開口され、そこに寝台の天板109に載置された被検体Pが挿入される。天板駆動部110は、電動機を備え、天板109をその長手方向にそって前後に移動する。通常、天板109の長手方向は、回転リング102の回転中心軸と平行に設けられる。

被検体Pを透過したX線はX線検出器103で電気信号に変換される。この電気信号はDASと呼ばれるデータ収集システム104で増幅され、またディジタルデータに変換される。なお、このディジタルデータを生データと称する。生データは、スリップリング(接触式)又は光や磁気を利用した非接触式のデータ伝送装置105を経由して前処理部106に送られる。

前処理部106では、データ収集システム104で検出された生データに対して、チャンネル間の感度不均一を補正したり、またX線強吸収体、主に金属部による極端な信号強度の低下又は信号脱落を補正する等の前処理を実行する。前処理を受けた生データを、投影データと称する。

この前処理部106を含むコンピュータシステムは、システムコントローラ112、ディスプレイ113、スキャンコントローラ114、キーボードやマウス等を備えた入力機器115、投影データやスキャノグラムデータ、さらに断層像データ等のコンピュータシステムで扱うデータの記憶のためのデータ記憶装置117、データ記憶装置117に記憶された投影データに基づいて断層像データを再構成する再構成処理部116、データ記憶装置117に記憶された投影データに基づいてスキャノグラムデータを再構成するスキャノグラム生成処理部118、投影データやスキャノグラムデータ、断層像データ等の差分を担当する差分処理部119、および表示処理部120を備えている。

次に、本実施形態の動作を説明する。本実施形態の動作として第1,第2,第3の動作モードが用意される。第1,第2,第3の動作モードは、入力機器115を介してユーザが任意に選択可能である。

図2には第1の動作モードの流れを示している。まず、スキャノグラム撮影が行われる(S1)。スキャノグラム撮影では、スキャンコントローラ114の制御のもとで、回転リング102の回転が停止された状態でX線の発生及びデータ収集が繰り返され、その間、天板109が被検体Pとともに一定速度で移動する。このスキャノグラム撮影で収集された投影データはデータ記憶装置117を介してスキャノグラム生成処理部118で収集位置に応じて配列され、また必要に応じて補間される。それによりスキャノグラムデータが生成される。このスキャノグラム撮影で得られたスキャノグラムデータを、第1のスキャノグラムデータと称する。

この第1のスキャノグラムデータはデータ記憶装置117に記憶され、またディスプレイ113に送られる。ディスプレイ113は第1のスキャノグラムデータに基づいてスキャノグラムを表示する。そして、このスキャノグラム上でヘリカルスキャンによりデータを収集する範囲が設定される(S2)。この設定は一般的に、「位置決め」と呼ばれる。位置決めが完了し、またスキャンサイクル(回転速度)やヘリカルピッチ等の設定といった様々な事前準備が完了すると、被検体Pに造影剤が注入される(S3)。

造影剤注入後、任意のタイミングでヘリカルスキャンが行われる(S4)。ヘリカルスキャンでは、スキャンコントローラ114の制御のもとで、回転リング102が一定の速度で回転された状態でX線の発生及びデータ収集が繰り返され、その間、天板109が被検体Pとともに一定速度で移動する。このように回転と移動とが組み合わされた動きにより、図3に示すように、被検体Pに対してX線管101は相対的に螺旋状に軌道を描く。このヘリカルスキャンで収集された投影データはデータ記憶装置117に記憶される。

ヘリカルスキャン終了後、システムコントローラ112の制御のもとで、初期的な再構成位置を中心とした1又は2周分の投影データがデータ記憶装置117から再構成処理部116に読み出される。再構成処理部116は1又は2周分の投影データに基づいて当該再構成位置に応じた断層像データを再構成する(S5)。この断層像データはデータ記憶装置117に記憶される。

再構成処理と並行して、システムコントローラ112の制御のもとで、ヘリカルスキャンで収集した投影データの中から、スキャノグラム撮影と同じ方向(ビュー角)で収集された投影データ、またはスキャノグラム撮影と同じ方向で収集された投影データ及びその対向データがデータ記憶装置117からスキャノグラム生成処理部118に読み出される(S6)。スキャノグラム撮影の方向は典型的には図4に示すように0°(トップビュー)又は90°(サイドビュー)に設定される。この場合、ヘリカルスキャンで収集した投影データの中から、X線管101が0°(又は90°)にある時に収集した投影データ、またはX線管101が0°(又は90°)にある時に収集した投影データ及び180°(又は270°)にある時に収集した投影データ(対向データ)とが、がデータ記憶装置117からスキャノグラム生成処理部118に読み出される。

スキャノグラム生成処理部118では、読み出された投影データがそれぞれの収集位置に応じて配列され、また必要に応じて補間される。それによりスキャノグラムデータが生成される(S7)。このヘリカルスキャンで収集された投影データの一部から生成されたスキャノグラムデータを、第2のスキャノグラムデータと称し、第1のスキャノグラムデータと区別する。この第2のスキャノグラムデータはデータ記憶装置117に記憶される。

システムコントローラ112の制御のもとで、第1のスキャノグラムデータと第2のスキャノグラムデータとがデータ記憶装置117から差分処理部119に読み出される。差分処理部119は、第1のスキャノグラムデータを第2のスキャノグラムデータから差分する(S8)。なお、差分により得られたスキャノグラムデータを第3のスキャノグラムデータと称する。第1のスキャノグラムデータは造影前に得られたデータであり、一方、第2のスキャノグラムデータは造影後に得られたデータである。従って、第3のスキャノグラムデータには、背景組織等が消され、造影血管だけが強調されている。

第3のスキャノグラムデータは、断層像データと共に、表示処理部120に送られる。表示処理部120は、第3のスキャノグラム上に、断層像データの再構成位置を表すマーカを合成し(S9)、さらに断層像データと第3のスキャノグラムデータとを1フレームに合成して(S10)、表示のためのフレームデータを生成する。表示処理部120は、フレームデータを例えばNTSC方式のビデオ信号に変換し、ディスプレイ113に供給する。それにより図5に示すようにディスプレイ113に断層像121が、断層像の再構成位置を表すマーカ122が合成された第3のスキャノグラム120と同じ画面に並んで表示される(S11)。ユーザは、入力機器115を介してマーカ122をスキャノグラム120上で移動することにより、再構成位置の変更が可能である(S12)。再構成位置が変更された場合、S5、S9,S10を経て、S11において、変更された再構成位置の断層像121が、変更された再構成位置を表すマーカ122が合成された第3のスキャノグラム120とともに表示される。

このように断層像が、X線撮影で慣用されているDSA(ディジタル・サブトラクション・アンジオグラフィ)と同様な平面画像(第3のスキャノグラム)とともに表示されるので、観察者は、断層像から得られる情報に加えて、DSAから得られる情報、つまり造影剤の平面的な流れ、そして背景組織等が消され、造影血管、しかも濃度の薄い部分、例えば血管狭窄部分の情報とともに、断層像の再構成位置情報を得ることができる。

なお、図6には他の表示例を示している。上記図5の例では、ヘリカルスキャンの全体範囲を対象として、第2のスキャノグラムデータを生成し、第1のスキャノグラムデータを差分したが、図6に示すように、ヘリカルスキャンの全体範囲の中の位置、ここではヘリカルスキャンの開始位置から再構成位置までの範囲を対象として、第2のスキャノグラムデータを生成し、第1のスキャノグラムデータを差分し、その部分的な差分により得られたスキャノグラム123を、表示するようにしても良い。この表示例では、ユーザは、ヘリカルスキャン開始から再構成位置までの範囲で造影剤の流れを観察することができ、再構成位置を自由に移動することで造影剤の動きを動的に把握することができる。

また、上記説明では、S5からS11の処理は、ヘリカルスキャン(S4)で設定範囲全域のデータの収集を完了して、ヘリカルスキャンが終了した後に、行われている。しかし、S5からS11の処理は、ヘリカルスキャンと並行して実時間で行うようにしても良い。

つまり、ヘリカルスキャンにより投影データを次々と収集しながら、一定角度毎に最新の360°又は180°+α(ファン角)分の投影データに基づいて断層像データの再構成を繰り返し、またそれと並行して、スキャノグラムと同じ方向の投影データを次々と抽出し、その抽出した投影データを次々と加えることで、ヘリカルスキャンの進行に伴って対象範囲が体軸方向に拡大していくスキャノグラムデータを生成し、そして造影前のスキャノグラムとの差分を行って、血管が強調されたスキャノグラムを生成し、そのスキャノグラムと断層像データとを1フレームに合成して表示する。この場合、断層像データは最新データから再構成され、またスキャノグラムはヘリカルスキャン開始から現時点までに収集した投影データの中から抽出した所定方向の投影データから生成されるので、ヘリカルスキャンの進行に伴って、再構成位置がヘリカルスキャンの開始位置から終了位置に向かって移動し、またスキャノグラムの差分範囲が動的に徐々に拡大していくように動画のように表示される。つまり、ヘリカルスキャンの進行に追従して、スライス位置の異なる断層像がリアルタイムで再構成されていくと共に、ヘリカルスキャンにより得られた投影データから生成され得る部分スキャノグラムが体軸方向に次々と積み重ねられていくように表示される。

上記再構成処理はいわゆるリアルタイムCTで実行され、つまり断層像再構成に要する角度分(360°又は180°+α)の投影データセットを収集する時間よりも短時間で、その投影データセットに対応する断層像データを再構成し、この再構成処理をヘリカルスキャン動作に追従して、ヘリカルスキャンによるスライス位置が変化する毎に繰り返される。また、スキャノグラムデータの生成もヘリカルスキャン動作に追従してリアルタイムで行われ、つまり、再構成処理と並行して、最新の特定方向の投影データを抽出し、その抽出した投影データからスキャノグラムデータを生成し、その生成に要する時間は、ヘリカルスキャンでX線管がある特定方向から次の特定方向に移動するのに要する時間より短い。

図7には第2の動作モードの流れを示している。まず、スキャノグラム撮影(S1)、位置決め(S2)が行われた後、造影剤注入(S3)の前に、ヘリカルスキャン(第1のヘリカルスキャン)が行われる(S13)。この第1のヘリカルスキャンで収集された投影データはデータ記憶装置117に記憶される。

また、この第2動作モードでも、S14、S15,S9,S10,S11,S6,S7,S8の処理は、ヘリカルスキャンと並行して実時間で行うようにしても良い。つまり、投影データを収集しながら、造影前後の投影データを差分し、一定角度毎に最新の360°又は180°+α(ファン角)分の差分投影データに基づいて血管強調された断層像データの再構成を繰り返し、またそれと並行して、スキャノグラムと同じ方向の投影データの抽出、その抽出した投影データを配置してスキャノグラムの生成、そして造影前のスキャノグラムとの差分を行って、血管が強調されたスキャノグラムを生成し、そのスキャノグラムと断層像データとを1フレームに合成して表示する。ヘリカルスキャンの進行に伴って、差分断層像の再構成位置がヘリカルスキャンの開始位置から終了位置に向かって移動し、またスキャノグラムの差分範囲が動的に徐々に拡大していくように動画のように表示される。

図10には第3の動作モードの流れを示している。第2の動作モードと相違するのは、造影血管を強調したスキャノグラム(第3のスキャノグラム)の生成法にあり、第3の動作モードでは、差分投影データからスキャノグラム(第3のスキャノグラム)を生成する。
