Description

【発明の詳細な説明】
【０００１】 （技術分野） 本発明は骨折の固定のための骨板システム、特にロックねじおよび非ロックね
じの両方のための板穴を有する骨板を含むシステムに関する。
【０００２】 （背景技術） 骨折の内部固定のための板およびねじシステムの臨床上の成功が良く報告され
ている。しかしながら、或る種の骨折、例えば、関節周囲の骨折（これらは骨板
とねじの間の一定の角度関係を必要とする）の治療は問題が多く残っている。こ
れらの骨折の治療用の一定角度装置が利用でき、パオリ、ＰＡのシンセス（米国
）から市販されているダイナミック・コンディラーねじシステムおよび多種のブ
レード板を含む。これらの装置の全てが高レベルの外科スキル、好適な骨の量お
よび質、並びにその装置の骨折パターン適合性を必要とする。
【０００３】 これらの要件が満足されない場合、例えば、ひどく微粉砕された骨又は失われ
た骨セグメントの場合でも、通常の骨板およびねじシステムを使用しなければな
らない。これらの通常のシステムは骨折端部を一緒に圧縮し、骨をその他の断片
および骨板とぴったりに並置させるように引っ張ることにより骨折の治癒を促進
するのに特に良く適しているが、板とねじの間の角度関係は固定されず、手術後
に変化し得る。これはアライメント不良および不十分な臨床結果をもたらし得る
。
【０００４】 角度関係の変化に関する主たるメカニズムはエネルギー蓄積に関係している。
既に注目されたように、骨への骨ねじのねじ込みは骨を板に対し圧縮する。圧縮
は骨中の高い歪、その結果として、エネルギー蓄積をもたらす。生理条件から生
じる動的負荷により、板およびねじのゆるみおよび蓄積エネルギーの損失が生じ
得る。
【０００５】 板へのねじの固定は板とねじの間の一定角度関係を与え、ゆるみの発生を減少
する。ねじを板に固定する一つの方法は所謂“ロックねじ”の使用を伴う。ロッ
クねじは板穴の表面における相当するねじ切りとかみ合うそのヘッドの外面にお
けるねじ切りを有してねじを板にロックする。ロックねじを収容するためのねじ
穴を有する骨板が知られている。例えば、ドイツ特許出願第4343117号はロック
ねじ用のねじ穴を備えた骨板を開示している。ロックねじと板の間の関係が固定
されるので、ロックねじはせん断力又は捻り力に対する高い抵抗を与える。しか
しながら、ロックねじが骨断片を圧縮する能力は制限される。
【０００６】 要するに、通常の骨ねじ、即ち、板とねじの間の一定角度関係が維持されるよ
うには板に固定されないねじ（以下、“非ロックねじ”）は骨断片を有効に圧縮
するが、せん断力に対する低い抵抗を有し、これがねじのゆるみをもたらし得る
。ロックねじは骨ねじ／板穴界面で安定性を確保するせん断力に対する高い抵抗
を有するが、骨断片を圧縮する制限された能力を有する。こうして、非ロックね
じをロックねじと組み合わせる骨板システムが或る種の臨床状況に理想的であろ
う。
【０００７】 米国特許第5,601,553号はロック板および骨ねじを開示している。その板はロ
ックねじを受け取るための複数の板ねじ穴を有する。また、その板はロックねじ
が挿入される間に板を適所に保つ一時的ねじを受け取るためのねじ切りされてい
ない板穴を有する。ロックねじが挿入された後に、一時的ねじが除去される。こ
うして、非ロックねじをロックねじと組み合わせることの長期の利益が得られな
い。米国特許第5,709,686号は部分ねじ切りされた板穴を有する骨板を開示して
いる。部分ねじ切りされた穴は非ロックねじ又はロックねじが使用されることを
可能にする。板穴は部分的にのみねじ切りされているので、使用されるロックね
じは生理的負荷のもとにねじと板の間の一定角度関係を維持することができない
かもしれない。詳しくは、板内のロックねじは部分的にのみ捕捉され、こうして
ねじ山により部分的にのみ包囲される。高い応力条件および負荷条件下では、ロ
ック板穴が歪んで、ロックねじと板の間の一定角度関係が変化することを可能に
するかもしれない。これが固定の損失又は確立された手術時の板配向の損失をも
たらし得る。更に、板穴の形状寸法のために、非ロックねじとの板の転置が一方
向のみに制限される。これは断片の整復および操作には不利となるかもしれない
。
【０００８】 こうして、従来技術の欠点を解消する改良された骨板システムに対する要望が
存する。
【０００９】 （発明の開示） 本発明の骨の固定のための骨板システムは上面、骨接触面、上面および骨接触
面を通過し、ねじ山を有する少なくとも一つの第一穴、および上面および骨接触
面を通過する少なくとも一つの第二穴を有する骨板を含む。骨板システムはまた
骨とかみ合うためのねじ山を備えたシャフトおよびヘッドを有する第一ねじ（ね

に骨板を変位するために骨接触面に向かって上面に対し或る角度で傾斜された端
部を有する。
【００１２】 例示の実施態様において、骨板が骨の骨幹端に合致するような形状および寸法
にされたヘッド部分および骨の骨幹に合致するような形状および寸法にされたシ
ャフト部分を含む。ヘッド部分が第一板穴のみを有し、シャフト部分が第一板穴
および第二板穴の両方を有する。一つの実施態様において、ヘッド部分が湾曲表
面を有し、シャフト部分の前面に実質的に平行な前フォークを含み、かつシャフ
ト部分の後面から延びる後フォークを含む。別の実施態様において、ヘッド部分

質的に骨板が移植される長い間にわたって第一ファスナーおよび第二ファスナー
の両方が骨中に留まる。
【００１４】 （発明を実施するための最良の形態） 本発明の骨板システムは骨板、非ロックねじ、およびロックねじを含む。図１
は本発明により使用し得る非ロックねじ１０の例を示す。一般に、また以下に更
に詳しく記載されるように、骨板の選択された板穴に適した寸法および形状の、

第一板穴３６のねじ山４０はまた二重リードねじ山であることが好ましい。二重

【００１９】 骨板３０の骨接触面３４は骨との接触を最小にするように成形し得る。骨板と
骨との間の接触を制限することは幾つかの生物学的および機械的利点を有し、例
えば、血液供給への損傷を低減し、板除去を一層容易にする。第一板穴３４と第
二板穴３６の間の領域に台形断面（図１１）を有する骨板３０を提供することが
接触を最小にする一つの方法である。その他の方法が米国特許第5,151,103号、
同第5,053,036号、同第5,002,544号および同第4,838,252号に開示されている。

【００２０】 同じ骨板でロックねじと非ロックねじを組み合わせることにより、本発明は新
規な混合固定を提供する。非ロックねじにより、骨折整復が骨板と骨の間の摩擦
により保持される。この摩擦は骨中で非ロックねじをしっかりと締めることによ
り生じられる。しかしながら、非ロックねじと骨の間の微小運動が骨吸収、およ
び整復の損失をもたらす。更に、非ロックねじの挿入は骨がねじの締めの応力に
耐えることを必要とする。これは非ロックねじの周囲の骨中に高い応力をもたら
す。通常、高い応力は非ロックねじの山が骨中で突き出し（骨中のねじ山がせん
断で骨折する）、かつ／又は徐々に動く（骨が粘弾性物質であるので）ことを生
じ得る。これらの現象のいずれか一つがまた整復のロスをもたらす。
【００２１】 少なくとも一つのロックねじを加えることにより、整復のロスが本発明により
最小にされ、又は排除される。詳しくは、ロックねじを骨ではなく骨板に固定す
ることにより、骨の粘弾性挙動の効果が軽減され、ねじ山が突き出さず、微小運
動が防止される。ロックねじと骨板の間の付着はロックねじが骨中で側面を見せ
て食い込んで動かなくならなければならない高強度連結である。
【００２２】 或る種の関節周囲の骨折の処理は典型的には骨板に対して種々の角度のねじの
挿入を伴い、個々のねじと骨板の間の初期の角度関係を維持することが高度に望
ましいので、本発明の骨板システムはこれらの臨床適用に特に良く適している。
図７−１９は遠位大腿骨中の使用に特に設計された本発明の骨板５０を示す。骨
板５０は主としてホッファ型骨折を含むひどく微粉砕された骨折に使用されるで
あろうが、これに限定されない。
【００２３】 骨板５０は上面５２および骨接触面５４を有する。骨板５０はロックねじ２０
を受け取るための複数の板ねじ穴５６ａ、５６ｂ、５６ｃ（集約して板ねじ穴５
６と称される）および非ロックねじ１０を受け取るための複数のねじ切りされて
いない板穴５８を有する。板ねじ穴５６およびねじ切りされていない板穴５８の
夫々が上面５２および骨接触面５４を貫通する。骨板３０の場合のように、板ね

時的に一定の角度配向でロックねじ２０を骨板５０に固定し、ねじ切りされてい
ない板穴５８中の非ロックねじ１０の挿入が骨を骨接触面５４に向かって引っ張
って骨を圧縮する。
【００２４】 骨板５０は遠位大腿骨の骨幹端に合致するような形状、寸法にされたヘッド部
分６０および骨の骨幹に合致するような形状、寸法にされたシャフト部分６２を
含む。図８に最良に見られるように、ヘッド部分６０の骨接触面５４は遠位大腿
骨の輪郭にフィットするために湾曲表面である。ヘッド部分６０はシャフト部分
６２の前方サイド６６に実質的に平行な前フォーク６４およびシャフト部分６２
の後面７０から横に延びる後フォーク６８を含む。
【００２５】 ねじ切りされていない板穴５８は第二板穴３８と実質的に同様の動的圧縮ユニ
ット（ＤＣＵ）ねじ穴であることが好ましい。シャフト部分６２はロックねじお

ｂ、５６ｃの種々の角度配向を示す。一般に、ねじ穴５６ｂ、５６ｃは挿入され
たロックねじが互いに向かって収束するように配置される。外科医が選択する場
合、非ロックねじが板ねじ穴５６のいずれか中で使用し得ることが注目されるべ
きである。最後に、骨板５０はペンシルバニア、パオリのシンセス（米国）から
市販されているコンディラー・バットレス板とは幾つかの構造上の相違を有する
ことがまた注目されるべきである。例えば、コンディラー・バットレス板のヘッ
ドは長さ方向および横方向の両方で輪郭付けられ、一方、骨板５０のヘッド部分
６０は一層の解剖学的フィットのために長さ方向のみで輪郭付けられる。更に、
テール７２は組織の下に達する高くした端部を有する。
【００２７】 図２０〜２６は近位脛骨中の使用のために特別に設計された本発明の骨板８０
を示す。骨板８０は横近位脛骨プラトーの骨折に主として使用されるであろうが
、これに限定されない。骨板８０は上面８２および骨接触面８４を有する。骨板
８０はロックねじ２０を受け取るための複数の板ねじ穴８６ａ、８６ｂおよび８
６ｃ（集約して板ねじ穴８６と称される）および非ロックねじ１０を受け取るた
めの複数のねじ切りされていない板穴８８を有する。板ねじ穴８６およびねじ切
りされていない板穴８８の両方が上面８２および骨接触面８４を通過する。骨板
３０の場合のように、板ねじ穴８６のねじ山はロックねじ２０のねじ切りされた
ヘッド２２とかみ合ってロックねじ２０を一定の角度配向で骨板８０に固定する
。ねじ切りされていない板穴８８中の非ロックねじ１０の挿入は骨接触面８４を
骨に向かって引っ張って板を骨に圧縮する。
【００２８】 骨板８０は横近位脛骨の骨幹端に合致するような形状、寸法にされたヘッド部
分９０および横近位脛骨の骨幹に合致するような形状、寸法にされたシャフト部
分９２を含む。図２０および図２６に見られるように、ヘッド部分９０の骨接触
面８４は湾曲され、テーパー付けられ、また捻られて横近位脛骨プラトーの輪郭
にフィットする。ヘッド部分９０はまた縫合糸固定のため、また骨板８０の暫定
的固定のための縫合糸穴を特徴とする。
【００２９】 ねじ切りされていない板穴８８は第二板穴３８と実質的に同様の動的圧縮ユニ
ット（ＤＣＵ）ねじ穴であることが好ましい。シャフト部分９２は板ねじ穴８６
ａおよびねじ切りされていない板穴８８の両方を有し、その結果、ロックねじお
よび非ロックねじの両方がシャフト部分９２中で使用し得る。ロックねじをシャ
フト部分９２中で使用することができることは骨幹の一部の遠位皮質が失われて
おり、又はひどく損傷されている場合に特に有益である。何となれば、非ロック
ねじによる固定は遠位皮質の状態に問題があるからである。図２４に最良に見ら
れるように、板ねじ穴８６ａおよびねじ切りされていない板穴８８の間の領域は
シャフト部分９２の骨接触面８４と脛骨の間の接触を制限する矩形断面を有する
。シャフト部分９２はテーパー付きテール１０２中で終端する（図２５）。
【００３０】 シャフト部分９２と同様の様式で、ヘッド部分９０はねじ穴８６およびねじ切
りされていない穴８８を含む。ヘッド部分９０は板ねじ穴８６ｂおよび８６ｃを
特徴とする。穴８６ｂおよび８６ｃは５．０ｍｍの直径を有し、図２３および図
２６に示されるように配向される。一般に、ねじ穴８６ｂ、８６ｃは挿入された
ロックねじが互いに向かって収束するように配置される。図２３に示されるよう
に、板穴８６ｂは板表面８４から所定の距離で収束するように配向されて脛骨プ
ラトー内のロックねじ２０の位置を最適化する。図２６に示されるように、板穴
８６ｃは所定の距離で板穴８６ｂと収束するように配向されて付加的な安定性を
ロックされた一定角度構築物に与える。外科医が選択する場合、非ロックねじが
板ねじ穴８６のいずれか中で使用し得ることが注目されるべきである。
【００３１】 本明細書に開示された本発明の例示実施態様は上記目的を達成することが明ら
かであるが、多くの改良およびその他の実施態様が当業者により推考し得ること
が認められるであろう。例えば、幾つかの特徴について一つの第一板穴および一
つの第二板穴のみが必要とされるが、少なくとも二つの夫々が有利である。更に
、ねじのない付加的な板穴が所望により板中に存在して使用中の外科医の更なる
融通性を可能にし得る。それ故、特許請求の範囲は本発明の範囲内に入る全ての
このような改良および実施態様を含むことが意図されていることが理解されるで

【０００７】 米国特許第5,601,553号はロック板および骨ねじを開示している。その板はロ
ックねじを受け取るための複数の板ねじ穴を有する。また、その板はロックねじ
が挿入される間に板を適所に保つ一時的ねじを受け取るためのねじ切りされてい
ない板穴を有する。ロックねじが挿入された後に、一時的ねじが除去される。こ
うして、非ロックねじをロックねじと組み合わせることの長期の利益が得られな
い。米国特許第5,709,686号は部分ねじ切りされた板穴を有する骨板を開示して
いる。部分ねじ切りされた穴は非ロックねじ又はロックねじが使用されることを
可能にする。板穴は部分的にのみねじ切りされているので、使用されるロックね
じは生理的負荷のもとにねじと板の間の一定角度関係を維持することができない
かもしれない。詳しくは、板内のロックねじは部分的にのみ捕捉され、こうして
ねじ山により部分的にのみ包囲される。高い応力条件および負荷条件下では、ロ
ック板穴が歪んで、ロックねじと板の間の一定角度関係が変化することを可能に
するかもしれない。これが固定の損失又は確立された手術時の板配向の損失をも
たらし得る。更に、板穴の形状寸法のために、非ロックねじとの板の転置が一方
向のみに制限される。これは断片の整復および操作には不利となるかもしれない
。 ＥＰ−Ｂ０４８６７６２ＬＩＮから別の骨板が知られており、これは、ねじ山
のない多数の細長い板穴、並びに対応する外部ねじ山を具備するヘッドを有する
ねじを適合するための多数の内壁ねじ山付き円形板穴を有する。細長い板穴に挿
入するためのねじは、長四角形の断面を有しナットに対応するねじ山付きヘッド
が設けられ、もって全てのねじが板に固着される。

    【特許請求の範囲】
  1. 【請求項１】 上面（３２、５２、８２）、 骨接触面（３４、５４、８４）、 上面（３２、５２、８２）および骨接触面（３４、５４、８４）を貫通し、ね
    じ山（４０）を有する少なくとも一つの第一穴（３６、５６、８６）、および 上面（３２、５２、８２）および骨接触面（３４、５４、８４）を貫通する少
    なくとも一つの第二穴（３８、５８、８８） を有する骨板（３０、５０、８０）、 骨とかみ合うためのねじ山を備えたシャフト（２６）およびヘッド（２２）を
    有する第一ねじ （ねじ山（２４）は第一穴（３６、５６、８６）のねじ山とかみ合うような形状
    および寸法にされている）、および 骨とかみ合うためのねじ山を備えたシャフト（１４）およびヘッド（１２）を
    有する第二ねじ を含む骨の固定用の骨板システムであって、 実質的に骨板（３０、５０、８０）が移植される長い間にわたって第一ねじお
    よび第二ねじがそれらの夫々の穴（３６、５６、８６、３８、５８、８８）中に
    固定されて留まることを特徴とする骨板システム。
  2. 【請求項２】 骨板（３０、５０、８０）が複数の第一穴（３６、５６、８
    ６）および第二穴（３８、５８、８８）を含み、相当する複数の第一ねじおよび
    第二ねじが用意される請求の範囲第１項記載の骨板システム。
  3. 【請求項３】 第一ねじがセルフタッピングねじである請求の範囲第１項記
    載の骨板システム。
  4. 【請求項４】 第一ねじが自己ドリル性ねじである請求の範囲第３項記載の
    骨板システム。
  5. 【請求項５】 第一ねじがねじ配置を案内するためのガイドワイヤの挿入の
    ために管状にされる請求の範囲第１項記載の骨板システム。
  6. 【請求項６】 第二ねじがセルフタッピングねじである請求の範囲第１項記
    載の骨板システム。
  7. 【請求項７】 第一板穴（３６、５６、８６）が実質的に円錐の形状を有す
    る請求の範囲第１項記載の骨板システム。
  8. 【請求項８】 第一板穴（３６、５６、８６）が二重リードねじ山（４０）
    を有する請求の範囲第７項記載の骨板システム。
  9. 【請求項９】 骨板（３０、５０、８０）が骨と骨接触面（３４、５４、８
    ４）の間の接触を最小にするために第一板穴（３６、５６、８６）と第二板穴（
    ３８、５８、８８）の間の領域に台形断面を有する請求の範囲第１項記載の骨板
    システム。
  10. 【請求項１０】 第二板穴（３８、５８、８８）の少なくとも一つが長さ方
    向に細長く、かつ第二骨ねじのヘッドによりかみ合わされた場合に骨板（３０、
    ５０、８０）を変位するために骨接触面（３４、５４、８６）に向かって上面（
    ３２、５２、８２）に対し或る角度で傾斜された端部（４１）を有する請求の範
    囲第２項記載の骨板システム。
  11. 【請求項１１】 骨板（３０、５０、８０）が骨の骨幹端に合致するような
    形状および寸法にされたヘッド部分（６０、９０）および骨の骨幹に合致するよ
    うな形状および寸法にされたシャフト部分（６２、９２）を含む請求の範囲第２
    項記載の骨板システム。
  12. 【請求項１２】 ヘッド部分（６０、９０）が第一板穴（３６、５６、８６
    ）のみを有する請求の範囲第１１項記載の骨板システム。
  13. 【請求項１３】 シャフト部分（６２、９２）が第一板穴（３６、５６、８
    ６）および第二板穴（３８、５８、８８）の両方を有する請求の範囲第１１項記
    載の骨板システム。
  14. 【請求項１４】 ヘッド部分（６０、９０）が湾曲表面を有し、シャフト部
    分（６２、９２）の前面に実質的に平行な前フォークを含み、かつシャフト部分
    （６２、９２）の後面から延びる後フォークを含む請求の範囲第１１項記載の骨
    板システム。
  15. 【請求項１５】 シャフトが骨と骨接触面（３４、５４、８４）の間の接触
    を最小にするために第一ねじ穴（３６、５６、８６）と第二ねじ穴（３５、５８
    、８８）の間の領域に台形断面を有する請求の範囲第１１項記載の骨板システム
    。

  17. 【請求項１７】 ヘッド部分（６０、９０）が縫合糸穴を備えている請求の

  18. 【請求項１８】 破損を整復して骨断片を並列させる工程、 骨板（３０、５０、８０）を骨に対し少なくとも一つの第一ファスナーで圧縮
    して破損整復を保持する工程、および 少なくとも一つの第二ファスナーを骨板（３０、５０、８０）に対し一定角度
    関係で固定する工程 を含む骨の破損固定方法であって、 少なくとも一つの第一ファスナーを少なくとも一つの第二ファスナーの前に挿
    入し、実質的に骨板（３０、５０、８０）が移植される長い間にわたって少なく
    とも一つの第一ファスナーおよび少なくとも一つの第二ファスナーが骨中に留ま
    ることを特徴とする骨の破損固定方法。
