背景
片頭痛は、1つのタイプの血管性頭痛である。片頭痛は、一部は、関連する視
覚および胃腸管障害を伴うかまたは伴わない、頭痛の再発性発作として特徴づけ
られる。片頭痛の症状は、通常、各患者において1つのパターンをたどる。発作
は、毎日または数カ月に1回のみであり得る。無処置の発作は、数時間または数
日間さえも持続し得る。悪心、嘔吐、光恐怖症、および音恐怖症(sonophobia)が
よく起こる。四肢は、冷たくそしてチアノーゼになり得、患者は感応性になり、
隔離を要求し得る。米国のみで、毎年、約1800万人の女性および570万人の男性
が、重篤な片頭痛に罹患すると推定されている。片頭痛は、職場における時間損
失の主要な原因であると考えられる。
患者の症状に基づく片頭痛の分類は、国際頭痛学会(International Headache
Society)の頭痛分類委員会(Committee on the Classification of Headache)に
よって提唱されている(Cephalalgia 8:1-96(1988))。この委員会は、片頭痛が、
前兆を伴わない片頭痛(以前は普通の片頭痛として知られた)、前兆を伴う片頭痛
(以前は古典的片頭痛として知られた)、および片麻痺性片頭痛(以前は合併片頭
痛として知られた)と分類することを提案する。前兆を伴う片頭痛および前兆を
伴わない片頭痛は、片頭痛の2つの最も頻繁な形態である。
家族性片麻痺性片頭痛についての遺伝子座は、第19染色体に存在することが報
告されている(Joutelら,Nature Genetics 5,40-45(1993);Joutelら,Am.J.
Hum.Genet.55,1166-1172(1994);OphoffらGenomics 22,21-26(1994);Ellio
ttら,Ann.Neurol.39,100-106(1996))。しかし、他の形態の片頭痛疾患(例え
ば、前兆を伴う片頭痛および前兆を伴わない片頭痛)の基礎となる遺伝学的根拠
および対応する生化学的メカニズムは報告されておらず、そして異なるタイプの
片頭痛は、種類が異なるのかまたは程度のみが異なるのかは不明である。
現在、前兆を伴うかまたは伴わない片頭痛の陽性診断に利用可能である、特異
的な遺伝子テストまたは生化学テストはない。診断および処置は、現在、患者の
自己申告および症状の説明にのみ基づく。抗炎症薬、麦角、5-HT1レセプターア
ゴニスト、および制吐剤のような広範囲の薬剤が、片頭痛の処置にいくらか有益
であることが報告されているが、それらの使用は、片頭痛に付随する臨床上の異
質性、薬物の副作用、および患者の申告の限界によって複雑になる(Cavinessら

よびメトクロプラミドのような制吐剤が、何人かの片頭痛患者に有益な効果があ
るとのいくつかの報告が存在する。しかし、このような薬物が有効であり得る片
頭痛患者のサブタイプは公知でなく、そしてこれらの薬物の可能性のある副作用
が、有益性が明確に期待されない投与を減少させるので、これらの薬物は、広く
投与されておらずそして片頭痛の処置についてFDAによって承認されていない。
したがって、現在、片頭痛が効率的に診断も管理(manage)もされないと結論づけ
られている(Cephalalgia 8,96(1988))。
片頭痛、不安、および抑鬱の疫学的特徴に類似性が存在する(Robinsら,Arch
.Gen.Psychiatry 41:949-958(1984);Stewartら,JAMA 267:64-69(1992);Ra
smussenおよびEisen,J Clin Psychiatry 55:5-14(1994);Kesslerら,Arch.Ge
n.Psych.51:8-19(1994)、Marazzitiら,Biol.Psychiatr.31:125-129(1995))
。3つのすべての疾患は、人生のある時期の一般集団の約10〜25%を苦しめ、そ
して女性で男性よりも約2倍もよく起こる。3つのすべての疾患についての予防
的投薬は、亜急性に作用開始し、臨床的改善を測定するためには3〜6週間の投
与を必要とする。
片頭痛、抑鬱、および不安についての遺伝パターンおよび遺伝学的根拠の同定
は、これらの疾患の診断および処置を大いに容易にする。本発明は、この必要性
および他の必要性を満たす。
発明の要旨
1つの局面では、本発明は、前兆を伴う片頭痛、抑鬱、および/または不安の
症状によって特徴づけられる症候群に対する感受性について患者を診断および処
置する方法に関する。この方法は、患者における1つ以上のドーパミン作動性遺
伝子の変異体対立遺伝子を検出する工程を伴う。この症候群に関連するドーパミ
ン作動性遺伝子には、DRD1、DRD2、DRD3、およびDATが挙げられる。例えば、DRD
2 NcoI A1遺伝子のホモ接合型A1対立遺伝子の存在は、ヘテロ接合型A1/A2対立遺
伝子の存在に比して症候群への増加した感受性を示し、そしてヘテロ接合型A1/A

別の局面では、本発明は、上記の症候群を処置するために有効な薬物について
スクリーニングする方法を提供する。このような薬物は、ドーパミン作動性レセ
プターまたはDATへのドーパミンの結合を拮抗する能力を決定することによって

本発明は、さらに、治療、予防、または診断における使用のための診断剤を製
造するための、1つ以上のドーパミン作動性遺伝子の変異体対立遺伝子を検出す
るための薬剤の使用を提供する。好ましいドーパミン作動性遺伝子は、DRD1、DR
D2、DRD3、およびDATから選択される。例えば、適切な変異体対立遺伝子は、DRD
2 NcoI A1である。診断剤は、代表的には、前兆を伴う片頭痛、抑鬱、および/ま
たは不安の症状によって特徴づけられる症候群の治療、予防、または診断に使用
される。さらに、診断剤は、前兆を伴う片頭痛の治療、予防、または診断に使用
され得る。
本発明は、さらに、1つ以上のドーパミン作動性遺伝子の変異体対立遺伝子の
存在に関連する前兆を伴う片頭痛、抑鬱、および/または不安の症状によって特
徴づけられる症候群の処置における使用のための医薬を製造するための、DRD1、

詳細な説明 I.一般原理
本発明は、前兆(aura)を伴う片頭痛(migraine)、抑鬱(depression)、および/
または不安(anxiety)の症状またはこれらに対する感受性によって特徴づけられ
る症候群(mada)(これは、少なくとも一部が、1つ以上のドーパミン作動性遺伝
子の変異に起因する)またはそれへの感受性を診断および処置する方法を提供す
る。これらの方法は、ドーパミン作動性遺伝子DRD1、DRD2、DRD3、およびDATの
変異が、前兆を伴う片頭痛、抑鬱、および/または不安への増加した感受性に関
連するという見識を一部前提とする。
DRD1、DRD2、およびDRD3は、ドーパミンが一次神経伝達物質であるようである
5つのGタンパク質カップリングレセプター(DRD1〜DRD5)のうちの3つである。

DRD3遺伝子は、DRD2遺伝子と高度の配列同一性を有する(O'Dowdら,Handbook
of Receptors and Channels:G Protein-coupled Receptors(Peroutka,S.J.編
)95-123(CRC Press,Boca Raton,1994);Sokoloffら,Nature 347:146-151(19
90))。DRD2と同様に、DRD3レセプターは、シナプス後レセプターおよびドーパミ

ドーパミン輸送体(DAT)遺伝子は、ドーパミンの放出および再取り込みの量を
調整するドーパミン経路における重要な調節タンパク質である(Shimadaら,Scie
nce 254:576-577(1991))。
ドーパミン作動性遺伝子DRD1、DRD2、DRD3、およびDATの変異体対立遺伝子は
、おそらく、増加したドーパミン作動性伝達の結果として、前兆を伴う片頭痛、
抑鬱、および/または不安への増加した感受性を引き起こす。前兆を伴う片頭痛
におけるドーパミン作動性遺伝子の提案された役割は、この疾患のいくつかの臨
床的特徴に一致する。例えば、悪心および/または嘔吐は、おそらくドーパミン
刺激である片頭痛の共通の特徴である。胃運動の変化(gastrokinetic changes)
、低血圧、および他の自律神経系の変化は、ドーパミン作動性神経伝達における
障害に一致するさらなる片頭痛症状である。提唱されたメカニズムはまた、これ
までの、片頭痛患者におけるドーパミンアゴニストに対する誇張された自律神経

1));A1およびA2と命名された対立遺伝子を有するDRD3における開始コドン
から25bp下流でのA→G置換(Rietschelら,Psychiatr.Res.46:253-259(1993)
);ならびに9および10と命名された対立遺伝子を有するDATの3'非翻訳領域での
多型40bpリピート(Vandenberghら,Genomics 14:1104-1106(1992))。DRD2 TaqI
A1対立遺伝子(Noble,Science & Medicine 3,52-61(1996))は、DRD2 NcoI A1と
連鎖不平衡にあることが公知であり、これは、DRD2 NcoI A1と類似の方法で診断
方法に使用され得ることが予期される。
前兆を伴う片頭痛、不安、および/または抑鬱と相関するDRD1、DRD2、DRD3、
およびDAT遺伝子の他の変異体形態は、以下のように同定され得る。第1の工程
は、これらの遺伝子の1つにさらなる多型部位を同定することである。このよう

Acid Res.17,12427-12448(1989))、制限酵素分析、対立遺伝子特異的プローブ

は一本鎖コンホメーション分析(Oritaら,Proc.Natl.Acad.Sci.86,2766-27
70(1989))によって検出され得る。例えば、NcoI A1およびA2対立遺伝子は、
NcoI消化によって区別され得る。A2対立遺伝子のみが切断される。対立遺伝子特
異的プローブおよびプライマーのような、変異体対立遺伝子を検出するために使

またはその症状の診断における使用についての適切性を示す標識を有し得る。III .感受性分析
本発明のデータは、患者におけるDRD1、DRD2、DRD3、および/またはDAT遺伝子
の分析が、前兆を伴う片頭痛、抑鬱、および/または不安に対する感受性の尺度
として使用され得ることを示す。例えば、DRD2のNcoI A1対立遺伝子の一方また
は両方のコピーの検出は、患者における前兆を伴う片頭痛、抑鬱、および/また
は不安に対する増加した感受性を示し、そして両方のコピーの検出は、一方のコ
ピーに比して増加した感受性を示す。DRD1 B1の一方または両方のコピーの検出
は、ホモ接合型DRD1 B2に比して、前兆を伴う片頭痛の増加した危険性を示す。
ホモ接合型DRD3 A2の検出は、ホモ接合型またはヘテロ接合型DRD3 A1に比して、
前兆を伴う片頭痛の増加した危険性を示す。ホモ接合型DAT 10の検出は、ヘテロ
接合型DAT 10/9およびおそらく9/9遺伝子型に比して、前兆を伴う片頭痛の増加
した危険性を示すが、後者は、本発明の分析に包含されているためには不十分な
頻度で生じる。
任意の1つの変異体対立遺伝子を有する個体がmada症候群を発現する確率は低
い(20%を越えない)が、個々の確率は相加様式で組み合わさる。mada症候群に関
連する各変異体対立遺伝子の存在は、実施例に記載のように、確率に関連する危
険因子を割り当てられ得る。図2は、存在する危険因子の数の関数として、前兆
を伴う片頭痛を有する個体の割合を示す。いずれのドーパミン作動性危険因子も
有さない個体は、mada症候群の症状を有さず、そして5つのすべてのドーパミン
作動性危険因子を有する個体の約75%が、mada症候群の症状を有することが理解
され得る。1〜4つの危険因子を有する個体は、存在する危険因子の数に関して
、mada症候群の症状の中程度の頻度を示す。
mada症候群の危険性に関連する変異体形態を有する、上記のドーパミン作動性
遺伝子以外の他の遺伝子がおそらく存在する。このような遺伝子の変異体形態の
存在は、検出され得、そしてドーパミン作動性遺伝子の分析と類似の様式で症候
群に対する感受性の確率と相関され得る。ドーパミン作動性遺伝子と他の遺伝子
との組み合わせ統計学的分析は、診断の的中率をなおさらに増加させる。
分析は、mada症候群を生じる共通の遺伝学的基礎を有する患者のサブセットを
同定することに有用である。このような患者は、以下に論じるようにドーパミン
作動性遺伝子のアンタゴニストでの処置になじみやすい。このようなアンタゴニ
ストでの処置は、症候群を有する患者と同様な症状を示す他の患者では、異なる
遺伝的基礎のため、効果的ではないかもしれない。この分析はまた、前兆を伴う
片頭痛を、前兆を伴わない片頭痛および類似の症状を示し得る他の疾患(例えば
、脳卒中)と区別することに有用である。すなわち、mada症候群について高い数
の危険因子を有する患者は、前兆を伴わない片頭痛または脳卒中の危険よりも、
前兆を伴う片頭痛の危険がより高い。IV .処置の方法
本発明は、さらに、mada症候群に罹患しているかまたは感受性である患者を処
置する方法を提供する。これらの方法では、前兆を伴う片頭痛、抑鬱、および/
または不安の症状を有するかまたは感受性である患者は、1つ以上のドーパミン
作動性遺伝子DRD1、DRD2、DRD3、およびDATに対するアンタゴニストの治療有効
量で処置される。このような用量は、前兆を伴う片頭痛、抑鬱、および/または
不安の症状を予防、停止、または検出可能に緩和するために十分である。用量は
、予防的にまたは治療的に投与され得る。用量はまた、その症状を明瞭に表現で

ることが知られている小児患者または障害者の患者に投与され得る。
一般に、ドーパミン作動性タンパク質へのアンタゴニストの結合が強いほど、
その効力は大きい。Glaxo Wellcome製造のブプロプリオン(2-tert-ブチルアミノ
-3'-クロロプロピオフェノンヒドロキシド)は、DATアンタゴニストの一例である
。DRD2アンタゴニストおよび適切な投与量の一覧を表1に提供する。これらのア
ンタゴニストの多くはまた、DRD2に最も密接に関連する他のドーパミン作働性レ
セプター(特に、DRD3およびDRD4)に結合する。DRD2アンタゴニストとしては、フ
ェ
ノチアジン類(クロルプロマジン、フルフェナジン、プロクロルペラジン、プロ
メタジン、チオリダジン、およびトリフルオペラジン)、ブチロフェノム類(ドロ
ペリドール、ハロペリドール、ピモジド、スピペロン)、チオキサンチン類(クロ
ルプロチキセン、チオチキセン)、および他の薬物(例えば、クロザピン)が挙げ
られる。いくつかのアンタゴニストは、血液−脳関門を横断し得、したがって、
中枢および末梢の両方のドーパミン作動性レセプターを拮抗し得る。ドンペリド
ンのような他のアンタゴニストは、血液−脳関門を横切らず、したがって、末梢
レセプターのみを拮抗する。ドンペリドン、メトクロプラミド、クロルプロマジ
ン、プロクロペラジン、およびフルナラジンのようないくつかの薬剤は、ある片

アンタゴニストは、症候群の処置における使用のための医薬を製造するために
使用され得る。アンタゴニストは、薬学的キャリアとともに混合され得、薬学的
キャリアは、患者にアンタゴニストを送達するために適切な任意の適合可能な非
毒性物質であり得る。滅菌水、アルコール、脂肪、ワックス、および不活性固体
がキャリアとして使用され得る。薬学的に受容可能なアジュバント、緩衝化剤、
分散化剤なども、薬学的組成物に組み込まれ得る。薬学的組成物中の活性薬剤の
濃度は、幅広く変化し得、すなわち、約0.1重量%以下、通常は少なくとも約1
重量%から20重量%程度またはそれ以上であり得る。医薬は、静脈内、筋肉内、
皮下、鼻内、皮膚に、坐剤によって、吸入によって、または経口によって投与さ
れ得る。非経口投与可能な組成物を調製する方法は、例えば、Remjngton's Phar
maceutical Science(第15版,Mack Publishing Company,Easton,Pennsylvania
,1980)(その全体がすべての目的について参考として援用される)においてより
詳細に記載される。
経口投与については、活性成分は、固体投与形態(例えば、カプセル剤、錠剤
、および粉剤)または液体投与形態(例えば、エリキシル剤、シロップ剤、および
懸
濁液)で投与され得る。活性成分は、不活性成分および粉末化されたキャリア(例
えば、グルコース、ラクトース、スクロース、マンニトール、スターチ、セルロ

ッカリンナトリウム、タルカム、炭酸マグネシウムなど)とともにゼラチンカプ
セル中にカプセル化され得る。所望の色、味、安定性、緩衝能、分散、または他
の公知の所望され得る特徴を提供するために添加され得る追加の不活性成分の例
は、赤色酸化鉄、シリカゲル、ラウリル硫酸ナトリウム、二酸化チタン、食用白
色インクなどである。同様の希釈剤が、圧縮錠剤を製造するために使用され得る
。錠剤およびカプセル剤の両方ともが、数時間にわたって薬物の連続放出を提供
するための持続放出生成物として製造され得る。圧縮錠剤は、何らかの不快な味
覚をマスクしそして大気から錠剤を保護するために糖コーティングまたはフィル
ムコーティングされ得、あるいは胃腸管での選択的崩壊のために腸溶性コーティ

本発明は、さらに、症候群の処置のためのDRD1、DRD2、DRD3、および/またはD
ATの新規アンタゴニストについてスクリーニングする方法を提供する。可能性の
ある薬剤が、随意にドーパミンとの競合において、ヒトDRD1、DRD2、DRD3、また
はDATへの特異的結合(Kd<μM)についてスクリーニングされる。好ましい薬剤は

ka,Cancer Treatment Reports 70,637(1986);PeroutkaおよびSnyder,Am.J
.Psychiatry 137,12(1980)に記載されるように行われ得る。いくつかの薬剤は
、少なくとも1つのドーパミン作動性レセプターDRD4またはDRD5への特異的結合
の欠如についてスクリーニングされる。薬剤はまた、副作用を最小にするために
、他のレセプターへの特異的結合の欠如についてスクリーニングされ得る。例え
ば、α-アドレナリン作動性レセプターへの結合の欠如は、起立性低血圧の副作
用を
最小にする。好ましい薬剤は、約24時間の血清半減期を有し、そして投与の約15
〜60分以内にピークの血漿レベルに達し得る。
実施例 前兆を伴う片頭痛とドーパミン作動性遺伝子との間の相関関係
この実施例は、コントロール、前兆を伴わない片頭痛(MO)、およびMWA個体に
おける、5つのすべてのドーパミンレセプター遺伝子(DRD1、DRD2、DRD3、DRD4
、およびDRD5)およびDAT遺伝子における対立遺伝子変異体の分析を記載する。
A.方法
1.被験体
医師の推薦によって本研究のための被験体を同定した。個体を、国際頭痛学会
(IHS)によって確立されたMOおよ哉4WAについての診断基準(Cephalalgia 8,1-96
(1988))を使用して評価した。片頭痛のIHS定義における基準のそれぞれについて
生涯での有無を決定した。面接を、医師、看護婦、および/または訓練された面
接官によって行った。すべての面接官を、IHS基準の使用において本発明者らに
よって訓練し、そしてすべての臨床データを同じ神経学者によって再調査した。
コントロール群の個体は、(直接面接に基づいて)片頭痛についてのIHS基準に適
合せず、そして主として片頭痛を有する個体の罹患していない配偶者であった。
インフォームドコンセントを得、そしてDNA試料を収集した。すべての臨床デー
タを、遺伝子型データとは独立して得た。研究参加者の平均年齢は、53±1歳で
ある。年齢、性別、民族起源のいずれに関しても3つの研究群間に偏りは観察さ
れなかった。
2.遺伝子型決定
35歳以上の関連のない個体からの合計246のDNA試料を分析した(115コントロー
ル個体;77 MO個体;54MWA個体)。ゲノムDNAを、Puregene DNA単離キット(Gentr
a Systems,Research Triangle Park,North Carolina)を使用して単離した。遺
伝子型を、臨床状態をブラインドにした2名の個体によって独立してスコア付
けした。
3.DRD1 、DRD2、DRD3、およびDRD5
DRD1、DRD2、DRD3、およびDRD5を、以下のように増幅した。簡単にいえば、40
ngのゲノムDNAを、1×Perkin Elmer PCR増幅緩衝液、400μMの各dNTP、0.5U Ta
qGoldポリメラーゼ(Perkin Elmer,Foster City,CA)、および1μMのプライマ
ーを含む10μLの溶液中で増幅させた。酵素を、94℃で10分間の最初のインキュ
ベーションで活性化し、次いで94℃で20秒間の変性、63℃で1分間のアニーリン
グ、72℃で30秒間の伸長での増幅の14サイクル(各アニーリング工程について0.5
℃および3秒減少させた)、ならびに94℃で30秒間の変性、56℃で30秒のアニー
リング、および72℃で1分間の伸長のさらなる40サイクルを行った。増幅後、2
×制限酵素緩衝液および4Uの適切な制限酵素を含む10μLの溶液を、増幅反応物
に直接添加し、そして37℃で4時間以上インキュベートした。消化産物を、1.5
%SFRアガロースゲル(Amresco,Solon,OH)上で分離した。各マーカーの増幅に
使用したプライマー、遺伝子型を区別するために使用した制限酵素、および各対
立遺伝子の予測されるサイズを、表2に挙げる。 4.DRD4
DRD4遺伝子型を、10μLの1×ThermoPol緩衝液(New England Biolabs)、400uM
dNTP、1uMの各プライマー(表2を参照)、および0.2U Vent(exo-)Polymerase
中で50ngのゲノムDNAを増幅させることによって評価した。増幅は、98℃で1分
間の変性および70℃で5分間のアニーリング/伸長の35サイクルからなった。増
幅産物を、1.2%アガロースゲル上で分析した。
5.DAT
DAT遺伝子型を、0.5μMの蛍光標識したdUTP(Applied Biosystems,Foster Cit
y,CA)の存在下で40ngのゲノムDNAを増幅させることによって評価した。反応を
、94℃で1分間の変性および72℃で1分間のアニーリング/伸長の35サイクルに
よって進行させた。増幅産物を、6%ポリアクリルアミドゲルを使用してABI 37
3配列決定機で分析した。分析をこれまでに記載されたように行った(Vandenberg
hら,Mol.Brain Res.15,161-166(1992);Doucette-StammらGenet.Epidemio
l.12,303-308(1995))。
B.結果
1.DRDI 5 ’UTR B1およびB2対立遺伝子頻度
DRD1遺伝子の5'非翻訳領域におけるA→G多型が記載されている(Cichonら,H
um.Mol.Genet.3,209-209(1994))。本研究のデータセット(n=246個体)におい
て、DRD1 B1対立遺伝子頻度は0.37であり、そしてB2対立遺伝子頻度は0.63であ
る。これらの値は、白色人種で報告されたDRD1 B1およびB2対立遺伝子頻度と類
似している(同上)。全体のデータセットにおいて、16%の個体がB1/B1遺伝子型
を有し、41%がB1/B2遺伝子型を有し、そして43%がB2/B2遺伝子型を示す。
MWAとDRD1 B1対立遺伝子との関連は、遺伝子型分布の分析で明らかである(表
3)。コントロール群とMOを有する個体との間の遺伝子型分布に有意差は観察さ
れない。データセット中のB2/B2遺伝子型は、コントロール個体およびMOを有す
る個体より、MWAを有する個体において有意に頻度が低い(28%)(χ2=6.28;p<0
.006)。MWAは、B2/B2個体の14%、B1/B2個体の26%、およびB1/B1個体の31%に
観察される(図1A)。
DRD1 B1よびB2対立遺伝子頻度もまた被験体の各亜群で決定した。類似の対
立遺伝子頻度を、コントロール群およびMOを有する個体の両方で観察する。対照
的に、MWAを有する個体は、コントロール群(χ2=6.71;p<0.01)またはMOを有す
る個体(χ2=3.91;p<0.02)のいずれよりもDRD2 B1対立遺伝子の有意に大きい
頻度を有する(0.47)。
2.DRD2 NcoI A1 およびA2対立遺伝子頻度
DRD2遺伝子中のアミノ酸313でのサイレント変異を生じるC→T多型が記載さ
れている(Sarkarら,Genomics 11,8-14(1991))。本研究のデータセットにおい
て、DRD2 NcoI A1対立遺伝子頻度は0.73であり、そしてA2対立遺伝子頻度は0.27
である。これらの値は、北米集団で報告されたDRD2 NcoI対立遺伝子頻度と同様
である(同上)。全体のデータセットにおいて、54%の個体がA1/A1遺伝子型を有
し、37%がA1/A2遺伝子型を有し、そして8%がA2/A2遺伝子型を示す。
MWAとDRD2 A1対立遺伝子との関連は、遺伝子型分布の分析で明らかである(表
4)。コントロール群とMOを有する個体との間の遺伝子型分布に有意差は観察さ
れない。データセット中のA1/A1遺伝子型は、コントロール個体およびMOを有す
る個体より、MWAを有する個体において有意に頻度が高い(69%)(χ2=5.50;p<0
.01)。MWAは、A2/A2個体の5%、A1/A2個体の17%、およびA1/A1個体の28%で観
察される(図1A)。
DRD2 NcoI A対立遺伝子頻度もまた被験体の各亜群で決定した(表5)。類似の
対立遺伝子頻度を、コントロール群およびMOを有する個体の両方で観察した。対
照的に、前兆を伴う片頭痛を有する個体は、コントロール群または前兆を伴わな
い片頭痛を有する個体のいずれよりもDRD2 A1対立遺伝子の有意に大きい頻度を
有する(0.83)。
3.DRD3 A1 およびA2対立遺伝子頻度
DRD3レセプターのN末端部分中の9位でのグリシン→セリン置換を生じる多型
を分析した(Lannfeltら,Psychiatr.Genet.2,249-256(1992))。多型は、開始
コドンから25bp下流でのA→G置換からなり、BalIの制限部位を生じる。この多
型は、細胞膜へのレセプター挿入において役割を果たすと仮定されている(Reits
chelら,Psychiatr.Res.46,253-259(1993))。
全体のデータセットにおいて、DRD3 A1対立遺伝子頻度は0.62であり、そしてA
2対立遺伝子頻度は0.37である。これらの値は、これまでの研究で報告されたDRD
3対立遺伝子頻度と同様である(同上)。全体のデータセットにおいて、37%の個
体がA1/A1遺伝子型を有し、50%がA1/A2遺伝子型を有し、そして12%がA2/A2遺
伝子型を示す。
コントロール群とMOを有する個体との間の遺伝子型分布に有意差は観察されな
かった(表5)。しかし、データセットにおけるDRD3 A2/A2遺伝子型は、コントロ
ール個体およびMOを有する個体よりもMWAを有する個体(20%)で有意に頻度が高
い(χ2=4.32;p<0.02)。MWAは、A1/A1個体の22%、A1/A2個体の19%、およびA
2/A2個体の37%において観察される(図1C)。DRD3 A2対立遺伝子頻度は、コント
ロールおよびMO個体の両方と比較してMWAで増加している。しかし、この差は、
統計学的有意に達しない。
4.DRD4 対立遺伝子頻度
DRD4遺伝子は、レセプターの第3の細胞質ループに、2〜8重のリピート単位
の範囲にある48bp配列を含む(Van Tolら,Nature 358,149-152(1992))。本研究
のデータセットにおいて、246個体のうちの238について遺伝子型を得た。観察さ
れた対立遺伝子頻度分布は、北米集団で報告されたDRD4対立遺伝子頻度と同様で
ある(同上):2リピート(n=38;0.08対立遺伝子頻度)、3リピート(n=26;0.0
5)、4リピート(n=310;0.65)、5リピート(n=1;<0.01)、および7リピー
ト(n=101;0.21)。本研究のデータ七ットでは、106個体(45%)が最も一般的な
遺伝子型(すなわち、4/4)を示すが、91個体(38%)は少なくとも1つの7対立遺
伝子を有する(表6)。
コントロール群、MOを有する個体およびMWAを有する個体の間で遺伝子型分布
に有意差は観察されない(最も一般的な遺伝子型のデータを表6にまとめる)。MW
Aは、7対立遺伝子を有する個体の21%および7対立遺伝子を有さない個体の23
%で観察される。コントロール群、MOを有する個体、およびMWAを有する個体に
おいて、同様な対立遺伝子頻度が観察される(表6)。
5.DRD5 A1 およびA2対立遺伝子頻度
本研究のデータセットにおいて、DRD5 A1対立遺伝子頻度は0.68であり、そし
てA2対立遺伝子頻度は0.32である。これらの値は、北米集団で報告されたDRD5対
立遺伝子頻度と同様である(Sommeretら,Hum.Genet.92,633-634(1993))。全
体のデータセットにおいて、47%の個体がA1/A1遺伝子型を有し、43%がA1/A2遺
伝子型を有し、そして10%がA2/A2遺伝子型を示す。
コントロール群、MOを有する個体およびMWAを有する個体の間で遺伝子型分布
に有意差は観察されなかった(表7)。MWAは、A1/A1個体の22%、A1/A2個体の23
%、およびA2/A2個体の16%において観察される。コントロール群、MOを有する
個体、およびMWAを有する個体において、同様な対立遺伝子頻度が観察される(表
7)。
6.DAT 9 およびDAT 10対立遺伝子頻度
DAT遺伝子中の3'非翻訳領域の多型40bpリピートが同定されている(Vandenberg
hら,Genomics 14,1104-1106(1992))。全体のデータセットにおいて、DAT9対立
遺伝子頻度は0.22であり、DAT 10対立遺伝子頻度は0.77であり、そしてさらにま
れな対立遺伝子は0.01の頻度を有する。これらの値は、これまでの研究で報告さ
れたDAT対立遺伝子頻度と同様である(同上)。全体のデータセットにおいて、35
%の個体が9/10遺伝子型を有し、そして59%が10/10遺伝子型を示す。より少な
い頻度のDAT遺伝子型多型は以下の通りである:9/9(n=11)、1/2(n=2)、および
2/8(n=1)。14個体(5.7%)のみがより少ない頻度のこれら遺伝子型を有するので
、これらの個体は本研究で独立して分析しなかった。
コントロール群とMOを有する個体との間で遺伝子型分布は有意差は観察されな
かった。MWAと10/10遺伝子型との関連は、本研究で観察される(表8)。MWAは、9
/10個体の13%および10/10個体の25%において観察される(図1D)。10/10遺伝子
型頻度は、コントロール被験体(38%;χ2=4.46;p<0.02)およびMO群(40%;
χ2=4.41;p<0.02)における9/10遺伝子型頻度と比較してMWA群(69%)で有意に
高い。MWA群における10/10遺伝子型頻度が、コントロール群とMO群との組合せに
おける9/10遺伝子型と比較される場合、より高いレベルの統計学的有意性が観察
される(39%;χ2=6.46;p<0.006)。 7.対立遺伝子の「危険因子」評価
危険因子分析は、心臓血管疾患の評価および管理への重要なアプローチである
ことが判明している(Kannel,Hosp.Pract.25,119-130(1990);Wilson,Am.J
.Hypertens.7,7S-12S(1994);Hancock,Scientific American Medicine(Dal
e,D.C.およびFederman,D.D.編)1-12(Scientific American,Inc.,New York
,1995)。MWAに対して危険因子分析の原理を適用して、データの分析を、MWAと
の独立した関連を示す対立遺伝子変異体に基づいて行った。この分析の目的は、
相加および/または相乗効果が、MWAに関してドーパミン作動性遺伝子間に存在す
るかどうかを決定することであった。
本研究のデータに基づいて、ドーパミン作動性遺伝子内の同じ分子位置での別
の遺伝子型と比較して、MWAに対して増加した感受性と独立して関連する、以下
の5つの特定の対立遺伝子「危険因子」を同定した:DRD1 B1/B1またはB1/B2遺伝
子型、DRD2 A1/A1遺伝子型、DRD2 A1/A2遺伝子型、DRD3 A2/A2遺伝子型、および
DAT 10/10遺伝子型。関連の強さは異なる遺伝子の間で異なったが、MWAについて
の「危険因子」プロフィールを開発する最初の試みは、1つの例外(DRD2 A1対立遺
伝子)を除いて、それぞれの対立遺伝子「危険因子」に等しい重みを割り当てた。M
WAの頻度が、個体に存在するDRD2 A1対立遺伝子の数に関連するようである(表4
)ので、DRD2 A1/A1遺伝子型を有する個体に、危険因子「2」を割り当てた。
対立遺伝子「危険因子」の数を、本研究における各個体について決定した(すな
わち、1個体当たり0〜5のドーパミン作動性対立遺伝子「危険因子」)。次いで
、MWAの頻度を、各対立遺伝子「危険因子」群で決定した。図2にまとめるように
、MWAは、「危険因子」を有さない個体(n=6)の0%、1つの「危険因子」を有する
個体(n=19)の11%、2つの「危険因子」を有する個体(n=69)の14%、3つの「危
険因子」を有する個体(n=92)の18%、4つの「危険因子」を有する個体(n=56)の3
9%、および5つすべてのドーパミン作動性対立遺伝子「危険因子」を有する個体(
n=4)の75%に存在した。
最後に、0、1、または2つの対立遺伝子「危険因子」を有する個体対4または
5つの対立遺伝子「危険因子」を有する個体でMWA頻度を決定した。4または5つ
の対立遺伝子「危険因子」を有する個体(42%)は、0、1、または2つの対立遺伝
