【発明の詳細な説明】
抗精神病薬ならびに鎮痛薬を製造するための、1,1−ジアルキル−1,2,3
,4−テトラヒドロイソキノリンの使用
技術分野
本発明は新規な抗精神病薬に係り、より詳細には、1,1−ジアルキル−1,
2,3,4−テトラヒドロイソキノリンまたはその薬理学的に許容される塩の抗
ドパミン作用に基づく抗精神病薬、ならびに鎮痛薬に関する。
背景技術
現代人の生活環境の著しい変化にともない、問題となる疾患も変化してきてい
る。特に、現代の激変する社会、ならびに経済状況下ではヒトの精神的なストレ
スが大きく、精神的な要因に基づくそう病、うつ病、精神***病等の疾患が現代
病としてクローズアップされてきている。
抗精神病薬は、精神科領域で精神***症、非定型精神病、そう病、中毒性精神
病、器質性精神病などの精神病状態の治療に用いられている。幻覚、妄想や、精
神運動興奮に特異的な効果をもつ薬剤であることから、最近、特にその重要性が
注目されつつある。これら抗精神病薬としては古くから、フェノチアジン誘導体
、ブチロフェノン誘導体、チオキサンテン誘導体、ベンズアミド誘導体等の薬剤
が提案されており、これらの薬剤の抗精神病作用はドパミンD2受容体遮断作用
によるものと考えられている。また抗精神病薬は、精神機能のみならず神経機能
へも強い作用を有することから、神経遮断薬とも称されている。
またその鎮静作用から強力な精神安定薬(いわゆるメジャートランキライザー
)とも称されている。実際の医療の現場で使用されてきている抗精神病薬も数多
いが、最近では、現代社会の外的要素に起因する複雑な精神病について問題とな
り、この分野でより有効な医薬品の開発が精力的に行われているのが現状である
。この場合にあっては、上記疾患に対する新たな治療薬を開発するために、従来
問題とされた副作用をより軽減し、かつ治療効果の強い薬物の開発が必須であ
るといえる。
発明の開示
本発明者らはこれまでに、優れた治療効果を有する種々の医薬品の開発を鋭意
検討してきており、そのなかでイソキノリン誘導体の薬理作用に着目してきてい
る。イソキノリン誘導体に関しては、これまでに末梢血管拡張作用、交感神経刺
激作用、鎮痛作用、鎮痙作用等の薬理作用を有するものが知られており、実際の
医薬品として使用されているものもいくつか存在する。
本発明者らは、イソキノリン誘導体のなかでも従来積極的に取り上げられてい
なかった、単純な化学構造式を有する1,1−ジアルキル−1,2,3,4−テ
トラヒドロイソキノリンまたはその薬理学的に許容される塩について検討を加え
た結果、これら化合物がドパミンD2受容体遮断作用、すなわち、抗ドパミン作
用による抗精神病薬として、有効なものであることを新規に見いだした。さらに
本発明者らは、これらの化合物には通常麻薬性鎮痛剤にみられるモルヒネ様の依
存作用のない、強力な鎮痛作用が存在することを新規に見いだした。したがって
本発明は、1,1−ジアルキル−1,2,3,4−テトラヒドロイソキノリンま
たはその薬理学的に許容される塩の抗ドパミン作用に基づく抗精神病薬および鎮
痛剤を提供する。
しかして、本発明の一つの態様は、下記の一般式(I):
(式中、R1およびR2は、それぞれ独立して、低級アルキル基をあらわす。)で
示される1,1−ジアルキル−1,2,3,4−テトラヒドロイソキノリンまた
はその薬理学的に許容される塩を有効成分とする抗精神病薬を提供する。
上記一般式(I)で示される化合物のなかで、いくつかのものはすでに公知の
化合物であるが、従来これらの化合物にドパミンD2受容体遮断作用があるこ
とは全く知られていなかったものである。
したがって、詳細には、本発明は、その具体的な薬理作用である抗ドパミン作
用(特に、ドパミンD2受容体遮断作用)に基づく、上記式(I)で示される1
,1−ジアルキル−1,2,3,4−テトラヒドロイソキノリンまたはその薬理
学的に許容される塩を有効成分とする抗精神病薬を提供する。
本発明の別の態様は、上記式(I)で示される1,1−ジアルキル−1,2,
3,4−テトラヒドロイソキノリンまたはその薬理学的に許容される塩を有効成
分とする鎮痛薬を提供する。
さらに本発明は、上記の各作用を基本とする医薬の提供にあり、具体的には上
記式(I)で示される、1,1−ジアルキル−1,2,3,4−テトラヒドロイ
ソキノリンまたはその薬理学的に許容される塩からなる医薬を提供し、より具体
的には、式(I)で示される1,1−ジアルキル−1,2,3,4−テトラヒド
ロイソキノリンまたはその薬理学的に許容される塩からなる精神***病治療薬を
提供する。

発明を実施するための最良の形態
本明細書ならびに請求範囲の記載において、「医薬」とは、薬事法にいう医薬
品、および医薬部外品のなかでヒトの疾患の治療に使用されることが目的とされ
るものをいう。
また本明細書において「低級」なる語は、この語が付された置換基または化合
物における炭素数が1〜7個、好ましくは1〜4個であることを意味する。した
がって、上記一般式(I)中において置換基「R1」および「R2」の定義におけ
る「低級アルキル基」としては、炭素原子数1〜7の鎖状または分枝鎖状のアル
キル基を意味し、例えばメチル、エチル、n−プロピル、イソプロピル、n−ブ
チル、イソブチル、sec−ブチル、tert−ブチル、n−ヘキシル、イソヘ
キシル、n−ヘプチル、イソヘプチル等が挙げられる。そのなかでも、メチル、
エチル、n−プロピル、イソプロピル、n−ブチル、イソブチル、sec−ブチ
ル、tert−ブチルが好ましい。
以下は、式(I)で示される1,1−ジアルキル−1,2,3,4−テトラヒ
ドロイソキノリンの具体的例示である。

上記に例示した、式(I)で示される1,1−ジアルキル−1,2,3,4−
テトラヒドロイソキノリンのいくつかは、すでに公知の化合物であり、例えば、
J.Med.Chem.,16(2),151〜156(1973)(H.E.L
apeら)に記載の方法に従って、収率良く製造することができる(後記製造例
参照)。
かかる方法によって製造された化合物には、その1位が不斉炭素原子である場
合もあり得る。したがって、理論的には2種の光学異性体が存在するが、本発明
はこれら光学異性体およびその混合物のいずれをも包含する。
式(I)で表される1,1−ジアルキル−1,2,3,4−テトラヒドロイソ
キノリンまたはその薬理学的に許容される塩(以下、「有効成分(I)」と略称する
場合もある。)は、抗精神病作用、より詳細には、抗ドパミン作用(特に、ドパ
ミンD2受容体遮断作用)に基づく精神***症治療作用を示す。
一般に、精神病薬の作用機序的なものをみると、その第一段階は、前頭葉・周
縁系、線条体、視床下部などに分布するドパミン受容体の遮断であるといわれて
いる。そのなかで精神***病にあっては、脳のドパミンD2受容体が顕著に増加
していること、またドパミン代謝も増加していることより、精神***病のある種
の症状におけるドパミン神経系の過剰活動の関連が問題となっていた。これまで
に以上の発見を基に、ドパミンD2受容体遮断作用を有する他種の薬物が検討さ
れてきた。その中で、ブチロフェン誘導体であるハロペリドール、ベンズアミド
誘導体であるスルピリド、あるいはジベンゾチアゼピン誘導体であるクロザピン
等が登場し、実際の治療に用いられてきている。
本発明の有効成分(I)は、これら治療薬であるハロペリドール、スルピリド
よりも副作用の発現(例えば、カタレプシーの発現)がなく、毒性が極めて低い
ドパミンD2受容体遮断作用を示すものである。特に、本発明の有効成分(I)
のうち、1,1−ジメチル−1,2,3,4−テトラヒドロイソキノリン・塩酸
塩は、この種の薬物のなかで最も効果が優れているといわれているハロペリドー
ルと比較して、同等の薬効を示しながら、カタレプシー等の副作用がないもので
ある。したがって、安全域が極めて広いものであることが判明した。
したがって、本発明のより好ましい具体的態様としては、1,1−ジメチル−
1,2,3,4−テトラヒドロイソキノリンまたはその薬理学的に許容される塩
を有効成分とする抗精神病薬、特にドパミンD2受容体遮断作用に基づく精神分
裂病治療薬であり、またかかる医薬、特に精神***病治療薬としての医薬である
。
さらに本発明の有効成分(I)は、ドパミンD2受容体遮断作用に加えて、強
力な鎮痛作用があることが判明した。すなわち、本発明の有効成分についてHa
ffner(ハフナー)法による鎮痛作用を検討したところ、アミノピリンより
強く、最も強力な鎮痛作用を有するモルヒネの、3分の1程度の鎮痛効果を示す
ことが明らかになった。その一方で、化合物には麻薬系の鎮痛剤であるモルヒネ
に認められる耽溺性形成、すなわち、モルヒネ様の依存性形成が認められないも
のであった。加えて本発明の化合物は、消化管おける潰瘍形成等の副作用をなん
ら示すものではなかった。ところでモルヒネは、その強力な鎮痛効果により、現
在癌末期患者の疼痛緩和に使用されているが、習慣性などの耽溺性形成により使
用域が限られている。その点で本発明の有効成分(I)は、モルヒネに代わる鎮
痛薬としてその効果が期待されるものである。そのなかでも1,1−ジメチル−
1,2,3,4−テトラヒドロイソキノリン・塩酸塩は、最も鎮痛効果が優れた
ものであった。
したがって、本発明のより好ましい別の具体的態様としては、一般式(I)で
表される1,1−ジメチル−1,2,3,4−テトラヒドロイソキノリンまたは
その薬理学的に許容される塩を有効成分とする鎮痛薬である。
本発明が提供するこれらの化合物の毒性は、極めて低いものである。例えば、
1,1−ジメチル−1,2,3,4−テトラヒドロイソキノリン・塩酸塩は、1
日当たりその100mg/kgをマウスに単回皮下投与して、連続3日間観察し
たが、なんら死亡例は認められなかった。
式(I)で示される1,1−ジアルキル−1,2,3,4−テトラヒドロイソ
キノリンは、遊離塩基の形態、もしくはその薬理学的に許容される塩のいずれの
形態でも、本発明の目的に使用することができる。式(I)の化合物は、有機ま
たは無機酸により、そのような塩に変換することができる。式(I)の遊離塩基
を、有機酸または無機酸と処理することにより行うことができる適切な有機酸と
しては、例えばギ酸、酢酸、プロピオン酸、酪酸、トリフロロ酢酸、トリクロロ
酢酸等の低級脂肪酸;コハク酸、マレイン酸、フマル酸等の有機酸;安息香酸、
p−ニトロ安息香酸等の置換または未置換の安息香酸;メタンスルホン酸、トリ
フロロメタンスルホン酸等の(ハロ)低級アルキルスルホン酸;ベンゼンスルホ
ン酸、p−ニトロベンゼンスルホン酸、p−ブロモベンゼンスルホン酸、p−ト
ルエンスルホン酸、2,4,6−トリイソプロピルベンゼンスルホン酸等の置換
または未置換のアリールスルホン酸;ジフェニルリン酸等の有機リン酸を挙げる
ことができる。適切な無機酸としては、例えば塩酸、硫酸、臭化水素酸、ヨウ化
水素酸、ホウフッ化水素酸、過塩素酸、硝酸等を挙げることができる。
かくして得られた塩のなかでも、とりわけ好ましいものは、コハク酸塩、マレ
イン酸塩、フマル酸塩、メタンスルホン酸塩のような有機酸との付加塩、さらに
塩酸塩、臭化水素酸塩、硫酸塩、およびリン酸塩のような無機酸との付加塩が包
含される。
式(I)で示される1,1−ジメチル−1,2,3,4−テトラヒドロイソキ
ノリンまたはその薬理学的に許容される塩は、その有効量を含有する製剤学的に
許容される製剤組成物として、経口的または非経口的に、ヒトに投与することが
できる。その投与量は、治療を目的とする患者の年齢、体重ならびに状態、疾患
の程度、医師の診断等によっても異なるが、経口投与の場合の通常の推奨1日投
与量は、0.002〜100mg/kg、好ましくは0.01〜50mg/kg
、より好ましくは0.02〜30mg/kgであり、また非経口投与の場合の通
常の推奨1日投与量は、0.0005〜100mg/kg、好ましくは0.00
2〜50mg/kg、より好ましくは0.005〜30mg/kgである。
本発明のこれら化合物は、種々の経口投与製剤に調製することができ、そのよ
うな製剤として、錠剤、カプセル剤、散剤、トローチ剤、液剤を包含する。これ
らの製剤化は、この分野の当業者にとって自他公知の方法により行うことができ
る。すなわち、本発明の化合物を、例えば、デンプン、マンニトール、乳糖、グ
ルコース、トウモロコシデンプン等の適切な賦形剤;カルボキシメチルセルロー
スナトリウム、ヒドロキシプロピルセルロース等の結合剤;タルク、ステアリン
酸マグネシウム等の滑沢剤;形質無水ケイ酸等の流動性向上剤等と混合され、錠
剤、カプセル剤、散剤、顆粒剤またはトローチ剤を製造することができる。
化合物はまた、非経口的投与として注射製剤の形態で投与することもできる。
このような製剤は、界面活性剤、乳化剤および/または分散剤等により、予め生
理食塩水のような水担体に、分散または可溶化した注射剤を包含することができ
る。あるいはまた、化合物は、注射時に先立って分散または可溶化し得るような
結晶製剤または凍結乾燥製剤とすることもできる。水担体には、pH調整剤、安
定化剤、あるいは必要な不活性成分を加えてもよい。かかる注射剤は、患者の症
状に合わせて静脈内投与、動脈内投与、皮下投与、腹腔内投与、あるいは他の適
当なルートにより投与される。これらの投与は一気にしてもよいし、適切な時間
をかける点滴等により行うことができる。
式(I)で表される1,1−ジアルキル−1,2,3,4−テトラヒドロイソ
キノリンまたはその薬理学的に許容される塩は、ドーパミンD2受容体遮断作用
を有し、抗精神病薬、特に精神***病治療薬として有効なものである。
さらに、これらの化合物は耽溺性形成のようなモルヒネ様の依存性作用や、潰
瘍形成を含むその他の副作用を示さずに、有効な鎮痛作用が認められる。したが
って、これらの点からみて、本発明の化合物は、特異的な作用メカニズムを有し
ているものであり、優れた抗精神病薬として、また優れた鎮痛薬を提供する点で
、その医療上の貢献度は多大なものであるといえる。
以下の実施例ならびに薬理試験例は、本発明をより詳細に説明するためのもの
である。実施例1:1,1−ジメチル−1,2,3,4−テトラヒドロイソキノリンの製 造
J.Med.Chem.,16(2),151〜156に記載の方法に従って製
造した1−メチル−3,4−ジヒドロイソキノリン9.6g(66.2ミリモル
)をアセトニトリル40mlに溶解し、さらにベンジルブロマイド12.4g(
72.6ミリモル)を加え、反応混合物を3時間加熱還流した。反応終了後、反
応
液にエーテルを加え、析出した結晶を濾取し、1−メチル−2−ベンジル−3,
4−ジヒドロイソキノリニウムブロマイドを19.1g(91%)得た。
次いで、この化合物17g(53.8ミリモル)の無水テトラヒドロフラン1
00ml懸濁液に、メチルマグネシウムブロマイド(3モル溶液)71.7ml
を徐々に滴下した。滴下終了後、さらに3時間加熱還流した。反応終了後、反応
液を1M塩化アンモニウム水溶液中に加え、溶媒を減圧留去した。得られた残留
物をエーテル抽出し、硫酸マグネシウムにて乾燥後、溶媒を留去し、1,1−ジ
メチル−2−ベンジル−1,2,3,4−テトラヒドロイソキノリンを13g油
状物として得た。
上記で得られた1,1−ジメチル−2−ベンジル−1,2,3,4−テトラヒ
ドロイソキノリン3.6gを、酢酸60mlに溶解し、室温下で10%パラジウ
ム−炭素触媒(3.6g)を用いた1.0気圧による接触還元に付した。理論量
の水素の添加終了後、触媒を除き、濾液に10%水酸化ナトリウム水溶液を加え
て塩基性とした。次いで、溶液を酢酸エチルにて抽出し、有機層を10%塩酸水
溶液で抽出した。塩酸抽出液を減圧下に留去し、残渣にエーテルを加え、析出す
る結晶を濾取し、無色結晶として1,1−ジメチル−1,2,3,4−テトラヒ
ドロイソキノリン・塩酸塩を4.0g得た。1

,m).実施例2:1−メチル−1−エチル−1,2,3,4−テトラヒドロイソキノリ ンの製造
実施例1の製造工程で得た1−メチル−2−ベンジル−3,4−ジヒドロイソ
キノリニウムブロマイドを用い、メチルマグネシウムブロマイドに代えてエチル
マグネシウムブロマイドを反応させ、その後実施例1と同様に処理を行い、無色
結晶として1−メチル−1−エチル−1,2,3,4−テトラヒドロイソキノリ
ン・塩酸塩を得た。1

上記の実施例1および実施例2の方法に準じて、式(I)で表される、前記し
た他の1,1−ジアルキル−1,2,3,4−テトラヒドロイソキノリンならび
にその薬理学的に許容される塩を、また製造することができた。薬理試験例1:抗精神病薬としての作用
1,1−ジメチル−1,2,3,4−テトラヒドロイソキノリン・塩酸塩を被
験薬として使用した。対照薬としてハロペリドールおよびスルピリドを用い、抗
精神病薬としての作用を検討した。
1)抗アポモルヒネ作用(最小有効用量の算出):
方法:
アポモルヒネ3mg/kgをマウスに投与し、20分後に観察ケージ内に入れ
た。3分間の観察時間中に上方に留まっている秒数(climbing時間)を
累積測定した。
上記の被験薬と対照薬をそれぞれアポモルヒネ投与の30分前にマウスに皮下
投与した。アポモルヒネ単独投与時のclimbing時間を50%以上抑制す
る最小用量を、被験薬の最小有効用量として算出した。なお、マウスは1群3匹
を使用した。
2)カタレプシー惹起作用(カタレプシー発現最小用量の算出):
方法:
被験薬をそれぞれマウスに皮下投与し、経時的にマウスの前肢を針金にかけて
カタレプシー発現(少なくとも30秒以上の不動)の有無を、投与後5時間まで
観察した。それぞれの化合物につき、カタレプシー発現最小用量を算出した。な
お、マウスは1群3匹を使用した。
3)致死作用(最小致死量の算出):
方法:
被験薬をそれぞれマウスに皮下投与した後、3日間にわたり死亡の有無を観察
し、それぞれの化合物につき最小致死量を算出した。なお、マウスは1群3匹を
使用した。
結果:
上記した3件の薬理試験例の結果を、表1−1)にまとめた。
また表1−2)中には、被験薬の安全域として、以下の値を示した。
A:カタレプシー発現最小用量/抗アポモルヒネ作用最小有効量
(最小有効抗精神病薬量)
B:最小致死量/最小有効抗精神病薬量
表1−1)中の結果からも明らかなように、本発明の有効成分の最小有効用量
(最小抗精神病薬量)は、スルピリドよりも小さなものであり、現在入手可能な
抗精神病薬の中でも、ドーパミンD2受容体遮断で最も強力な作用を有するとい
われているハロペリドールと同程度のものであった。
しかしながら、副作用として問題となるカタレプシー発現最小用量は、本発明
の化合物は100mg/kg以上であり、さらに最小致死量も100mg/kg
以上と高いものであった。この値はハロペリドール、スルピリドに比較して極め
て大きなものである。したがって表1−2)からも明らかなようにその安全域(
A:副作用発現最小用量と薬効発現最小用量の比、B:最小致死量と薬効発現最
小用量の比)が極めて広い。
以上の結果から、本発明の有効成分である1,1−ジメチル−1,2,3,4
−テトラヒドロイソキノリン・塩酸塩は、強力なドパミンD2受容体遮断作用を
有すると共に、その安全性も極めて高いものであることが理解される。薬理試験例2:鎮痛作用および潰瘍形成作用
方法:
試験化合物としては、1,1−ジメチル−1,2,3,4−テトラヒドロイソ
キノリン・塩酸塩を使用し、投与量は、30mg/kgおよび100mg/kg
とした。対照薬として、モルヒネ・塩酸塩(30mg/kg)、インドメタシン(
100mg/kg)ならびにアミノピリン(100mg/kg)を用いた。
正常な疼痛反応を示すマウスに、試験化合物および対照薬を皮下投与し、投与
後5時間まで、Haffner法(Tail Pinch Method)によ
り経時的にその鎮痛作用の有無を観察した。また無投与対照群として溶媒のみを
投与したマウスをコントロール群としておいた。
試験終了後、マウスを屠殺し、その胃部を摘出し、実体顕微鏡下で潰瘍形成の
有無を観察した。
なお、マウスは本発明の試験化合物投与群、モルヒネ・塩酸塩投与群ならびに
インドメタシン投与群では1群3匹を用い、アミノピリン投与群では1群2匹を
用いた。
結果:
その結果を表2にまとめた。 表中の結果からも明らかなように、1,1−ジメチル−1,2,3,4−テト
ラヒドロイソキノリン・塩酸塩の鎮痛効果は、モルヒネよりも弱いものの、イン
ドメタシン、アミノピリンより強いものであった。さらに対照薬であるインドメ
タシン、アミノピリン投与群には潰瘍形成が認められ、特にアミノピリン投与群
では、全例に胃粘膜に軽度の出血が観察されたのに対して、本発明の試験化合物
にはなんら潰瘍形成が認められなかった。薬理試験3:依存性形成能の評価試験
方法:
試験化合物として、1,1−ジメチル−1,2,3,4−テトラヒドロイソキ
ノリン・塩酸塩を用い、ポジティブコントロール薬としてモルヒネ・塩酸塩を使
用した。
マウスに、被験薬として試験化合物を初日に10mg/kg、次いで、20m
g/kg,40mg/kg,60mg/kg,80mg/kg,100mg/k
gまで連日増量して6日間、その後100mg/kgを維持用量としてさらに3
日間の合計9日間、1日2回(9:00および18:00)皮下投与し、さらに
その翌日(10日目)の9:00にも100mg/kgを皮下投与した。最終投
与の2時間後に、ナロキソンを3mg/kg皮下投与して、ナロキソン誘発退薬
症候を60分間にわたって観察した。また体重変化を観察するために、ナロキソ
ン投与後経時的に体重を測定した。
