【発明の属する技術分野】この発明は、特にセメント硬
化大腿股関節ステムの交換(cemented femoral hip stem
replacement)に使用される加圧装置に関するものであ
る。加圧装置はまた他のセメント硬化接合プロテーゼの
用途(cemented prosthetic applications)にも適するも
のである。

【従来の技術および発明が解決しようとする課題】交換
用股関節ステムプロテーゼのために、ブローチ(broach)
やラスプ（rasp−骨膜剥離子）等を使用して大腿骨の内
腔を拡張しておく。ブローチにより拡大された内腔をこ
の股関節ステムプロテーゼの挿入前または挿入後のいず
れかの時点において骨セメントで充填する。
【０００３】ブローチにより拡大した内腔へのセメント
の導入後に股関節ステムプロテーゼを挿入する方法にお
いては、準備された大腿骨孔内でステムを中心に配置
し、また股関節ステムプロテーゼ周辺のセメントの圧力
を維持することが重要である。この股関節ステムプロテ
ーゼの中心への配置及びセメントの加圧は、股関節交換
手術におけるステムの長期間存続と密接な関係がある。
【０００４】従来の加圧装置は、ステム導入前に取り外
すか、あるいはまたその一部を取り外すか独立した部品
に変更するか等によって、セメント及び圧力の発生に支
障を与えていた。本発明の目的は、セメントに対する圧
力を障害なく維持することである。

【課題を解決するための手段】本発明によれば、プロテ
ーゼ構成部品を収容するための開口を形成するカラー
と、該カラーの少なくとも中央開口の一部を覆い得る、
カラーに取り付けたフードとを有する加圧装置を備えて
いる。

延びるフープを有することが好適である。さらに、フー
ドのボディはカラーからフープに至って延びていること
が好適である。
【０００９】前記フープはプロテーゼ構成部品の形状に
合致することが好適であり、また、フープは、ばね付き
ヒンジによりカラーに付着され、また該ヒンジの付勢力
に抗して横方向に折曲する構成であってもよい。
【００１０】前記フードのボディは、挿入されるプロテ
ーゼ構成部品の形状に合致することが最適である。ま
た、フードは、前記カラーの前方及び後方に沿って該カ
ラーに付着される構成であってもよい。
【００１１】前記フードは破れやすく可撓性のある薄膜
により付着されるのが好適である。
【００１２】前記フードは、該フードを硬化すべくイン
サートに裏打ちされているのが好適である。また、前記
インサートはプロテーゼ構成部品の形状に合致するもの
であってもよい。

【発明の実施の形態】以下、この発明による加圧装置の
一実施形態を図１のみを参照して説明する。
【００１４】図１は本発明による加圧装置の斜視図であ
る。図によれば、加圧装置１はカラー４及びフード３の
２部分から成っている。カラー４は医学用シリコンなど
の可撓性物質により形成されるものであり、フード３
は、高濃度ポリエチレンのインサートを有するシリコン
から形成されている。加圧装置１のボディ２は上面６、
側面８及び底面１２を有するカラー４の形状を呈してい
る。
【００１５】カラー４は中央開口１４を形成している。
このカラー４の中央開口１４は、この例では股関節ステ
ムであるプロテーゼ構成部品の形状に合致するものであ
る。開口１４は、股関節ステムの基部にフィットする内
部壁１６を有している。
【００１６】ボディ２は下方部１０及び外部壁１８を有
している。下方部１０の外部壁１８はブローチにより拡
大された大腿管(femoral canal)に合致するものであ
り、その中に、ボディ２が患者の骨のブローチ拡大され
た大腿管への入り口に据えられような形で加圧装置１が
挿入される。
【００１７】加圧装置１は、カラー４の開口１４を横切
ってアーチ状に延びるフープ(hoop)２０付きのフード３
を有する。カラー４の開口１４は股関節ステムの基部に
合致するのでその形状は概して楕円形であり、フープ２
０は楕円形の狭い幅の中央部に跨っている。フープ２０
は股関節ステムの形状に合致する。またフープ２０はば
ね付きヒンジ２２によりカラー４に付着される。さら
に、フープ２０はばね付きヒンジ２２に抗して横方向に
折曲する。
【００１８】フード３は、フード裏地(lining)２４の形
態を取るポリエチレンインサートで裏打ちされており、
これによりフード３を硬化してステムを容易に導入する
ことでステムの導入を助成している。フード裏地２４は
また股関節ステムの形に準拠するものである。
【００１９】フード３は、それを開く際に裂けたり引き
延されたりするのを想定した、破れやすく可撓性を有す
る薄膜により、カラー４の上面６の前方あるいは後方に
沿って加圧装置１に付着される。
【００２０】本発明の目的は、加圧注入器を使用してＰ
ＭＭＡ骨セメントをブローチにより拡大された大腿管に
導入させることである。加圧装置１は大腿腔内でのセメ
ントの加圧を許容する。
【００２１】加圧装置１はまた一つの加圧部品も交換あ
るいは取り除くことなくステムの導入を許容する。大腿
腔内のステムの最終的な基部位置の制御は可能である。
【００２２】本発明の加圧装置１の利点は、セメントの
導入から硬化に至るまで、加圧器からの悪影響を受ける
ことなくセメントへの圧力を維持できることである。本
発明から逸脱しない範囲での上記装置に対する改善及び
変更は可能である。
【００２３】好適な実施態様を以下に示す。 （１） 前記フードは該フードが開口をカバーする第１
位置と、該開口を露出する第２位置の２つの位置で移動
可能である請求項１記載の加圧装置。 （２） 前記カラーがプロテーゼ構成部品が挿入される
骨腔の形状に合致する外部表面を有する請求項１または
実施態様（１）に記載の加圧装置。 （３） 前記フードが前記カラーの開口に跨るフープを
有する請求項１または実施態様（１）または実施態様
（２）に記載の加圧装置。 （４） 前記フープはプロテーゼ構成部品の形状に合致
する実施態様（３）に記載の加圧装置。 （５） 前記フープは、ばね付きヒンジによりカラーに
付着され、また該ヒンジの付勢力に抗して横方向に折曲
するよう構成される実施態様（３）または実施態様
（４）記載の加圧装置。
【００２４】（６） 前記フードのボディは前記フープ
からカラーに至るまで延びている実施態様（３）乃至実
施態様（５）のいずれか１項に記載の加圧装置。 （７） 前記フードのボディは、挿入されるプロテーゼ
構成部品の形状に合致する請求項１または実施態様
（１）乃至実施態様（６）のいずれか１項に記載の加圧
装置。 （８） 前記フードは、前記カラーの前方及び後方に沿
って該カラーに付着される請求項１または実施態様
（１）乃至実施態様（７）のいずれか１項に記載の加圧
装置。 （９） 前記フードは破れやすく可撓性の薄膜により付
着される、請求項１または実施態様（１）乃至実施態様
（８）のいずれか１項に記載の加圧装置。 （１０） 前記フードは、該フードを硬化させるべくイ
ンサートで裏打ちされている請求項の１または実施態様
（１）乃至実施態様（９）のいずれか１項に記載の加圧
装置。
【００２５】（１１） 前記インサートはプロテーゼ構
成部品の形状に合致するものである実施態様（１０）に
記載の加圧装置。 （１２） 図面を参照して実質的に発明の実施形態で説
明された加圧装置。

【発明の効果】前記したように、本発明においては、プ
ロテーゼ部品を収容するための開口を形成する（明確に
する）カラーと、該カラーの少なくとも中央開口の一部
を覆い得る、カラーに取り付けたフードとを有する加圧
装置を備えたので、セメントの導入から硬化まで加圧器
に由来する障害なくセメント上の圧力を維持できる。
