It is known that calcium phosphates are materials of considerable interest for the production of scaffolds for tissue engineering, thanks to their good biocompatibility and their osseointegrative properties. However, the use of these materials is limited due to their slow biodegradability. Young-Sang Park et al. in J. Mater. Sci. 41 (2006), 4357-4364 describe the production of macroporous three-dimensional scaffolds using calcium phosphate glass by the polyurethane foam method. The calcium phosphate glass used has an atomic ratio Ca / w of 0.6.
The scope of the invention includes both porous materials (scaffolds) consisting of glass only, obtained with the use of the aforementioned compositions, and consisting of composite materials with a vitreous matrix, reinforced by ceramic particles, such as hydroxyapatite, in which the glass phase is the majority component; for example, the hydroxyapatite component can be between 5% and 50% referred to the total weight of the porous material.
The vitreous or glass-ceramic materials in porous form, object of the invention, are preferably obtained through the method of heat treatment (burn-out) of a compact of powders containing the previously described vitreous powders and powders of an organic phase, or by the method replication of a sponge, although porous materials obtained by other production processes known in the state of the art fall within the scope of the invention,
Sci. Mater. in Med. The burn-out method of an organic phase has already been reported by the authors of the present invention for silica-based glasses (see C. Vìtale-Brovarone et al. J. Mater. Sci. Mater. In Med.
17 (2006), 1069-1078). In particular, the technique provides for the production of a compact of glass powders and organic particles, the dimensions of which are chosen appropriately according to the porosity to be achieved; this compact of powders is then thermally treated leading to the sintering of the glass in the viscous phase and to the oxidation of the organic component with formation of porosity.
Preferably the organic phase content is between 40 and 70% by volume; the preferred organic phase is constituted by polyethylene powders with dimensions preferably between 50 and 600 microns. The heat treatment of the powder compact is generally carried out at temperatures between 500 and 700 ° C, preferably between 500 and 600 ° C, for times, for example, from 1 to 3 hours. At these temperatures, the phosphate glass softens and, under the action of the organic component that burns and develops fumes, the compact of dust swells considerably giving rise to very high porosities, generally between 50 and 90% by volume and more specifically between 70 and 90 % by volume.
According to a preferred embodiment, the process according to the invention contemplates that, in the heat treatment step, the direction of evacuation of the combustion fumes of the organic phase is controlled so as to generate an anisotropy in the shape and orientation of the porosities. and so also in the mechanical characteristics of the scaffold; this aspect is extremely positive as it is capable of generating the highest mechanical strengths in one or more directions, in analogy to what happens for spongy bone tissue.
The replication process of a sponge for obtaining porous materials is described in the technical literature by the publication of Y. Park et al. previously cited, in relation to phosphate glasses with compositions very rich in CaO (therefore potentially not very resorbable) and by C. Vitale-Brovarone et al. in Acta Biomar. 3 (2007), 199-208 for silice-based glass.
This process contemplates the preparation of an aqueous suspension (slurry) of glassy powders, possibly including particles of a second ceramic reinforcing phase, such as hydroxyapatite, where this aqueous suspension is preferably added with dispersing and / or binder agents such as polyvinyl alcohol, polyethylene glycol and dimethylformamide. The polymeric sponge, typically polyurethane, is then impregnated with the aforementioned aqueous suspension and left to dry at room temperature. The impregnated sponge is then subjected to heat treatment, at temperatures generally between 600 and 700 ° C with a heating up to the sintering temperature, preferably between 1 ° C / minute and 5 ° C / minute, with temperature maintenance with times 1 to 3 hours. At this temperature the glass or glass ceramic powders soften and sinter, generating a glass ceramic or composite replica of the sponge. According to a preferred embodiment, following the step of impregnating the sponge, a step of selective removal of the excess suspension is carried out along one or more directions, in such a way that, following the heat treatment, an anisotropic distribution of the porosity and / or thickening of the pore walls and trabeculae and consequently an anisotropy in the mechanical properties of the scaffold.

Esempio 1: produzione di uno scaffold mediante metodo di burn-out Example 1: production of a scaffold by the burn-out method
In the example glass powders having the previously reported preferred composition were used. These powders were mixed with 50% by volume of polyethylene with particle size between 100 and 300 microns and subjected to compaction to obtain a compact (figure 1), the heat treatment was carried out in an oven at 550 ° C for three hours, with directional evacuation of combustion fumes. The scaffold obtained (figures 3 and 4) has a total porosity of 90 ± 3% by volume and is therefore decidedly higher than the quantity of organic phase used thanks to the combined effect of the thrust of the fumes and the softening of the glass.
Contrary to the phosphate glasses, object of the invention, in the case of silica-based glasses, during the heat treatment, the removal of the fumes and the softening of the glass occur at considerably different temperatures and therefore it is not possible to exploit the swelling effect. . Figure 4 shows an SEM micrograph of the material obtained, in which it is possible to observe the size and type of porosity introduced. The porosities are partially closed, with dimensions of a few hundred microns and windows of a few tens of microns. Figure 4 also shows a certain anisotropy in the porosities which are mainly oriented and elongated along the direction of removal of the fumes.
Furthermore, a diffuse microporosity is observed along the walls which makes these scaffolds easily permeable to fluids thanks to capillary forces. In this regard, figure 5 shows a scaffold before and after an impregnation step in bovine serum diluted to 25% in water colored by a few drops of ink; the homogeneous absorption of the fluid also inside the scaffold is high as can be seen from the images relating to the section of the scaffold. The partial closure of the porosities makes these scaffolds easily workable in order to obtain simple geometries; as regards bone regeneration, it is known that the presence of open and strongly interconnected porosities is necessary to favor a rapid and effective vascularization of the scaffold. Despite the presence of closed macroporosities, the immersion of the scaffolds in SBF showed that during the resorption process, the thin walls of the pores are gradually eroded, opening the initially closed porosities as can be seen in figure 6. The high interconnected porosity it makes possible the vascularization of the scaffold and the flow of nutrients and cells within it.
The peculiar composition of these materials makes them particularly bioabsorbable, making it possible, once implanted, to stay inside the human body only for the time necessary for the bone tissue to regenerate.
In this regard, by optimizing the sintering treatment it is possible to induce a more or less high crystallization of the glass, thus modifying its resorption kinetics. The composition of the example showed, following the sintering treatment, the presence of two different crystalline phases identified as: Ca2P2O7 and Na2Mg (PO4) 3. In particular, the first phase to crystallize is calcium phosphate, while magnesium and sodium phosphate crystallizes at slightly higher temperatures.
By varying the heat treatment to obtain the scaffolds, it is therefore possible to modulate the type and abundance of the crystalline phases that nuclean, as well as the quantity and composition of the residual amorphous phase, so as to modulate the resorption kinetics of the material.
The reabsorption characteristics of the scaffolds obtained with different sintering temperatures were tested both in simulated physiological solutions (SBF) and in double distilled water, immersing the scaffolds for up to four months, carrying out a change of the physiological solution per week and monitoring both changes in pH that the associated weight changes. Furthermore, solubility tests in Tris-H1l buffer were conducted in accordance with the provisions of ISO 10993-14 ("Biological Evaluation of Medical Devices - part 14 Identification and Quantification of Degradation Products from Ceramica").
For example, as regards the scaffolds produced with the organic phase burn-out method and treated at 550 ° C for three hours, after four months in SBF, the weight loss is equal to 75% without disintegration. of the scaffold.
It is important to underline that the tests in SBF give partial results as regards the reabsorption since the precipitation of calcium phosphates on the surface of these materials is observed, following the bioactivity of the residual amorphous phase and of the Ca2P2O7 which is reported in the literature. The tests carried out in water and in Tris-HC1 are from this point of view more significant, showing a value equal to 50% for the composition object of the example.
Esempio 2: produzione di scaffold mediante il metodo della spugna polimerica Example 2: production of scaffolds by the polymer sponge method
A suspension of the vitreous powders having the previously reported preferred composition was prepared in water with the addition of polyvinyl alcohol (glass: 35% by weight, polyvinyl alcohol 6% by weight the remaining distilled water). A polyurethane sponge was then impregnated with the above suspension and subjected to heat treatment at 610 ° C for three hours obtaining a final porosity of 80 ± 1%.
In particular, the impregnation technique has been modified in order to achieve a distribution of porosities and a different thickness of the walls of the pores and trabeculae in the various directions. This result was achieved by modulating the removal step of the excess suspension in order to coat the trabeculae of the polymeric sponge more in one or more directions, thus creating, after the heat treatment, a scaffold with a direction of greater mechanical strength. This expedient is particularly useful in the case of underload applications where a high mechanical resistance is required.
The reabsorption characteristics of the scaffold thus obtained were tested in physiological solution (SBF) resulting in a weight loss after three months of 18%. The most significant test performed in water and in Tris-HC1 resulted in a weight loss value of 38%.
Both in the case of the scaffold obtained with the burn-out method and in the case of the scaffold obtained by the polymeric sponge method, extremely high and decidedly higher weight loss values were obtained than those reported in the literature for phosphates. of crystalline calcium, such as for example tricalcium phosphate in its beta form (β-Ca3 (PO4) 2) or for other crystalline calcium phosphates.
The biological characterization carried out on the composition object of the examples has shown good results in terms of cell adhesion and proliferation, as shown in Figure 7, in which the good degree of distension of some osteoblasts on the surface of the material being reabsorbed is observed.
Whatever the technique used for the production of the scaffold, through its controlled crushing it is possible to obtain porous granules of the desired size, typically between 0.2 and 5 mm to be used in the field of bone fillers.
The porous materials object of the invention have, with respect to other materials proposed for similar applications, such as SiO2-based glasses, as well as with respect to materials based on phosphate glasses known in the prior art, improved resorbability characteristics. The resorption of an implant biomaterial is a highly sought after feature, but difficult to obtain from a material for bone replacement which at the same time has adequate mechanical properties.
Furthermore, unlike other materials (polymers) which have acid dissolving products, the scaffolds object of the invention do not modify the pH of the local environment during their reabsorption. The observed resorption characteristics are also higher than the crystalline calcium phosphates currently marketed (β-TCP) and compared to the glass-ceramic phosphates reported in the literature which on average contain significantly higher quantities of CaO which stabilizes the glass network through the formation of crosslinks.
The degradation times can be modulated by acting on the composition of the glass and on the presence of more or less soluble crystalline phases to meet specific clinical needs.
