RIASSUNTO SUMMARY
La chirurgia resettiva a carico del colon-retto prevede in oltre un terzo dei casi la necessità di proteggere temporaneamente l’anastomosi intestinale mediante il confezionamento di una ileostomia di protezione. Una ileostomia temporanea inoltre è spesso necessaria in pazienti affetti da occlusione intestinale in attesa che sia completato l’iter diagnostico e terapeutico specifico. Per ricanalizzare un paziente portatore di ileostomia temporanea è ad oggi necessario un intervento chirurgico che nella quasi totalità dei casi viene eseguito in anestesia generale. L’intervento d chiusura di ileostomia presenta una significativo tasso di complicanze e morbilità e non è scevro da rischio di mortalità. Al momento attuale non esiste un dispositivo che sia in grado di deviar temporaneamente all’esterno il contenuto intestinale e che possa, una volta svolta la propri funzione, essere rimosso senza la necessità di un intervento chirurgico. La presente invenzion riguarda un dispositivo che, opportunamente posizionato all’interno del lume intestinale, poss deviare all’esterno il contenuto dell’intestino e che possa essere rimosso senza la necessita’ di u intervento chirurgico. La presente invenzione cade nel campo dei drenaggi utilizzati per permettere la fuoriuscit dallorganismo di liquidi e materiali biologici. Al momento attuale non esiste e non è comunemente in uso un catetere che permetta la deviazione temporanea del contenuto intestinale di fuori del corpo umano al fine di proteggere una o piu’ anastomosi intestinali per il temp necessario al loro consolidamento; tale obiettivo e’ al momento attuale raggiunto con l’utilizzo d una ileostomia o di una colonstomia che per essere chiusa necessita di un intervento chirurgico d eseguirsi quasi esclusivamente in anestesia generale. La ricanalizzazione di dette stomie e’ gravat da morbilità’ e mortalità’ significative. La presente invenzione e’ in grado di evitare la necessita’ d una ileostomia o colonstomia chirurgica e la relativa chiusura, riducendo i tempi del primo intervento chirurgico ed evitando un successivo intervento chirurgico. In figura 1 e 2 sono mostrati gli elementi che costituiscono il sistema nei suoi componenti base. Ad una estremità’ di detto tubo e’ connesso in modo stabile o removibile lo stent di fissaggio e drenaggio intestinale, mentre all’ altra estremità e’ presente un raccordo di congiunzione con la busta di raccolta del materiale intestinale. b) Stent di fissaggio e drenaggio intestinale: Costituisce il meccanismo di ancoraggio al lume intestinale e di drenaggio del contenuto dell’intestino. E’ connesso in modo stabile removibile ad una estremità’ del tubo di raccordo. E’ rappresentato da un o stent di materiali biocompatibile plastico e/o composito e/o metallico e/o da una lega della lunghezza compresa tra 1 mm e 100 cm ed un diametro compreso tra 1 mm e 30 cm e dotato di un memoria di forma. Il materiale e’ dotato di memoria ed e’ espandibile rendendo lo stent capace di assumere una posizione di riposo quando e’ contenuto all’intemo del meccanismo di rilascio-rimozione e di espandersi aumentando il diametro della estremità’ libera (non fissata alla cannula) quando e’ all’esterno del meccanismo di rilascio-rimozione per poi tornare alla posizione di riposo quando reinserito nel manicotto di rilascio-rimozione. Si trova all’ estremità’ del tubo di raccordo opposta a quella in cui si trova lo stent. E’ costituita di materiale plastico e/o metallico e/o sintetico. L’utilizzo della presente invenzione e’ particolarmente semplice; il sistema in posizione chiusa viene inserito all’ interno del lume intestinale a monte della anastomosi che si vuole proteggere o comunque nella regione intestinale oltre la quale non si voglia far transitare il contenuto intestinale. L’inserimento aH’intemo del lume intestinale avviene attraverso una piccola enterotomia, ossia una soluzione di continuo della parete intestinale subito a valle della zona dove verrà’ posizionata l’estremità’ del dispositivo caratterizzata dallo stent. Una volta aH’intemo del lume intestinale lo stent verrà’ portato dalla posizione chiusa alla posizione aperta mediante il semplice scorrimento del manicotto di rilascio-adattamento. Lo stent in posizione aperta, dotato di memoria, si adatterà’ quindi alla parete dell’intestino ancorando i sistema e drenando all’interno del tubo di raccordo il contenuto intestinale. La altra estremi del tubo, raccordata ad una busta di raccolta, sara’ portata al di fuori del corpo attraverso piccola incisione praticata nella parete addominale. Una volta svolta la sua funzione d drenaggio, il sistema sara’ rimosso dall’esterno del corpo, semplicemente scorrendo il manicoto in avanti e riportando lo stent in posizione chiusa; ciò’ permette al sistema di essere agevolmente sfilato senza necessita’ di interventi invasivi o anestesia. I piccoli fori chi rimangono a livello della parete addominale ed a livello della parete dell’intestino, attraverso quali passava il tubo di raccordo, si chiuderanno spontaneamente in un breve tempo per le favorevoli condizioni pressorie determinate dalla fisiologica peristalsi intestinale che determinano il passaggio preferenziale del materiale intestinale all’ interno del lume delle anse in senso ab-orale. d) Junction between the connecting pipe and the intestinal material collection bag: this constitutes the mechanism by which it is possible to join the connecting pipe with a closed bag for the collection of intestinal material.
