Description
"STRUMENTO PER AGOBIOPSIA CORIALE" "CHORIAL AGOBIOPSY INSTRUMENT"
TESTO DELLA DESCRIZIONE TEXT OF THE DESCRIPTION
The present invention relates to a medical instrument and in particular an instrument for chorionic needle biopsy, that is, for the sampling of chorionic villi.
Chorionic villus sampling in the first trimester of pregnancy (CVS) is known to be an improvement in the prenatal diagnostic technique known as amniocentesis. This procedure was introduced in 1983-1984 in order to obtain the same diagnostic information of the amniocentesis but at an earlier time (one month earlier) and through faster laboratory techniques (already in 24 hours), with a considerable reduction of time required for early prenatal diagnosis of hereditary diseases and chromosomopathies in the fetus.
The chorionic villi are the building blocks of the placenta. Their cells contain the same genetic heritage as the fetus, as they derive from the division, differentiation and specialization of cells originating from the fertilized egg cell. Diagnosis is obtained by studying chromosomes in chorionic villus cells in spontaneous or cultured stimulated mitosis (cell division).
It is also known that the chorionic villi originally collected with a cannula introduced into the uterus through the vagina and the cervical canal, are now preferably collected through a needle introduced into the uterus through the abdominal wall, free hand, and under direct ultrasound control. , at the site of development of the placenta.
There is no need for any type of analgesia or anesthesia in this type of operation. The needle, which is normally of the "spinal" type, is usually rigidly connected directly to a standard 20ml syringe and the plunger depression is created by hand or with a fine needle gun.
Alternatively, the depression can be created with a mechanical aspirator, and in this case a connecting tube of variable dimensions is connected to the needle, connected to a well, where, due to the vacuum created by the aspirator, the material is collected. of the withdrawal; the sump is in turn directly connected, by means of an additional connection, with the suction nozzle.
All the system, for both types of sampling, must be sterile, starting from the needle up to the place where the villi coriaii are collected, as the material is subsequently treated in culture media, where the growth of bacteria and molds is possible. . In addition, sterile, single-use syringes tested for cell toxicity of plastic components are currently commercially available.
It has been proven that the sampling of villi coriaii should be performed no earlier than the ninth week of gestational age and that it is advisable not to collect quantities of villi coriaii greater than 30-40 mg. The sampling in an earlier period, and / or in larger quantities, is more traumatic for the placental insertion site, and can cause a sudden and important reduction in the blood supply of the fetus.
This can determine, in some cases, fetal malformations affecting above all the bone structures (limbs and bones of the face), especially if the villi obtained are not sufficient for the analysis, and the puncture must therefore be repeated.
The difficulty in carrying out the sampling of villi coriaii lies in reaching, with the needle, the place of placental insertion, in the least traumatic way, and possibly in a single attempt, taking the minimum quantity "of villi coriaii sufficient for the diagnostic investigation. undertaken.
The placental insertion is often found, with respect to the cutaneous plane of the abdominal wall, on an angled biopsy trajectory, not easy to follow, even if under direct ultrasound control. This angle may vary after the insertion of the needle into the skin, due to the changing of the anatomical relationships of the uterus, which is a mobile organ within the pelvis. The position of the uterus also varies depending on the state of filling of the bladder, to which the uterus is attached anteriorly.
The chorionic villi have a solid nature, and a gelatinous consistency, which means that they offer resistance to aspiration through a thin needle: a long needle (150 mm) and therefore of a larger gauge (1 mm - 1, 2) must often be used. mm = 19-18 Gauge). If the biopsy angle is also not very favorable, the connection with the suction instrument becomes difficult, and the suction itself is not very effective, due to the reduced vacuum seal of the system. On the other hand, the freehand suction determines a variable and inconstant depression, depending on the force exerted by the operator, while the mechanical suction does not allow the depression to be modulated according to the case, making the collection more traumatic and less accurate.
Ulteriori caratteristiche vantaggiose dell'invenzione vengono rivendicate nelle rivendicazioni dipendenti. The instrument for chorionic needle biopsy according to the present invention allows to brilliantly solve the problems and drawbacks encountered in the instruments of the prior art, the instrument according to the invention having the characteristics indicated in the characterizing part of claim 1. Further advantageous characteristics of the invention are claimed. in dependent claims.
The features, objects and advantages of the invention will certainly be clearer and more evident from the detailed description that follows and from the drawings of a currently preferred embodiment thereof, in which:

la Figura 3 è una vista parziale dello strumento della Figura 1 che mostra una possibile alternativa al raccordo tra ago e collegamento flessibile; e Figure 3 is a partial view of the instrument of Figure 1 showing a possible alternative to the connection between needle and flexible connection; And
Figure 4 is a schematic view of the instrument of Figure 1 associated with a fine needle aspiration gun.
With reference to the figures, the instrument 10 of the invention (Fig. 1), in the preferred embodiment, consists of a syringe 12 (for example in polypropylene of 20 ml in another biocompatible plastic material) with a plunger 14 with a conical end. To the usual connection cone 16 with the needle 18 (in the form of two truncated cones superimposed with respective diameters of the base circumference, for example, approximately 6 mm and approximately 4.5 mm) , a portion of semi-rigid pipe 20 (for example made of polyethylene or other biocompatible plastic material). Conventionally it is of the type used for infusion sets or low pressure extensions, it may have a length between about 20 and 80 mm, an external diameter between about 2 and 6 mm, an internal diameter between 1 and 3 mm. approximately, and a thickness of approximately 0.35 and 0.75 mm. At the opposite end of the semi-rigid pipe, a fitting 22 (also preferably made of polyethylene or polypropylene) with a diameter between 1 and 3 mm is joined, by sealing interlocking, consisting, in a first preferred embodiment, of two trunks of superimposed cone, of different diameter, and in which the base of greater diameter is opposite to the base of greater diameter of other two truncated identical cones. Conventionally it is of the type used to connect the infusion tubes to the injection needle (preferably Round-Lock type). This connection allows an easy connection of the instrument with the common needles used for prenatal diagnosis (spinal or echo-reflective type) (Fig. 2), with excellent resistance to the depression necessary for biopsy sampling. At the opposite end of the semi-rigid pipe, a 22 'Luer-Lock fitting (Fig. 3) can also be joined, whose truncated cone connection however must protrude at least between about 5 and 7.5 mm from the ring nut. mobile threaded fixing, to guarantee an optimal seal against depression, after it has been screwed on.
The tubular system thus obtained has a fairly homogeneous internal diameter (about 2 mm) by virtue of the elasticity of the semi-rigid tube, wedged at the ends on the two truncated connecting cones, thus allowing the collected villi not to stop along the tube, but to be drawn into the syringe when the needle is removed.
The instrument is, in its rest position, "straight" (ie coaxial to the axis of the fittings) but, by virtue of the elastic properties of the tube 20, it can be flexed up to 90 ° without the tube becoming occluded (this allows an excellent connection of the system to the needle, even when the biopsy angle is not very favorable, and a maintenance of the optimal depression for aspiration even with the hose flexed). Also in this case, the villi collected do not stop along the tube, but are aspirated into the syringe when the needle is extracted.
The actuation of the plunger 14 of the syringe, for aspiration of the villi, can be controlled by a needle aspiration gun 24, in which the syringe 12 is fitted (Fig. 4). Up to now, this solution presented not a little difficulty for biopsy, as the needle-syringe and gun system assembled together was bulky, rigid, unwieldy and did not allow the operator to easily change the inclination of the needle, if this was necessary to move to the placental insertion site during aspiration. The instrument of the invention has a certain flexibility and is therefore more versatile, easily allowing the biopsy operation and avoiding the risk of accidentally disconnecting the needle from the syringe during sampling, with consequent loss of depression (Fig. 4).
The gun 24 also allows, being operated by hand, to be able to vary and graduate the extent of the depression according to the cases, while maintaining a valid and constant suction force. This reduces the likelihood of sudden changes in depression at the sampling site, reducing the invasiveness of the technique. In this sense, the fitting 20 also functions as a compensation chamber between the lumen of the needle 18 and the cylinder of the syringe 12 due to the flexion-extension property of its elastic wall, which thus partially absorbs the variations in the force of depression. It is thus possible to use short (90 mm - 100 mm) and thin (20 Gauge, ie 0.9 mm) needles for sampling, without problems of connection with the syringe, managing to standardize the "yield in villi" of the biopsy about 20 mg per single puncture. The use of a thinner needle reduces the likelihood that it will become clogged during the collection, after the extraction of the mandrel, due to the coagulation of the blood flowing into its lumen, in contact with the walls, thus determining the aspiration of clots blood instead of chorionic villi.
Studies carried out show that, with this new sampling technique, the abortion rate determined by early prenatal diagnosis through chorionic biopsy stabilizes decidedly below 0.5%.
There are currently no particular maternal risks reported in early diagnosis via chorionic biopsy. It is also intuitive that, with the same diagnostic results obtained, puncturing the abdominal wall and the uterus with a thinner needle is less annoying for the patient and less traumatic for pregnancy.
In addition, the instrument of the invention used for early prenatal diagnosis through chorionic villus sampling can be built quite easily by assembling pieces of sterile and disposable devices (medical surgical devices) already used in the health field for other purposes. It is therefore envisaged that the instrument of the invention can be proposed in an original sterile and disposable kit, already assembled, and ready to be connected to the needle and used with the fine needle aspiration gun.
Although the above invention has been specifically described for the sampling of chorionic villi, it is intuitive that it will be usable for the sampling of other body material and / or tissue in a fluid or solid state, and therefore with the term "villi chorionics "should also be understood as other aspirates.
It is clear that the embodiment described and illustrated in this report has been given purely by way of non-limiting example of the invention and that numerous modifications, variations, additions, replacements and additions to parts with equivalent elements may be made to it without however, they fall outside the scope of the invention as defined in the following claims.
