Description
SISTEMA DI ANASTOMOSI PER UN’ENDOPROTESI VESCICALE ANASTOMOSIS SYSTEM FOR A BLADDER ENDOPROTESIS
CAMPO DELL'INVENZIONE FIELD OF THE INVENTION
The present invention relates to the field of medical prostheses, and in particular to bladder prostheses that can be implanted in a body district of the human organism, or, in general, of mammals. The invention was developed with particular attention to the portions of a bladder prosthesis that receive the ureters and the urethra of the organism in which it is implanted, in order to facilitate the anastomosis operation.
STATO DELL’ARTE STATE OF THE ART
As is known, some pathologies can irreversibly damage some internal organs of the body, including the bladder. In these cases it may be necessary to replace the bladder thus compromised with an artificial prosthesis, to guarantee the correct biological functions of the organism.
The artificial bladders are made using membranes obtained from highly biocompatible materials, which also allow the same appropriate structural deformations; in this way an artificial bladder can fill and empty itself, adapting its volume and shape according to whether it is in one of these two conditions.
The artificial bladder conceived and produced by the Applicant herself, for which she submitted the international patent application with publication number â € œW02007039159â € entitled â € œOrthotopic endoprosthesis of artificial bladder and relative production methodâ € has the characteristics described above and , in particular, it is made with a soft silicone membrane coated with a pyrolytic turbostratic carbon layer to increase its biocompatibility. Furthermore, it has a pouch shape that resumes the original shape of a bladder, and includes at least three openings to allow the insertion of the ureters and urethra so that the normal physiological cycle is maintained, that is the urine produced by kidneys can reach there through the ureters and then flow out through the urethra.
The ureters and urethra have anatomical constraints that the structure of the artificial bladder must respect. In fact, the access holes for the ureters must be positioned so that the latter can maintain the original length and curvature with which the renal pelvis reaches the bladder. Furthermore, the stitching operation with which the ureters and urethra are connected to the bladder, known in the medical-surgical jargon with the term anastomosis, is of particular importance and very delicate for the final positive outcome of the operation. of replacement of a bladder. For this purpose, in correspondence of each access hole, there are cannulas that emerge with respect to the external surface of the prosthesis, on which the ureters and urethra are anastomosed. Therefore, the anastomosis will involve fixing the ureters and urethra to the respective cannula by means of one or more points made with a surgical suture thread.
SOMMARIO SUMMARY
The Applicant has realized that the known techniques partially described do not allow to have a sufficiently reliable artificial endoprosthesis, which does not require subsequent interventions after implantation in the organism for which it is intended. The cannulas, on which the anastomosis of the ureters and urethra is performed, are made of soft silicone. This material provides a high degree of biocompatibility, ensuring rapid resorption of the points and good re-growth of the tissues around it; however it makes the mechanical bond between ureters and urethra and the cannulae themselves unstable. The consistency of the material is such that the applied points can detach before reabsorption, therefore requiring further intervention. Furthermore, the anastomosis operation produces a slight and physiological inflammation of the tissues of the ureters and urethra. This inflammation manifests itself with a swelling of these tissues, which in a short time burn out and return to their natural size. In the transition between these two conditions, the applied points are rather stressed, thus increasing the risk that they detach before natural reabsorption. Furthermore, a portion of the ureters and a portion of the urethra will be contained within the defined containment volume of the endoprosthesis, therefore exposed to the bacteria that proliferate there and that in this way propagate through the ureters and the urethra. This situation is particularly disadvantageous, as the implantation of an artificial bladder solves the physiologically unsustainable situation of having a compromised bladder, however, leading to an equally unsustainable infection of the organism.
The general purpose of the present invention is to obviate the aforementioned drawbacks and, in particular, to provide a bladder endoprosthesis aimed at facilitating and making reliable the anastomosis operation between ureters, urethra and relative portions of endoprostheses suitable for receiving them.
According to the invention, this object is essentially achieved by providing a fixing body in correspondence with the portions of endoprostheses which connect ureters and urethra to the artificial bladder itself.
Further advantageous technical characteristics of the present invention are set out in the dependent claims, which are to be considered an integral part of the present description.

DESCRIZIONE DETTAGLIATA DETAILED DESCRIPTION
Both this description and these drawings are to be considered for illustrative purposes only and therefore not for limitation purposes; therefore, the present invention can be implemented according to other and different embodiments; furthermore, it must be borne in mind that these figures are schematic and simplified.
With reference to Figure 1, it is possible to visualize a bladder endoprosthesis according to the present invention comprising a containment portion comprising, in turn, a surface 5 defining a volume suitable for containing organic liquids (in particular urine) and having a plurality holes 45a, 45b and 45c for the inflow and outflow of said organic liquids. Preferably, the surface 5 is made of soft silicone and is coated with a pyrolytic turbostratic carbon layer; in this way an excellent biocompatibility is guaranteed and at the same time an excellent resistance to the corrosive action caused by the organic liquids contained, in use, in the aforementioned volume. A prosthesis according to the present invention further comprises connecting means connected to the surface of the containment portion and adapted to receive respectively a portion of a first ureter, a portion of a second ureter and a portion of urethra. Furthermore, the aforementioned connecting means are coupled in use with a suture 60: in this way it is possible to fix the ureters and the urethra to the implanted bladder endoprosthesis.
According to the present invention, the aforementioned connecting means comprise - a containment collar 30 which emerges from the surface 5 at each hole of said plurality of holes 45a, 45b and 45c.
- an insertion element 20a, 20b and 20c, also emerging from the surface 5 in correspondence with each hole of said plurality of holes 45a, 45b and 45c.
It is worth specifying that the configuration of use of a bladder prosthesis according to the present invention will provide that:
- un primo uretere verrà collegato all’endoprotesi mediante il primo foro 45a e accoppiandolo all’elemento di inserimento 20a e il relativo collare 30; - a first ureter will be connected to the endoprosthesis by means of the first hole 45a and coupling it to the insertion element 20a and the relative collar 30;
- un secondo uretere verrà collegato all’endoprotesi mediante il secondo foro 45b accoppiandolo all’elemento di inserimento 20a e il relativo collare 30; - a second ureter will be connected to the endoprosthesis through the second hole 45b, coupling it to the insertion element 20a and the relative collar 30;
- l’uretra verrà collegata all’endoprotesi mediante il terzo foro 45c e accoppiandolo all’elemento di inserimento 20c e il relativo collare 30. - the urethra will be connected to the endoprosthesis by means of the third hole 45c and coupling it to the insertion element 20c and the relative collar 30.
The aforementioned connecting means, defined at each hole, will have some structural differences based on whether they receive a portion of the ureter or a portion of the urethra, as will become clearer from the following description.
Essi comprendono preferibilmente un collare 30 di contenimento che presenta inviluppo sostanzialmente tubolare e si estende da Figure 2 refers to the detail of an endoprosthesis according to the present invention, in particular to the connecting means suitable for receiving a portion of ureter defined in correspondence with a hole 45a. They preferably comprise a containment collar 30 which has a substantially tubular envelope and extends from
- un primo estremo coincidente con una base del collare stesso collegata a una porzione 6 della superficie 5 dell’endoprotesi vescicale - a first end coinciding with a base of the collar itself connected to a portion 6 of the surface 5 of the bladder endoprosthesis
a to
- un secondo estremo coincidente con un’apertura di detto collare 30 - a second end coinciding with an opening of said collar 30
Preferably, the collar 30 is made of soft silicone coated with a pyrolytic turbostratic carbon layer as well as the surface 5 itself.
L'elemento di inserimento 20a comprende una superficie a inviluppo sostanzialmente tubolare e si estende da The insertion member 20a comprises a substantially tubular envelope surface and extends from
- un primo estremo coincidente con una base collegata alla porzione 6 della superficie 5 - a first end coinciding with a base connected to the portion 6 of the surface 5
a to
- a second extreme coinciding with an opening of the collar 20.
Preferably, the height up to which the element 20 extends is greater than that of the collar 30, in fact the element 20 is inserted inside the ureter to be connected; It is therefore good to have a clear visual reference: the greater length therefore allows easier execution of the operation of coupling the ureters to the bladder endoprosthesis.
The portion 6 actually constitutes a flat portion of the surface 5 on which the collar 30 and the insertion element 20 are defined. The surface 5 will be configured so that the endoprosthesis preferably has a spherical shape. On such a surface it can be problematic; the substantially flat portion 6 facilitates this connection operation, as well as making the operation with which the holes 45a, 45b and 45c are made more precise and simple.
It should be noted that in correspondence with the base defined by the collar 30 there are a plurality of holes 35; these holes 35 guarantee the complete outflow of the organic liquids which from the ureter end up in the containment volume of the endoprosthesis. According to the present invention, the ureter is flush with the surface 5, that is, it does not occupy the containment volume of the bladder endoprosthesis. This configuration, which is particularly advantageous to avoid the propagation of bacteria (especially in the hospitalization phases following the intervention) could potentially create a stagnation of organic liquids in the base of the collar 30. The holes 30 prevent precisely this potential risk, guaranteeing the outflow of the liquids within the containment volume.
The element 20a (naturally hollow) is inserted inside the ureter cavity so that the organic liquids are conveyed inside the containment volume of the bladder endoprosthesis through the hole 45a. In addition, it also acts as a support for the ureter itself and guarantees accessibility to the upper urinary tract: in case of need for an intervention aimed at accessing the kidneys through the ureters (for example if it is necessary to remove kidney stones) , access to the former is not compromised in this way by tissue growth which could tend to reduce the diameter of the ureteral cavities.
Again with reference to figure 2, it is possible to note that in correspondence with the surface that externally delimits the insertion element 20a there is at least one slit 25 able to guarantee the complete flow of organic liquids inside the containment volume. Preferably, the element 20a comprises four slits 25 defined in an unequally spaced way on the surface itself: in this way an optimal flow is guaranteed and anomalous potentially dangerous stagnations are avoided.
The same considerations can be made for the element 20b and the collar 30 defined in correspondence with the hole 45b: in fact they receive the second ureter of the organism in which the endoprosthesis according to the present invention is implanted.
Figure 3 refers to the detail of an endoprosthesis according to the present invention, in particular to the connection means suitable for receiving a portion of the urethra defined in correspondence with a hole 45c. In this case the element 20c, defined concentric with respect to the collar 30, has no slit, just as no hole is provided on the base of the body 30. It is in fact necessary for the liquids inside the containment volume to be conveyed, through the hole 45c, to the inside of the urethra without dispersion.
Figures 4 and 5 allow to describe the dimensional characteristics of the connecting means of a bladder endoprosthesis according to the present invention.
The diameter d1 of said collar 30 is between 1 cm and 3 cm, preferably between 1.8 cm and 2.5 cm.
