È noto che i pazienti emiplegici hanno molta difficoltà nel riprendere a camminare. Una delle modalità di tentativo di recupero funzionale di pazienti con simili esiti è rappresentata da una serie di esercizi finalizzati ad allenare il paziente a spostare quanto più rapidamente possibile il carico dall’arto sano a quello plegico. Gli esercizi consistono in movimenti guidati dal fisioterapista che obbligano il paziente a cambiare la direzione del movimento in modo casuale, senza preavvisare il paziente. Il recupero della capacità di spostare il carico da un arto all’altro è prerequisito fondamentale nel tentare di recuperare una locomozione sicura che permetta al paziente di potersi nuovamente spostare in modo autonomo, considerando che il pieno recupero dell’andatura fisiologica è praticamente impossibile a causa dei danni irreversibili del sistema nervoso. CHT ha recentemente progettato e realizzato un dispositivo meccatronico in grado di sostituire il fisioterapista nella somministrazione dell’esercizio riabilitativo. Questa apparecchiatura può guidare il paziente a esercitarsi con continuità sia in una struttura clinica che a domicilio, ed è dotata di un portale per sostenere il paziente in posizione eretta in modo da impedirgli di cadere. Rivendicazioni 1. Nuovo sistema meccatronico per riabilitazione del cammino di pazienti emiplegici composto da una piattaforma a forma di C di larghezza fissa o variabile grazie al collegamento della parte centrale anteriore con le sponde laterali tramite due quadrilateri articolati collegati da due ruote dentate cui è affidato il compito di invertirne il moto, in modo da passare dalle porte, su cui è montato in posizione centrale un manubrio di altezza regolabile, mentre sulle ali laterali sono posti due strutture ad arco cui viene collegata un’imbragatura regolabile che, afferrando il paziente sotto le ascelle o alla vita, o in ambedue modi, garantisce l’impossibilità di caduta, essendo il sistema in grado anche di alzare tramite uno o due argani il paziente dalla posizione seduta a quella eretta, sempre utilizzando l’imbragatura sotto-ascellare, mentre sotto alla struttura a C sono sistemati tre o quattro gruppi di ruote multidirezionali in grado di adattarsi anche ad eventuali irregolarità del pavimento su cui si deve muovere, essendo munite di sospensioni (nel caso le ruote siano quattro), o con l’aggiunta di due ruote pivotanti che non toccano terra ai lati della ruota centrale anteriore, o anche semplicemente due perni in modo da impedire l’eventuale ribaltamento del sistema, permettendo il sistema di muoversi in qualsiasi direzione, sia seguendo un preciso programma terapeutico stabilito dal riabilitatore, che muoversi su comando del paziente utilizzando una tastierina o un piccolo joystick posizionabile su uno dei due manici del manubrio, consentendo al paziente il pieno controllo del moto, inclusa la possibilità di girare a destra o sinistra od anche di girare su se stesso mentre, nel caso di terapia programmata, uno schermo touch permette sia di inserire lo stesso programma, sia di fornire al paziente indicazioni sia visuali che vocali su quanto dovrà eseguire, essendo ovviamente il tutto alimentato da una batteria ricaricabile, e controllato da un sistema elettronico che può anche essere collegato sia in wifi che in Bluetooth con altri sistemi presenti in casa, essendo anche il sistema costretto a fermarsi automaticamente se il paziente non tiene premuto uno dei bottoni di controllo, per incrementare il livello di sicurezza del sistema. Claims 1. New mechatronic system for rehabilitation of the gait of hemiplegic patients consisting of a C-shaped platform of fixed or variable width thanks to the connection of the front central part with the side rails through two articulated quadrilaterals connected by two toothed wheels to which the task is entrusted to reverse the motion, so as to pass through the doors, on which a handlebar of adjustable height is mounted in a central position, while on the side wings there are two arched structures to which an adjustable harness is connected which, grabbing the patient under the armpits or at the waist, or in both ways, guarantees the impossibility of falling, as the system is also able to lift the patient from sitting to standing position by means of one or two winches, always using the underarm sling, while under the C-shaped structure there are three or four groups of multidirectional wheels able to adapt to any irregularities in the pa vehicle on which it must move, being equipped with suspensions (if there are four wheels), or with the addition of two pivoting wheels that do not touch the ground on the sides of the front central wheel, or simply two pins so as to prevent the '' possible overturning of the system, allowing the system to move in any direction, either following a precise therapeutic program established by the rehabilitator, or moving at the command of the patient using a keyboard or a small joystick that can be positioned on one of the two handlebar handles, allowing the patient full control of motion, including the ability to turn right or left or even turn on itself while, in the case of programmed therapy, a touch screen allows both to enter the same program and to provide the patient with both visual and voice on what to do, obviously being all powered by a rechargeable battery, and controlled by an electronic system c he can also be connected both in wifi and in Bluetooth with other systems in the house, as the system is also forced to stop automatically if the patient does not hold down one of the control buttons, to increase the system's security level.
