The present invention relates to a gastro-reactive device comprising a component capable of expanding gas by expanding with gastric juice and a component comprising the active ingredient. The invention thus relates to a device for delaying the passage of orally administered drugs to the gastric gate (pylorus) which, when in contact with the gastric juice, comprises an expanding component and a fluid permeable to gas, but a gas-sealing polymeric cover. The device may contain an active ingredient, which is predominantly delivered to the gastric fluid by the drug formulation. In contrast to conventional formulations that delay pylori passage, release of the active ingredient is less dependent on the nature and condition of the polymer casing and is largely dependent on the embedded form. The device according to the invention can be readily folded or folded and filled into capsules.
Egy, folyadékkal való érintkezésbe lépéskor aktivált gázelőállító szer a hatóanyagnak a falban lévő kilépőnyíláson keresztül történő kijutását biztosítja.FR 2323440 is a device for delivering active ingredients to a fluid-containing environment by osmosis, which is provided with a wall made of a material impermeable to the active ingredient. A gas-producing agent activated upon contact with a liquid provides an exit of the active ingredient through the outlet in the wall.
Patent No. 4,207,890 discloses a device for controlled release of active ingredients that is subjected to local retention by expansion in the stomach, thereby prolonging the residence time in the stomach. The device is provided with a polymer cover (a) which is collapsed prior to use. The polymer casing itself does not have openings and consists of a material that is practically non-hydrating, but permeable to body fluids. The device further comprises an element (b) that controls the release of the active ingredient. This element, according to claim 2, may be the polymer casing itself. The device further comprises (c) an expanding component in contact with body fluids.
The retention (retention) effect is based on osmosis, where one element (c) represents an osmotic active agent which drives water through the polymer casing (a) and thus assumes the formation of a solution in the device. The device thus contains an aqueous solution in an expanded state and allows the active substance to diffuse from the device due to the concentration conditions, but corresponding to the apparent density in the chimus. Thus, the device floats or sinks in the chimus. Only the size of the device acts as a retention factor. No possible deflation mechanism is disclosed and thus the removal of the device from the stomach and its disposal after release of the drug is not clarified.
A retentio elve a chimuson való felúszás (lásd az US 4 996 058 3. oszlopának 6. A hatóanyag-leadás vezérlése mindkét esetben lényegében a polimer burkolaton keresztül történik. A polimer burkolat diffúziós állandója döntő paramétert képvisel. A modification of the retention principle is described in EP 0 307 904 A1 and US PS 4 996 058. Substances capable of gaseous components, such as carbon dioxide or nitrogen, which add polymeric coating (a), are added as an expandable component. they swell and form a chimuson floating device. The principle of retention is suppression of chimuson (see columns 6 and 7 of column 3 of US 4 996 058), thereby preventing the device from leaving the stomach through the pylori. These devices have significant drawbacks. In both cases, the release of the drug is controlled essentially through the polymer casing. The inflation kinetics of the device are also substantially limited by the polymer casing, since water and hydronium ions have to diffuse through the polymer casing and, on the other hand, the drug molecules have to diffuse in the opposite direction. The diffusion constant of the polymer casing represents a crucial parameter. In the case of damage to the relatively thin and thus sensitive polymeric coatings by mechanical or chemical action, there is a risk of leakage of the drug in the form of dose-dumping. The amount of expanding gas produced depends, inter alia, on the acidity of the environment and thus on the composition of chimus and the secretion (secretion) of gastric acid. In case of strongly acidic environment and thus high gas generation, the use of the device should not reach the break limit and, on the other hand, in case of less acidic environment, sufficient gas generation should be ensured. This dilemma leads to strongly changing states of devices due to circadian changes in gastric acidity.
A találmány révén megoldandó feladat, hogy olyan gasztroretentív eszközt hozzunk létre, amely a technika állásából ismert eszközök hátrányait kiküszöböli.It is an object of the present invention to provide a gastro-responsive device which overcomes the drawbacks of the prior art devices.
To solve the problems described above, the present invention provides a gastro-responsive device comprising the following elements:
Annak érdekében, hogy a pórusok adott esetben jobban nedvesíthetek legyenek, a membránok a találmány szerint nedvesítőszerekkel vagy egyéb hidrofil komponensekkel lehetnek átitatva.a) A polymeric casing consisting of a microporous membrane or a partial microporous membrane or a combination of these and another non-porous film-like polymer, wherein the production of pores according to the invention may be carried out using arbitrary methods such as stretching films, applying multiphase systems and evaporating part of the system, targeted by polymerization, for example in the form of ionomers, by mechanical processes such as needles, thermal processes such as lasers, irradiation with ionized rays and subsequent milling, etc. For the microporous membrane according to the invention, the following materials can be used: polyurethanes, polypropylene, poly (vinyl alcohol), polyvinyl acetate, polyacrylic acid and derivatives, polymethyl methacrylic acid and derivatives, polycarbonates, polyvinyl idendifluoride, polytetrafluoroethylene and any further polymers with pores of suitable size. The size of these pores according to the invention is in the range of 0.1 to 20 µm, preferably 0.3 to 10 µm, particularly preferably 0.5 to 1 µm. In order for the pores to be more wettable, the membranes may be impregnated with wetting agents or other hydrophilic components according to the invention.
b) Expandálásra képes komponenst, amely gyomornedvvel történő érintkezés során, különösen savb) Expandable component for contact with gastric juice, especially acid
Suitable materials for this purpose include, for example, alkali and earth alkali metals, carbonates and carbonates of ammonium cations, or sodium acid, or mixtures thereof. Optionally, these expandable components may be modified, for example, by coating with lipophilic components or incorporating lipophilic components such as waxes or fats or suitable coatings such as polymethacrylates or polymethyl methacrylates and derivatives, or the like, in order to modify the gas production kinetics. using materials known to those skilled in the art. According to the invention, the expandable component may be present in the form of a foam forming agent, for example polyvinyl alcohol, or in the form of a semi-solid ointment composition to allow control of the deflation kinetics.
An active ingredient component such as multiparticulates, tablets, capsules or semi-solid ointment formulations or foams. As an active ingredient, basically all physiological agents can be used, especially medicaments for human and veterinary use, preferably those which are absorbed by the gastric mucosa or which act on the surface of the gastric mucosa. Such pharmaceutical agents are known to those skilled in the art. The components listed under b) and c) may also be present in a common formulation.
The device may be filled into a container of physiologically acceptable material, such as a hard gelatin capsule, to facilitate application and treatment.
Preferably, the inflated shape of the device is a bulging triangular shape to obtain a planar, maximally bulky structure. Thanks to the planes, the chimuson floating state has two distinguished positions that allow the chimus to pass by the gastro-refractory device so that the chimus cannot push through the pylor on the puffed shape. The triangular shape represents a compromise between the flatness and rigidity of the structure when it is inflated from the planar shapes and can therefore be advantageously recommended for forming the retention device. The bulky structure in the stomach encapsulates between the stomach walls and allows the chimus to escape through the pylori, and the elimination in the inner world can only withstand the flow resistance in chimus. The closer the shape of the device is to the spherical shape, the more it is possible to seal the stomach outlet with the consequence that the evacuation pressure will push the device through the pylor. According to the invention, four or polygonal structures as well as rounded corners can be used.
According to the invention, several problems are solved by the use of a microporous or porous polymer coating. Due to the surface tension of the liquid, especially aqueous systems, the discharge of the fluid-filled pore requires a specific pressure, which is essentially dependent on the pore diameter, surface tension and interface tension and the contact angle. By selecting a suitable pore diameter, a specific pressure can be set for a particular system, under which the pores cannot be emptied, so that the membrane becomes gas-tight. However, liquids can easily pass through the membrane, the flow being determined by the number of pores, the size of the pores, and the viscosity of the medium. By using microporous membranes having a certain pore size according to the invention, an impermeable device may be imparted to a gas which is permeable to liquids under a specified pressure. Thus, the internal pressure of the device is limited to a certain value. If the limit pressure is exceeded, the membrane becomes permeable and as a non-destructive protection prevents the device from being subjected to a stress that exceeds the permissible break pressure. Thus, by means of the device according to the invention, it is possible to provide an almost constant internal pressure during the life of the device. The duration of application of the device can be controlled by the type and amount of component capable of expanding. By capillarity, the particles on the inside of the membrane get wet and release the active ingredient through the microporous membrane with little delay.
