Abstract
Description
Συνθετική διαδικασία σκευάσματος συμπληρώματος διατροφής για την προληπτική θεραπεία των ανήσυχων κάτω άκρων Synthetic process of formulating a dietary supplement for the preventive treatment of restless legs
(3) Restless lower limb syndrome is a problem that afflicts many people. It is an unusual sensation (paresthesia) in the legs that typically occurs at bedtime and is a common cause of insomnia. The sensation has been described as uncomfortable, not painful, but more like a "creepy reptilian tingling", which is temporarily relieved (relieved) by moving the legs. (1) Most people with restless legs syndrome also move their legs when they sleep. These movements can further disrupt their sleep and add to the daytime sleepiness the syndrome causes. In some people, it can be so extreme that it becomes excruciating. Restless legs syndrome is surprisingly common. Various estimates put it at 2% of the adult population, with the actual number possibly around 6% (1). It is more common in women (2). It is rare in young children, and for those over 65 it ranges from about 10% to 28%. The cause of restless legs syndrome is not known. Increased syndrome has also been associated with magnesium and filic acid deficiency. Magnesium therapy (12.4 mmol/day = 301 mg/day) has been shown to be beneficial. (3)
A number of lifestyle factors have been associated with restless legs syndrome. These include smoking, advanced age, obesity, hypertension, loud snoring, use of antidepressants, diabetes and lack of exercise (4). Conventional medical treatment for the disease is drugs for the nervous system. Some of these drugs are quite strong and dangerous, and should be reserved for more serious cases. On the herbal side, there are plants for the nervous system, which also help improve sleep quality, and help with blood circulation.
Circulatory: Many studies found that restless legs syndrome was very common in people with varicose veins (22% of cases) (5). After treatment for superficial varicose veins (sclerotherapy or vein stripping) 98% reported an immediate improvement in their symptoms. This was only the treatment for the superficial veins, while the deeper veins carry most of the burden of returning blood from the extremities. When the blood does not circulate properly, the walls of the deeper veins can stretch, resulting in an unpleasant feeling in the legs. The sluggish circulation can cause red blood cell aggregation which can further add to paresthesia and restless legs syndrome (6). Varicose veins are a chronic condition characterized by the appearance of dilated veins due to a degeneration in the vascular wall. The most common location of varicose veins is in the legs, where venous drainage is provided by two systems: one deep (tributary of the inferior vena cava) and one superficial (tributary to the two saphenous veins-and further into the deep network), which are joined by communicating vessels (which, just like the blood vessels of the superficial network have non-return valves). The disease is very common, affecting 30-50% of adults and a further 60% of women (7-8-9).

RUSCUS ACULEATUS (R.A). RUSCUS ACULEATUS (R.A). R.A is a small evergreen shrub native to Western Europe (10-11). The extract is often used with other ingredients, especially hesperidin methylchalcone, and ascorbic acid. The effects of R.A are venous tone, reduction of edema associated with venous stasis and inflammation. Venous stenosis is explained by the peripheral postsynaptic action of alpha-nonadrenergic (12). Another activity that of microcirculation is also involved. This combines an inhibitory effect on capillary permeability, a protective effect on the endothelium against hypoxia through stimulation of mitochondrial enzymes, and an inhibition of endothelial leukocyte adhesion observed in ischemia/reperfusion models. Oral administration of the R.A extract combination inhibited histamine-induced plasma secretion in mildly diabetic hamsters without affecting blood glucose levels (22).
Εντός 2,5 ωρών εφαρμογής ποσότητας κρέμας RA που περιέχει 64 έως 96 mg εκχυλίσματος η διάμετρος της μηριαίας φλέβας μειώθηκε κατά μέσο όρο 1.25 mm, σε σύγκριση με την αύξηση της διαμέτρου κατά 0,5 mm για την κρέμα εικονικού φαρμάκου (19) Venous activity: In an experimental microcirculatory model, R.A extract causes constriction in venules, but not in arterioles, after intravenous administration (13). The inhibitory effect of the extract R.A. in histamine-induced microvascular permeability was attributed to calcium blockade and a selective activation of alpha 1-adrenergic receptors (and to a lesser extent alpha 2-receptors) (14-15). R.A extract enhanced the action of norepinephrine (noradrenaline) released in nerve endings (16). Vasoconstriction is independent of the endothelium and, as mentioned above, is achieved by the activation of smooth muscle adrenergic receptors. This adrenergic action of R.A cream enhances venous return. (17-18). A venoconstrictive activity of R.A cream under orthostatic condition, demonstrated by ultrasonography in a randomized blind study involving 18 healthy volunteers. Within 2.5 hours of application of an amount of RA cream containing 64 to 96 mg of extract the diameter of the femoral vein decreased by an average of 1.25 mm, compared to an increase in diameter of 0.5 mm for the placebo cream (19).
HESPERIDIN METHYLCHALCON decreased capillary filtration rate (p,0.01) and increased blood volume (flow), due to dehydration of lower limb tissue. Blood volume after using the combination was between the values for R.A and hesperidin methylchalconate alone, and the effects on other parameters corresponded to those given the individual substances (20). In addition to experimentally induced reduction in capillary permeability a combination of R.A extract, methylesculetin methylchalconate hesperidin and ascorbic acid reduced experimentally induced hyperaemia, infrared erythema and dextran or carrageenan in local edema after either oral administration or intraperitoneal injection (21). In topical and oral use during pregnancy a significant reduction in femoral vein dilatation (P, o. o5) was observed in an uncontrolled study in 18 women evaluating the use of R.A cream (22). Similar beneficial effects were observed in a previous study (23). A therapeutic study involving 20 pregnant women (21 to 24 weeks) found that oral use of a combination of RA extract improved the symptoms of venous insufficiency and was harmless to the fetus as assessed by routine clinical and ultrasound surveillance criteria. pregnancy, including umbilical artery flow and the condition of the infant and placenta after delivery (24). In a much larger earlier study, the efficacy and tolerability of the R.A extract combination was evaluated in an uncontrolled multicenter clinical trial (25). The product was administered for 2 months to 214 women who experienced chronic venous insufficiency as a result of their pregnancy. Symptoms and signs were significantly improved compared to baseline at 30 and 60 days of treatment. Tolerability was rated as excellent, with only nine patients reporting mild gastrointestinal or skin reactions.
Hemorrhoids: The efficacy of a combination extract of R.A for the treatment of acute attack of hemorrhoids was investigated in an open multicenter study (26). One hundred twenty-four patients were treated for 1 week. The protocol was six capsules of the combination daily for three days, then four capsules per day. Each capsule contained 150 mg of R.A extract, 150 mg of hesperidin methylchalconate and 100 mg of ascorbic acid. Parameters studied were painful syndromes, accompanying symptoms, local signs, overall symptom severity score and clinical efficacy and safety. Analysis of the results showed a statistically significant decrease in all scores between day 0 and day 7 (26).
AESCULUS HIPOCASTANUM (Α.Η). AESCULUS HIPOCASTANUM (A.H). The seed extract, standardized to about 20% aescinis, is used for its phlebotonic effect, vascular protection, anti-inflammatory and free radical scavenging properties (27-28). Primary indication is chronic venous insufficiency (29). The extract is also rich in a number of flavonoids, such as quercetin and kaempferol derivatives. Flavonoids also have protective effects on blood vessels, and are well-known, powerful antioxidants (30). Safe, well tolerated, and accepted by patients, the fresh plant extract offers a real alternative in the treatment of patients with mild to moderate venous insufficiency (31). Finally, there are studies on AH that show it increases the body's antioxidant defenses and prevents high-fat diet-induced lipid peroxidation in male mice (32).
