Abstract
Description
ΦΑΡΜΑΚΕΥΤΙΚΟ ΣΚΕΥΑΣΜΑ ΠΟΥ ΠΕΡΙΛΑΜΒΑΝΕΙ ΕΝΑ ΠΕΠΤΙΔΙΚΟ ΦΑΡΜΑΚΟ ΚΑΙ ΜΕΘΟΔΟΣ ΓΙΑ ΤΗΝ ΠΑΡΑΣΚΕΥΗ ΑΥΤΟΥ PHARMACEUTICAL FORMULATION COMPRISING A PEPTIDE DRUG AND METHOD FOR PREPARATION THEREOF
ΤΕΧΝΙΚΟ ΠΕΔΙΟ ΤΗΣ ΕΦΕΥΡΕΣΗΣ TECHNICAL FIELD OF THE INVENTION
The present invention concerns a suspension for intramuscular injection, which contains a therapeutically effective amount of a peptide drug, such as octreotide or a pharmaceutically acceptable salt thereof, as well as the method of its preparation.
ΥΠΟΒΑΘΡΟ ΤΗΣ ΕΦΕΥΡΕΣΗΣ BACKGROUND OF THE INVENTION
Peptide drugs are usually administered systemically, i.e. parenterally, due to their low absorption when administered orally and their high instability in gastric fluids. However, parenteral administration can be laborious and cause discomfort, especially in case of multiple daily doses. To reduce the number of injections in a patient, the medicinal substance is progressively administered as a long-acting formulation (depot). The parenteral administration of peptide drugs, such as the long-lasting formulation in biodegradable polymer, e.g. microspheres or implants, is considered to provide the possibility of gradual release after a residence time within the polymer that protects the peptide from the enzymatic and hydrolytic effect of biological media.
Octreotide binds to somatostatin receptors. These receptors are coupled via G proteins sensitive to pertussis toxin (PTX), which contribute to the inhibition of adenyl cyclase. Octreotide binding to these receptors also stimulates phosphotyrosine phosphatase and activates the Na(+)/H(+) exchanger through pertussis toxin-resistant G proteins.
Octreotide, which is a form of somatostatin, is a peptide medicine consisting of eight amino acids. Octreotide has a stronger affinity for somatostatin receptors than native somatostatin and therefore inhibits the release of growth hormone, glucagon and insulin more effectively than somatostatin. In addition, octreotide suppresses the release of luteinizing hormone (LH) via gonadotropin-releasing hormone, reduces visceral blood flow, and inhibits the release of serotonin, gastrin, vasomotor intestinal peptide, secretin, motilin, and related substances. Based on these pharmacologic actions, octreotide has been used in the treatment of large patients with functional gastrointestinal/pancreatic endocrine tumors and advanced midgut neuroendocrine tumors, as well as in the treatment of thyrotropin (TSH)-secreting pituitary adenomas.
The chemical name of Octreotide is [(4R,7S, 10S, 13R, 16S, 19R)- 10-(4-aminobutyl)-19-[[(2R)-2-amino-3-phenyl-propanol]amino]- 16-benzyl-N-[(2R,3R)-1,3-dihydroxybutan-2-yl]-7-(1-hydroxyethyl)-13-(1H-indol-3-ylmethyl)-6,9,12, 15,18-pentatoxo-1,2-dithia-5,8,11,14,17-pentacycloeicosane-4-aminocarbonyl and its molecular formula is C49H66N10O10S2, which corresponds to a molecular weight of 1,019.24.
The patent EP-B-1 151746 concerns the pharmaceutical preparation of microcapsules of lactic co-glycolic copolymer that includes a peptide of pharmaceutical interest, which is characterized by the fact that the copolymer that forms the microcapsules includes a citric acid ester as an additive.
US-B-6521599 relates to a stable pharmaceutical composition for intravenous or intramuscular injection of Octreotide, which is characterized by the fact that the medium used for the infusion of the peptide or pharmaceutically acceptable salts thereof comprises glycine in concentrations from 10 to 60 mM as well as an aqueous solution of hydrochloric acid in sufficient quantity to adjust the pH of the formulation to values ranging from 3.0 to 4.2.
Although all the aforementioned patents are attempts to prepare long-acting injectable formulations that will provide satisfactory levels of release, there is still a need to prepare parenteral formulations to avoid any fluctuations in plasma levels and unwanted pharmacological side effects.
ΠΕΡΙΛΗΨΗ ΤΗΣ ΕΦΕΥΡΕΣΗΣ SUMMARY OF THE INVENTION
The present invention aims to create a sustained gradual release pharmaceutical formulation which will contain a peptide drug such as Octreotide or a pharmaceutically acceptable salt thereof for intramuscular administration.
The present invention therefore aims at the creation of a formulation, which on the one hand will have the physical and chemical characteristics of the reference product and on the other hand will overcome the disadvantages associated with formulations of the previous technological generation.
Another objective of the present invention is the preparation of Octreotide acetate microparticles produced from Octreotide acetate which is delivered through a biodegradable poly(lactic-co-glycolide) star polymer in order to achieve a gradual release of the active substance of the drug.
A further object of the present invention is the preparation of an injectable suspension of Octreotide or a pharmaceutically acceptable salt thereof for parenteral administration, which effectively addresses the problems associated with fluctuations in plasma levels such as extremely high levels or near-zero levels by the entire release period.
According to the above objectives of the present invention, microparticles are provided which contain a peptide drug such as Octreotide or a pharmaceutically acceptable salt thereof as well as an active amount of a pharmaceutically acceptable biodegradable polymer such as the PLGA star polymer in order to achieve a gradual release of the active substance.
Βάσει μιας άλλης προσέγγισης της παρούσας εφεύρεσης παρέχεται μέθοδος για την παρασκευή PLGA μικροσωματιδίων βαθμιαίας αποδέσμευσης, τα οποία περιέχουν ένα πεπτιδικό φάρμακο και συγκεκριμένα οξική Οκτρεοτίδη, η οποία μέθοδος περιλαμβάνει τα κάτωθι στάδια: According to another approach of the present invention, there is provided a method for the preparation of PLGA microparticles of gradual release, which contain a peptide drug, namely Octreotide acetate, which method includes the following steps:
- Dissolve the active substance in methanol by stirring constantly.
- Preparation of the polymer solution by dissolving the PLGA star polymer in methylene chloride (DCM) with constant stirring.
- Addition of the solution of the active substance formed in the polymer solution in methylene chloride.
- Emulsification of the resulting mixture in an aqueous phase containing polyvinyl alcohol (PVA).
-The resulting microemulsion is then subjected to solvent removal through extraction and evaporation in order to create solid microparticles.
Through the following detailed description, other advantages and objectives of the present invention will become apparent to those skilled in the art.
ΑΝΑΛΥΤΙΚΗ ΠΕΡΙΓΡΑΦΗ ΤΗΣ ΕΦΕΥΡΕΣΗΣ DETAILED DESCRIPTION OF THE INVENTION
Peptide drugs such as Octreotide often have low blood bioavailability when parenterally or orally administered due to their short biological half-life due to their metabolic instability. Therefore, it is difficult to achieve a therapeutically sufficient blood concentration over a prolonged period of time.
Adherence to treatment is an important factor in clinical outcomes involving patients in a wide range of clinical settings. Adherence is particularly important in the case of serious diseases in which patients undergo multi-month or multi-year treatments and premature discontinuation of treatment may have significant effects on the patient's health and quality of life.
Regardless of the specific reason for non-adherence to treatment, the inability of patients to continue taking medications as prescribed leads to high rates of relapse, hospitalization, and in some cases a high risk of death.
For a wide range of disease states, recent advances in drug delivery technologies have led to the development of innovative delivery systems that aim to improve therapeutic outcomes. One possible solution to the problem of poor medication adherence is the development of new, long-acting drug delivery systems that release the active substance over days or weeks through a single dose. Long-acting injectable technologies may outperform conventional drugs by improving safety and efficacy through prolonged duration of action and reducing compliance issues as well as side effects. By enabling patients to take medication less frequently, these technologies are creating drugs that may be particularly beneficial for treating serious diseases in which medication adherence is closely associated with improved outcomes.
The present invention proposes a sustained release drug delivery system for parenteral delivery of peptide drugs, such as Octreotide, in a bioavailable polymer, such as microparticles, which enable a sustained release of the active substance after a residence time within the polymer that protects the peptide from the enzymatic and hydrolytic effect of biological media.
According to the present invention, Octreotide acetate microparticles are produced from Octreotide acetate, which is delivered through a biodegradable poly(lactic co-glycolide) glucose star polymer.
Special measures must be taken to achieve sustained peptide release to maintain the therapeutically active plasma drug concentration level, avoiding very high plasma drug concentrations that cause unwanted side effects. The release model of the peptide drug depends on a multitude of factors, such as the type of peptide and its presence in a free or some other form, e.g. in salt form. Another important factor is the choice of polymer and its inherent characteristics.
Suitable glucose star polymers used in the preparation of microparticles of the present invention have particular intrinsic characteristics such as lactide-glycolide ratio ranging between 45:55 and 50:50, weight average molecular weight (Mw) ranging between 45,000-65,000 Da, glucose content 0.2%, intrinsic viscosity 0.5-0.6 dL/g in CHCl 3 .

Organic solvents of PLGA that can be used in the present invention include, but are not limited to, ethyl acetate, tetrahydrofuran, ethanenitrile, dichloromethane, chloroform, and acetone.
In the present invention the continuous phase consists of an aqueous solution with a surfactant, ideally polyvinyl alcohol (PVA). Octreotide acetate microparticle sterile bulk powder is a free-flowing white to off-white powder. The medicinal substance in the form of Octreotide acetate is uniformly distributed within the microparticles which are composed of a star polymer of glucose, poly[D,E-lactide-co-glycolide]. Sterile mannitol is added to the microparticles to improve suspension formation.
The resulting powder is filled into vials. The powder in the vials is reconstituted with a diluent to create an octreotide acetate injectable suspension. A milky suspension is formed and the contents of the vial are transferred to the syringe for administration to the patient via a needle-free access device.
