Abstract

A pharmaceutical preparation useful in alleviating the symptoms of arthritis comprises in combination a fish oil, preferably cod liver oil, and pectin. Additionally, the preparation can include a plant gum and/or Vitamin C, and can be formulated as either an emulsion or as a capsule, or incorporated into a foodstuff such as a yoghurt or salad cream.

Description

PREPARATIONS FOR ALLEVIATING ARTHRITIS The present invention relates to pharmaceutical preparations useful in alleviating the symptoms of arthritis.
Fish oils, notably cod liver oil, are well known as health promoting substances, particularly because of the presence therein of Vitamin D. Other known constituents of such oils include fatty acid triglycerides and iodine-containing oils which are themselves also known to be physiologically useful.
For alleviating the symptoms of arthritis, cod liver oil has been tried and in a study carried out by Dr.
R. Pemberton reported in Archives of Internal Medicine, 1920 positive results were achieved using this oil for the treatment of chronic arthritis.
The main problem, however, of taking cod liver oil for the treatment of chronic arthritis is that the daily dose that is required in order to have any effect for the majority of patients is quite high, e.g. several tablespoonfulls, and the taste of the oil is generally found to be disagreeable. Attempts have been made to disguise the taste of the oil, for example, by adding orange juice, but large amounts are needed because of the strong taste of the oil and this greatly increases the volume of the mixture that must be ingested each day.
There is therefore a need to find a way of reducing the daily dosage of oil needed whilst at the same time, if possible, rendering the oil less unpleasant to take by mouth.
In accordance with the present invention there is provided a pharmaceutical preparation for the alleviation of the symptoms of arthritis comprising in combination a fish oil and pectin.
It has now been realised that because in the duodenum pectin absorbs cholates it will have the effect of hindering uptake by the liver through portal vein resorption of the oil and therefore, according to Dr.
A.C. Fraser (Journal of Physiology Vol. 103, 1944-45), of allowing the oil to be drawn primarily into the lymphatic system and therefore into the joints. With hindsight it can be postulated that it is to the lymphatic system that Dr. T.S.P. Strangeways was alluding in Journal of the American Medical Association, 1920 where he suggested that cartilage was, in some way unknown to him, able to take nourishment through the mechanism of osmosis for its regeneration, for example in order to counteract the symptoms of all types of arthritis.
Dr. Fraser's work was, however, concerned with emulsified oil, i.e. an intimate mixture of oil and water, but since pectin is itself an emulsifying agent it means that a readily absorbable emulsion of fish oil will always be formed to some extent within the stomach and duodenum whenever the oil and pectin are taken together, particularly if an oil/pectin emulsion is ingested.
Thus according to one aspect of the present invention there is provided a pharmaceutical capsule consisting of a fish oil and pectin in combination, for example having a liquid centre comprising a fish oil and a solid outer case comprising pectin admixed with gelatin or the like. Alternatively the liquid centre could consist of a fish oil/pectin dispersion, with the outer case being of gelatin or the like.
In accordance with a second aspect of the present invention there is provided a pharmaceutical emulsion consisting of an intimate mixture of a fish oil, pectin and water.
Surprisingly it has been found that by preparing a fish oil in the form of an aqueous emulsion with pectin, not only can the daily amount of oil taken be reduced because the oil is being "pipelines to where it is needed, but also if sufficient pectin is used the unpleasant taste of the oil is found to disappear.
In addition to pectin other emulsifiers such as plant gums can be added to the oil to modify its taste and/or appearance. Furthermore, Vitamin C can be added, for example in an amount of at least 25 mg per 100 mls of emulsion, since it is known to be of benefit in the treatment of arthritis.
Because of the bland taste of the emulsion of the present invention it can be added to yoghurt or fruit juice or the like or used as the basis of a foodstuff such as a salad cream or salad dressing.
The preferred fish oil is cod liver oil but other oils such as halibut liver oil can be used.
Since the prognoses of the various forms of arthritis are generally the same and are related to the degradation of cartilage in the joints, the present invention is believed to be of use in alleviating the symptoms of all forms of arthritis.
One embodiment of the present invention will now be described, by way of example.
1 g of dry pectin was ground in a mortar with 25 ml of cod liver oil. 25 ml of water was then added and grinding continued until an intimate dispersion of the pectin in the oil/water mixture was obtained.
Emulsification was seen to begin at this point and so the mixture was allowed to stand for about 10 minutes. Further grinding then took place until a thick creamy emulsion was formed. 250 mg cf powdered Vitamin C, 25 ml more of oil and 15 ml more of water were then added with continual mixing in the mortar until a homogeneous emulsion was formed.
A daily dosage of four 15 ml tablespoons of the final mixture was found by arthritis sufferers greatly to ease the discomfort they experienced in their joints.
