Abstract
An acetabular cup moulded or machined from a synthetic plastics material such as high density polyethylene has an external part spherical envelope and defines a substantially part-spherical internal recess for receiving the head of the femoral endoprosthesis. Concentric bone cement receiving grooves 2 are formed in the external surface of the cup and ungrooved circumferential portions of that surface support integral or inserted studs 7, 8, 9 which act as spacers to facilitate, in use, the formation of a layer of bone cement of substantially constant thickness between cup and acetabulum. An integral flange 6 around the circular periphery of the cup serves, in use, to apply pressure to the bone cement to push it into the bone and the grooves 2 of this cup. <IMAGE>
Description
SPECIFICATION
An acetabular cup
The present invention relates to an acetabular cup for use in the production of a prosthesis.
The cup is particularly, although not exclusively, intended for the femoral endoprosthesis of my copending United Kingdom patent application No. 8005254 filed on the 1 5th
February, 1980. Whilst the cup provides the socket the femoral endoprosthesis provides the base of a ball and socket hip joint.
Such acetabular cups are well known and, in their installation, the acetabulum is cleared of any loose bone and cartilage and the cup fixed therein in the correct position by means of bone cement. The optimum layer thickness of bone cement between cup and acetabulum is considered to be 4 to 6 mm over the whole external surface of the cup. In existing cups uniform distribution of cement around the cup is difficult to achieve.
According to the present invention, there is provided an acetabular cup for insertion in the acetabulum in the production of an artificial hip joint, the cup having an external envelope of substantially part-spherical shape and defining a substantially part-spherical internal recess for receiving the head of a femoral endoprosthesis, in which the external surface of the cup is formed with a plurality of grooves, a plurality of spacers extend out from the external envelope of the cup and a flange extends around the circular periphery of the envelope such that when the cup is placed in position in the acetabulum a space of substantially constant thickness is defined between the external envelope of the cup and the external surface of the acetabulum for the insertion of cement, the grooves act to receive cement when inserted to improve keying between the cup and the surrounding bone and the flange acts to retain cement so inserted.
A preferred embodiment of the invention may comprise any one or more of the following advantageous features:~
(a) The cup is made of synthetic plastics material.
(b) The synthetic plastics material is high density polyethylene.
(c) The cup is moulded from synthetic plastics material.
(e) The flange is integral with the remainder of the cup.
(f) The flange is flexible so that in operation it can be used to apply pressure to cement in the acetabulum.
(g) The grooves extend circumferentially around the cup.
(h) The grooves are circular.
(i) The spacers comprise small individual projecting members.
(j) The spacers are fixed in complementary holes formed in the external surface of the cup.
(k) The spacers are integral with the remainder of the cup.
(I) The spacers comprise individual members projecting from an ungrooved circular belt extending circumferentially around the external surface of the cup and the grooves are formed in those parts of the external surface extending to both sides of the circular belt.
In order that the invention may be more clearly understood, two embodiments will now be described, by way of example, with reference to the accompanying drawings, in which:
Figure 1 shows a side elevational view of a first form of acetabular cup,
Figure 2 shows a sectional view of the cup illustrated in Fig. 1, and
Figure 3 shows a plan view of the cup illustrated in Fig. 1.
Referring to Figs. 1-3, the acetabular cup is turned down from a solid block of synthetic plastics material. The preferred material is high density polyethylene (RCH 1000), but other synthetic plastics material may equally well be used. The cup is of substantially hemispherical shape and defines a substantially hemi-spherical recess 1 in the base thereof.
This recess, the centre of which is offset in relation to the centre of the cup as can be seen from Fig. 2, forms the socket into which the ball head of the femoral endoprosthesis described in my copending United Kingdom patent application No. 8005254 filed 1 5th
February, 1980 fixed to form a complete joint. The external surface of the cup is provided with a series of parallel circumferentially extending grooves 2. These grooves 2 are disposed so as to leave two circumferentially extending ungrooved bands 3 and 4. An ungrooved area 5 is also left at the very top of the cup. An integral flange 6 extends circumferentially around the base of the cup. In the bands 3 and 4 and the area 5 a plurality of spacers are formed. These spacers are in stud form and are machined out of the base material in the same manner as the remainder of the cup.The length of the spacers 7, 8 and 9 is such that they form with the free edge of the flange 6 a substantially hemi-spherical envelope 10 which is spaced from the hemispherical envelope 11 of the actual surface of the cup by a distance of between 2-6 mm.
As can be seen from Fig. 3, provision is made in this particular cup for four spacers 7 and for four spacers 8. In order to simplify the drawings, some of the spacers have been omitted in Figs. 1 and 2 and all of them have been omitted in Fig. 3. The stud form of the spacers ensures a uniform distribution of cement and enables a satisfactory spacing to be achieved without any unacceptable division of the cement layer between the acetabulum and the cup. In use the offset recess 1 localises the centre of the axis of rotation of the hip joint enabling leg length equalisation to be achieved more easily as the cup can be placed in a precise position in the acetabulum.
In installation of this cup, the acetabulum is cleaned to remove any loose bone or carti lage. The acetabulum is filled with cement and the cup pushed in to express the excess cement. The spacers 7, 8 and 9 hold the main body of the acetabular cup the correct distance away from the internal wall of the acetabulum so that a layer of bone cement of uniform thickness of between 2-6 mm is obtained between the acetabulum itself and the acetabular cup. If the cup is pushed in, pressure will be exerted on the cement via the flange 6, thus producing a cement pressure injection effect. This pressurisation forces the bone cement against the inner wall of the acetabulum and into the grooves 2 of the acetabular cup thereby helping to provide a good key between the acetabulum, the acetabular cup and the bone cement.Naturally, the size of the acetabular cup is chosen to fit the particular acetabulum for which it is intended.
However, certain dimensions of the cup advantageously remain the same whatever size of cup is chosen. Thus, in the preferred form of cup the width F of the flange 6 and the preferred extension E of the spacers from the surface of the cup is 3 mm. The dimension W is also constant at 7 mm and the diameter of the recess 1 is also constant.
Dimension V therefore varies depending on the size of cup. This arrangement facilitates manufacture of the cup whilst maintaining desired cup performance in use irrespective of cup size. In use the cup is placed in the acetabulum with the recess lowermost. This increases the stability of the joint, increases or restores leg length, tensions the musculature and reduces the tendency to dislocate.
It will be appreciated that the above embodiments have been described by way of example only and that many variations are possible without departing from the scope of the invention. For example, the cup could be formed with appropriate apertures and the spacers 7, 8 and 9 inserted into these apertures. This cup may be moulded instead of being machined.
It will be appreciated that the above described embodiment, apart from ensuring an optimum distribution of bone cement around the cup, a precise fitting of the cup in the acetabulum, and the ability to pressurise the bone cement in order to provide for better keying also ensures that the locus of rotation of the ball and socket and joint are always in the optimum position to equalise leg length and give a firm reduction to guard against dislocation.
Claims (14)
1. An acetabular cup for insertion in the acetabulum in the production of an artificial hip joint, the cup having an external envelope of substantially part-spherical shape and defining a substantially part-spherical internal recess for receiving the head of a femoral endoprosthesis, in which the external surface of the cup is formed with a plurality of grooves, a plurality of spacers extend out from the external envelope of the cup and a flange extends around the circular periphery of the envelope such that when the cup is placed in position in the acetabulum a space of substantially constant thickness is defined between the external envelope of the cup and the external surface of the acetabulum for the insertion of cement, the grooves act to receive cement when inserted to improve keying be tween the cup and the surrounding bone and the flange acts to retain cement so inserted.
2. An acetabular cup as claimed in Claim 1, in which the cup is made of synthetic plastics material.
3. An acetabular cup as claimed in Claim 1 or 2, in which the cup is moulded from synthetic plastics material.
4. An acetabular cup as claimed in Claim 1 or 2, in which the cup is machined from synthetic plastics material.
5. An acetabular cup as claimed in Claims 2, 3 or 4, in which the synthetic plastics material is high density polyethylene.
6. An acetabular cup as claimed in any preceding claim, in which the flange is integral with the remainder of the cup.
7. An acetabular cup as claimed in any preceding claim, in which the flange is flexible so that in operation it can be used to apply pressure to cement in the acetabulum.
8. An acetabular cup as claimed in any preceding claim, in which the grooves extend circumferentially around the cup.
9. An acetabular cup as claimed in Claim 8, in which the grooves are circular.
10. An acetabular cup as claimed in any preceding claim, in which the spacers comprise small individual projecting members.
11. An acetabular cup as claimed in any preceding claim, in which the spacers are fixed in complementary holes formed in the external surface of the cup.
12. An acetabular cup as claimed in any preceding claim, in which the spacers are integral with the remainder of the cup.
13. An acetabular cup as claimed in claim 1, in which the spacers comprise individual members projecting from an ungrooved circular belt extending circumferentially around the external surface of the cup and the grooves are formed in those parts of the external surface extending to both sides of the circular belt.
14. An acetabular cup for insertion in the acetabulum in the production of an artificial hip joint substantially as hereinbefore described with reference to the accompanying drawings.
