Abstract

1,243,808. Ligaturing with straight wires. UNITED STATES SURGICAL CORP. 2 Oct., 1968 [2 Oct., 1967], No. 46819/68. Heading A5R. A surgical instrument for placing wire ligatures around tubular organs comprises an anvil 18 having a curved surface 36, clamping means for holding an organ to be ligatured in a position adjacent the curved surface of the anvil, pushing means for advancing a substantially straight wire ligature 101 in a direction parallel to its length towards the anvil and curling the ligature thereagainst, and means for guiding the ligature as it is advanced towards the anvil by the pushing means tangentially part one side of a tubular organ when clamped in the instrument by the clamping means, the pushing means and curved surface together being effective to curl a straight wire ligature around to effect the ligaturing of a clamped organ. As described the instrument comprises a steel rail 12 on a handle 16 ligatures with a curved leading end are contained in two cartridges 64 on a carriage 44, the forward end of which is shaped to provide a clamping face 108 to clamp an organ and a camming surface 112 which engages a probe 34 of the anvil to depress the anvil head to present to a ligature a section of the curved surface 36 appropriate to the diameter of the clamped organ. Between the two cartridges is a knife with a blade 82. A pusher 46 has attached two ligature pushing members 70 and a lost motion mechanism for operating the knife. In use the head of the instrument is placed behind a tubular organ and a trigger is squeezed to move pusher 46. Spring 54 moves carriage 44 until the organ is clamped and the anvil is suitably depressed. Pusher 46 continues to advance, first ejecting wire ligatures from the cartridges and encircling the clamped organ, then the knife is advanced to sever the organ between the ligatures.
