Abstract

The cage (Ci) has elementary cages (11, 12) respectively with two parts (18, 19) defined by closed side walls (20, 22) having two faces (21, 24) with two planes. The elementary cages are mounted in co-operation relative to each other such that the parts are mounted in rotation relative to each other around a rotational axle (13). The elementary cages are mounted in co-operation relative to each other such that the planes form a non-zero angle (alpha) relative to each other when the parts are in an open position, and the faces contact each other when the parts are in a closed position. An independent claim is also included for a method for bringing an intersomatic cage from an open configuration to a closed configuration.

Description

CAGE INTERSOMATIQUE APTE A ETRE INTERCALEE ENTRE DEUX VERTEBRESINTERSOMATIC CAGE THAT CAN BE INTERCALE BETWEEN TWO VERTEBRATES

The present invention relates to intersomatic cages capable of being interposed between two consecutive vertebrae, comprising a plurality of elementary cages, as well as methods for forming them. It is known that, according to the trauma suffered by a spine, Practitioners are led to perform two kinds of surgery. When the trauma concerns only one intervertebral disc and the two consecutive vertebrae surrounding this disc are in good condition, the practitioner may opt for the replacement of the disc by a prosthesis whose functionality is substantially equivalent to that of the original disc. that is to say leaving the two vertebrae the ability to move relative to each other in rotational and / or translational movements. On the other hand, when the trauma is greater, the practitioner can choose to secure the two consecutive vertebrae by osteosynthesis. To do this, the intervertebral disk is partially or completely destroyed and replaced by an intersomatic cage generally associated with an osteosynthesis promoting element, for example a bone graft. There are currently two types of intersomatic cages which make it possible to carry out these interventions, with, for each type, a relatively large range of products so as to be able to provide the most appropriate solution to the case of each patient, which requires a stock of important products. It is therefore conceivable that the cost price of these intersomatic cages is relatively high. In addition, these intersomatic cages are essentially implantable by two ways, one called "anterior", the other called "posterior", each of these routes having its advantages and disadvantages. However, it would seem obvious that the posterior implantation 30 is the most normal because the path used by the practitioner for the surgical procedure is relatively short. But it is very delicate because this path passes very close to essential elements for the patient's life, namely the spinal cord and the nerves that control the movements of the patient.

It is for this reason that some Practitioners opt for the so-called "anterior" pathway which involves a much longer intervention path, but in theory less dangerous for the patient. Conversely, the intervention time is longer than that by the posterior route, which can be a disadvantage for the health and recovery of the patient. Thus, the purpose of the present invention is to provide an intersomatic cage, the structure of which makes it possible to implant by the posterior route much easier than that of the intersomatic cages of the prior art, and therefore less dangerous for the patient.

More specifically, the subject of the present invention is an intersomatic cage capable of being inserted between two consecutive vertebrae in replacement of at least a part of the intervertebral disc situated between these two vertebrae, comprising: at least two elementary cages each formed of a piece and means for pivoting about an axis of rotation between the two parts so that they are able to take different angular positions relative to each other, • each part being defined between two substantially parallel faces, the distance between these two faces being substantially equal to the thickness of the original intervertebral disk, • one of the two parts being delimited by a closed side wall comprising at least a first face defining a first plane containing the axis of rotation, • the other part being bounded by a closed side wall having at least a second face defining a cond plane also containing the rotationaln axis, • the two elementary cages being mounted in cooperation with each other so that the two parts respectively forming these two elementary cages are rotatably mounted relative to each other; the other around the same axis of rotation and so that, when in their position called "unfolded", the first and second planes form between them a non-zero angle, and that, when in their position said "folded", the first and second faces are in contact with each other, characterized in that it further comprises locking means for locking the two parts relative to each other in their folded position. The present invention also aims to implement a method for forming this cage.

Other features and advantages of the invention will emerge in the course of the following description given with reference to the accompanying drawings for illustrative but not limiting purposes, in which: FIG. 1 represents, in plan view and schematically, a method of embodiment of an intersomatic cage according to the invention, in its so-called "unfolded" configuration, FIG. 2 represents, in plan view and schematically, the intervertebral cage according to FIG. 1, in its so-called "folded" configuration when it is interposed between two vertebrae, and Figure 3 shows a diagram for explaining the formation of the ntersomatic cage. With reference to the three figures attached to the present description, the subject of the invention is an interbody cage C1 which can be implanted by the posterior route, more particularly the so-called "posterolateral" pathway VP, that is to say a path whose entrance is located in the back of the patient. The intersomatic cage Ci according to the invention comprises at least two elementary cages 11, 12, ... each formed of a part 18, 19, and pivoting means 14 about an axis of rotation 13 between the two parts 18 , 19 so that they are able to take different angular positions with respect to each other. According to an advantageous embodiment, the pivoting means 14 of one with respect to the other of the two parts 18, 19 are constituted by a pivot comprising a bearing, a rotation shaft adapted to pivot in this bearing, and means for linking, for example by securing, the bearing and the rotation shaft respectively to the two parts 18, 19. Each part 18, 19 is defined between two parallel faces or forming between them a non-zero angle of small value, the distance between these two faces being substantially equal to the thickness of the original intervertebral disc or part of this disc to be replaced. One of the two parts, for example the part 18 forming the elementary cage 11, is further delimited by a closed side wall 20 connecting its two parallel faces, which may have different shapes but comprises at least one first face 21 which may be of any shape, but advantageously flat or can be likened to a plane face to define as a whole a first plane containing the axis of rotation 13 defined above. As for the other piece 19, that forming the other elementary cage 12, it is also delimited by a side wall 22 of different possible shapes but having a second face 24 which may also be of any shape but advantageously flat or likened to a plane face to define as a whole a second plane also containing the axis of rotation 13.

The two elementary cages 11, 12 are mounted in cooperation with each other so that the two parts 18, 19 respectively forming these two elementary cages 11, 12 are mounted in rotation relative to one another. other around the same axis of rotation 13 and so that, when they are in their so-called "unfolded" position, Figure 1, the first and second planes form between them a non-zero angle for example of a value I, and that when in their so-called "folded" position, FIG. 2, the first and second faces 21, 24 are in contact with one another, preferably in a substantially congruent manner. The intersomatic cage Ci further comprises blocking means 30 for blocking with respect to one another the two parts 18, 19 of the two elementary cages 11, 12 in their folded position. These locking means 30 may advantageously be unlockable and designated in this case, in the following description, by "reversible locking means" to be able to pass the two elementary cages 11, 12 from their folded position to their unfolded position. These locking means 30 may be constituted in different ways. However, as illustrated in FIGS. 2 and 3, they advantageously consist of a male-female socket 31 which, in the case where they are reversible, can be unlocked under the action of a traction force greater than a given value. . According to a preferred embodiment such as that illustrated, this male-female engagement consists of a male portion 32 integral projecting from the first face 21 of one 18 of the two parts, comprising a rod portion of which one end is secured this first face 21 and a head of a section greater than that of the rod portion and secured to the other end of the rod portion, and a female portion: 33 hollowed in the other piece 19 This recessed female portion 33 comprises a piercing portion of a cross section at least equal to that of the stem portion but less than that of the head, this piercing portion emerging, at one end, on the second face 24 of the other piece 19 being of a length substantially equal to the length of the rod portion, and an open cavity on the second end of the piercing portion, this cavity having a section greater than c her head.

These two male and female parts 32, 33 are respectively made on and in the two parts 18, 19 of the two elementary cages 11, 12 so that, when in their folded position, Figure 2, the male portion 32 is plugged in the female part 33, the head having been forced by elastic compression in the piercing portion to become lodged in the cavity while relaxing. The value of the force for passing the two elementary cages 11, 12 in their unfolded position to their folded position, and vice versa when the male-female interlock is reversibly retentive, is a function of at least the following parameters: the coefficient of elastic deformation of the material in which the two pieces 18, 19 are made, the value of the section difference between that of the head and that of the breakthrough portion, and the values of these sections. The skilled person will, of itself, determine the value of these parameters to obtain the value of the desired force.

Each elementary cage 11, 12 may of course also include all the elements that a normal cage known from the prior art can comprise. For example, although not illustrated, it may include notches on two parallel faces of each piece 18, 19, one or more housings in the rooms to receive one or more bone grafts, breakthroughs adapted to cooperate with tools or the like etc. These elements are well known in themselves have not been described here for the sole purpose of simplifying the present description. These parts 18, 19 may be made of different materials, for example titanium, stainless steel or a biodegradable material such as that known under the name PEEK (Polyether Ether Ketone), polymer, or even a shape memory alloy . The formation of the intersomatic cage Ci according to the invention, that is to say its passage from the unfolded configuration to its folded configuration, is carried out in the following manner described more particularly with reference to FIG. 3: In this figure 3 is schematically represented a vertebra Vt, as well as the spinal cord Moe and two nerves N1 and N2 controlling for example movements of the lower limbs of the patient on which is to be implanted an interbody cage Ci according to the invention.

To do this, the structure of the intersomatic cage Ci allows first of all to introduce it between the two nerves N1 and N2 when it is in its unfolded configuration referenced PCE, the elementary cages 11, 12, ... being substantially aligned on a straight line Di. In this configuration, it has a lateral bulk at most equal, and advantageously lower, the distance between the two separating nerves N1 and N2. The cage Ci is introduced by passing flush with the spinal cord Moe or, if necessary, by shifting very slightly the spinal cord by means of a suitable ancillary, then is positioned in the intervertebral space Eiv between the two bodies of vertebrae in its folded configuration schérnatisée in PCI, as explained below. To bring the intersomatic cage Ci from its unfolded configuration to its folded configuration, when it consists of elementary cages formed of parts 18, 19 all substantially identical, as illustrated in FIGS. 1 to 3, it is first guided on a rectilinear path in its unfolded configuration, for example by being maintained in a chute Go or the like, FIGS. 1 and 3. When the first downstream elementary cage 18, 19 reaches a determined point, it is subjected to a rotation of a value a, for example by means of a curved abutment Bc, as schematically illustrated in Figures 1 and 3, the next elementary cage still remaining on the straight path. Since the value of the angle between the first and second planes of two consecutive elementary cages is equal to a, with this rotation of value a, the two male and female parts 32, 33 of the locking means 30 cooperate with each other, the part male 32 plugging into the female part 33 to block the first downstream elementary cage with its next.

This is so with all the successive elementary cages as the intersomatic cage Ci is pushed on the rectilinear path and they reach, one after the other, the curved stopper Bc, to obtain, at the outlet of the chute Go, the folded configuration shown in FIG. 2. The structure of the intersornatic cage Ci presents undeniable advantages. In particular, the locking means 30 allow secure automatic positioning of the cage between two vertebrae without special ancillary instrument. When these locking means are reversible, if necessary, for example for a new intervention, the cage can be easily removed from the intervertebral space in the reverse way to that which allowed its implantation. It has been described above an intersomatic cage Ci comprising two elementary cages 11, 12, but it is obvious that such a intersomatic cage Ci may comprise several successive elementary cages as suggested in the figures. In this case, each elementary cage has two planar faces, or similar, defining two planes forming between them an angle f3 as a function of the final shape that the intervertebral cage will have to take when all the elementary cages 11, 12 are in their folded position. . Between two consecutive elementary cages, are of course provided blocking means 30, possibly reversible, for locking the two parts in their folded position.
