Abstract
Description
The present invention relates to safety catheters for fluid injection including embolic for endovascular treatments, for example to treat arteriovenous malformations (AVM). Such treatment consists of the injection of an embolic liquid by means of a catheter, which makes it possible to block the vessel or vessels supplying the sick area. However, it is known that sometimes this embolic liquid envelops the distal end of the catheter used for injection and traps it in such a way that the catheter must be pulled to try to unblock it. It is easy to see that such a maneuver can have disadvantages. For example, the embolic liquid is removed with the catheter and no longer plays its role, or it may be deposited in an unwanted place, which can cause many small arteries to rupture. According to another drawback, the tensile stress is insufficient and can not be increased without risk of causing large arterial lesions. In this case, the only solution is to leave the catheter in place almost in its entirety, in the body of the patient. Accordingly, it is an object of the present invention to provide a safety catheter for fluid injection, particularly embolic fluid, which at least substantially overcomes the aforementioned disadvantages of similar catheters of the prior art. More specifically, the subject of the present invention is a safety catheter for fluid injection, in particular an embolic fluid, comprising: a tubular body defined between a proximal end and a distal end, said tubular body comprising a first channel opening at its two first and second ends respectively at both proximal and distal ends of the tubular body, and injection means adapted to inject said fluid into the first channel through the first end of said first channel, characterized in that it further comprises : A tubular nozzle defined between two first and second ends, said tubular nozzle having a second channel opening through its two first and second ends respectively to the first and second ends of the tubular nozzle, and means for making a tight connection between said tubular body and said tubular ferrule so that the second end of the first channel and the first end of the second channel are substantially coaxial, said connecting means being furthermore arranged in such a way that by exerting a pair of forces of determined value respectively on the tubular body and the tubular endpiece , there is rupture of said connection to obtain the separation of the tubular body and the tubular endpiece.
Other features and advantages of the invention will become apparent from the following description given with reference to the accompanying drawings for illustrative but not limiting purposes, in which: FIG. 1 represents the block diagram of the safety catheter according to the invention for In particular, FIGS. 2 to 4 show three partial longitudinal sectional views, respectively of three embodiments of the safety catheter according to the invention in accordance with the block diagram shown in FIG. 1, and FIG. represents an embodiment constituting an improvement to the catheter shown in FIG.
It is first of all specified that, in the figures, the same references designate the same elements, whatever the figure on which they appear and whatever the form of representation of these elements. Similarly, if elements are not specifically referenced in one of the figures, their references can be easily found by referring to another figure. It is also stated that the figures essentially represent three embodiments of the object according to the invention, but that there may be other embodiments that meet the definition of this invention. It is furthermore specified that, when, according to the definition of the invention, the object of the invention comprises "at least one" element having a given function, the embodiment described may comprise several of these elements. Conversely, if the embodiment of the object according to the invention as illustrated has several elements of identical function and if, in the description, it is not specified that the object according to this invention must obligatorily comprise a number particular of these elements, the object of the invention may be defined as comprising "at least one" of these elements. Lastly, it is specified that when, in the present description, an expression defines, by itself, without a specific particular mention concerning it, a set of structural characteristics, these characteristics may be taken, for the definition of the object of the protection requested, when technically possible, either separately, or in full and / or partial combination. With reference to FIG. 1, the fluid injection safety catheter, in particular an embolic catheter, comprises a tubular body 1 defined between a proximal end 2 and a distal end 3, the tubular body comprising a first channel 4 opening at its first and second ends. ends 5, 6 respectively at the two proximal and distal ends of the tubular body, and injection means 10 adapted to inject the fluid into the first channel 4 by its first end 5. These injection means 10 are well known in themselves. same and therefore will not be more fully described here, for the sole purpose of simplifying the present description. According to an important feature of the invention, the catheter further comprises a tubular nozzle 20 defined between two first 21 and second 22 ends, this tubular tip having a second channel 23 opening through its two first 24 and second 25 ends respectively to the two first and second ends 21, 22 of the tubular nozzle 20, and means 30 for sealingly connecting the tubular body 1 and the tubular nozzle 20 so that the second end 6 of the first channel 4 and the first end 24 of the second channel 23 are substantially coaxial 40 so that the first and second channels 4, 23 form a substantially continuous channel, these connecting means 30 being furthermore arranged so that by exerting a pair of forces of determined value respectively on the tubular body 1 and the tubular end 20, there is rupture of the connection to obtain the separation of the tubular body 1 and the tubular endpiece 20. According to an advantageous embodiment, such as that more particularly illustrated in FIG. 2 and taken in combination with the representation according to FIG. 1, the sealing connection means 30 defined above by their primary function, consist of a ring 31 covering the lateral face portions 32, 33 respectively of the tubular body 1 and the tubular tip 20 respectively at the distal end 3 of the tubular body and the first end 21 of the tubular tip, and by sealing means 50 for sealing between the ring 31 and the two lateral face portions 32, 33. Preferably, whatever the material in which the tubular body 1 and the tubular endpiece 20 are made, the ring 31 is made of platinum and the sealing means 50 are constituted by adhesive bonding the side wall of the ring and the two side wall portions 32, 33 respec tively the distal end 3 of the tubular body and the first end 21 of the tubular nozzle. This adhesive is, for example, as illustrated in FIG. 2, in the form of two cords which seal respectively the two end edges of the ring 31 and the two side wall portions 32, 33 defined above. . In general, the material in which the tubular body 1 and the tip 20 are made is for example polyurethane, a polyamide, or a material known under the trademark "Pebax", or a mixture of these three materials.
In the diagram according to Figure 1, the tubular body 1 and the tubular nozzle 20 are shown relatively distant from each other. But, advantageously, as illustrated for example in Figure 2 and for a purpose to be explained below, the tubular body and the tubular end are abutted. FIGS. 3 and 4 show two other embodiments of the catheter according to the invention, according to which the sealing connection means 30 consist of a spacer 70 comprising a third channel 71, this spacer being interposed in contact between the tubular body 1 and the tubular endpiece 20 so that the first 4, second 23 and third 71 channels form a substantially continuous channel, and by means for sealing between the spacer and the tubular body and the tubular end respectively. According to the embodiment illustrated in Figure 3, the tubular body 1, the spacer 70 and the tubular nozzle 20 are made of a single tube 60, the spacer 70 consisting of a part of the wall of the single tube having a thickness lower than that of the wall of the remaining portion of the single tube 60, which creates a point of weakness in this single tube 60 and thus a means to obtain the breaking of the connection. In this embodiment according to FIG. 3, the means for sealing between the spacer and the tubular body 1 and the tubular end-piece 20 are constituted by the fact that the tubular body 1, the spacer 70 and the tubular end 20 are made of a single tube 60. This embodiment may have advantages over that of FIG. 2, in particular the fact that it does not require the use of a material other than that of the tube 60, as a glue and / or solder. As for the embodiment according to Figure 4, it can be interesting, especially when the tubular body 1 and the tubular nozzle 20 are of different materials. In this case, the spacer 70 is advantageously constituted by one of the following elements: an adhesive ring, a solder ring.
By its nature, the spacer 70 according to the embodiment illustrated in Figure 4 is both the connecting means between the body 1 and the nozzle 20 and the means for sealing between the spacer itself. same and respectively the tubular body and the tubular endpiece. The breaking of the connection can be obtained, as mentioned before, by applying a pair of forces of determined value respectively to the tubular endpiece 20 and the tubular body 1. This pair of forces is at least one of the following pairs : torque of tensile forces, torsional torque, or a combination of these two pairs. The catheter according to the invention described above is used in the following manner.
When it is necessary to perform an operation such as that mentioned in the preamble of the present description, the catheter is introduced, through the second end 22 of the tip 20, into the artery to be treated, from a point of the patient's body that is relatively accessible. The tubular body 1 is pushed until the second end of the tip reaches the place to be treated, its proximal end 5 with the injection means 10 being held outside the body of the patient. It should be noted that, during this thrust, the tubular body pushes on the tip, and that there is therefore no risk of rupture of the connection between the body 1 and the tip 20.
The treatment liquid is then injected into the catheter under a determined pressure which, to allow ejection of the liquid through the second end of the second channel 23, is relatively higher than the atmospheric pressure and / or blood pressure prevailing in an artery, and flows into the artery by this second end, to the place in this artery chosen by the Practitioner. If, as previously mentioned, the injected liquid forms a plug around the second end 22 of the tubular tip 20, which prevents the removal of the catheter, the practitioner exerts a tensile force and / or a torsion on the proximal end of the catheter. tubular body 1. Under the action of this traction and / or torsion, because the connecting means 30 have the characteristics defined in the present description, or the tip emerges from the plug, or there occurs a rupture of the connection 30 and thus a separation of the tubular endpiece 20 and the tubular body 1.
In the case of the embodiment according to Figure 2, the connecting means are generally arranged so that the weld seam and / or adhesive connecting the ring 31 and the tip 20 is broken first, which allows to separate the nozzle and the assembly formed of the ring and the tubular body. The embodiment of the catheter according to the invention illustrated in this figure 2 has a significant advantage. Indeed, the ring 31 constitutes a resistance to lateral deformations of the ends in contact, the tubular body 1 and the tubular tip 20, which could occur under the pressure of the injected fluid, but does not exert tensile strength on the tubular body and thus does not interfere with the separation of the tip 20 and the body 1 as previously explained.
In the case of the embodiment according to Figure 3, the tube 60 breaks at the level of the thinning 61. In the case of the embodiment according to Figure 4, it is the welding ring and / or glue 70 which breaks under the tensile force and / or torsion, thus separating the tubular nozzle 20 and the tubular body 1 which is then easily removed.
In any case, it is no longer necessary to leave in place almost the entire catheter. At worst, only the tip is left in place in the body of the patient, without presenting any danger.
FIG. 5 shows another embodiment of the catheter according to the invention, which is in fact an advantageous improvement, for certain applications, to the embodiment illustrated in FIG. 2. The catheter according to this improved mode has the same characteristics as those described in FIG. according to the embodiment described with reference to Figure 2, with the addition that at least a portion of the adhesive bonding the side wall of the ring 31 and the two side wall portions 32, 33 respectively of the distal end 3 the tubular body 1 and the first end 21 of the tubular nozzle 20 is adapted to be destroyed by the fluid 70, in this case an endovascular treatment liquid, as mentioned before. In an advantageous embodiment, the sealing means defined before are constituted by two beads of adhesive 501, 502 coating at least partially, if not completely, the two ends 311, 312 of the ring 31 and respectively covering at least a part of the two portions of side walls 32, 33, preferably all around their periphery, respectively of the distal end 3 of the tubular body 1 and the first end 21 of the tubular nozzle 2C). In this way, at least one of the two beads of adhesive 501, 502 can be destroyed by the fluid 70. Preferably, the bead of adhesive which is capable of being destroyed by the fluid 70 is the one 502 which is closest to the second end 22 of the tubular nozzle 20, the other cord 501 being preferably non-destructible by the fluid 70. It is clearly stated that, for the purposes of the present description, by "destruction", it is one of the following effects: disintegration, decomposition, melting, fracturing, dissolution, whether this "destruction" is total or partial, so that it considerably reduces the properties of the glue's hold so that it can no longer play its role as an integral assembly of two parts with each other. The catheter according to the embodiment according to Figure 5 as described above is used and operates as follows: As previously described, the tubular body 1 is pushed until the second end 22 of the mouthpiece tubular 20 arrives at the place to be treated, its proximal end 5 with the injection means 10 being held outside the body of the patient.
The fluid 70, that is to say more particularly the treatment liquid, is then injected and, after having traveled the first and second channels 4, 23, flows at 71 through the end 25 of the second channel 23. However, very quickly, there is produced, in front of this end, a "plug" which leads, de facto, a fluid reflux 72 between the wall of the vein or the blood vessel V and the outer wall of the tubular nozzle 20. Since the adhesive bead 502 is intended to be dissolved by the fluid 70, it is at least partially eliminated and the ring 31 is able to be released from the tubular endpiece 20.
The structural characteristics of the glue defined above allow, firstly during the injection of the fluid 70, the holding of the tubular endpiece 20 and the tubular body 1 against each other by means of the ring 31 stuck on them and, in a second time when the reflux 72 occurs and while continuing the injection 71 of the fluid, gradually reduce the strength of the glue so as to further maintain the ring 31 secured to the tip and the tubular body to complete the injection 71 of the fluid and to allow, at the end of the injection, the detachment of the ring 31 under traction exerted on the tubular body. The tubular body 1, the ring 31 and the adhesive bead 501 can then be removed, as explained previously.
The Applicant has experimented with a catheter according to this embodiment and obtained very good results with the embolization liquid known under the trademark ONYX which comprises a solvent which allows a reduction of at least 50% of the behavior of the glue when the latter is based on polyurethane, which is sufficient to detach the tubular body 1 of the tubular nozzle 20 and obtain the desired result.
