Abstract

The device is a tube (6) with two balloons used in medicine to ensure the control of the airways thus allowing ventilatory support. It can be called "laryngeal probe" or "glottic mask". The device makes it possible to remedy, in particular, the disadvantages of the laryngeal mask related to the leaks and its displacement. It is a tube (6) resembling an intubation probe with a proximal orifice and a distal orifice but having two balloons, a first one of twenty milliliters of air (4) located about one centimeter from the distal end (2) and a second (5) from one hundred milliliters of air located behind the first to about one centimeter. The distal end (2) is placed under laryngoscopy just after the vocal cords (3) hence the name of laryngeal probe (1). The first balloon is then just before the vocal cords (4) and then inflated with twenty milliliters of air.The second balloon (5) is then inflated by about one hundred milliliters of air so as to lodge and settle at the level of the oropharynx. It will also make it possible to maintain the first balloon (4) under pressure on the glottis to ensure its sealing, hence the name of the glottic mask of the device.

Description

Le domaine technique de l'invention est médical et concerne plusThe technical field of the invention is medical and concerns more

spécifiquement le contrôle des voies aériennes.  specifically the control of the airways.

This control is carried out conventionally by several devices: the tracheal intubation which allows a good control of the airways, but requires to cross the vocal cords then a part of the trachea which is painful and requires a very good relaxation.

- the laryngeal mask: its insertion requires a lighter general anesthesia, but its disadvantages are the failures of the technique during the installation, the leaks and its secondary displacement.

The device according to the invention overcomes the disadvantages of the two techniques without completely replacing them. It is indeed an intubation limited to part of the larynx (1) avoiding the crossing of the painful trachea. Its pose requires a general anesthesia less deep than for endotracheal intubation. Compared to the laryngeal mask, this device is more watertight with much less leakage and less risk of inhalation.

It is a tube (6) resembling an intubation tube with a proximal orifice and a distal orifice but having two balloons, a first (4) of twenty milliliters of air located about one centimeter from the distal end ( 2) and a second (5) of one hundred milliliters of air located behind the first (4) at about one centimeter.

Patient under general or comatose anesthesia, laryngoscopy and local anesthesia of the glottis by Xylocaine 5% (7 to 10 instillations) and then placement of the distal end (2) of the device at the level of the larynx (1) just after or between the vocal cords (3). Inflate the first balloon (4) with twenty milliliters of air and remove the laryngoscope by exerting light pressure with the hand on the device in the caudal direction to keep it in place.

Inflate the second balloon (5) with one hundred milliliters of air, thus allowing the device to be fixed at the level of the oropharynx and to complete the sealing of the first balloon (4) around the glottis. pushing it towards the larynx (1).

The device can make it possible to provide ventilatory assistance in general anesthesia, especially for acts of short to medium duration. It could be useful in difficult intubations or often only a part of the glottis is visible. The device could provide ventilatory support during the realization of the percutaneous tracheotomy without hindering it.
