Abstract

L'appareil de mise en place et de positionnement sous forme sensiblement d'une potence, présente deux bras équerrés (1) et (2) dont l'un est destiné à être positionné en regard du fémur en étant apte à recevoir des canons de visée (12) avec écrou de blocage (13), pour la mise en place de vis au niveau distal de la tige, tandis que l'autre est monté, avec capacité de réglage en translation, sur l'extrémité supérieure et présente des agencements d'accouplement coopérant avec le col de la tige.La tige de reprise (5) présente en partie proximale, une empreinte profilée (5a) formée sur le côté externe de ladite tige et apte à coopérer avec une broche de rigidification (4) que présente le bras de visée (1).

Description

L'invention se rattache au secteur technique des implants
orthopédiques, et plus particulièrement aux tiges fémorales de reprise.
Il est connu d'utiliser, pour la mise en place de tiges fémorales de reprise, des appareils de conception spécifique, généralement connus sous le nom de viseurs.
Pour l'essentiel, un viseur se présente sous forme sensiblement d'une 10 potence constituée de deux bras équerrés avec capacité de réglage. L'un des bras est destiné à être positionné en regard du fémur, en étant apte à recevoir, en partie inférieure, les canons de visée pour la mise en place de vis au niveau distal de la tige de reprise. A l'opposé, l'extrémité libre de ce bras reçoit, dans un plan orthogonal, l'autre bras monté généralement avec 15 capacité de réglage en translation. L'extrémité libre de ce deuxième bras présente des agencements pour l'accouplement temporaire de la tige fémorale de reprise au niveau de sa partie proximale supéro-exteme ou supéro-interne formant un col recevant, ultérieurement, une tête sphérique d'articulation apte à coopérer avec un implant cotylodien. 20 Les solutions connues de viseur génèrent certains inconvénients pour la mise en place des vis d'ancrage au niveau distal et plus accessoirement pour le réglage en hauteur de l'implant avant le verrouillage.
En effet, l'extrémité de la tige, siège du verrouillage, peut, sous l'effet d'un fléchissement induit par la grande longueur de l'implant, se déplacer par rapport au cadre du viseur lors de la première visée. Les visées ultérieures pour les deuxième et troisième vis s'avèrent alors impossibles.
L'invention s'est fixée pour but de remédier à ces inconvénients, de manière simple, sre, efficace et rationnelle, en spécifiant des agencements propres à l'implant.
Le problème que se propose de résoudre l'invention est de pouvoir rigidifier la liaison tige/viseur avant la mise en place des vis.
Pour résoudre un tel problème, il a été conçu et mis au point une tige fémorale de reprise adaptée et son appareil de mise en place et de 10 positionnement sous forme sensiblement d'une potence présentant deux bras équerrés dont l'un est destiné à être positionné en regard du fémur en étant apte à recevoir des canons de visée pour la mise en place de vis au niveau distal de la tige, tandis que l'autre est monté, avec capacité de réglage en translation, sur l'extrémité supérieure et présente des 15 agencements d'accouplement coopérant avec le corps de la tige.
Le côté externe de la tige de reprise, en partie proximale, dispose d'une empreinte profilée apte à coopérer avec une broche de rigidification solidaire du bras de visée.
Il est apparu également que la mise en place d'une tige de reprise s'avère beaucoup plus délicate à réaliser que dans le cas d'une tige de première intention. Pour valider la hauteur du centre de la tête, le positionnement en hauteur de la tige dans le fémur s'effectue par rapport à des repères osseux, généralement constitués par le grand trochanter. Or, 25 dans le cas d'une reprise, le plus souvent le grand trochanter est détruit. Il n'y a plus réellement de repères anatomiques.
Avantageusement, en vue de faciliter le réglage en hauteur de la tige de reprise, après son introduction dans le canal médullaire, l'appareil de visée et la tige fémorale de reprise présentent des agencements complémentaires d'accouplement avec un col fantôme rendu solidaire du viseur. Dans ce but, l'extrémité proximale de la tige de reprise présente une empreinte tronconique apte à recevoir le col fantôme, le fond de ladite empreinte présentant des agencements d'indexation angulaire coopérant 10 avec des formes complémentaires du col, lesdites empreintes étant en communication avec un trou taraudé pour le vissage du col fantôme.
Le col fantôme est constitué par une tige présentant, à l'une de ses extrémités, une portée tronconique et, à son extrémité opposée, une portée filetée, ladite tige étant rendue solidaire, avec capacité de déplacement en 15 translation limitée, d'une embout tronconique solidaire de l'extrémité du bras, ledit embout étant terminé par une portée polygonale apte à coopérer avec l'empreinte de la tige.
L'invention est exposée ci-après plus en détail à l'aide des figures 20 des dessins annexés dans lesquels: - la figure 1 est une vue de face de la tige de reprise; - la figure 2 est une vue de côté correspondant à la figure 1 - la figure 3 est une vue considérée selon la flèche F de la figure 1 - la figure 4 est une vue de face du viseur seul; - la figure 5 est une vue de côté correspondant à la figure 4; - la figure 6 montre la tige de reprise accouplée au viseur; - la figure 7 montre la mise en place du viseur et de la tige; - la figure 8 est une vue correspondant à la figure 7, après mise en place d'une broche de rigidification; - la figure 9 montre la mise en place des vis au niveau distal; - la figure 10 montre la tige de reprise impactée dans le canal médullaire, après retrait du viseur; - la figure 11 est une vue semblable à la figure 10, après mise en place d'un col modulaire.
On désigne par (V) l'appareil de mise en place et de positionnement 10 dénommé ci-après viseur, et par (T) la tige fémorale de reprise.
D'une manière connue, le viseur (V) se présente sous forme sensiblement d'une potence, avec deux bras équerrés (1) et (2). Le bras (1), destiné à être positionné en regard du fémur, présente, dans sa partie 15 inférieure, plusieurs trous (la) pour l'engagement de canons de visée, sous forme de douilles (12), pour la mise en place de vis au niveau distal, comme il sera indiqué dans la suite de la description. Un écrou papillon (13) assure le blocage en position de chacune des douilles.
L'autre bras (2) est disposé perpendiculairement à l'extrémité libre 20 supérieure du bras de visée (1), notamment avec capacité de réglage en translation. Par exemple, ce bras (2) est monté libre en translation dans un bloc (lb) du bras (1), en étant immobilisé par une vis (3).
A noter que, dans le cas o la tige de reprise (T) présente, d'une manière connue, un profil longitudinal en S, correspondant à la courbure 25 anatomique du fémur, le bras (1) peut présenter également un profil longitudinal en S (figure 5).
Selon une autre caractéristique de l'invention, le bras de visée (1) présente, sensiblement dans sa partie médiane et au-dessus du bloc comportant les trous de passage (la) pour les canons de visée (12), une broche de rigidification et de centrage (4) apte à coopérer avec une 5 empreinte profilée (5a) formée sur le bord externe de la tige de reprise (5), en partie proximale. Cette empreinte (Sa) est formée en alignement avec les trous débouchants (5b) que présente la tige de reprise pour l'engagement des vis d'ancrage distales.
L'empreinte (5a) est concave et conique pour permettre un auto 10 centrage de la broche, en combinaison avec l'extrémité de la broche (4).
La broche de rigidification (4) est montée dans un support (6), rendu solidaire du bras de visée (1) et présentant des moyens de manoeuvre (7) pour un déplacement en translation de la broche (4).
Selon une autre caractéristique, l'extrémité libre du bras (2) présente des agencements d'accouplement spécifique de la tige fémorale (5), au niveau de sa partie proximale.
Dans ce but, l'extrémité libre du bras (2) présente une platine support (2a) disposée angulairement, afin d'être orientée perpendiculairement à 20 l'axe cervical de la tige de reprise (5). Cette platine support (2a) reçoit un col fantôme (8) qui peut être accouplé, d'une manière démontable, à l'extrémité proximale de la tige de reprise (5).
Le col fantôme (8) est constitué par une tige (9) présentant, à l'une de ses extrémités, une portée tronconique (9a) et, à son extrémité opposée, 25 une portée filetée (9b). Cette tige (9) est rendue solidaire, avec capacité de déplacement limité en translation, d'un embout (10) solidaire de la face de dessous de la platine support (2a).
L'embout (10) présente une portée tronconique (lOa) prolongée par une portée cylindrique (lOb) de section polygonale. L'ensemble de l'embout (10), tel que défini, coopère avec une empreinte complémentaire formée, au niveau de la partie proximale de la tige de reprise. Cette 5 empreinte est constituée par une portée creuse tronconique (5c) de forme complémentaire à la portée (lOa) de l'embout (10), le fond de ladite portée (5c) étant prolongé par une empreinte creuse (5d) de section polygonale complémentaire à la portée (lOb) de l'embout (10). L'empreinte (5d) est en communication avec un trou taraudé destiné à recevoir par vissage la portée 10 filetée (9b) de la tige (9).
On renvoie notamment aux figures 6, 7, 8, 9, 10 et 1 l qui montrent schématiquement le mode opératoire pour la mise en place de la tige de reprise au moyen du viseur, selon les caractéristiques de l'invention: 1. Réglage sur table du viseur sur la tige (réglage latéral en fonction de l'épaisseur de la cuisse, vérification de la visée pour les trous de verrouillage requis).
2. Descente de la tige dans le ft fémoral montée avec le viseur jusqu'à un niveau mesuré sur la radio pré-opératoire de façon approximative.
3. Réduction articulaire après adaptation d'une tête d'essai sur le col fantôme du viseur.
4. En fonction des résultats de la réduction (longueur de membre satisfaisante ou non), luxation du fémur et enfoncement ou relèvement de la tige via le cadre du viseur. Si besoin, contrôle de la bonne longueur 25 de membre par une nouvelle réduction.
5. Perçage du fémur pour le passage de la broche de rigidification.
6. Mise en place de la broche de rigidification.
7. Mise en place des douilles (12) de guidage jusqu'à l'appui cortical blocage des écrous (13).
8. Verrouillage successif distal.
Les avantages ressortent bien de la description.

Claims (3)

    REVENDICATIONS
  1. - 1 - Ensemble constitué d'une tige fémorale de reprise (T) et de son appareil 5 (V) de mise en place et de positionnement, l'appareil (V) adoptant la forme sensiblement d'une potence présentant deux bras équerrés (1) et (2) dont l'un est destiné à être positionné en regard du fémur en étant apte à recevoir des canons de visée (12) avec écrou de blocage (13), pour la mise en place de vis au niveau distal de la tige, tandis que l'autre est monté, avec capacité 10 de réglage en translation, sur l'extrémité supérieure et présente des agencements d'accouplement coopérant avec le col de la tige, caractérisé en ce que: la tige de reprise (5) présente en partie proximale, une empreinte profilée (5a) formée sur le côté externe de ladite tige et apte à coopérer avec une 15 broche de rigidification (4) que présente le bras de visée (1).
  2. -2- Ensemble selon la revendication 1, caractérisé en ce que l'empreinte profilée (5a) de la tige (5) coopérant avec la broche de rigidification (4) est concave et conique pour permettre un auto centrage en combinaison avec 20 l'extrémité de ladite broche.
  3. -3- Ensemble selon la revendication 1, caractérisé en ce que la broche (4) est montée dans un support (6) rendu solidaire du bras (1) et est assujettie à des moyens de déplacement en translation. 25 -4- Ensemble selon la revendication 1, caractérisé en ce que les agencements d'accouplement de l'appareil (V) sont constitués par un col fantôme (8) rendu solidaire de l'extrémité du bras (2) et apte à être accouplé, d'une manière démontable, à l'extrémité proximale de la tige de reprise (5).
    -5- Ensemble selon la revendication 1 ou 4, caractérisé en ce que l'extrémité proximale de la tige de reprise (5) présente une empreinte tronconique apte à recevoir le col fantôme (8), le fond de ladite empreinte présentant des agencements d'indexation angulaire coopérant avec des formes complémentaires du col (8), lesdites empreintes étant en 10 communication avec un trou taraudé pour le vissage du col fantôme.
    -6- Ensemble selon la revendication 5, caractérisé en ce que le col fantôme est constitué par une tige (9) présentant, à l'une de ses extrémités, une portée tronconique et, à son extrémité opposée, une portée filetée, ladite tige 15 (9) étant rendue solidaire, avec capacité de déplacement en translation limitée, d'un embout tronconique (10) solidaire de l'extrémité du bras (2), ledit embout (10) étant terminé par une portée polygonale apte à coopérer avec l'empreinte de la tige.
