Abstract

The trocar (2) for use esp in coelioscopic surgery contains an inner transparent bag (1) that is initially pushed out of the trocar by a rod and then inflated with CO2. The bag is made from an elastic material that offers good visibility, is impermeable to fluids and becomes rigid when inflated. During use the bag adheres to the walls of the peritoneum, allowing surgery to be performed through a window in it. After use it is deflated and withdrawn through the trocar by a clamp.

Description

<Desc/Clms Page number 1> <Desc / Clms Page number 1>

The present invention relates to an intraperitoneal trocar-sac device (SIP) for laparoscopic surgery.

This surgery has many advantages that were the basis of its development. But, to date, it is not free from inconveniences. Surgeons are still forced to use carbon dioxide, which can lead to subcutaneous emphysema or metabolic acidosis. They must also work in a reduced operating field which may be the cause of certain late complications (eg, loss of micro stones in the peritoneal cavity during laparoscopic cholecystectomy may cause an abscess).

The present invention aims to avoid these complications of laparoscopic surgery, by proposing a trocar device integrated with an inflatable bag (intraperitoneal-SIP bag).

Such a device has several advantages. We see its role in; - protection of the peritoneum against: gas, intraoperative trauma in general and against infection from operated organs, - picking up objects which can be considered as foreign bodies; stones, clips, blood, bile, washing fluid, - protection of the intestine (especially of the small intestine) against iatrogenic accidents.

Comme ces derniers, le SIP ne prétend pas remplacer le sac d'extraction de la vésicule biliaire  This device can be useful for the separation of the left lobe of the liver, often annoying during laparoscopic cholecystectomy, but without replacing the devices which are described in FR-A-2700 110 and FR-A-2 737 401 Like these, SIP does not pretend to replace the gallbladder extraction bag.

The airbag (1), which integrates fully with the trocar (2) does not present the danger of getting lost in the peritoneal cavity. The proclive position, recommended for laparoscopic cholecystectomy, promotes its placement in the vesicular region in a natural way. Its fixation will be ensured by the trocars usually used for laparoscopic cholecystectomy. Its volume proposed at the start should reach between 1/2 and 1/3 of the volume of the coelomic capacity of the adult subject (therefore between 2 and 3 liters). It will be made of an elastic, transparent material, with good visibility, impermeable to the fluid, chosen so that it is rigid in the inflated state.

The walls of the bag will be glued to the walls of the peritoneum using the pneumoperitoneum created inside the bag (1). The intervention will be carried out through the window in the bag (1), suitable for the organs and elements operated.

The proposed device requires no additional fluid for its implementation. At the end of the intervention, it is deflated and at the same time removed using a gripper through the integrated trocar (2).
