Abstract

Description

The present invention relates to femoral stems for hip prostheses, and more particularly to femoral stems comprising, partly male of the acetabular patella intended to replace the head of femur damaged following for example a disease, an accident, etc., a head hemispherical in a material like ceramic or the like.

It is first of all recalled that certain hemispherical heads of acetabular ball joints for hip prostheses are made of metallic material, others of ceramic or the like, while the prosthetic legs capable of being implanted in the medullary canal of the femur and on which the hemispherical heads must be adapted, are made of metallic material. Consequently, if the adaptation of a hemispherical head of an acetabular ball joint of metallic material on a femoral leg does not pose major technical problems, it is not the same for the hemispherical heads of ceramic or the like.

However, as the heads of ceramic prostheses have advantages compared to metal heads, solutions have been sought to solve the problems of adaptation of these heads on the metal femoral legs.

Among the solutions which have given good results, mention may for example be made of the embodiment described in FR-A-2 580 170.

In this document, a femoral stem for hip prosthesis is described, comprising a prosthetic leg having two ends respectively distal and proximal, this leg being able to be implanted in the medullary canal of a femur by its distal end, its end proximal remaining outside said channel being shaped as a truncated cone, a sleeve comprising, on the one hand a hollow housing in the form of a truncated cone substantially complementary to the proximal end of the prosthetic tab so that this sleeve can be fitted on this proximal end by male-female conical fitting, and on the other hand an external surface of cylindrical shape, the external cylindrical surface being bordered at one end by a shoulder ring defined in a plane perpendicular to the axis of the housing in a truncated cone, and a hemispherical cap comprising a cylindrical cavity of substantially shaped complementary to the cylindrical outer surface of the sleeve, the cavity emerging through a flat formed on the cap, this flat being able to rest on the shoulder ring of the sleeve.

In the embodiment according to this document, the connection between the cap and the sleeve was obtained by welding or the like. However, despite all the precautions that could be taken during the manufacture of these prostheses, this type of connection did not always make it possible to compensate, for example, the differences in machining tolerances and, for security reasons, it was necessary to eliminate a relatively large number large number of prostheses thus produced in order not to run the slightest risk to the patients on whom these prostheses could be implanted.

The object of the present invention is therefore to produce a femoral stem for a hip prosthesis, in which, in particular in the case where it comprises a hemispherical ceramic head or the like, the nature of the connection between this hemispherical head and the femoral leg overcomes the drawbacks mentioned above of the stems of similar prostheses of the prior art.

More specifically, the present invention relates to a femoral stem for hip prosthesis, comprising a prosthetic leg having two ends respectively distal and proximal, said prosthetic leg being able to be implanted in the medullary canal of a femur by its end. distal, its proximal end remaining outside said channel being shaped as a truncated cone, an insert comprising, on the one hand a hollow housing in the form of a truncated cone substantially complementary to the proximal end of the prosthetic tab so that said insert can be fitted onto this proximal end by conical male-female conical fitting, and on the other hand an outer surface of cylindrical shape, said cylindrical outer surface being bordered, at one end, by a shoulder ring defined in a plane substantially perpendicular to the axis of the recess in a truncated cone shape, and a cal hemispherical cap comprising a cylindrical cavity substantially complementary to the cylindrical outer surface of the insert, but of an upper section, the cavity opening onto a flat formed on the hemispherical cap, this flat being able to rest on the shoulder ring of the insert, characterized in that it further comprises a stop formed on the proximal end of the prosthesis tab to limit an additional depression of this proximal end in the hollow housing of the insert by radial elastic deformation of this insert, after the congruent interlocking has been produced, and a sheath made of an elastically compressible and expandable material interposed at least partially in contact between the wall of the cavity of the hemispherical cap and the cylindrical outer surface of the insert, the respective values of the section of the cavity of the hemispherical cap and of the section of the cylindrical outer surface of the insert being determined so that, when the proximal end of the prosthesis tab is pressed into the hollow housing of the insert and said insert comes into contact with said stop, the compression of said sheath does not reach the value beyond which the sheath exceeds its elastic compressibility limit.

Other characteristics and advantages of the present invention will appear during the following description given with reference to the drawing annexed by way of illustration but in no way limiting, in which the single figure represents, in a view in longitudinal section and partially cut away, a mode for producing part of a femoral stem for hip prosthesis according to the invention.

The femoral stem for hip prosthesis illustrated in the single figure comprises a prosthetic leg l having two ends respectively distal 2 and proximal 3.

This prosthetic leg is adapted to be implanted in the medullary canal of a femur in a known manner by its distal end 2, its proximal end 3 remaining outside the canal. In addition, this proximal end 3 is shaped as a truncated cone, as visible in the figure.

It also includes an insert 4 comprising, on the one hand a hollow housing 5 in the form of a truncated cone substantially complementary to the proximal end 3 of the prosthesis tab so that this insert 4 can be fitted onto this proximal end by Conical male-female fitting congruent, and on the other hand an outer surface 6 of cylindrical shape, this cylindrical outer surface 6 being bordered, at one end 7, by a shoulder ring 8 defined in a plane 9 substantially perpendicular to the axis 10 of the recessed recess 5 in a truncated cone, it also comprises a hemispherical cap 11 comprising a cylindrical cavity 12 substantially complementary to the cylindrical external surface 6 of the insert 4, but of an upper section, the cavity 12 opening onto a flat 13 made on the hemispherical cap, this flat being able to rest on the shoulder ring 8 of the insert 4. This femora rod the further comprises, according to essential characteristics of the invention, a stop 20 produced on the proximal end 3 of the prosthesis tab 1 to limit the further insertion of this proximal end into the hollow housing of the insert by radial elastic deformation of this insert, after the congruent nesting has been produced, and a sheath 30 of an elastically compressible and expandable material interposed at least partially between the wall of the cavity 12 of the hemispherical cap 11 and the external surface cylindrical 6 of the insert.

In addition, the respective values of the section of the cavity 12 of the hemispherical cap 11 and the section of the cylindrical outer surface 6 of the insert 4 are determined so that, when the proximal end 3 of the prosthetic tab 1 is pressed into the hollow housing 5 of the insert 4 and the insert comes into contact with the stop 20, the compression of the sheath 30 does not reach the value beyond which the sheath exceeds its limit of elastic compressibility.

In a preferred embodiment, the stop 20 is constituted, as illustrated in the figure, by a shoulder ring produced by machining in the same part as the femoral leg 1 defined above.

It is firstly specified that the frustoconical shapes, respectively of the proximal part 3 of the femoral leg and of the hollow housing 5 of the insert, are determined so that there is a congruent conical male-female fitting before the insert 4 comes into contact with the stop 20 and when the insert 4 is fitted onto the femoral leg 1, it does not normally come into contact with the stop 20. It takes, in relation to this stop, a position as that which is illustrated in the single figure.

If a pressure overload, for example accidental, applies to the hemispherical cap 11, this overload is transmitted to the insert 4 via the flat 13 which rests on the shoulder ring 8. The insert then has tendency to descend along the proximal end 3 of the femoral leg 1. Relatively, this proximal end 3 will sink into the housing 5 of the insert 4 which consequently undergoes deformation by elastic radial expansion. However, due to the presence of the stop 20, this expansion of the insert will be automatically limited to a certain threshold since, as soon as it is in contact with this stop, the insert 4 can no longer expand radially.

Correlatively, the sheath 30 will compress under the effect of the radial expansion of the insert, but without reaching its elastic compressibility limit. There will therefore be, following this accidental overload, transmission of radial expansion forces on the hemispherical cap 11, and any risk of rupture of the hemispherical cap is therefore almost completely annihilated.

In addition, so that the hemispherical cap 11 can remain permanently integral with the insert in a reliable manner, the sheath 30 is advantageously made of a material capable of creating an adhesive bond between the outer cylindrical surface 6 of the insert 4 and the wall of the cavity 12 of the hemispherical cap, for example a silicone-based adhesive or the like.

To improve the adhesion between the sheath 30 and at least the cylindrical external surface 6 of the insert 4, the cylindrical external surface of the insert comprises recessed portions 22 constituted by at least one of the following elements: thread, grooves, grooves, grooves, schematically illustrated in the single figure attached to this description.

The sheath 30 is advantageously constituted by a thickness of adhesive applied to the cylindrical surface 6 of the insert 4 before the latter is introduced into the cavity 12 of the hemispherical cap and, to be certain of obtaining good adhesion, it is best to apply an excess amount of glue. However, this excess of glue should not hinder the establishment of the cap on the insert, or the contact of the flat 13 on the shoulder ring 8 as shown in the figure.

For this purpose, the femoral stem further comprises at least one cup 24 capable of receiving an overflow of glue. This bowl, preferably of annular shape, is produced in the shoulder ring 8 so as to be in fluid communication with the space 26 between the wall of the cavity 12 of the hemispherical cap 11 and the cylindrical outer surface 6 of the insert 4, this communication being obtained by the fact that, as illustrated in the figure, an edge of the bowl opens onto the space 26 defined above.

In known manner, such a prosthesis rod is intended to be mounted in cooperation with an acetabulum or the like placed in the acetabular cavity of the hip, and this rod must be able to undergo, relative to the pelvis, rotations substantially identical to those of a valid leg.

It is therefore necessary that the rotation of the femoral stem relative to the acetabulum is in no way limited to an angle of small value.

Also, according to another characteristic of the present invention, when the flat 13 of the hemispherical cap 11 rests on the shoulder ring 8 of the insert 4 as illustrated in the attached figure, the shoulder ring 8 is inscribed in the sphere 28 defined by the hemispherical cap 11. This characteristic allows the femoral stem to perform, relative to the acetabulum, rotations in a solid angle of a value (3 relatively large. Indeed, if the values of the angles solids of opening of the acetabulum and of the lower part 14 of the insert are respectively equal to y and to a, the value P is equal to y - a. In the case where the shoulder ring 8 is inscribed in the sphere 28 defined by the hemispherical cap 11, a is smaller than if the shoulder ring protruded outside of this sphere 28, and the value 5 is therefore relatively large.

In the single figure appended to this description, the value y of the opening angle of the acetabulum is shown diagrammatically and, by way of example, is equal to 7E.

Of course, preferably, the trunks of cones respectively forming the proximal end 3 of the prosthesis tab 1 and the hollow housing 5 of the insert 4 are surfaces of revolution.

In addition, to obtain, when the proximal end 3 fits into the hollow housing 5, a very good congruence between the wall of the hollow housing and the surface of the proximal end, it is very advantageous that the angle at the top of the truncated cone forming the proximal end 3 of the prosthetic leg 1 is slightly greater than the angle at the top of the truncated cone forming the hollow housing 5 of the insert 4, for example a few tens of seconds of angle for a prosthesis intended for an adult person.

Finally, it should be noted that the femoral stem structure according to the invention as illustrated in the single figure appended to this description, is particularly advantageous in the case where the hemispherical cap 11 is produced in at least one of the following materials ceramic, monocrystalline material, polycrystalline material, the insert 4 being made of a metallic material, for example a titanium alloy or the like.
