Abstract

Cet élément fémoral est du type comportant une tige (2), destinée à être ancrée et, éventuellement, scellée dans le cana médullaire d'un fémur réséqué, et supportant une partie cervicale (3) qui reconstitue le col anatomique du fémur et comporte une portée conique (4) destinée à recevoir et retenir une tête sphérique (5) reconstituant la tête du fémur. La partie cervicale (3) est solidaire d'une embase (3a) fixée, de manière amovible et avec le calage angulaire désiré, à l'extrémité proximale (2a) de la tige (2).

Description

ELEMENT FEMORAL POUR PROTHESE DE LA HANCHE
La présente invention concerne un élément fémoral pour prothèse de la hanche, ctest-à-dire un élément comportant une tige destinée à être ancrée dans le canal médullaire du fémur reséqué, et supportant une partie cervicale qui reconstitue le col anatomique du fémur et porte une portée conique destinée à recevoir et retenir une tête sphérique reconstituant la tête du fémur.
Dans ces éléments fémoraux connus de prothèse de hanche, la partie cervicale fait, généralement, partie intégrante de la tige avec laquelle elle forme donc un ensemble monobloc.
Par ailleurs, le col anatomique d'un fémur présente généralement une antéversion d'environ 200.
En outre, lors de la pose d'une prothèse de hanche, il est nécessaire de retendre le système musculaire en latéralisant le fémur.
Il en résulte que, pour pouvoir réaliser des poses de prothèses de hanche, en respectant l'antéversion droite ou gauche, décalées ou non selon qu'il y a nécessité ou non de retendre les muscles, il est nécessaire de disposer de quatre types différents d'éléments fémoraux pour chaque taille de prothèses.
Etant donné qu'il est, généralement, prévu dix tailles différentes de prothèses, il est donc nécessaire de disposer de quarante implants différents.
La présente invention vise à remédier à ces inconvénients en permettant de simplifier considérablement les stocks d'implants.
A cet effet, dans l'élément fémoral qu'elle concerne et qui est du type précité, comportant une tige et une partie cervicale, la partie cervicale est solidaire d'une embase fixée, de manière amovible et, avec le calage angulaire désiré, à l'extrémité proximale de la tige.
Ainsi, il suffit de disposer de dix tiges et de quatre parties cervicales, à savoir, antéversée droite, antéversée gauche, décalée antéversée droite, et décalée antéversée gauche, pour remplacer les quarante éléments fémoraux habituellement nécessaires.
Suivant une forme d'exécution simple de l'invention, les moyens de fixation amovible de l'embase de la partie cervicale à l'extrémité proximale de la tige sont du type à cônes mâle et femelle, avec effet de coincement, éventuellement complèté par vis coaxiale et les moyens de calage angulaire de l'embase sur l'extrémité proximale de la tige peuvent être constitués, tout simplement, par un ergot excentré porté par l'une de ces parties et engageable dans un logement prévu pour le recevoir dans l'autre partie.
De toute façon, l'invention sera bien comprise, à l'aide de la description qui suit, en référence au dessin schématique annexé représentant une forme d'exécution de cet élément fémoral pour prothèse de hanche

Cet élément fémoral est du type comportant une tige 2 supportant une partie cervicale 3 qui reconstitue le col anatomique du fémur et qui présente, à son extrémité libre, une portée conique 4 destinée à recevoir une tête sphérique 5 reconstituant la tête du fémur. La tige 2 est du type destine à être ancrée et, éventuellement, scellée avec du ciment, dans le canal médullaire du fémur.
Selon l'invention, pour permettre de recréer l'antéversion anatomique du fémur réséqué, droite ou gauche, selon le cas, décalée ou non selon qu'il y a nécessité de retendre les muscles, sans avoir à disposer d'un stock important d'éléments fémoraux de dimensions et d'antéversions différentes, la partie cervicale 3 est solidaire d'une embase 3a fixée, de manière amovible et avec le calage angulaire désiré, à l'extrémité proximale 2a de la tige 2.
Dans l'exemple illustré sur le dessin, la fixation amovible de l'embase 3a de la partie cervicale 3 à l'extrémité proximale 2a de la tige 2 est réalisée par l'engagement d'un puits tronconique 3b, ménagé dans la face d'assemblage 3c de l'embase 3a, sur une portée conique 2b de volume complémentaire de celui du puits 3b, même à l'extrémité proximale 2a de la tige 2, en correspondance de ce puits, la pente commune du puits 3b et de la portée 2b étant déterminée pour l'obtention d'une bonne fixation par coincement.
Cette fixation peut d'ailleurs être améliorée au moyen d'une vis 9 traversant axialement la partie cervicale 3 et engagée dans un trou taraudé au même pas 6 et ménagé axialement à la portée conique 2b.
Ce mode de fixation de l'embase 3 à l'extrémité proximale 2a de la tige 2 permet donc une fixation amovible très solide tout en autorisant le calage angulaire rendu nécessaire par la valeur de l'antéversion à recréer, propre à retendre les muscles du patient.
Grâce à ces dispositions, une même embase peut servir à recréer plusieurs antéversions, de sorte que, pour pouvoir recréer pratiquement toutes les conditions opératoires possibles, y compris en taille, au lieu de 40 modèles différents d'éléments fémoraux, comme avec les éléments fémoraux connus, il suffit de disposer de dix tiges 2 et d'un jeu de quatres embases 3, soit une embase antéversée droite, une antéversée gauche, une décalée antéversée droite et une décalée antéversée gauche, soit quatorze pièces au lieu de quarante.
Pour améliorer la tenue du calage de la position angulaire choisie de l'embase 3, il est prévu, en saillie sur la face d'assemblage 3c de l'embase 3, un ergot cylindrique 7 engageable dans l'un d'une série de logements 8 de même dimension, ménagés dans la face d'assemblage 2c de la tige 2, concentriquement à l'axe de la portée conique 2b et à même distance de cet axe que l'ergot 7 précité. Naturellement, chaque logement 8 correspond à l'une des positions angulaires possibles de l'embase 3.

Claims (5)

REVENDICATIONS
1. Elément fémoral pour prothèse de hanche, du type comportant une tige (2), destinée à être ancrée et, éventuellement, scellée dans le canal médullaire d'un fémur réséqué, et supportant une partie cervicale (3) qui reconstitue le col anatomique du fémur et comporte une portée conique (4) destinée à recevoir et retenir une tête sphérique (5) reconstituant la tête du fémur, caractérisé en ce que la partie cervicale (3) est solidaire d'une embase (3a) fixée, de manière amovible et avec le calage angulaire désiré, à l'extrémité proximale (2a) de la tige (2).
2. Elément fémoral selon la revendication 1, caractérisé en ce que les moyens de fixation amovible de l'embase (3a) de la partie cervicale (3) à l'extrémité proximale (2a) de la tige (2) sont du type à cônes mâle (2b) et femelle (3b), avec effet de coincement.
3. Elément fémoral selon la revendication 2, caractérisé en ce que la face d'assemblage (3c) de l'embase (3a) de la partie cervicale (3) présente, en son centre, un puits tronconique (3b) à faible pente et la face d'assemblage (2c) de la tige (2) porte, en son centre, une portée conique (2b) de même dimension que le puits précité et propre à y être engagé et retenu dans la position angulaire précitée.

Elément fémoral selon la revendication 2 ou 3, caractérisé en ce que les moyens de fixation amovible de l'embase (3a) de la partie cervicale (3) à l'extrémité proximale (2a) de la tige (2) comprennent, en outre, une vis e: traversant coaxialement la partie cervicale (3) et engageable dans un trou taraudé (6) au même pas, prévu au centre de la portée conique (2b).

Elément fémoral selon l'une quelconque des revendications, caractérisé en ce que les moyens de fixation amovible de l'embase (3a) de la partie cervicale (3) à l'extrémité proximale (2a) de la tige (2) comprennent, en outre, pour améliorer le calage dans la position angulaire choisie, sur les faces d'assemblage (3c) de l'embase (3a) et (2c) de la tige (2), d'une part, un ergot (7) et, d'autre part, une série de logements (8) dont chacun est propre à recevoir et loger l'ergot (7) et qui sont disposés en arc de cercle autour de l'axe commun du puits (3b) et de la portée conique (2b), le choix du logement (8) dans lequel est engagé l'ergot (7) déterminant la position angulaire de la portée cervicale (3) par rapport à l'extrémité distale (2a) de la tige (2).
