Abstract

CE PROCEDE EST REMARQUABLE EN CE QU'ON EXECUTE SUR TOUT OU PARTIE DU DEVELOPPEMENT DU FILETAGE PERIPHERIQUE D'ANCRAGE, UNE PLURALITE DE SAILLIES ET DE CREUX DE PETITES DIMENSIONS DESTINEES A MULTIPLIER LES SURFACES DE CONTACT DESDITS ELEMENTS AVEC LE TISSU OSSEUX.

Description

L'invention a pour objet un procédé et des moyens d'ancrage d'éléments d'implants vissés dans les tissus osseux et les éléments d'implants obtenus.
L'objet de l'invention se rattache au secteur technique des moyens de réfection des articulations du corps humain.
Notamment, l'invention concerne les cotyles artif i- ciels métalliques qui sont vissés dans le cotyle osseux du bassin après restructuration, et dont la partie intérieure est amé- nagée directement par l'intermédiaire d'un noyau pour coopérer avec une tête fémorale artificielle disposée en bout d'une tige fémorale implantée dans le fémur.
Une autre application intéressante se situe au niveau des queues de tiges fémorales qui sont vissées dans le canal intramédullaire du fémur préparé à cet effet.
Selon l'invention, on a voulu améliorer l'ancrage de ces éléments de prothèses, et surtout obtenir un maximum de surface de contact avec les tissus osseux pour faciliter l'ostéogé- nase.
Pour cela, selon une première caractéristique, le filetage périphérique d'ancrage des éléments d'implants dans les tissus osseux, présente sur tout ou partie de son développement une pluralité de saillies et de creux de petites dimensions destinées a multiplier les surfaces de contact desdits éléments avec le tissu osseux.
Ces caractéristiques et d'autres encore ressortiront de la description qui suit.
Pour fixer l'objet de l'invention sans toutefois le limiter, dans les dessins annexés
La figure 1 est une vue en coupe illustrant un cotyle selon un exemple non limitatif de réalisation, et dont le fond du filetage d'ancrage présente les saillies et creux selon l'invention.
La figure 2 est une vue en plan correspondant a la figure 1.
La figure 3 est une vue partielle en coupe à grande échelle correspondant a la figure 2.
La figure 4 est une vue partielle correspondant a la figure 3, illustrant une variante de réalisation des saillies et creux selon l'invention.
La figure 5 est une vue de face illustrant une tige fémorale dont la queue présente les saillies et creux selon l'invention.
La figure 6 est une vue partielle à grande échelle selon la figure 5.
La figure 7 est une vue partielle a plus grande échelle montrant un autre exemple de réalisation des saillies et creux selon l'invention.
Afin de rendre plus concret l'objet de l'invention. on le décrit maintenant dans des formes non limitatives de réalisation illustrées aux figures des dessins.
Aux figures 1 à 4. on a illustré une première application du procédé concernant les cotyles artificiels. Le cotyle artificiel illustré et référencé en (1) est du type comprenant un fond (la) qui présente des orifices (lb) permettant d-intro- duire des moyens de vissage du cotyle dans son logement cotyloïdien. Ledit filetage tic) est par exemple à profil triangulaire mince assurant une pénétration aisée sans taraudage préalable, et il est établi sur une forme extérieure en ogive tron quée, sans pour cela exclure d'autres formes (cylindro-coniques, coniques, sphériques, etc...
Intérieurement, le cotyle présente différents alésages aménagés pour recevoir et retenir une cupule en polyéthylène (non représentée) qui est indexée en rotation par coopération d'une saillie avec une ouverture axiale (ld) réalisée sur le fond (la). On voit encore que sur la périphérie sont réalisées des encoches (le) équidistantes destinées à faciliter l'autotaraudage.
Selon l'invention, pour multiplier les surfaces en contact du cotyle (1) dans son logement, on prévoit de réaliser sur la surface filetée une multitude de saillies et de creux constituant ainsi des mini surfaces supplémentaires pour développer les surfaces d'ancrage.
Suivant une réalisation préférée quoique non limitative, ces saillies et creux sont exécutées avec un outil coupant approprié a surface active simple ou multiple, pour constituer, en une seule passe ou par plongées successives des mini ou micro filetages (if) à profil triangulaire ou d'autres sections.
Selon les figures 1 et 3, ces filetages (if) sont réalisés seulement au fond des filets du filetage d'ancrage tic) tandis que selon la figure 4, ils sont réalisés à la fois au fond des filets et sur les flancs. Lesdits filetages peuvent encore être réalisés seulement sur les flancs des filets (tic).
On comprend qu'ainsi les surfaces de contact sont multipliées de manière importante et le tissu osseux du logement cotyloïdien peut alors s'introduire dans toutes les micro aspérités lors de la repousse osseuse en améliorant l'ostéogé- nèse.
Aux figures 5 et 7, on a illustré une autre application du procédé concernant les queues (2a) de tiges fémorales (2) vissées dans le canal intramédullaire qui a été préparé pour recevoir cette queue.
Suivant le profil en section des filets (2b) de la queue, on réalise là encore avec un outil coupant approprié des mini ou micro-filetages (2c) soit seulement sur le fond des filets (figures 5 et 6) soit seulement sur les flancs de filets, soit encore sur les flancs et sur le fond (figure 7).
Bien entendu, le procédé selon l'invention peut entre mis en oeuvre pour d'autres éléments de prothèses destinés a être ancrés par vissage dans les tissus osseux.
Les avantages ressortent bien de la description, on souligne encore, la multiplication importante des surfaces de contact entre le cotyle et le logement cotyloidien, ou entre la queue de la tige fémorale et le canal intramédullaire.

Claims (1)

  1. REVENDICATIONS
    -l- Procédé d'ancrage d'éléments d'implants tels que prothèses vissés dans les tissus osseux, caractérisé en ce qu'on exécute sur tout ou partie du développement du filetage périphérique d'ancrage, une pluralité de saillies et de creux de petites dimensions destinées à multiplier les surfaces de contact desdits éléments avec le tissu osseux.
    -2- Procédé d'ancrage selon la revendication i, caractérisé en ce qu'on réalise les saillies et creux sous la forme de mini ou micro filetages (lf-2c).
    -3- Procédé d'ancrage selon la revendication 2, caractérisé en ce qu'on réalise les mini ou micro filetages (lf-2c) seulement au fond des filets du filetage d'ancrage (lc-2b) des éléments.
    -4- Procédé d'ancrage selon la revendication 2, caractérisé en ce qu'on réalise les mini ou micro filetages (lf-2c) seulement sur les flancs des filets du filetage d'ancrage < lc-2b) des 6 ments.
    -5- Procédé d'ancrage selon la revendication 2, caractérisé en ce qu'on réalise les mini ou micro filetages (if-2c) a la fois sur le fond et sur les flancs des filets du filetage d'ancrage (lc-2b) des éléments.
    -6- Moyens de réalisation des saillies et creux selon le procédé de la revendication 1, caractérisés en ce qu'ils sont constitués par un outil coupant à surface active simple ou multiple agissant en une seule passe ou par plongées successives.
    -7- éléments d'implants tels que prothèses obtenus selon le procédé de la revendication 1, du type cotyles artificiels métalliques (1) vissés dans le cotyle osseux du bassin par un filetage d'ancrage (tic) dont tout ou partie de son développement présente les saillies et creux de petites dimensions (If) destinées a multiplier les surfaces de contact desdits éléments avec le tissu osseux.
    -8- éléments d'implants tels que prothèses obtenus selon le procédé de la revendication 1, du type tiges fémorales (2) dont la queue (2a) est vissée dans le canal intramédullaire par un filetage d'ancrage (2b) dont tout ou partie de son développement présente les saillies et creux de petites dimensions (2c) destinées a multiplier les surfaces de contact desdits éléments.
