Abstract
Description
STIMULATEUR CARDIAQUE IMPLANTABLE
The present invention relates to an implantable pacemaker, of the type comprising connection means for at least one stimulation and detection electrode and which is provided with a metal housing which serves as a common or ground electrode for the stimulation and detection circuits.
In a stimulator of this type, the electrodes are used alternately to stimulate and to detect with the exception of a short refractory period following each stimulation pulse when the detection function is switched off. This is of course true for a stimulator operating in synchronous mode. When the pacemaker operates in asynchronous mode, the detection is switched off and the heart is stimulated according to a predetermined fixed rhythm. Thus, according to the asynchronous mode, the electrodes are available for other functions during the time which separates the stimulation pulses. For example, known for programmable pacemakers, first switch the pacemaker to asynchronous mode and then apply programming signals thereto by means of electrodes attached to the patient's body. The external programmer must be synchronized with pacing rate of the pacer so that programming information is sent during the intervals between pacing pulses and can be picked up by the electrodes. There are also known pacemakers provided with a number of electrodes greater than two such as for example atrioventricular stimulators which, in addition to the common or mass electrode, have an electrode intended to stimulate the atrium as well as detect information coming from it, as well as an electrode intended to stimulate the ventricle as well as to detect information coming from this one. However, in this case also, the two electrodes connected respectively to the atrium and at the ventricle are occupied alternately by their stimulation and detection functions.
The object of the present invention is to provide a stimulator which is capable of auxiliary functions, such as for example a programming function, without using the stimulation and detection electrodes for this purpose. In a stimulator according to the invention, there is provided, in order to perform functions other than stimulation or detection, an auxiliary electrode which is mechanically associated with the stimulator housing and which is electrically isolated from the latter.
It is known to mount the connection means for the stimulation and detection electrode (s) inside a piece of material which is not electrically conductive and which is fixed to the metal housing of the stimulator. According to the present invention, in such an embodiment, the auxiliary electrode may have the form of an electrically conductive element mounted in said piece of material which is not electrically conductive and having a contact surface exposed substantially to the level of the surface of said part.
According to an embodiment of the invention relating to a pacemaker comprising telemetry circuits for communication with an external programmer and or an information reading device, the auxiliary electrode is connected to said telemetry circuit and constitutes a transmitting and / or receiving electrode therefor.
In another embodiment, the auxiliary electrode is connected to the input of a circuit intended for controlling the operation of the stimulation circuits and of the electrode or electrodes of the stimulator. According to yet another embodiment of the invention, the auxiliary electrode is connected to the input of a circuit for monitoring physiological signals.
An embodiment of the invention will now be described in more detail, with reference to the accompanying drawings, in which FIG. 1 is a side view of a stimulator according to the invention; Figure 2 is a simple block diagram of the stimulator circuits.
The stimulator shown in Figure 1 has its. electronic components as well as its battery sealed hermetically in a metal case 1 which also serves as common electrode or ground pole for the circuits of the stimulator. At the upper end of the housing 1 is fixed a molded piece 2 of plastic material which houses connection means for the end of a probe or catheter 3 in which extends an electrical conductor which is connected to the opposite end from the probe 3 to a cardiac stimulation electrode 4. In the plastic molded part 2 which is not electrically conductive, there is mounted an electrically conductive element 5, either by molding or by assembly. Element 5 is electrically connected to a circuit inside the housing 1. The conductive element 5 forms an auxiliary electrode, the exposed contact surface of the latter being at the surface of the part 2, OR even slightly protruding therefrom, so that when the stimulator is implanted, the electrode 5 is in contact with the adjacent body tissue.
In FIG. 2, block 6 represents the conventional circuits and components of a stimulator, including the battery.
The electrode 4 is connected to the stimulation output, as well as to the input of the detection circuits of block 6. The common electrode, in the form of the metal housing li is also connected to block 6. The electrode auxiliary 5 according to the invention is connected to a circuit 7 which may consist. for example in a telemetry circuit, a circuit for controlling the operation of the output circuit of the stimulation pulses or a circuit for controlling physiological signals. Circuit 7 has an input link 8 coming from block 6 and an output link 9 to block 6. The power supply of block-7 is ensured by means of a connection 10 and a link 11 with the common electrode 1.
Thanks to the arrangement described above, the pair of electrodes 5, 1 can be used for telemetry or control as explained above independently of the pair of electrodes 4, 1 for stimulation and detection. The fact that the auxiliary electrode 5 is mechanically connected to the metal stimulator housing, there is no additional work involved during implantation.
The location and the exact shape of the electrode 5 can naturally be modified, as long as the electrode 5 is mechanically associated with the housing 1 and electrically isolated from the latter. Other alternative embodiments of the form described and shown in the figures are possible, within the scope of the claims and of the description.
