Abstract

Poultice comprises a perforated adhesive film (2) which maintains a plug (16) of non-adherent and physiologically inert foam expanded and cured in situ to bear on to the layer of grafted skin (14) covering the wounded zone (10) of flesh. Pref. the cellular plug is of silicone or polyurethane resin, pref. tinted/disinfected/scented to render it sterile and inconspicuous, and incorporating a rapid self-curing system which is not uncomfortably exothermic at ambient temps. Less painful and quicker to apply or remove than conventional bandage covers secured by stitching, the expanded plug also applies more uniform pressure upon the graft, esp. where this enters depressions in the damaged flesh, and permits inspection of the development of the graft. Film and plug are easily disengaged after the skin graft is established.

Description

PANSEMENT DESTINE A IMMOBILISER ET A COMPRIMER LES
GREFFES DE PEAU.
DRESSING FOR IMMOBILIZING AND COMPRESSING
SKIN GRAFT.

The invention relates to a new type of dressing for the compression and immobilization of skin grafts.

As we know, a transplant is intended to rem
place losses of skin substances.

Currently, after having trimmed the banks of the
wound, we place the graft on the loss of substance, then every centimeter, we sew on the periphery of the sewing threads, taking good care each time to allow an appreciable length of thread to exceed.

We then place a dressing made of various compresses on the graft, such as oily compresses, soaked compresses, lint, etc., we then tie the suture threads over these compresses, so as to compress and i-mmobilize the graft .

When the graft is taken, the sutures are cut, then the compresses are removed and the knots are removed.

- tout d'abord, elle est longue à réaliser;
- ensuite, la compression n'est pas toujours très homogène;
- en outre, on provoque toujours un certain traumatisme dans les régions de la peau sur lesquelles les fils sont noués et tendus;
obligation de recouvrir le tout d'un pansement;
This method, although widespread, still has many disadvantages
- first of all, it takes a long time to achieve;
- then, the compression is not always very homogeneous;
- moreover, it is difficult for the compresses to penetrate into narrow or closed areas,
- in addition, it always causes a certain trauma in the regions of the skin on which the threads are knotted and stretched;
obligation to cover the whole with a bandage;
- Finally, the dressing has the mower tendency to adhere sometimes to the skin, which is very troublesome when it is necessary to peel it often to check the progress of the graft.

L'invention pallie ces inconvénients. The invention overcomes these drawbacks.

It concerns a dressing which is easy to apply, gives uniform pressure and perfect immobilization of the graft.

This dressing is characterized in that it consists of:
a self-curing foam at room temperature, compatible with the skin and the graft and not adhering thereto, said foam being produced in situ in contact with the graft,
- a perforated field covering said foam and adhering to the banks of the loss of substances and to the foam.

- cette mousse est colorée pour bien être identifiée et lui donner un aspect proche de celui de la peau;
- cette mousse peut être parfumée pour atténuer les odeurs chimiques et même être désinfectée;;
in-situ,
la compression et l'immobilisation de cette
mousse sur la greffe,
le positionnement du pansement sur la péri
phérie de la perte de substances,
la surveillance de l'évolution de la greffe;
In practice
- the self-polymerizing foam is a foam that sets quickly at room temperature, in order to properly fill the entire volume of the graft; this foam must also be non-citotoxic; by way of example, mention may be made of silicone or polyurethane foams
- this foam is colored to be clearly identified and give it an appearance close to that of the skin;
- this foam can be scented to reduce chemical odors and even be disinfected;
- the perforated field is transparent and is self-adhesive to allow
the exudation of the excess foam produced
in-situ,
compression and immobilization of this
foam on the graft,
the positioning of the dressing on the peri
the loss of substances,
monitoring the progress of the transplant;
- the field has perforations of 1 to 2 millimeters distributed every centimeter, in order to obtain a homogeneous compression,
- the drape has two non-adhesive side strips to facilitate the placement and lifting of the drape.

The manner in which the invention can be implemented and the advantages which result therefrom will emerge more clearly from the embodiment which follows, given by way of indication but not limiting in support of the appended figures.

Figure 1 shows a top view of a field according to the invention.

FIG. 2 represents a dressing put in place according to the invention.

In known manner, the loss of substances (10) is countered by cutting the banks (7) in the epidermis (11), the dermis (12) and the hypodermis (13).

The graft (14) is then positioned by fixing it to the banks of the loss of substances (10) by a few traditional stitches (15) without long thread.

The cavity formed by the graft (14) is then filled with a silicone foam (16) during development.

Deux minutes après, la mousse ayant pris sa consistance caoutchouteuse, peut être manipulée c'est-à-dire versée par dessus la greffe (14).This silicone foam called RHODORSIL RTV 5370 from RHONE POULENC is in the form of a fluid paste which, after addition of its catalyst XY70 at room temperature, results in a foam of low elastic density and resistance. The expansion of this foam takes place at room temperature, the expansion being complete after three minutes. Two minutes later, the foam having taken its rubbery consistency, can be handled, that is to say poured over the graft (14).

As soon as this foam (16) is put in place, it is covered by means of a transparent plastic field (1) (see FIG. 1), the central part (2) of which has holes (3) of 1.5 millimeters in diameter spaced every inch, staggered. The side edges (4) and (5) of this field are made of a non-adhesive material while the reverse face (6) of the central part 12) is covered with an adhesive material. This field can be made of transparent plastic such as that marketed by SMITH NEPHEW under the name OP-S1TE, or by an incising field from the company 3M.

The establishment of this field (1) is carried out by placing the adhesive face (6) on the foam itself (16) during expansion and by sticking the periphery of this field on the banks (7) of the loss of substances (10), so that the non-adhesive lateral strips (4) and (5) are clearly released from these banks (7).

En d'autres termes, le champ adhésif (1-2) est collé sur la peau (7) en dépassant très largement la surface de la greffe;
In other words, the adhesive field (1-2) is bonded to the skin (7) by very much exceeding the surface of the graft;
The excess foam (16) is allowed to exude through the holes (3) in the drape (2) and the graft is allowed to set.

After a few days, the field is detached by pulling on the non-adhesive side strips (45), the foam (16) glued to the back (6) of the field (1) is then removed with the field (1).

In an advantageous and sterile form of presentation, the dressing according to the invention is available in a blister or the like, such as a sterile peelable sachet and comprises
- on the one hand, the ready-to-use field,
- on the other hand, means making it possible to prepare the foam in situ, namely, either a compact containing the dose of monomer, a syringe containing the catalyst and an agitator for mixing the whole, or the whole catalyst and monomer presented in a special double syringe.

- l'absence d'adhérence de la mousse à la peau;
- la possibilité lors de la surveillance, d'enlever la mousse, de voir si la greffe a bien pris et de remettre le pansement en place simplement en changeant le champ collant; ;
This new type of dressing has many advantages over previous solutions. We can cite
- the speed and ease of its implementation
- the possibility of obtaining a homogeneous compression and going in every corner of the volume of the graft
- absence of skin trauma
- the absence of a dressing intended to cover the whole
- the possibility of easily monitoring the development of the graft
- the absence of adhesion of the foam to the skin;
- the possibility during monitoring, to remove the foam, to see if the graft has taken well and to put the dressing back in place simply by changing the sticky field; ;
- the possibility of immobilizing the entire region concerned, which makes it unnecessary to immobilize the region on which the graft was placed, for example the lower eyelid.

In this way, this dressing can be used successfully in all skin grafting problems.
