Abstract
Implante quirúrgico para reparar cartílago hialino dañado en una articulación de mamífero, que comprende: una capa de anclaje (130) que está diseñada para presionar contra un área de superficie de hueso preparada de la cual se ha retirado cartílago, una capa de superficie de soporte (110), estando realizada dicha capa de superficie de soporte de un polímero hidrófilo flexible que proporcionará una superficie de articulación lisa y lubricante (112) que se parece al cartílago hialino después de la implantación del implante quirúrgico en una articulación de mamífero, caracterizado porque están previstos unos salientes de anclaje, siendo adecuados dichos salientes de anclaje para extenderse dentro del hueso de acomodación de una manera que resista tensiones de cizalladura impuestas sobre el implante quirúrgico después de la implantación quirúrgica en una articulación de mamífero; la capa de anclaje (130) está provista de por lo menos uno de dichos salientes de anclaje (200) o está diseñada para interactuar con por lo menos uno de dichos salientes de anclaje, y caracterizado además porque el implante quirúrgico es un implante quirúrgico flexible, de modo que dicho implante puede enrollarse e insertarse quirúrgicamente en una articulación a través de una incisión mínimamente invasiva con ayuda de unos medios artroscópicos.
Description
Implante de reparación de cartílago con
superficie de soporte blanda y dispositivo de anclaje flexible.
La presente invención se refiere al campo de la
cirugía y se refiere a unos implantes quirúrgicos para sustituir
cartílago dañado en articulaciones tales como rodillas, caderas y
hombros.
Se han realizados numerosos esfuerzos para
crear diversos tipos de procedimientos e implantes quirúrgicos que
puedan reparar o sustituir segmentos de cartílago dañados o enfermos
en articulaciones de mamíferos tales como rodillas, caderas y
hombros. Esos esfuerzos pueden dividirse generalmente en dos
categorías: (1) implantes resorbibles realizados en materiales que
se disolverán en un lapso de semanas o meses y se sustituirán por
materiales biológicos naturales; y (2) implantes no resorbibles
diseñados para permanecer en una articulación reparada durante
numerosos años y, preferentemente, durante toda la vida restante del
paciente.
La mayoría de tipos de implantes resorbibles
están diseñados para la regeneración del cartílago por células
vivas y no para la sustitución de cartílago por polímeros sintéticos
u otros materiales no vivos. Los esfuerzos por la regeneración del
cartílago utilizando implantes resorbibles implican el transplante
de condrocitos y/o células mesenquimales, y los implantes
resorbibles están diseñados para albergar y nutrir las células
trasplantadas, permitiéndoles que sean "sembradas" en el
implante resorbible una semana o así antes de la implantación,
dando con ello a las células una "ventaja de partida" antes de
que sean trasplantadas a la articulación dañada.
En el momento actual, los dispositivos descritos
están comprendidos en la categoría de implantes no resorbibles
realizados en materiales sintéticos y diseñados para permanecer en
una articulación durante años y, preferentemente, durante la vida
restante del pacientes. Estos implantes se utilizarán para
sustituir, y no para regenerar, un segmento de cartílago que se
haya dañado o esté enfermo. En consecuencia, esta presente
invención, como se describe y se reivindica más abajo, se limita a
implantes no resorbibles diseñados para la sustitución y no para la
regeneración de cartílago dañado.
No obstante, deberá reconocerse que, a medida
que progrese la investigación sobre estos dispositivos, puede ser
posible también adaptar estos dispositivos para uso en: (i)
implantes que utilicen materiales resorbibles; y/o (ii) operaciones
de trasplante de células diseñadas para promover la regeneración de
cartílago por células trasplantadas. En consecuencia, esta presente
invención no está destinada a excluir y negar esas posibilidades;
en lugar de esto, esta invención está concebida como un paso
enfocado y bien dirigido hacia delante en el desarrollo de
implantes no resorbibles, en el entendimiento de que este desarrollo
podría ser extensible también, en el futuro, a un enfoque diferente
y distinto de la reparación de cartílagos.
De una manera similar, en el momento actual,
cualesquiera referencias a "implante", "dispositivo" o
términos similares se limitan a dispositivos que se implantarán
quirúrgicamente en una articulación de mamífero para reparar o
sustituir un segmento de cartílago hialino. Como es conocido por lo
expertos en la materia, cartílago hialino es el término utilizado
para describir el tipo de cartílago que cubre los cóndilos (extremos
redondeados) de las superficies de huesos de articulación, en
articulaciones tales como rodillas, caderas, hombros, dedos,
muñecas, etc.
Otros dos tipos de cartílago (conocidos como
cartílago elástico y fibrocartílago) existen también en mamíferos y
están presentes en las orejas, la nariz, la tráquea, etc. Si se
desea, la invención descrita aquí puede adaptarse para uso en la
reparación de cartílago elástico y/o fibrocartílago. Sin embargo,
deberá reconocerse que, debido a las fuerzas de compresión y
cizalladura implicadas, la sustitución de cartílago hialino dañado
en articulaciones de soporte de carga (especialmente, las rodillas,
las caderas y los hombros) es mucho más difícil que la sustitución
de cartílago en las orejas, la nariz u otras ubicaciones en el
cuerpo. En consecuencia, la única técnica anterior que se considera
relevante aquí implica esfuerzos para reparar cartílago hialino
dañado en rodillas, caderas, hombros o tobillos. Cualquier otro tipo
de reparación de cartílago (incluyendo la reparación de cartílago
dañado en articulaciones de dedo) no tendrá que cumplir o satisfacer
los tipos de tensiones mecánicas que se impondrán a segmentos de
cartílago en articulaciones tales como las rodillas, las caderas o
los hombros.
Un objetivo de la presente invención es
proporcionar dispositivos no resorbibles que puedan insertarse en
una articulación dañada o enferma utilizando procedimientos e
instrumentos "mínimamente invasivos", lo que incluye
procedimientos e instrumentos artroscópicos. Sin embargo, deberá
reconocerse que algunos de los dispositivos descritos aquí pueden
ser capaces de proporcionar implantes mejorados que puedan
implantarse utilizando procedimientos quirúrgicos más antiguos
(denominados frecuentemente procedimientos "clásicos" o de
"articulación abierta"). Además, deberá reconocerse que no
siempre hay una clara línea divisoria entre los procedimientos de
articulación abierta y los procedimientos mínimamente invasivos,
puesto que cualquier cirujano competente minimizará siempre la
cantidad de corte y otros daños y trastornos en músculos,
ligamentos, vasos sanguíneos y otros tejidos, en cualquier
intervención quirúrgica, a la vista de las necesidades de la
operación y del paciente.
En consecuencia, cualquier utilización en la
presente memoria de términos amplios tales como "cirugía" o
"quirúrgico" se considera que incluye procedimientos
artroscópicos y otros procedimientos mínimamente invasivos; y,
siempre que se haga uso de un término restringido tal como
"artroscópico", deberá comprenderse que el uso artroscópico es
el objetivo principal de esta invención, pero que los dispositivos
descritos aquí pueden adaptarse también para uso con otros
procedimientos quirúrgicos (clásicos, de articulación abierta).
La información referente a los antecedentes
sobre implantes no resorbibles en la técnica anterior está contenida
en diversos libros de texto médicos, tales como Campbell's
Operative Orthopedics, y en numerosos artículos y patentes,
tales como la patente US nº 6.132.468 (del mismo inventor que aquí)
y en otras patentes y artículos allí citados. El estado de la
técnica según la patente
US-A-2001/0039455 es reconocido en
el preámbulo de la reivindicación 1.
En sus esfuerzos en curso para desarrollar
implantes mejorados para reparar o sustituir cartílagos, el
solicitante ha considerado que una combinación particular de
componentes dispuestos de una manera particular como la descrita a
continuación puede proporcionar un dispositivo mejorado con una
combinación de rasgos y ventajas altamente útiles. Dichos
dispositivos pueden implantarse completamente a través de incisiones
artroscópicas o mínimamente invasivas para minimizar daños y
trastornos en los tejidos y la vasculatura circundantes. Después de
la implantación, estos dispositivos pueden proporcionar una
combinación de rasgos altamente ventajosos tanto en el lado de
"anclaje" (es decir, el lado de un implante que presiona contra
una estructura de hueso de soporte, tal como un cóndilo femoral o
tibial en una rodilla) como en el lado de "articulación" (es
decir, el lado que está expuesto al fluido sinovial en una
articulación y que se deslizará y frotará contra otro cartílago o
superficie de implante en la articulación).
En consecuencia, un objetivo de la presente
invención es describir un implante mejorado no resorbible para
reparar o sustituir cartílago dañado o enfermo en una articulación
tal como una rodilla, muñeca, tobillo o dedo y, posiblemente, en una
articulación de rótula tal como una cadera o un hombro.
Otro objetivo de la presente invención es
describir un implante mejorado no resorbible para reparar o
sustituir cartílago que tenga (i) una "superficie de soporte"
relativamente dócil y no rígida que emule la superficie de
articulación de un segmento de cartílago natural, y (ii) una porción
de anclaje que sea suficientemente flexible para permitir que se
inserte en una articulación, durante la cirugía, a través de un tubo
de inserción artroscópico.
Otro objetivo de la presente invención es
describir un implante mejorado no resorbible para reparar o
sustituir cartílago que sea suficientemente delgado para permitir
que se enrolle en una forma cilíndrica que pueda insertarse en una
articulación a través de un tubo de inserción artroscópico, y que se
desenrolle después, dentro de la articulación, de una manera que
permite que se sujete con pasadores o se ancle de otra forma a una
superficie de hueso preparada.
Otro objetivo de la presente invención es
describir un implante mejorado no resorbible para reparar o
sustituir cartílago realizado en componentes que sean
suficientemente pequeños para insertarse en una articulación por
medio de procedimientos artroscópicos, y para ensamblarse a
continuación in situ, dentro de la articulación, a fin de
formar un implante completo y funcional.
Estos y otros objetivos de la invención se
pondrán más claramente de manifiesto mediante el sumario, los
dibujos y la descripción siguientes.
Se describe un implante no resorbible para
reparar o sustituir cartílago dañado o enfermo en una articulación
de mamífero (tal como una rodilla). Este implante utiliza una
"superficie de soporte" relativamente blanda y doblable que
está pegada o acoplada de otra forma (preferentemente por medio de
una o más capas o componentes intermedios que proporcionan mayor
resistencia) a un componente de anclaje flexible, tal como un
componente de cerco o de rejilla o una capa moldeada porosa no
planar. Todos los componentes que conforman el implante deberán ser
flexibles, de modo que el implante (o cada componente para un
dispositivo que se ensamble in situ) pueda enrollarse e
insertarse quirúrgicamente en una articulación a través de una
incisión mínimamente invasiva, de preferencia por medios
artroscópicos. En una forma de realización, un componente de anclaje
flexible, que no contiene una superficie de soporte, es insertado
en una articulación dañada a través de un tubo de inserción,
abierto hasta su forma final dentro de la articulación y anclado a
una superficie de hueso preparada, utilizando pasadores, cemento u
otros medios adecuados. A continuación, se inserta en la
articulación un componente con la superficie de soporte, y éste es
desenrollado y desplegado por el cirujano, y fijado al componente
de anclaje. En una forma de realización alternativa, puede usarse un
implante unitario que tenga una superficie de soporte de tipo gel
con alta porosidad en el lado de articulación, y que tenga clavos de
anclaje realizados en el mismo material o un material compatible,
pero con menor porosidad y mayor resistencia en el lado de anclaje
opuesto. En ambas estructuras, el lado de anclaje deberá hacerse de
un material que fomente el crecimiento de tejido de hueso (óseo,
calcificado) para una fijación más fuerte y permanente al hueso de
soporte.
La figura 1 representa un implante flexible de
sustitución de cartílago realizado a partir de componentes que se
insertan por separado en una articulación y se ensamblan a
continuación in situ (dentro de la articulación).
La figura 2 es una vista en perspectiva
recortada de un segmento pequeño de una capa de "barquillo
perforado" que puede utilizarse para proporcionar una interfaz
más fuerte entre una capa de hidrogel blanda y una capa de anclaje
mucho más rígida y dura.
La figura 3 representa un implante unitario
realizado a partir de una matriz fibrosa hidrófila que tiene un
gradiente de densidad/porosidad que permite que la superficie de
articulación lisa sea un hidrogel de alta porosidad, mientras que
los clavos de anclaje tienen mucho menos porosidad y son mucho más
rígidos y fuertes.
La figura 4 representa un cerco flexible de una
rejilla de anclaje de dos componentes que ha sido aplastado para
darle una anchura más pequeña y que ha hecho que sus pasadores de
anclaje sean girados hacia una posición de arrastre/inserción por
medio de bisagras o solapas flexibles que sujetan los pasadores al
cerco.
La figura 5 representa un implante de componente
único como se muestra en la figura 3, que se ha enrollado en un arco
cilíndrico que tiene un ángulo de desplazamiento designado como
\alpha.
Para ilustrar las formas de realización
preferidas de la presente invención, se supone que un cirujano
ortopédico utilizará un implante flexible como se describe aquí
para tratar un paciente que está padeciendo "condromalacia de
grado 4" en una articulación de rodilla. Tras hacer un análisis
artroscópico, el cirujano se da cuenta de que parte de un tabique
femoral medial y parte de la meseta tibial medial, que frota contra
el tabique femoral, han quedado ambas seriamente erosionadas. En
cada una de esas dos superficies, un área de cartílago generalmente
redonda se ha desgastado completamente, exponiendo el hueso
subyacente y generando dolor e incomodidad sustanciales que han
provocado que la rodilla del paciente tenga que ser examinada. Cada
área de hueso descarnado está rodeada por anillos aproximadamente
concéntricos de condromalacia de grado 3 (que es menos severa, con
algún cartílago permaneciendo sobre el hueso), a continuación
condromalacia de grado 2 (moderada), después condromalacia de grado
1 (superficie poco profunda) y con áreas de cartílago generalmente
no dañadas que rodean esas áreas erosionadas.
Utilizando instrumentos artroscópicos, el
cirujano preparará una superficie de hueso dura, usualmente en un
área aproximadamente circular o elíptica, retirando un área de
cartílago de cada uno de los huesos afectados (el fémur y la
tibia). Las superficies de hueso "espongiforme" expuestas son
después "refrescadas" por abrasión. Cuando se han preparado
esas superficies de hueso, el cirujano está listo para insertar dos
implantes, uno para cada superficie de hueso expuesta. Tal como se
utiliza en la presente memoria, cualesquiera referencias a
"superficie de hueso dura" excluyen la porción de canal
intramedular de un hueso o la porción de un hueso que comprenda la
porción interna relativamente blanda que contiene la médula del
hueso. La mayoría de las preparaciones de "superficie de hueso
dura" que proporcionarán las fuertes sujeciones de anclaje
necesarias para una reparación de rodilla o de una articulación
similar tendrá como objetivo una porción de la estructura de hueso
dura conocida como "placa subcondral".
Los implantes descritos en la presente memoria
pueden fabricarse en una variedad de tamaños y formas. Para
asegurar que el implante utilizado se adaptará o, por lo menos, se
aproximará estrechamente al área de superficie de hueso expuesta,
el cirujano puede dar uno cualquiera de varios pasos, incluyendo (i)
elegir un implante prefabricado que está próximo a la forma final
deseada; (ii) recortar el implante según sea necesario para
asegurar la mejor adaptación posible; y (iii) utilizar una pequeña
pieza de material fácilmente manipulada (que puede ser opaca y de
colores brillantes para un alto contraste visual o que puede tener
una rejilla de dimensionamiento impresa sobre ella) como
dispositivo de adaptación para ayudar al cirujano a comprobar el
tamaño y la forma exactos de la superficie de hueso preparada.
Llevando a cabo cuidadosamente el procedimiento, un cirujano
experto puede modificar el implante y/o la superficie del hueso para
asegurar una acomodación de ajuste perfecto entre los dos.
Si se desea, el implante puede tener una forma
total curvada (en vez de plana o planar) para ayudarle a adaptarse
más estrechamente a la superficie del hueso sobre la que él se
ubicará y a la superficie del cartílago que está siendo sustituida.
A menos que se diseñe de otra forma para un fin específico, se
anticipa generalmente que cada implante tendrá típicamente un
espesor de varios milímetros.
Cada implante tendrá una superficie de
"articulación" o de "soporte" en un lado y una superficie
de "anclaje" en el otro lado. Después de que se haya instalado
un implante, la superficie de anclaje será presionada contra el
hueso de soporte y la superficie de articulación o de soporte
permanecerá expuesta al fluido sinovial que lubrica la
articulación. Cuando el paciente camina, la superficie de soporte
sobre un implante femoral frotará y se deslizará contra la
superficie de soporte sobre un implante tibial.
Se anticipa que, para la mayoría de los
pacientes, tanto un implante de tabique femoral como un implante de
meseta tibial se instalará al mismo tiempo, en la misma operación.
Debido a que el cartílago es una forma de hidrogel natural
relativamente blanda, tan pronto como se dañe una superficie de
cartílago en una articulación, esta superficie comienza a ponerse
áspera de un modo que provoque el comienzo de la erosión y el daño
de la superficie del cartílago que frota contra ella. Para el
momento en que la condromalacia de grado 4 lleve a hueso expuesto,
las dos superficies de cartílago
que frotan y se deslizan una contra otra en ese compartimento de la articulación estarán ambas gravemente dañadas.

La figura 1 representa un implante de dos
componentes que se insertará en la articulación en por lo menos dos
piezas diferentes, que se ensamblarán in situ (es decir,
dentro de la articulación en la que funcionará el implante). Una
rejilla de anclaje 200 se implantará de forma segura en una
superficie de hueso preparada y a continuación se insertará un
subconjunto de implante flexible 100 en la articulación y se le
fijará de forma segura a la rejilla de anclaje 200.
El subconjunto de implante flexible 100 mostrado
en la figura 1 tiene tres capas o zonas diferentes. En una forma de
realización preferida, estas tres capas se pegan o se fabrican
juntas de otra forma antes de la implantación en una operación de
fabricación controlada; esto puede asegurar un control de calidad
apropiado y una máxima resistencia y estabilidad, y puede reducir la
cantidad de tiempo requerido para la cirugía si el cirujano no tiene
que llevar a cabo una etapa de ensamblaje que implique estas
capas.
Las tres capas utilizadas en el implante 100
mostrado en la figura 1 incluyen una capa de soporte de hidrogel
110, una interfaz 120 de "barquillo perforado" y una
almohadilla de crecimiento de hueso 130.
La capa de soporte de hidrogel 110 tendrá una
superficie de articulación o soporte muy blanda 112. Deberá
realizarse en un material tal como una matriz proteínica y/o
polimérica fibrosa realizada en fibras de colágeno o en cualquiera
de diversas fibras poliméricas hidrófilas. La mayoría de los
materiales de hidrogel tienen niveles de porosidad más altos que
aproximadamente el 90%, alcanzando frecuentemente el 95% o incluso
más. Puesto que están realizados en fibras hidrófilas y tienen
niveles muy altos de porosidad, permitirán que las moléculas de
agua permeen fácilmente a través de esta capa (o por lo menos a
través de las porciones más superiores de la misma). Si se desea,
los materiales de hidrogel pueden reforzarse, para proporcionar
mayor resistencia, por medio de una red tridimensional de fuertes
fibras tejidas, como se describe con más detalle en la solicitud de
patente US 09/818.811 del mismo solicitante que la presenta; esa
solicitud fue publicada el 21 de febrero de 2002 como solicitud
"A1" U.S. no examinada 2002-0022884 (publicada
con retraso). Si se desea, la capa 110 puede ser cubierta también
con una membrana "semipermeable" más exterior que permitirá una
rápida y fácil permeación por moléculas de agua, pero no por las
moléculas poliméricas mucho mayores que
conforman los lubricantes resbaladizos que están presentes en el fluido sinovial de una articulación de mamífero.

En su lado opuesto, el implante flexible 100
tendrá una almohadilla de crecimiento de hueso 130 con una
superficie de anclaje 132 que será presionada contra una superficie
de hueso después de la implantación. Con el fin de promover un
anclaje sólido y estable, la almohadilla de crecimiento del hueso
deberá promover e inducir preferentemente el crecimiento del tejido
de hueso (óseo, calcificado). Materiales biocompatibles que
promueven el crecimiento de tejido de hueso se han desarrollado
durante las dos décadas pasadas y son conocidos por los expertos en
la materia. Los ejemplos incluyen unas mallas realizadas en
aleaciones metálicas biocompatibles que puede ser "revestidas por
pulverización catódica", si se desea, con mezclas de fosfato de
calcio que emulen hidroxiapatito, la estructura cristalina que
proporciona a los huesos su dureza.
En una forma de realización preferida, un
componente de "interfaz no planar multiperforada" 120 (también
denominado, en la presente memoria, capa de "barquillo
perforado") puede estar dispuesto entre el material relativamente
blando del componente de soporte de hidrogel 110 y el material
sustancialmente más duro de la almohadilla 120 de crecimiento del
hueso. Este tipo de capa de interfaz 120 se describe con más detalle
en la solicitud de patente US número de serie 10/011.933, que se
publicó el 5 de diciembre de 2002 como solicitud "A1" US no
examinada 2002-0183845 (publicada con retraso). Un
segmento pequeño de este tipo de capa de "barquillo perforado"
mostrado en una vista recortada en perspectiva se proporciona en la
figura 2. Este dibujo recortado representa dos de las numerosas
"protuberancias ascendentes" 40 que se ordenarán en un patrón
geométrico en la capa de interfaz 120. Los patrones geométricos
adecuados pueden incluir ordenaciones rectangulares, romboidales o
hexagonales o cualquier otro patrón adecuado.
Cada protuberancia ascendente 40 en la capa 120
tiene uno o más lados o facetas semiverticales 42 (que ascienden
desde la capa 32 de "línea de base" o de "rejilla"), así
como una superficie superior relativamente horizontal 44 (todas las
referencias aquí a direcciones tales como vertical u horizontal
suponen que la rejilla 30 es horizontal). Los lados semiverticales

tener una superficie superior horizontal plana 44 para minimizar
cualquier riesgo de que una superficie afilada o en punta pueda
hincarse en o a través del material blando que la revestirá.
Como se muestra en la figura 2, cada faceta
aproximadamente vertical 42 y cada faceta horizontal 44 tienen un
orificio (que puede denominarse también perforación, orificio,
abertura, etc.) que pasa a través de ellas, mostrado en la figura 2
como orificios horizontales 54, orificios longitudinales 56 y
orificios transversales 58.
Actuando juntos, las protuberancias ascendentes
40 y las numerosas perforaciones 54-58 que pasan a
través de las protuberancias ascendentes en diversas direcciones
crean una superficie exterior multiperforada no planar compleja en
la capa de interfaz 120. Además, como puede verse por las "paredes
interiores" o "paredes inferiores" 45 visibles mostradas en
la figura 2, las facetas de superficie exterior 42 y 44 están
suplementadas además por todavía más superficies o facetas en el
lado inferior de la capa de interfaz 120.
Todas esas superficies o facetas están expuestas
y son accesibles a las moléculas de agua en un compuesto de gel.
Por tanto, todas esas superficies o facetas pueden resistir la
presión del fluido que se impone sobre esas facetas. De esta
manera, la geometría de superficie compleja de la capa de interfaz
120 puede permitir que esta capa utilice flujo de fluido dentro de
un hidrogel para redistribuir y diseminar, de una manera más
equilibrada, uniforme y reforzada, las fuerzas de compresión y/o de
cizalladura que se imponen sobre la superficie de articulación 112
del subconjunto de implante flexible 100.
La capa de interfaz 120 puede pegarse,
fusionarse o asegurarse de otra forma a la almohadilla de
crecimiento de hueso 130 utilizando cualesquiera procedimientos
adecuados (tal como un adhesivo químico, utilizando calor dirigido
para ablandar y fundir las superficies apropiadas de dos capas
poliméricas de modo que éstas se fusionen una con otra, utilizando
una etapa de soldadura si las dos capas están realizadas en metal,
etc.). Alternativamente, puede ser posible fabricar ambas capas a
partir de un único tipo de material, tal como por una combinación
de técnicas que pueden incluir, por ejemplo: (i) moldear un
componente polimérico en un molde que esté ocupado parcialmente, en
la capa inferior, por un componente granular (tal como sal, azúcar,
etc.) que, subsiguientemente, puede disolverse en agua o un
disolvente adecuado; (ii) disolver y retirar los gránulos, creando
así una estructura porosa en la capa inferior que promueve el
crecimiento de las células; y (iii) mecanizar la capa superior por
medio de rayos láser, brocas pequeñas u otros medios adecuados para
crear las múltiples perforaciones no planares en las protuberancias
ascendentes.
El subconjunto de implante 100, cuando esté
completamente fabricado, deberá ser suficientemente flexible para
permitir que se enrolle en forma de un cilindro y que se cargue en
un tubo de inserción artroscópico. Se insertará en una articulación
que ya se haya preparado por colocación de una rejilla de anclaje
200, como se muestra en la figura 1.

La figura 1 ilustra un diseño potencial para una
rejilla de anclaje 200 que tiene un cerco 202, un tabique
longitudinal 204, dos tabiques transversales 206 y 208 y pasadores
de anclaje 220.
Los pasadores de anclaje 220 están ilustrados en
la figura 1 para indicar su emplazamiento. En una forma de
realización preferida, los pasadores de anclaje 220 no estarán
fabricados como una parte integrante de la rejilla de anclaje 200,
puesto que es probable que interfieran con un procedimiento de
inserción artroscópica si están sujetos fijamente a la rejilla 200.
En consecuencia, pueden insertarse en la articulación por el
cirujano después de que la rejilla 200 ya esté en su sitio, y pueden
guiarse a través de ojetes de acomodo o orificios similares
posicionados alrededor de la periferia de la rejilla 200.
En una forma de realización preferida
alternativa, puede ser posible fabricar unos pasadores 220 como
parte de la rejilla de anclaje 200 sujetándolos al resto de la
rejilla 200 por medio de bisagras, solapas u otros medios que
permitirán a los pasadores enrollarse en una forma tubular para
permitir que la rejilla 200 sea aplastada en (y a través de) un
tubo de inserción artroscópico. Además, como se observa
anteriormente, todo el implante que se describe aquí puede
adaptarse para uso en cirugía de "articulación abierta"; si se
prepara para uso en esa realización, los pasadores pueden sujetarse
firme y rígidamente a la capa de anclaje.
En la forma de realización mostrada en la figura
1, la rejilla de anclaje 200 se muestra con "paredes" que
comprenden un cerco periférico 202 y unos tabiques
204-208. Para los fines de esta exposición, se cree
que este tipo de distribución puede ser capaz de proporcionar
varias ventajas potencialmente importantes debido al modo en que
los diversos componentes de pared y los compartimientos internos
creados por el cerco y los tabiques pueden interactuar con la
almohadilla de crecimiento de hueso 130.
Como un ejemplo de una ventaja potencial de esta
distribución, el cerco reforzado y relativamente fuerte 202 puede
proporcionar cualquiera de las diversas opciones potencialmente
útiles para sujetar de manera segura el implante flexible 100 a la
rejilla de anclaje 200, sin tener que utilizar uno o más pasadores
de anclaje o dispositivos similares que necesitarían penetrar la
porción central de la superficie de soporte 112. Además, los
tabiques 204-208 dentro del cerco 202 pueden ayudar
a asegurar que el cerco 202 vuelva a la forma apropiada después de
ser enrollado y forzado a través de un tubo de inserción
artroscópico. En tercer lugar, puede ser posible utilizar un
cemento fuerte (tal como un polimetilmetacrilato (PMMA) o una mezcla
de policarbonato y/o uretano) para pegar químicamente una rejilla
de anclaje 200 a un hueso de una manera que impedirá que el cemento
se aglomere e interfiera con la porosidad deseada del lado inferior
de la almohadilla de crecimiento de hueso 130. Y en cuarto lugar,
la distribución de la rejilla de anclaje 200 puede ayudar a asegurar
que, durante la recuperación inicial después de la cirugía, antes
de que haya ocurrido un crecimiento sustancial del hueso hacia
dentro de la almohadilla de crecimiento de hueso 20, la almohadilla
de crecimiento de hueso 20 permanezca inmovilizada y estacionaria,
y no se deslice o se desplace sobre la superficie del hueso que se
preparó al inicio de la cirugía.
Estos y otros factores sugieren que una forma de
realización del implante descrito aquí, que será preferida por por
lo menos algunos pacientes, es probable que utilice una rejilla de
anclaje que incluirá: (i) un cerco que interactuará con pasadores
de anclaje que serán hincados directamente en material de hueso
duro; (ii) uno o más tabiques internos que dividirán la rejilla de
anclaje en compartimientos internos en una disposición tal como la
disposición mostrada en la figura 1 (pero no necesariamente idéntica
a ésta). Ese tipo de rejilla puede ser capaz de interactuar con una
almohadilla de crecimiento del hueso de maneras que serían difíciles
de proporcionar utilizando una almohadilla de crecimiento del hueso
en solitario (o pegada a otras capas de cubierta).
En la forma de realización mostrada en la figura
1, las paredes que conforman el cerco 202 y los tabiques
204-208 de la rejilla de anclaje 200 están provistos
de una pluralidad de orificios (denominados también aberturas,
lumbreras, orificios, etc.). Dichos orificios (que pueden llenarse
de material permeable que promueva el crecimiento del tejido del
hueso) pueden utilizarse si los ensayos en animales indican que
promueven un mejor anclaje del implante al tejido circundante.
Dicho anclaje puede ser generado por tejido cicatricial, por
crecimiento de tejido óseo o alguna combinación de los mismos,
dependiendo de los pasos de corte y esculpido utilizados en un
paciente particular, que determinarán el tamaño, la forma y el
posicionamiento del área de contacto entre el implante y el
tejido
circundante.

Deberá observarse también que una rejilla de
anclaje de esta naturaleza puede proporcionar la opción de sustituir
periódicamente la almohadilla de soporte de un implante de una
manera que sea relativamente simple y no traumática para el
paciente.

En contraste con el sistema de dos componentes
descrito anteriormente, en el que una rejilla de anclaje 200 es
insertada en una articulación y firmemente anclada a un hueso antes
de que el resto del implante 100 se inserte y se fije a la rejilla
de anclaje, la figura 3 representa la superficie de anclaje 310 de
un implante "unitario" 300 que puede fabricarse como una sola
unidad en una instalación de fabricación y enrollarse a
continuación para su inserción en una articulación a través de un
tubo de inserción artroscópico.

contiene una agrupación de salientes o "clavos" 320 que tienen
un tamaño uniforme y un patrón geométrico simétrico, de modo que
pueden encajar de una manera bastante ajustada y segura en
orificios de acomodación que pueden taladrarse en una superficie de
hueso preparada con ayuda de una plantilla que hará que los
orificios coincidan con el patrón de los clavos 320.
Se anticipa que un tamaño común para un implante
unitario 300 estará típicamente en un intervalo comprendido entre
aproximadamente 1 y aproximadamente 3 centímetros de diámetro y que
los clavos de anclaje 320 estarán típicamente dentro de un intervalo
comprendido entre aproximadamente 3 y aproximadamente 7 mm de
diámetro.
El implante unitario 300 puede fabricarse como
un único elemento, creando un gradiente de densidad/porosidad
bastante grande entre la superficie de soporte de hidrogel 330 y los
clavos de anclaje 320. El hidrogel relativamente blando y abierto
de la superficie de soporte 330 tendrá una porosidad muy alta, que
se anticipa que estará en el intervalo de por lo menos un 80% del
espacio de poros (con menos de un 20% de su volumen ocupado por la
matriz fibrosa). Por contraste, los clavos de anclaje mucho más
rígidos y duros 320 tendrán una porosidad mucho más baja que se
anticipa que estará en el intervalo de aproximadamente 20% o menos
del espacio de poros, con el 80% de su volumen realizado en fibras.
Este tipo de gradiente puede crearse por diversos medios conocidos
que incluyen: (i) utilización de centrifugación, prensado en filtro
u otras técnicas de compactación o presurización para comprimir
fibras hacia dentro de los huecos de moldeo que crearán los clavos
de anclaje; (ii) pasos de reticulación y/o compactación
secuenciales (provocados o controlados por agentes químicos,
radiación, etc.) con material fibroso adicional añadido entre
diferentes pasos de reticulación; o, cualquier otro procedimiento
convencional o descubierto posteriormente que sea adecuado para
crear los tipos de gradientes de densidad y de compactación que se
anticipan aquí.

Tal como se utiliza en la presente memoria, un
cerco de anclaje "flexible", en un sistema bicomponente como
se muestra en la figura 1, indica que el cerco puede ser aplastado
hasta que su anchura sea de aproximadamente un 75% o menos de su
anchura relajada, como se muestra en la figura 4. La figura 4 indica
también que los pasadores de anclaje 206 se han doblado hasta una
posición de "seguimiento" o de "inserción" relativamente
plana por medio de bisagras o solapas 207 que acoplan los pasadores
206 al cerco 202.
Tal como se utiliza en la presente memoria, un
implante unitario "flexible" 300 puede enrollarse en forma de
un arco cilíndrico que tenga un "ángulo de desplazamiento"
(designado como \alpha en la figura 5) que sea de aproximadamente
110 grados o menos. Se anticipa que los implantes unitarios
descritos aquí serán capaces de enrollarse en forma de un tubo
completo.
Estos grados de flexibilidad (o mayores) pueden
ayudar a minimizar el trastorno de tejido cuando se está insertando
un implante en una articulación.

Para ser útil como se describe en la presente
memoria, un implante de reparación de cartílago debe ser flexible
de una manera que permita que se inserte en una articulación, sobre
la que se esté operando quirúrgicamente, a través de una incisión
mínimamente invasiva (de preferencia utilizando dispositivos y
procedimientos artroscópicos). Sin embargo, esto no significa que
el implante deba ser flexible de la misma manera en todas las
direcciones. En lugar de esto, como un ejemplo, puede realizarse
una capa de anclaje de material relativamente rígido (tal como
cordones metálicos o cordones poliméricos relativamente gruesos y
rígidos) en una dirección (tal como la dirección
"longitudinal", que es el eje más largo del implante). Puede
usarse en la dirección "transversal" un material diferente y
más flexible (tal como cordones de nailon o cordones poliméricos
sustancialmente más delgados) para mantener juntos los cordones
longitudinales más gruesos y más rígidos. Este enfoque puede
permitir que una capa de anclaje se enrosque de manera bastante
apretada en una dirección, pero no en la otra. Este tipo de
flexibilidad unidireccional o "selectiva" puede ser altamente
útil para permitir la inserción artroscópica de un implante
enrollado en una articulación.
Pueden utilizarse también (o alternativamente)
otros diversos enfoques para proporcionar un tipo o grado de
flexibilidad deseado y/o manipulable durante el procedimiento de
inserción del implante. Como un ejemplo, puede utilizarse una malla
de anclaje, realizada en un polímero "semicurado" o material
similar, que sea relativamente blanda y flexible para proporcionar
el componente de anclaje. En su forma prequirúrgica blanda y
flexible, esta malla puede enrollarse en forma de un cilindro o
espiral relativamente apretado. Después de que éste se haya
insertado en una articulación, desenrollado y fijado por puntos a
un hueso, dicho cilindro o espiral puede someterse a un tratamiento
(tal como radiación ultravioleta, a través de cables de fibra óptica
que terminan en una espátula aplanada, con un escudo opaco en un
lado para proteger el tejido frente a la radiación UV) que hará que
aquél pase a ser sustancialmente más rígido y más duro.
En algunos aspectos, los procedimientos que se
utilizarán para insertar y anclar un implante flexible como el
descrito aquí pueden ser análogos a desenrollar un segmento de
alfombra (que tiene diferentes capas de respaldo y empenachado) en
un suelo.
Brevemente, el implante flexible (que, en la
mayoría de los caos, presumiblemente no será de más de unos pocos
milímetros de grosor) se enrollará en una forma cilíndrica que debe
ser suficientemente delgada para ser empujada a través de una
incisión mínimamente invasiva, utilizando un tubo de inserción,
hacia dentro de una articulación que se ha preparado para recibir
el implante. En una forma de realización, cuando se expulsa el
implante fuera del extremo del tubo, dicho implante se desenrollará
espontáneamente, buscando recuperar su forma fabricada. Cuando se
ha desenrollado o en un momento posterior, el implante puede
posicionarse, fijarse por puntos según sea necesario y anclarse de
forma permanente.
En una forma de realización alternativa, el
material puede enrollarse alrededor de un núcleo central de una
manera que le permita desenrollarse cuando se mueve a través de una
superficie de hueso bajo el control del cirujano. Cuando se ha
desenrollado, se "fija" dicho material en algunos puntos
alrededor de su periferia por medio de suturas, pequeños pasadores
u otros dispositivos comparables que pueden hincarse en la capa de
hueso exterior relativamente esponjosa de una manera controlada que
les impide provocar subsiguientemente un corte o abrasión. Dichas
suturas o pasadores pueden conectarse al implante por medios
adecuados, tales como pequeños ojetes o bucles que se extienden
hacia abajo desde la almohadilla de crecimiento de hueso 130 o
desde cualquier material fibroso tejido que esté embebido en una
capa exterior blanda 110.
Independientemente de qué procedimiento de
desenrollado se utilice, después de que un implante se haya
desenrollado completamente y se haya posicionado y fijado por
puntos, tal implante puede anclarse permanentemente al hueso.
Deberá observarse que la capa de anclaje no
necesita ser altamente flexible hasta el punto de permitir que el
dispositivo se enrolle en forma de una espiral apretada. En tanto
que se proporcione flexibilidad suficiente en la capa de anclaje
para permitir que el implante se flexione adoptando una forma que no
sea tan ancha como lo será el implante después de que se haya
anclado completamente (por ejemplo, si dos lados o extremos opuestos
del dispositivo pueden enroscarse uno hacia otro para formar una
configuración simple en forma de U o similar), entonces ese grado
de flexibilidad permitirá que el implante se inserte a través de una
incisión "mínimamente invasiva", tal como se interpreta y se
aplica ese término. Al determinar el valor de este tipo de implante
en cirugía en una articulación dañada, la cuestión no es si un
dispositivo flexible encajará a través de un tubo de inserción
artroscópico convencional y estándar. Por el contrario, la cuestión
pertinente es si este nuevo tipo de implante flexible puede
insertarse a través de una incisión más pequeña (lo que provocará
menos daños y trastornos del tejido y de los vasos sanguíneos,
permitiendo así una recuperación más rápida con menos dolor y
cicatrización) que la que se requeriría para un implante no flexible
del mismo tamaño.
De este modo, se han mostrado y descrito unos
medios nuevos y útiles para crear implantes mejorados para reparar
cartílago en articulaciones tales como rodillas. Aunque la presente
invención se ha ejemplificado a título ilustrativo y descriptivo
haciendo referencia a determinadas formas de realización
específicas, resultará evidente para los expertos en la materia que
son posibles diversas modificaciones, alteraciones y equivalentes
de los ejemplos ilustrados. Cualesquiera de dichos cambios que
deriven directamente de las enseñanzas contenidas en la presente
memoria y que no se aparten, por ello, del alcance de la invención,
se consideran cubiertos por la presente invención.
Claims (5)
1. Implante quirúrgico para reparar cartílago
hialino dañado en una articulación de mamífero, que comprende:
- una capa de anclaje (130) que está diseñada para presionar contra un área de superficie de hueso preparada de la cual se ha retirado cartílago,
- una capa de superficie de soporte (110), estando realizada dicha capa de superficie de soporte de un polímero hidrófilo flexible que proporcionará una superficie de articulación lisa y lubricante (112) que se parece al cartílago hialino después de la implantación del implante quirúrgico en una articulación de mamífero,
caracterizado porque están previstos unos
salientes de anclaje, siendo adecuados dichos salientes de anclaje
para extenderse dentro del hueso de acomodación de una manera que
resista tensiones de cizalladura impuestas sobre el implante
quirúrgico después de la implantación quirúrgica en una articulación
de mamífero;
la capa de anclaje (130) está provista de por lo
menos uno de dichos salientes de anclaje (200) o
está diseñada para interactuar con por lo menos
uno de dichos salientes de anclaje,
y caracterizado además porque el implante
quirúrgico es un implante quirúrgico flexible, de modo que dicho
implante puede enrollarse e insertarse quirúrgicamente en una
articulación a través de una incisión mínimamente invasiva con ayuda
de unos medios artroscópicos.

2. Implante quirúrgico flexible según la
reivindicación 1, en el que la capa de superficie de soporte (110)
comprende un hidrogel.
3. Implante quirúrgico flexible según la
reivindicación 1, en el que se utiliza una pluralidad de componentes
de anclaje (200) que se extienden por debajo de la superficie de
hueso preparada para permitir que el implante quirúrgico resista
tensiones de cizalladura que se encontrarán en la articulación que
se está reparando.
4. Implante quirúrgico flexible según la
reivindicación 1, en el que el implante quirúrgico es
suficientemente flexible para permitir que dicho implante se
flexione adoptando una forma alargada que tiene una anchura de
aproximadamente el 75% o menos de su anchura relajada normal.
5. Implante quirúrgico flexible según la
reivindicación 1, en el que el implante quirúrgico es
suficientemente flexible para permitir que se enrolle en forma de un
arco cilíndrico en el que los bordes opuestos del arco tienen un
desplazamiento angular de 110 grados o menos.
