Abstract
Aparato de tratamiento de heridas para el control y/o retraso de la respuesta inflamatoria del cuerpo de un paciente, que comprende una compresa de espuma (11) para la inserción en el lugar de una herida (12), una fuente de vacío conectada en comunicación de fluidos con la compresa de espuma para favorecer el drenaje de fluido, y un vendaje (14) de la herida, caracterizado porque además comprende una compresa de calentamiento y enfriamiento (13) para la aplicación sobre la compresa de espuma, medios para la circulación de un fluido caliente o frío a través de la compresa de calentamiento y enfriamiento, y el vendaje (14) de la herida para el sellado del cierre de la compresa de espuma y la compresa de calentamiento y enfriamiento en el lugar de la herida.

Sistema de cierre asistido por vacío con
provisión de calentamiento y enfriamiento.
La presente invención se refiere a la
cicatrización de heridas. Más específicamente, la presente invención
se refiere al cierre asistido por vacío de heridas en las que se usa
calentamiento y enfriamiento para acelerar o retrasar la función
metabólica del sistema inflamatorio para facilitar la cicatrización
de la herida.
El cierre de las heridas implica la migración
hacia el interior de tejido epitelial y subcutáneo adyacente a la
herida. Esta migración está asistida normalmente por el proceso de
inflamación, por medio del cuál aumenta el flujo sanguíneo y se
activan varios tipos de células funcionales. A través del proceso de
inflamación, el flujo sanguíneo a través de los vasos dañados o
rotos se para por oclusión a nivel capilar, pudiendo empezar después
las operaciones de limpieza y reconstrucción. Desafortunadamente,
este proceso se dificulta cuando una herida es grande o se infecta.
En dichas heridas, se forma una zona de estasis (es decir, un área
en que la hinchazón localizada del tejido restringe el flujo de
sangre a los tejidos) cerca de la superficie de la herida.
Sin suficiente flujo sanguíneo, los tejidos
epiteliales y subcutáneos que rodean la herida no sólo reciben menos
oxígeno y nutrientes, sino que también son menos capaces de combatir
con éxito la infección bacteriana y por tanto son menos capaces de
cerrar la herida de forma natural. Hasta hace poco, estas heridas
difíciles se trataban sólo mediante el uso de suturas o grapas.
Aunque aún se practica extensamente y a menudo de forma efectiva,
dichas técnicas de cierre mecánico tiene su mayor desventaja en que
producen tensión en el tejido cutáneo adyacente a la herida. En
particular, la fuerza de tracción requerida para conseguir el cierre
usando suturas o grapas causa tensiones muy localizadas en el punto
de inserción de la sutura o la grapa. Estas tensiones tienen como

circundantes. Los ejemplos de heridas no tratables fácilmente con
grapas o sutura incluyen las heridas grandes, profundas y abiertas;
úlceras decúbito; úlceras resultantes de osteomielitis crónica; y
quemaduras de segundo grado que se transforman posteriormente en
quemaduras de tercer grado.
Como resultado de estas y otras insuficiencias
de los dispositivos de cierre mecánico se han desarrollado métodos y
aparatos para el drenaje de heridas aplicando presiones negativas
continuas. Se ha observado que cuando se aplican sobre un área
suficiente de la herida, dichas presiones negativas favorecen la
migración de tejidos epiteliales y subcutáneos hacia la herida. La
patente US 5,645,081 revela un aparato según el preámbulo de la
reivindicación 1. En la práctica, la aplicación de una presión
negativa a una herida, llamado normalmente terapia Vacuum Assisted
Closure® (V.A.C.®), implica típicamente la contracción mecánica de
la herida con eliminación simultánea del exceso de fluido. De esta
forma, la terapia V.A.C.® aumenta el proceso inflamatorio natural
del cuerpo, aliviando muchos de los efectos secundarios intrínsecos,
como la producción de edema causado por el incremento de flujo
sanguíneo y la ausencia de la estructura vascular necesaria para el
apropiado retorno venoso.
Aunque la terapia V.A.C.® ha tenido mucho éxito
favoreciendo el cierre de heridas, curando muchas heridas que antes
se pensaba que eran intratables, aún quedan algunas dificultades. Al
ser el proceso inflamatorio único para cada paciente individual,
incluso la aplicación de la terapia V.A.C.® no tiene como resultado
una respuesta lo suficientemente rápida en el cierre de algunas
heridas, especialmente cuando se aplica durante la oclusión y
limpieza inicial y las fases de reconstrucción. Por consiguiente, un
objeto principal de la presente invención es proporcionar un método
y un aparato por medio cuál mejoren las modalidades de la terapia
V.A.C.® a través de la aceleración controlada de la respuesta
inflamatoria.
Adicionalmente, y de nuevo al menos parcialmente
atribuible a la variancia entre pacientes, es posible que una
respuesta inflamatoria iniciada de forma adecuada se pueda llevar
demasiado lejos, teniendo como resultado edema y dolor. Por
consiguiente, otro principio objeto de la presente invención es
proporcionar un método y un aparato por medio cuál mejoren las
modalidades de la terapia V.A.C.® a través del retraso controlado de
la respuesta inflamatoria.
De acuerdo con los objetos anteriores, la
presente invención - un aparato para la aceleración y/o retraso
controlados de la respuesta inflamatoria del cuerpo -
comprende generalmente una compresa de espuma sustancialmente para la inserción en el lugar de una herida, una compresa de calentamiento y enfriamiento para la aplicación sobre el lugar de la herida y un vendaje para sellar el cierre de la compresa de espuma y la compresa de calentamiento y enfriamiento en el lugar de la herida. Según la invención, la compresa de espuma se sitúa en comunicación de fluidos con una fuente de vacío, para favorecer el drenaje de fluido, mientras circula fluido caliente o frío a través de la compresa de calentamiento y enfriamiento para la aceleración o retraso controlado, respectivamente, de la función metabólica de la respuesta inflamatoria de una parte del cuerpo.
comprende generalmente una compresa de espuma sustancialmente para la inserción en el lugar de una herida, una compresa de calentamiento y enfriamiento para la aplicación sobre el lugar de la herida y un vendaje para sellar el cierre de la compresa de espuma y la compresa de calentamiento y enfriamiento en el lugar de la herida. Según la invención, la compresa de espuma se sitúa en comunicación de fluidos con una fuente de vacío, para favorecer el drenaje de fluido, mientras circula fluido caliente o frío a través de la compresa de calentamiento y enfriamiento para la aceleración o retraso controlado, respectivamente, de la función metabólica de la respuesta inflamatoria de una parte del cuerpo.
De acuerdo con la realización preferente de la
presente invención, se añade un medio de calentamiento y
enfriamiento a la terapia de presión negativa conocida previamente,
para controlar la función metabólica local como parte de la
respuesta inflamatoria. Proporcionando calor localizado en
combinación con la terapia ordinaria de presión negativa, la
respuesta inflamatoria global se puede acelerar sinérgicamente
produciendo una rápida oclusión capilar y una iniciación anticipada
de las fases de limpieza y reconstrucción. De la misma forma, en
caso que el médico determine que la respuesta inflamatoria se ha
sobre-activado, se puede aportar enfriamiento
localizado en combinación con la terapia de presión negativa para
retrasar la respuesta inflamatoria del cuerpo, sin sacrificar el
control del edema y otros aspectos de la terapia de presión negativa
proporcionada.
En la realización preferente de la presente
invención, la compresa de calentamiento y enfriamiento comprende una
capa acuosa flexible y transpirable, que comprende generalmente dos
hojas de material unido por RF. Las dos hojas de la compresa están
unidas entre sí por RF en una estructura similar a un gofre, en la
que se forman una pluralidad de aberturas entre una pluralidad de
canales. Las aberturas permiten la transpiración del sudor de la
piel del paciente mientras los canales permiten la circulación,
mediante un tubo de suministro y un tubo de drenaje, de fluido
caliente o frío, según se requiera, a través de la compresa para su
calentamiento o enfriamiento.
Mientras la compresa de calentamiento y
enfriamiento puede situarse dentro o fuera del vendaje de la herida
durante un método de calentamiento, es crítico que la compresa de
calentamiento y enfriamiento se sitúe dentro del vendaje de la
herida durante un método de enfriamiento. De esta manera se puede
minimizar la formación de condensado en el interior del vendaje, que
podría causar que el vendaje adhesivo se aflojara y en último lugar
tuviera como resultado una pérdida de vacío en el lugar de la
herida. En particular, colocar la compresa de calentamiento y
enfriamiento dentro del vendaje de la herida limita el contenido de
sudor circundante al que existe y se genera dentro de la herida, lo
que se minimiza mediante el aspecto de succión de la terapia de
presión negativa.
Como el método de enfriamiento se debe
implementar de esta manera y el médico puede indicar la necesidad de
enfriamiento en cualquier momento tras la iniciación de la terapia
de presión negativa, el método preferente comprende la situación de
la compresa de calentamiento y enfriamiento debajo del vendaje de la
herida, adyacente a la compresa de espuma y el lugar de la herida,
indiferentemente de si el calentamiento o el enfriamiento está
indicado inicialmente. Sobre la situación de la compresa, el vendaje
de la herida se adhiere firmemente alrededor del tubo de suministro
y el tubo de drenaje para impedir pérdidas de vacío.
Finalmente, otras muchas características,
objetos y ventajas de la presente invención se harán aparentes a los
expertos en las técnicas relevantes, especialmente a la luz de las
discusiones anteriores, los dibujos siguientes y la descripción
detallada de un ejemplo y las reivindicaciones adjuntas a ello.
Aunque el ámbito de la presente invención es más
amplio que cualquier realización particular, a continuación se
presenta una descripción detallada de la realización preferente
junto con figuras ilustrativas, en las que los números de referencia
se refieren a componentes y donde:
La figura 1 muestra, en una vista en perspectiva
de un corte parcial, la realización preferente de la presente
invención aplicada al lugar de una herida de un mamífero; y
La figura 2 muestra, en una vista en planta en
sección transversal superior, la compresa de calentamiento y
enfriamiento de la invención de la figura 1.
Aunque los expertos en la técnica reconocerán
fácilmente muchas realizaciones alternativas, especialmente a la luz
de las ilustraciones proporcionadas aquí, esta descripción detallada
es un ejemplo de la realización preferente de la presente invención
- un sistema de cierre asistido por vacío con aporte de
calentamiento y enfriamiento, el ámbito del cuál está limitado sólo
por las reivindicaciones que se anexan aquí.
Refiriéndonos ahora a las figuras, la presente
invención 10 mostrada comprende generalmente una compresa de espuma
11 sustancialmente para la inserción en el lugar de una herida 12,
una compresa de calentamiento y enfriamiento 13 para la aplicación
sobre el lugar de la herida 12 y un vendaje 14 de la herida para el
sellado del cierre de la compresa de espuma 11 y la compresa de
calentamiento y enfriamiento 13 en el lugar de la herida 12. De
acuerdo con la invención, la compresa de espuma 11 se sitúa en
comunicación de fluidos con una fuente de vacío para favorecer el
drenaje de fluidos mientras circula el fluido caliente o frío a
través de la compresa de calentamiento y enfriamiento 13 para la
aceleración o retraso controlados, respectivamente, de la función
metabólica de la respuesta inflamatoria de una parte del cuerpo.
Según la realización preferente de la presente
invención, la compresa de espuma 11, el vendaje de la herida 14 y la
fuente de vacío se implementan de la forma conocida por la técnica
anterior, lo cuál se detalla en la solicitud de patente
estadounidense nº serie 08/517,901, registrada el 22 de agosto de
1995. Mediante esta referencia, la revelación completa de la
solicitud de patente estadounidense nº serie 08/517,901 ("la
solicitud '901"), que incluye las reivindicaciones y los dibujos,
se incorpora aquí como se expone en su totalidad. Adicionalmente,
dicho sistema V.A.C.® ya está disponible comercialmente en Kinetic
Concepts, Inc. de San Antonio, Texas, EEUU y/o sus compañías
filiales.
Como se detalla en la solicitud '901, la
compresa de espuma 11 comprende preferiblemente una espuma de
poliéter o poliuretano de célula abierta altamente reticulada para
una buena permeabilidad de los fluidos de la herida durante la
succión. Como también se detalla en la solicitud '901, la compresa
de espuma 11 se sitúa preferiblemente en comunicación de fluidos,
mediante un manguito 15 de plástico o material similar, con una
fuente de vacío, que comprende preferiblemente un recipiente
sometido a vacío de forma segura a través de comunicación de
fluidos, mediante un filtro de membrana hidrofóbica interpuesto, con
una bomba de vacío. Finalmente, la solicitud '901 también detalla el
vendaje 14 de la herida, que comprende preferiblemente un material
elastomérico cubierto al menos periféricamente con un adhesivo
acrílico sensible a la presión para aplicar un sellado sobre el
lugar de la herida 12.
Según un método revelado, estos componentes
descritos en la solicitud '901 se emplean generalmente según la
técnica, con la excepción que el aporte de calentamiento y
enfriamiento de la presente invención se añade para controlar la
función metabólica local como parte de la respuesta inflamatoria.
Proporcionando calentamiento localizado en combinación con la
terapia de presión negativa ordinaria, la respuesta inflamatoria
global se puede acelerar sinérgicamente produciendo una rápida
oclusión capilar y una iniciación anticipada de las fases de
limpieza y reconstrucción. De la misma forma, en caso que el médico
determine que la respuesta inflamatoria se ha
sobre-activado, se puede aportar enfriamiento
localizado en combinación con la terapia de presión negativa para
retrasar la respuesta inflamatoria del cuerpo, sin sacrificar el
control del edema y otros aspectos de la terapia de presión negativa
proporcionada.
En la realización preferente de la presente
invención, la compresa de calentamiento y enfriamiento 13 comprende
una capa acuosa flexible y transpirable 16, que comprende
generalmente dos hojas 17, 18 de material unido por RF. Las dos
hojas 17, 18 de la compresa están unidas entre sí por RF en una
estructura similar a un gofre, en la que se forman una pluralidad de
aberturas 19 entre una pluralidad de canales 20. Las aberturas 19
permiten la transpiración del sudor de la piel 21 del paciente
mientras los canales 20 permiten la circulación, mediante un tubo
de suministro 22 y un tubo de drenaje 23, de fluido caliente o frío,
según se requiera, a través de la compresa 13 para su calentamiento
o enfriamiento.
Mientras la compresa de calentamiento y
enfriamiento 13 puede situarse dentro o fuera del vendaje 14 de la
herida durante un método de calentamiento, es crítico que la
compresa de calentamiento y enfriamiento 13 se sitúe dentro del
vendaje 14 de la herida durante un método de enfriamiento.
De esta manera se puede minimizar la formación
de condensado en el interior y cerca de los límites del vendaje 14,
que podría causar que el vendaje adhesivo se aflojara y en último
lugar tuviera como resultado una pérdida de vacío en el lugar de la
herida 12. En particular, colocar la compresa de calentamiento y
enfriamiento 13 dentro del vendaje 14 de la herida limita el
contenido de sudor circundante al nivel existente y generado dentro
de los límites de la herida 12, lo que se minimiza mediante el
aspecto de succión de la terapia de presión negativa.
Como el método de enfriamiento se debe
implementar de esta manera y el médico puede indicar la necesidad de
enfriamiento en cualquier momento tras la iniciación de la terapia
de presión negativa, el método preferente comprende la situación de
la compresa de calentamiento y enfriamiento 13 debajo del vendaje 14
de la herida, adyacente a la compresa de espuma 11 y el lugar de la
herida 12, indiferentemente de si el calentamiento o el enfriamiento
está indicado inicialmente. Sobre la situación de la compresa 13, el
vendaje 14 de la herida se adhiere firmemente alrededor del tubo de
suministro 22 y el tubo de drenaje 23 para impedir pérdidas de
vacío.
Aunque la descripción anterior es un ejemplo de
la realización preferente de la presente invención, los expertos en
las técnicas relevantes reconocerán como posibles múltiples
variaciones, alteraciones, modificaciones, sustituciones y
similares, especialmente a la luz de esta descripción, los dibujos
que la acompañan y sus reivindicaciones adjuntas. Por ejemplo, los
expertos en la técnica reconocerán que la compresa de calentamiento
y enfriamiento 13 se puede construir en una amplia variedad de
formas, tamaños y estructuras internas. Una realización alternativa
de este tipo puede comprender la integración de la compresa de
calentamiento y enfriamiento 13 en una versión multicapa del vendaje
14 de la herida. En cualquier caso, como el ámbito de la presente
invención es más amplio que cualquier realización particular, la
descripción detallada anterior no se debe considerar una limitación
de la presente invención, la cuál está limitada sólo por sus
reivindicaciones adjuntas.
La presente invención es aplicable a las
técnicas de cicatrización de heridas.
Claims (4)
1. Aparato de tratamiento de heridas para el
control y/o retraso de la respuesta inflamatoria del cuerpo de un
paciente, que comprende una compresa de espuma (11) para la
inserción en el lugar de una herida (12), una fuente de vacío
conectada en comunicación de fluidos con la compresa de espuma para
favorecer el drenaje de fluido, y un vendaje (14) de la herida,
caracterizado porque además comprende una compresa de
calentamiento y enfriamiento (13) para la aplicación sobre la
compresa de espuma, medios para la circulación de un fluido caliente
o frío a través de la compresa de calentamiento y enfriamiento, y el
vendaje (14) de la herida para el sellado del cierre de la compresa
de espuma y la compresa de calentamiento y enfriamiento en el lugar
de la herida.
2. Aparato según la reivindicación 1 en el que
la compresa de calentamiento y enfriamiento comprende una capa
acuosa flexible y transpirable que comprende un par de hojas
interconectadas (17, 18) de material flexible.
3. Aparato según la reivindicación 2 en el que
las hojas están interconectadas mediante soldadura por RF.
4. Aparato según la reivindicación 2 ó 3 en el
que las hojas están interconectadas en una estructura similar a un
gofre, proporcionando una pluralidad de aberturas (19) entre una
pluralidad de canales (20), por medio de lo cuál las aberturas
permiten la transpiración del sudor de la piel del paciente mientras
los canales permiten la circulación de fluido caliente o frío a
través de la compresa.
