Definitions
- the invention is a dynamic cervical disc prosthesis with damping that fits between the cervical vertebrae by replacing the degenerate intervertebral disc.
- Patent FR2929105 discloses a device of which one of the elements comprises two independent parts of the other of which at least one includes a joint surface and each of the two parts comprising a joint seat delimited by an arcuate wall; said element comprises an O-ring ring of elastically deformable material and one of said seats is sized to receive a radially outer portion of said ring while the other seat is sized to receive a radially inner portion of said ring.
- the functions of this device are complex. Indeed, there is a combination of translational movement, rotation and ball-and-socket movement with damping, thanks to the ring which is elastically deformable.
- This O-ring whose section diameter is less than one millimeter seems undersized compared to the level of load that the vertebrae put on it.
- the complex motions of translation are favorable to vertebral displacements which could generate spondylolisthesis.
- This invention of the prior art therefore presents risks of fragility and inadequacy with the pathologies it is supposed to treat.
- the patent WO2012047279 describes a device comprising two rigid plates between which is placed a deformable core of viscoelastic material, surrounded by an annular fibrous structure bonded to the two plates, itself surrounded by a protective membrane.
- the damping is improved thanks to the significant increase in the amount of viscoelastic material which makes it possible to approach the behavior of a healthy cervical intervertebral disc.
- the wide variety of flexible materials and the large number of flexible parts that make up the device can induce parasitic shear movements potentially harmful to the patient and may compromise the life of the prosthesis.
- the complexity of assembly generates a high manufacturing cost which is an index of lack of inventive step.
- the present invention avoids the complexity by choosing a mode of deformation without stray stress of the viscoelastic element which is one of the essential risks that must be removed to ensure dynamic operation with damping of the device in the long term.
- the present invention combines a ball-type movement with optimized damping while limiting the number of means, and allowing the central viscoelastic means to deform freely during the stresses it undergoes.
- Figure 5 Plate 2/5, top view of the device representing the dynamic cervical disc prosthesis
- Figure 6 Plate 2/5, sectional view of the device representing the dynamic cervical disc prosthesis
- Figure 9 Plate 3/5, top view showing the specific shape of the lower tray ready to accommodate the upper tray of the dynamic cervical disc prosthesis
- Figure 1 1 - plate 3/5, perspective view of the dynamic cervical disc prosthesis being locked
- the dynamic cervical disc prosthesis 1 comprises two upper rigid plates 1 1 and lower 12 enclosing a viscoelastic element 10.
- the upper plates 1 1 and lower 12 have a contact face ridged and slightly curved so as to perfectly marry the concavity of the vertebral bodies and allow a good snap.
- Streaks 1 10, ( Figure 5) have a curvilinear sawtooth profile to impact by pressure in the bone and thus preventing any expulsion of the prosthesis out of its housing. This sawtooth profile follows a curvilinear extrusion that generates a good grip on its entire surface and facilitates the introduction of the prosthesis, which gives great stability to the prosthesis.
- the inner part of the upper plate 1 1 has a circular arch shape 1 1 1 (Figure 6) in the vertical plane, its outer face 1 1 10 (Figure 12) has a spherical geometry which in the outer edge has two flats 1 1 1 1 a and b diametrically opposed allowing the introduction of the viscoelastic element 10 in the lower plate 12 and the locking by rotation of a quarter turn of the upper plate 1 1 in the lower plate 12.
- the bottom of the lower plate 12 comprises a concave base 121 and forms with the circular arch 1 1 1 of the upper plate 1 1 a cavity 1 January 12 containing the viscoelastic means 10 to allow it to deform without loss of volume or stress which allows to damp the applied solicitations.
- the cavity 1 1 12 has a shape of revolution adapted to the deformation of the viscoelastic means 10.
- the lower tray 12 in the form of housing accommodates the lower part 1 January 1 of the upper plate 1 1, thanks to forms specially designed for nesting , said shapes being paired with those of the upper plate 1 1.
- the lower plate 12 has an opening 120 ( Figures 8 and 12) which allows the insertion of the upper plate 1 1 in the lower plate 12, and its locking quarter turn. This technique of mounting by rotating a quarter turn of the upper plate 1 1 in the lower plate 12 makes the prosthesis 1 unmountable once in place by preventing any separation of the two plates 1 1 and 12 in contact with the vertebrae.
- the lower tray 12 in the form of a housing has internal walls of spherical geometry 1 1 13 which are related to the sets of assemblies, to the spherical geometry 1 1 10 of the means 1 1 1, which allows to generate a ball movement with a minimum of friction, very well adapted to the stresses exerted by the vertebral column at the cervical level.
- the bottom of the lower plate 12 comprises a concave base 121 (FIGS. 7 and 12) on which the viscoelastic means 10 rest.
- the central viscoelastic element 10 has a shape of revolution with upper and lower faces 100a and b curved with rays. and r '( Figure 7).
- the base 121 of the lower plate 12 has a concave face of radius R (FIG. 7) greater than the radius r of the convex lower face 100a of the viscoelastic means 10.
- the circular arch 11 1 of the upper plate 11 has a radius of curvature R '(FIG.
- the prosthesis 1 behaves like a ball joint thanks to the combination of the previously described means.
- This movement of the patella is accompanied by flexion-extension movements and rotation of the plates 11 and 12 integral with the vertebrae between which the prosthesis 1 is inserted.
- the central viscoelastic means 10 by virtue of the physico-chemical characteristics of the polymer which constitutes it, provides a protective damping to the prosthesis 1 during mechanical stresses in compression and flexion-extension respecting the anatomical behavior of a healthy intervertebral disc.
- the prosthesis 1 guarantees the patient a functioning perfectly adapted to the biomechanics of the spine with an optimal life span.
