At the present time, many different fixation methods have been used for the problems related to femoral bones. Intramedullary locking nail fixation as being expanded the indications in the last years has been widely used for also femur like in other long (tubular) bones. New nail system searches are because of not being able to be developed the ideal intramedullary fixation method to the problems as especially for the passing problems of distal locking screws, nail and locking screw breakages in usages of nails in
small diameters, not being able to early loading, side effects in connection with long scopy usage period.
In this invention, the different, new intramedullary locking nail system that aims to eliminate the disadvantages of the present locking intramedullary fixation methods being used, and is applied in femur, having multi locking system design, multi functional, providing fixation possibility for all the femur problems except femur heads has been developed.
The invention is the Multi Functional Femoral Intramedullary Nail (FIN) System Having Multi Locking System; and its feature is that the numbers of the surgical devices (equipment, instrument) to be used are few.
The invention is the Multi Functional Femoral Intramedullary Nail (FIN) System Having Multi Locking System; and its feature is that it can be easiliy inserted and extracted and so it decreases the operation time. It can make compression under control. It provides multiplanary stability on distal and proximal. It can easily make locking on both ends (proximal and distal) by lowering to the least or eliminating the necessity of Scopy usage. It prevents nail migration on the ends (proximal and distal migration). It prevents screw and nail breakage. It prevents screw to get out (migration). It prevents translation with a single distal locking screw having a single plan. It provides the most strong fixation possible (maximum stability). It realizes intramedullar fixation allowing early moving by eliminating the need for external fixation. It provides stabilization with a single nail system for all femur fixation problems (except caput femoris). It brings solution with a single nail system for both right and left femur and also for both antegrade and retrograde usage. The features of the Multi Functional Femoral Intramedullary Nail (FIN) (1) Having Multi Locking System; it is solid and round, distal end is wedge-shaped that is compatible with DSLS (12), it can be used with or without reamer, it has titanium flexibility and biocompatibility, the 12- 13cm part (4) of the nail (1) is available in different diameters and lengths. On the one end (4), there are 7 locking holes as 1 oval oblique hole (5) that allows compression and provides locking in 5 different angles, two holes (6), (7) able to be angled 130° as distal and proximal to the nail (1) axis, and able to be angled 10° to right and left according to the multi intentional oblique hole (5) for right and left femur bones, 2 holes (15), (16) that 24 degrees angled with each other in
the sagittal plane, 2 holes (17), (18) that 12 degrees angled with each other in the sagittal plane. On the other end, there are 2 holes (19), (20) that 12 degrees angled with each other in the sagittal plane. The desired ones of these, Distal Supportive Locking Screw (DSLS) (12) especially designed for locking, setscrew or locking screw can be used together. The same nail (1) can be applied from antegrade or retrograde for right or left femur. Locking is succeeded 100% with usage of guide.
Its differences which could be superior to the other nails are those; it has multiple locking system. The desired ones of these, locking screw, (DSLS) (12) or setscrew can be used together. It's multifunctional: It can make static locking, dynamic locking (dynamization or auto-dynamization), primer compression; the same nail (1) system can be used from antegrade or retrograde for right or left femur bone, it provides solution with a single nail system for all femur fixation problems except femur head. It provides multiplanary stability. Because of not existing distal locking screw hole also in the nails in small diameters of the system, the solid nails in small diameters can be used for the femurs having narrow medulla canals; and they have enough biomechanical strength and early moving and loading can be done. Because of not existing distal locking screw hole in the nails in small diameters, nail breakage complication in the region of the screw hole that can be seen in classic nails doesn't happen. Owing to the Distal Supportive Locking Screw (DSLS) (12); lock screw breakage and joint in migration don't happen also in the nails in small diameters, distal locking (in condylar region) is provided with a single DSLS (12). (DSLS) (12) provides maximum resistance (multiplanary stability) to axial loading, rotation, translation and angulation forces, without any nail or screw breakages. Scopy usage for locking with DSLS (12) is not needed, it's used when desired. Locking with the guide system can be succeeded in 100%. If locking with DSLS (12) without scopy usage is provided or not can be understood by checking it with the setscrew in it. It is used for all femur fixation problems (except femur heads): Turn femurun (ba§ haric) tespit problemlerinde kullamhr:
-For all fractures that is open-closed, having simple fragments-segments • For Collum femoris fractures,
• For Trochanteric-subtrochanteric fractures,
• For Diaphiseal fractures,
• For Supracondylar fractures,
• For Condylar-intercondylar fractures,
• For simultaneous fractures in more than one regions of the same femur (ipsilateral fractures),
-In case of malunion or non-union (malunion or its pseudoarthroses)
