Abstract
The invention relates to an assembly comprising a support (1, 3, 4) for intervertebral prostheses and intervertebral prostheses (2) that are provided with different widths and are composed of two final plates (21, 22) with a ventral flange (23, 24) and an intermediate joint core (20). The support (1, 3, 4) is equipped with a base plate (10, 30, 40) which encompasses holes (12, 32, 42) for accommodating one of the intervertebral prostheses (2) and is larger than the maximum width of the intervertebral prostheses (2). A couple of steps (17) are embodied on at least one longitudinal edge (13) of the hole (12, 32, 42). The facing sides (18) of said couple of steps (17) are disposed at a specific distance from each other so as to center the flange (24) of an intervertebral prosthesis (2) that is inserted into the hole (12, 32, 42), thus making it possible to insert intervertebral prostheses (2) having different widths into the holes (12, 32, 42) and retain the same in a centered position in the holes (12, 32, 42) regardless of the actual width thereof.

Die Zwischenwirbelprothesen werden mittels eines speziellen Einsetzinstruments, dass zur Implantation der Zwischenwirbelprothesen ausgebildet ist, aus dem Halter entnommen. Zwischenwirbelprothese und Einsetzinstrument weisen ineinander greifende Vorsprünge und Ausnehmungen auf, um eine positionsgenaue Fixierung der Zwischenwirbelprothese an dem Einsetzinstrument zu erreichen. Damit die Zwischenwirbelprothesen präzise mit dem Einsetzinstrument aufgenommen werden können, sind die Zwischenwirbelprothesen in einem speziellen Halter angeordnet. Dieser ist passgenau auf die Abmessungen der jeweiligen Zwischenwirbelprothese abgestimmt. Für Zwischenwirbelprothesen mit unterschiedlichen Abmessungen sind verschiedene Halter erforderlich. Zur einfachen Handhabung sind die Halter oft zu Magazinen zusammengefasst, die Aufnahmeöffnungen für Zwischenwirbelprothesen mit unterschiedlichen Breiten oder Höhen aufweisen. Dies hat zwar den Vorteil, dass dem Operateur auch intraoperativ eine große Auswahl an Zwischenwirbelprothesen

Falls dies im Interesse einer leichten Entnehmbarkeit nicht gewünscht ist, kann auch ein geringes Spiel von wenigen zehntel Millimetern, vorzugsweise 0,1 bis 0,7 mm, vorgesehen sein. Die Konizität der Flanken 18 erlaubt ein vereinfachtes Einsetzen der Zwischenwirbelprothese 2 in die Öffnung 12 des Halters 1, und zwar sogar in dem Fall eines strammen Sitz.
An den Querkanten 14 der Öffnungen 12 sind Führungsaufsätze 15 angeordnet. Sie weisen an ihren einander zugewandten Innenseiten eine Führungsrinne 16 auf, die mit den Querkanten 14 und den jeweiligen Enden der Längskanten 13 fluchtet. Die Führungsrinne 16 dient dazu, beim Entnehmen der Zwischenwirbelprothese 2 aus dem Halter 1 die Spitze des Einsetzinstruments derart zu führen, dass dessen als Stift und Leiste ausgebildete Vorsprünge in die schlitzartige Vertiefung 25 und die Bohrung 26 der Flansche 23, 24 eingreifen. Ein zweites Ausführungsbeispiel ist in den Fig. 5 und 6 dargestellt. Es kann sich dabei bspw. um tiefgezogenes Stahlblech oder um einen autoklavierbaren Kunststoff handeln. Die Bahn 30 des Halters 3 ist so geformt, dass sich zwei taschenartig ausgebildete Öffnungen 32 ergeben. Anders als in dem ersten Ausführungsbeispiel sind die Öffnungen 32 nach unten und zur Seite hin geschlossen. Ein weiterer Unterschied zu dem ersten Ausführungsbeispiel liegt darin, dass die Längskanten 33 versenkt ausgeführt sind. Sie fluchten nicht mit der oberen Ebene des Bahnmaterials 30, sondern sind um einen etwa der Stärke der Flansche 23, 24 entsprechenden Betrag versenkt. Damit können, wie im Fall des ersten Ausführungsbeispiels, Zwischenwirbelprothesen mit unterschiedlichem Breitenmaß B in die Öffnungen 32 eingesetzt werden, wobei die Seitenflächen 28 des Flansches 24 in Kontakt mit den Flanken 38 des Stufenpaares kommen, um so die Zwischenwirbelprothese 2 in der Öffnung 32 zu zentrieren.A second embodiment is shown in Figs. 5 and 6. Bei dem zweiten Ausführungsbeispiel des erfindungsgemäßen Halters 3 ist dank der versenkten Anordnung der Zwischenwirbelprothese 2 eine sterile Aufbewahrung ermöglicht. Dazu ist eine Abdeckfolie 39 vorgesehen, die sich über den oberen, als Tragplatte 30 fungierenden Teil des Bahnmaterials derart von einem zum gegenüberliegenden Ende erstreckt, dass die beiden Öffnungen 32 bedeckt sind. Damit ist nicht nur eine rationelle und billige, den Halter 3 als Wegwerfprodukt ausführbare Konstruktion ermöglicht, sondern auch eine vereinfachte Handhabung, bei welcher die Zwischenwirbelprothese 2 bis zur Implantation im sterilen Halter verbleibt.In the second embodiment of the holder 3 according to the invention a sterile storage is possible thanks to the recessed arrangement of the intervertebral prosthesis 2. Insbesondere für als Wegwerfhalter aus tiefgezogenem Bahnmaterial 30 ausgeführte Halter 3 kann sich auch eine Ausführung mit lediglich einer Öffnung 32 anbieten.
