Description

Technisches GebietTechnical area

The invention relates to a device for correct positioning of a patient in a recording with a panoramic medical radiography device.

Stand der TechnikState of the art

When taking a panoramic radiograph, the tomographic zone ideally passes through the patient's alveolar ridge. In order to achieve a high recording quality, it is necessary to bring the two elements tomography zone and alveolar ridge arch in the best possible spatial agreement. In this case, the tomography zone on the side of the panoramic X-ray device can, in principle, be adapted within certain limits to the individual shape and position of the jaw of a patient. However, the prerequisite is that they are known to the operator. In this case, the knowledge of the following features is particularly important:
  • the spatial orientation of the occlusal plane (occlusion plane) with respect to a reference point of the panoramic X-ray device; and
  • the individual anatomical expression of the jaw, according to size, shape, anomalies and the like.

Conventionally, the patient's head is fixed to the device for panoramic X-ray exposure via a patient's head positioning with forehead support and / or earlobe, tooth rack, nasion, or chin rest. The necessary alignment of the head is typically via light lines that are projected onto the patient's head and depicting the Frankfurt horizontal and the median levels. The Frankfurt horizontal runs through the right and left porion, the highest point of the external auditory meatus, and the orbital, which represents the lowest point of the orbital bony margin. The projected median plane runs as a sagittal plane from the vertex over the middle of the nose to the chin. It supports the symmetrical positioning of the head.

The position of the occlusal plane is not recorded directly, but only indirectly via the Frankfurt horizontal. For this purpose, the Frankfurt horizontal and the incisors projecting from the panoramic X-ray device onto the head of the patient are closed to the position of the occlusal plane. Although the correlation between the occlusal plane and the horizontal plane of Frankfurt is generally accepted by the anatomy, positioning errors with respect to the patient's Frankfurt horizontal are not excluded.

In order to determine the individual size of the patient's jaw, an optional face size measurement can be performed. However, the derivation of the jaw size is not sufficiently reliable.

Also, there is currently no reliable method for determining the shape of the alveolar ridge arch and possible anomalies of the jaw.

Today's panoramic X-ray devices offer as a result, only a few modification options, since the operator could not use such setting options due to the insufficient knowledge of the individual expression of a patient's jaw anyway.

From the US Pat. No. 6,424,694 B1 For example, a positioning apparatus is known which uses to generate transversal tomograms becomes. This positioning apparatus comprises a fixedly arranged with respect to the X-ray apparatus holder and a pivotable plate, wherein means are provided, with which the angle of rotation between the plate and the holder can be detected. With this positioning, it is possible to adjust the inclination of the occlusal plane by means of the adjustment. However, the positioning apparatus disclosed in D1 is only suitable for creating transversal tomograms.

From the US 6,118,842 X-ray device is known for the production of tomographic images and panoramic panoramic photographs. The patient's head is positioned by means of a horizontally and vertically linearly movable chin rest, wherein input means are provided, with the aid of which desired positions of the chin rest can be adjusted.

The position of the chin rest can be read out via means for storing the object position, wherein a change of the position is to be made via controlled motors, for example via stepper motors.

The invention, as characterized in the claims, the object of the invention is to provide a device for correct positioning of a patient in a recording with a panoramic medical X-ray recorder, a simple and robust detection and / or adjustment of the inclination of the occlusal plane of a patient allowed.

Darstellung der ErfindungPresentation of the invention

This object is achieved by the biting device according to claim 1. Advantageous embodiments of the invention are the subject of the dependent claims.

The invention builds on the state of the art in that a bite device contains a directionally fixed to the X-ray machine holder part and a pivotable against the holder part plate having a Aufbissteil, in which the patient bites. The bite apparatus further comprises means for detecting the tilt angle α associated with the inclination of the occlusal plane between the plate and the holder part, wherein the means for detecting the pivot angle α are arranged in a radiation-free region of the bite device in an X-ray exposure. The area covered by X-rays can thus be kept as free of metal as possible.

The invention is thus based on the idea of detecting the relative position of the patient's occlusal plane to the device via the position determination of a plate lying in the occlusal plane. For this purpose, the angle between the plate and a holder part is measured, which can be arranged relative to the X-ray device with a fixed orientation. From this angle, a signal can then be derived that indicates the inclination of the occlusal plane.

The means for detecting the pivot angle α expediently comprise one or more sensors arranged in the holder part. For example, the sensors may be formed by a light barrier arranged in the holder part, which detects the position of a recess moved up and down by the pivoting of the plate.

The bite device according to the invention may expediently include means for indicating the angular position of the pivotable plate.

Furthermore, drive means are provided for adjusting the height of the holding part and thus for pivoting the plate in a predetermined angular position. Thereby an adaptation to the body size of the patient is possible and the patient can be forced to adjust the inclination of his head of the inclination of the plate, in which a desired angular position of the occlusal plane of the patient is achieved relative to the X-ray device.

In addition, it can be provided that the means indicate the achievement of the predetermined angular position of the plate by optical and / or acoustic signals.

In a preferred embodiment of the bite device according to the invention, the pivotable plate is connected to a rail movable up and down in the holder part, which has a recess for determining the position of the rail in the holder part. From the position of the recess in the holder part can then be closed to the swivel angle α.

The Aufbissteil the bite device is provided in an expedient embodiment for reasons of hygiene with a removable protective cover.

Alternatively, the bite portion may be formed by a replaceable bite insert. The bite insert is preferably formed of a soft material, in particular of a substantially radiolucent material. The production of the bite insert made of closed-cell ethylene foam has proven particularly suitable.

The Aufbissinsatz expediently takes an angle range β of the mandibular arch, which is between 20 ° and 40 °, in particular at about 30 °. As a result, lateral tilting or tilting of the patient's head is largely avoided.

In an advantageous embodiment, the bite insert on its top and bottom underside a bite groove for receiving a portion of the dental arch of the upper and lower jaw of the patient.

The Aufbisseinsatz is advantageously integrally formed and hinged along a folding edge. Suitably, it has on opposite sides of a wedge-shaped tapered projection and a corresponding recess for receiving the projection, for releasably attaching the Aufbisseinsatzes to the pivoting plate.

Further advantageous embodiments, features and details of the invention will become apparent from the dependent claims, the description of the embodiments and the drawings.

Fig. 1 shows a schematic representation of a panoramic X-ray recording device 10, wherein a pivotable Unit 12 carries a radiation source 14 with a diaphragm 16, and an opposite detector camera 18 with a diaphragm 20. The X-ray 22 emanating from the radiation source 14 transilluminates the jaw region of the head 24 of a patient and generates an image signal in the detector camera 18. This is fed to an evaluation unit 26 for evaluation and display in a conventional manner.

The rotary unit 12 is attached to a boom 11 rotatably or pivotally mounted, wherein the boom 11 is mounted vertically adjustable on a column 13. To carry out the height adjustment, a drive 15 is provided. In this way, the X-ray source 14 and the detector camera 18 can be aligned to the size of the patient.

Essential for a high-quality, error-free panoramic radiograph is the alignment of the tomographic zone 30 (FIG. Fig. 2 ) on the mandibular arch comb 32 of the patient. Fig. 2 shows a perfect spatial alignment of the two elements, which ensures a qualitatively perfect recording. Inadequate alignment can lead to the need for a repeat scan, resulting in increased patient exposure and additional material and time.

Fig. 3 shows a perspective view of a bite device generally designated 40 according to an embodiment of the invention in a schematic representation, Fig. 4 a section through the bite device 40 of Fig. 3 , The bite device 40 has a holder 42, to which a thin plate 46 is pivotally connected via a joint 44. On its side facing away from the hinge 44, the thin plate 46 merges into a receiving region 48 formed parallel to the thin plate.

The receiving area 48 has a recess 62, by means of which in a manner described in more detail below, a replaceable Aufbisseinsatz 50 can be quickly and easily attached to the thin plate. At its top and bottom, the bite insert 50 is provided with a bite groove 64 and 66 respectively, each receiving the dental arch of the upper and lower jaw of the patient. This design ensures that the thin plate 46 is parallel to the patient's occlusal plane as it bites into the bite insert 50 during X-ray exposure.

The pivoting angle α of the thin plate 46 is measured via a movable up and down rail 54, which is connected in a region 56 with the thin plate 46, and in the interior 52 of the holder member 42 extends vertically downwards. In its lower portion, the rail 54 is provided with a hole 58. The altitude of the hole 58 is determined by two light barriers, which are indicated by the arrows 60. From the altitude can be deduced the pivot angle of the plate 46. By transmitting the inclination of the plate 46 on the movable rail, the measurement of the pivot angle α is moved far down in a radiation-free region of the holder 42. The X-ray region can thus be kept largely free of metal.

The bite device acts with a in Fig. 1 shown drive means 15 together, so that the rail 54 is moved up or down and thus the thin plate 46 can be brought into a desired position. The associated with the height adjustment pivoting of the thin plate 46 of the patient's head is gently guided in until he has reached the correct slope for the panoramic radiograph.

The height adjustment of the plate 46 can be done interactively by the operator. For example, the operator may press on an adjustment button until the desired inclination is reached. Achieving a previously set angular position, for example α = 105 °, which corresponds to an angle between flap 46 and the horizontal of 15 °, can be indicated by optical and / or acoustic signals.

Alternatively, the drive can drive the plate 46 automatically in the set angular position and stop when reaching the desired position. In addition, the angular position of the plate can be displayed for the operator to check.

The in the FIGS. 3 and 4 Bite insert 50 will now be described with reference to FIGS FIGS. 5 to 7 described in more detail. It shows Fig. 5 in (a) and (b) is a perspective view of the unfolded insert, seen in Fig. 5 (a) diagonally from below and in Fig. 5 (b) diagonally from above. FIG. 6 shows a section through the bite insert along the line VI-VI of Fig. 5 (b) and Fig. 7 represents the Aufbissinsatz in folded position, as he at the bite of the Fig. 3 is appropriate.

In the exemplary embodiment, the bite insert 50 is produced in one piece from closed-cell ethylene foam, a soft, largely X-ray transparent material. On its upper side, the bite insert 50 has bite grooves 64 and 66 which receive a portion of the dental arch of the patient's upper and lower jaws. The bite insert 50 may be folded along a central fold edge 70 defined by notches 72 on the bottom and a center groove 74 on the top of the insert.

On its underside, the bite insert 50 has a wedge-shaped tapered projection 76 which, when folded, engages a corresponding recess 78 disposed on the opposite side of the insert to produce a stable but easily releasable connection. The planar receiving region 48 of the bite device contains a recess 62 (FIG. Fig. 4 ), through which the projection 76 engages when attaching the Aufbissinsatzes, so that the insert 50 is fixedly connected after folding with the thin plate 46 of the bite device. After use, however, the insert can be unfolded again without great effort and thrown away for reasons of hygiene.

In the embodiment of the FIGS. 5 to 7 shown insert takes at a width of 40 mm on an angular range β of the dental arch of about 30 °. In order to meet the different jaw size of the patients, bite inserts are also manufactured and used in other widths. The different widths can be easily distinguished by different color markings or other markings for the user. With such a bite insert the fixation of the patient is very accurate and yet comfortable for the patient.
