Abstract
L'invention concerne un dispositif pour remédier à l'obstruction des sondes d'intubation
chez les patieents intubés et des canules de trachéotomie chez les patients trachéotomisés. Il est
constitué de deux éléments introduits dans la sonde d'intubation ou la canule de trachéotomie par leur extrémité distale:
d'une part une tige flexible (1), arrondie à son extrémité distale (2), munie d'un système repliable (3) à proximité de son ex
trémité distale et pourvue d'une saillie (5) à l'autre extrémité permettant la manipulation; d'autre part d'un tube souple (4),
muni d'un repère bloquant (6) à son extrémité proximale, à l'intérieur duquel la tige (1) est inséré et peut coulisser. Le sys
tème repliable (3) est replié dans le tube souple (4) et le coulissage de la tige centrale (1) à l'intérieur du tube souple (4)
après introduction du dispositif dans la sonde d'intubation ou dans la canule de trachéotomie libère le système repliable

A device for unblocking intubation tubes in intubated patents and tracheotomy cannulas in
tracheotomized patients comprises two elements which are introduced into the intubation tube or
tracheotomy cannula by their distal end, namely a flexible rod (1) rounded at its distal end (2),
provided with a collapsible system (3) near its distal end, and provided with a projection (5) at the
other end to permit manipulation, and a flexible tube (4) provide with a buffer (6) at its proximal
end, into which the rod (1) is inserted and in which it can slide. The collapsible system (3) is col
lapsed in the flexible tube (4) and the sliding of the central rod (1) inside the flexible tube (4) after
the introduction of device into the intubation tube or into the tracheotomy cannula releases the
collapsible system (3), which sweeps the tube and the tracheotomy cannula as the device is with
drawn. This device may be sterile and disposable. Its use is recommended one to four times daily.
Description
DISPOSITIF DE DESOBSTRUCTION DES SONDES D'INTUBATION ET DES CANULES DE TRACHEOTOMIE IN VIVO
La présente invention concerne un dispositif pour prévenir par ramonage l'obstruction des sondes d'intubation chez les patients intubés et des canules de trachéotomie chez les patients trachéotomisés.
Habituellement, chez les patients intubés ou trachéo¬ tomisés le passage d'une sonde d'aspiration trachéale permet l'aspiration des sécrétions bronchiques du patient et la vé¬ rification de la perméabilité de la sonde d'intubation ou de la canule de trachéotomie, mais ne prévient pas l'obstruction progressive ou brutale de la sonde ou de la canule par des dépôts organiques. Les conséquences de cette obstruction sont multiples et parfois graves, pouvant mettre en jeu la vie du patient.
Le dispositif selon l'invention permet de remédier à cet inconvénient. Il comprend deux éléments ayant chacun une extrémité distale introduite dans la sonde d'intubation ou dans la canule de trachéotomie et une extrémité proximale ma¬ nipulée par l'opérateur.
Le premier élément est une tige centrale, flexible dont l'extrémité distale est arrondie pour être non trauma- tique. A proximité de l'extrémité distale est inséré tout au¬ tour de la tige un système repliable qui va permettre le net¬ toyage de la sonde par ramonage. La partie proximale de la tige se termine par une saillie permettant la bonne préhen¬ sion de la tige.
Le deuxième élément est un tube souple à l'intérieur duquel le premier élément est inséré et peut .coulisser. Le tube est plus court que la tige elle-même. Le diamètre ex¬ terne du tube est approprié au calibre des différentes sondes ou canules pour permettre un passage facile. La longueur de l'ensemble du dispositif est fonction de la longueur des sondes d'intubation ou canules de trachéotomie. Elles est toujours largement supérieure à celle de la sonde ou de la
canule. Le système repliable inséré sur la tige et replié à l'intérieur du tube. L'extrémité distale du tube s'applique sur l'extrémité distale arrondie de la tige.
L'ensemble du dispositif est introduit dans la sonde d'intubation ou dans la canule de trachéotomie par l'extré¬ mité distale*. Un repère bloquant sur le tube permet de posi¬ tionner le dispositif par rapport à l'extrémité distale de la sonde d'intubation ou de la canule de trachéotomie.
L'extrémité proximale du tube est traitée pour assu¬ rer une bonne préhension du tube. En faisant coulisser le tube sur la tige de façon à bloquer celui-ci contre la sail¬ lie de la tige servant de poignée, l'utilisateur libère le système replié dans le tube. Il retire alors le dispositif et le système replié assure le ramonage de la sonde d'intu¬ bation ou de la canule de trachéotomie.
Selon une forme de réalisation préférentielle, le sys¬ tème repliable est constitué de poils insérés tout autour de la tige centrale. Un petit cylindre de mousse est fixé à l'extrémité distale de la tige entre les poils et la partie arrondie, également replié à l'intérieur du tube. Libéré en même temps que les poils, il permet de collecter les débris décrochés par les poils. Ce cylindre de mousse est facultatif.
Selon la variante, c'est le ballonnet gonflable qui est intercalé entre l'extrémité distale arrondie et les poils de la tige. Ce ballonnet peut être gonflé à l'aide d'une se¬ ringue par un canal inclu dans la tige.
Selon une autre variante, le système repliable est constitué de lamelles de plastique insérées sur la tige, à nouveau, il est possible d'intercaler un cylindre de.mousse ou un ballonnet gonflable entre 1'extrémité distale arrondie et les lamelles.
Les caractéristiques et avantages de l'invention res- sortiront d'ailleurs de la description qui va suivre à titre d'exemple en référence aux dessins annexés.
La figure 1 représente en coupe, une forme de réalisa¬ tion du dispositif avant utilisation.
Le dispositif représenté sur la figure 1 comprend d'une part une tige centrale flexible (1) dont l'extrémité distale (2) est arrondie, un système repliable (3) inséré tout autour de la tige à proximité de l'extrémité distale, un cylindre de mousse (8), replié, inséré sur la tige (1) entre le système repliable (3) et l'extrémité arrondie (2), une saillie (5) terminant l'extrémité proximale de la tige (1) et permettant sa manipulation. Le dispositif représenté sur la figure 1 comprend d'autre part un tube souple (4) à l'intérieur duquel la tige (1) est insérée et peut coulisser, le système replia¬ ble (3) et le cylindre de mousse (8) sont repliés à l'inté¬ rieur du tube (4). L'extrémité distale du tube (4) s'applique sur l'extrémité distale arrondie (2) de la tige (1). Le tube (4) est muni r'un repère bloquant (6) permettant de position¬ ner le dispositif par rapport à l'extrémité distale de la sonde d'intubation ou de la canule de trachéotomie. L'extré¬ mité proximale du tube (4) est traitée (7) pour assurer une bonne préhension du tube.
La figure 2 représente, en coupe, la même forme de réa¬ lisation du dispositif que celle représentée sur la figure 1 après avoir fait coulisser le tube (4) sur la tige (1). L'ex¬ trémité proximale du tube (4) est alors bloquée sur la sail¬ lie (5). Le système repliable (3) et le cylindre de mousse (8) sont dépliés.
La figure 3 représente, en coupe, la même forme de réa¬ lisation du dispositif que celle représentée sur la figure 1 et sur la figure 2, en place chez un patient intubé après in¬ troduction dans la sonde d'intubation (9) par l'opérateur et avant d'avoir fait coulisser le tube sous la tige. La confor¬ mation du dispositif est celle de la figure 1. Le repère blo¬ quant (6) bloqué contre l'extrémité proximale de la sonde d'intubation permet à l'extrémité distale (2) du dispositif d'être positionnée correctement par rapport à l'extrémité de la sonde d'intubation.
La figure 4 représente, en coupe, la même forme de réa¬ lisation du dispositif que celle représentée sur la figure 1,

la figure 2 et la. figure.3,en place chez le patient intubé après introduction dans la sonde d'intubation (9) par l'opé¬ rateur et après avoir fait coulisser le tube sur la tige. La conformation du dispositif est celle de la figure 2 : le sys¬ tème repliable (3) et le cylindre de mousse (8) sont dépliés à l'intérieur de la sonde d'intubation, l'extrémité proximale du tube (4) est bloquée contre la saillie (5). L'opérateur peut alors remonter le dispositif en le tenant par l'extré-. mité (7) , la tige (1) remontant nécessairement avec le tube (4) car la saillie (5) est bloquée contre l'extrémité distale du tube (4). C'est la remontée du dispositif par l'opérateur qui assure le ramonage de la sonde d'intubation (9).
Le dispositif peut être stérile réalisé avec des maté¬ riaux non toxiques, présenté en étui.
Le dispositif peut être à usage unique. Il est préco¬ nisé d'utiliser ce dispositif de une à quatre fois par jour suivant, les patients.

Claims
REVENDICATIONS
1) Dispositif pour prévenir par ramonage l'obstruction des sondes d'intubation chez les patients intubés et des canules de trachéotomies chez les patients trachéotomisés, caracté¬ risé en ce qu'il comporte deux éléments ayant chacun une ex¬ trémité distale introduite dans la sonde d'intubation ou dans la canule de trachéotomie et une extrémité proximale manipulée par l'opérateur, le premier élément étant constitué d'une tige centrale (1) dont l'extrémité est arrondie (2), un système repliable (3) étant inséré tout autour de la tige à proximité de l'extrémité distale, le deuxième élément étant constitué d'un tube souple (4) à l'intérieur duquel le premier élément est inséré et peut coulisser, le système repliable inséré sur le premier élément étant replié à l'intérieur du second élé¬ ment, l'ensemble étant introduit par l'extrémité distale dans la sonde d'intubation ou la canule de trachéotomie (9) et le coulissage du tube sur la tige permettant de libérer le sys¬ tème repliable.
2) Dispositif selon la revendication 1 caractérisé en ce que le tube comporte un repère bloquant (6) permettant de posi¬ tionner le dispositif par rapport à l'extrémité distale de la sonde d'intubation ou de la canule de trachéotomie.
3) Dispositif selon la revendication 1 ou la revendication 2 caractérisé en ce que l'extrémité proximale du tube (4) est traitée pour assurer la bonne préhension du tube.
4) Dispositif selon l'une quelconque des revendications pré¬ cédentes caractérisé en ce que l'extrémité proximale de la tige (1) se termine par une saillie (5) permettant la bonne préhension de la tige et le blocage du tube (4) contre la saillie (5) pendant le retrait du système hors de la sonde d'intubation ou de la canule de trachéotomie.
5) Dispositif selon l'une quelconque des revendications pré¬ cédentes caractérisé en ce que le système repliable (3) est constitué de poils insérés tout autour de la tige centrale.
6) Dispositif selon l'une quelconque des revendications 1,2,
3, 4, caractérisé en ce que le système repliable (3) est constitué de lamelles de plastique insérées sur la tige.
7) Dispositif selon l'une quelconque des revendications pré¬ cédentes caractérisé en ce qu'un cylindre de mousse (8) est fixé à l'extrémité distale de la tige (1), entre le système repliable (3*) et la partie arrondie (2) terminant la tige (1).
8) Dispositif selon l'une quelconque des revendications 1,2, 3,4,5,6, caractérisé en ce qu'un ballonnet gonflable à l'aide d'une seringue par un canal inclu dans la tige est intercalé entre l'extrémité distale arrondie (2) et le système replia¬ ble (3).
9) Dispositif selon l'une quelconque des revendications pré¬ cédentes caractérisé en ce que le matériel peut être stérile, à usage unique, présenté en étui, réalisé avec des matériaux non toxiques.
