Opfindelsen angår hidtil ukendte methylen-bisphosphonsyrederivater til brug ved human og veterinær terapi, farmaceutisk acceptable salte og let hydrolyserbare estere heraf, fremgangsmåder til fremstilling af de nye forbindelser og farmaceutiske sammensætninger 5 indeholdende de nye forbindelser.

Rj betegner navnlig phenyl eller benzyl, der eventuelt er substitu-30 eret med hydroxy eller methyl, og Rg er navnlig hydrogen eller ligekædet eller forgrenet Cj-C^-alkyl; idet Rj fortrinsvis betegner phenyl eller tolyl, og R,, betegner hydrogen, methyl eller ethyl.
Som ovenfor nævnt angår opfindelsen også salte af forbindelserne med 35 formlen I, som er tetrabasiske syrer, og som således danner mono-, di-, tri- og tetrabasiske salte med baser. Som eksempler på salte dannet af farmaceutisk acceptable, ikke-toxiske baser kan nævnes alkalimetalsalte og jordalkalimetalsalte, såsom lithium-, natrium-, kalium-, magnesium- og calciumsalte, såvel som salte af ammoniak og DK 16/8O8 Bl 2 hensigtsmæssige, ikke-toxiske aminer, såsom lavere al kyl aminer, f.eks. triethylamin, lavere al kanolaminer, f.eks. di ethanolamin eller triethanolamin, procain, cycloal kyl aminer, f.eks. dicyclo-hexylamin, benzyl aminer, f.eks. N-methylbenzylamin, N-ethylbenzyl -5 amin, N-benzyl-jS-phenethylamin, N,N'-di benzyl ethylendiamin eller di benzylamin og heterocykli ske aminer, f.eks. morpholin, N-ethyl-piperidin og lignende.
Da forbindelserne med formlen I som nævnt er tetrabasiske syrer, kan 10 de danne mono-, di-, tri- eller tetraestere. Esterne ifølge den foreliggende opfindelse er sådanne, som er let hydrolyserbare in vivo.
Særligt foretrukne er pivaloyloxymethyl- og ethoxycarbonyloxyethyl-15 estere.
Normale knogler er levende væv, som konstant undergår resorption og genafsætning af calcium, idet nettoeffekten er, at der opretholdes en konstant mineral balance. Den dobbelte proces omtales almi ndelig-20 vis som "knogleomsætning". I normale voksende knogler overstiger mi neral afsætningen mineral resorptionen, hvorimod knogleresorptionen overstiger knogleafsætningen under visse patologiske tilstande, hvilket resulterer i f.eks. hypercalcemi, f.eks. som følge af ondartede lidelser eller primær hyperparathyroidisme eller i osteo-25 porosis. Calciumafsætningen kan finde sted i uønskede mængder og på uønskede steder, hvilket fører til andre patologiske tilstande, f.eks. osteoarthritis, rheumatisk arthritis, atheroscl erose og Paget's sygdom, som karakteriseres af en kombination af en unormalt stor knogleresorption efterfulgt af en unormal calciumafsætning.

Forskning inden for andre områder, som påvirkes af calciumafsætning og/eller -resorption, såsom udviklingen af rensemidler til brug i områder med hårdt vand, eller tandpastaer, som kunne tilvejebringe beskyttelse mod calciumresorption som følge af syreopløsning eller 35 calciumafsætning i form af tandsten, er blevet udført med forbindelser, der danner komplekser med calcium. Eksempler på sådanne kompleksdannende forbindelser er visse bisphosphonater. Det har længe været kendt, at sådanne forbindelser ikke alene kan modvirke krystalvæksten, men også kan forsinke opløsningen af allerede dannede DK 167808 B1 3 krystaller.
På trods af, at der kendes en lang række bisphosphonsyrer, er kun få blevet anvendt klinisk. Et eksempler herpå er 1-hydroxyethyliden-5 1,1-bisphonsyre (etidronat), som er den eneste markedsførte forbindelse af denne type. Den er ikke særlig aktiv og modvirker knoglemineral i sering i doser, der ligger tæt på de doser, der er nødvendige for at opnå en mere hensigtsmæssig modvirkning af knogleresorption.

Dichlormethylen-bisphosphonsyre (clodronat) har vist sig at hæmme knogleresorption uden at påvirke knoglemineral i sering, men har været forbundet med visse ugunstige virkninger.
15 3-Amino-l-hydroxypropyliden-l,l-diphosphonsyre (APD) er mere aktiv på vægtbasis end de to ovenfor nævnte forbindelser, men har også uønskede virkninger.
En fælles ulempe ved de tre ovenfor nævnte forbindelser er deres 20 meget ringe orale absorptionsevne (ca. 2-6%).
Den ideelle forbindelse til formålet er en, som er selektiv, dvs. en forbindelse, som kun påvirker en af processerne i mi neral balancen, enten resorptionen eller afsætningen afhængigt af den patologiske 25 tilstand, der behandles, og den ideelle forbindelse bør ydermere absorberes i stort omfang, når den indgives oralt, og skal være fri for bivirkninger.
Det har nu overraskende vist sig, at de omhandlede forbindelser har 30 de ønskede egenskaber. Sammenlignet med de forbindelser, som vides at have en systemisk aktivitet, udviser de en stor grad af selektivitet kombineret med en god oral absorptionsevne og en lav toxi-citet. 1
Aktiviteten, for så vidt angår hæmning af knogleresorption, bestemmes på følgende måde: Hunrotter (140 - 160 g) anbragtes i metaboliske bure, og urinen opsamledes i 2 dage. Den .forbindelse, der skulle undersøges, opløstes i en 2% bicarbonatpuffer og administreredes dagligt subkutant i en mængde på 16 ømol/kg i 7

4 dage. Urin opsamledes dagligt, og urinens indhold af hydroxyprolin (som er en indikation for knogleresorption, jfr. P.H. Reitsma et al., Calcif. Tissue Int. 32, 145-157 (1980), bestemtes ved den af Prockop og Udenfriend beskrevne metode (Anal. Biochem. 1, 228-239, 5 1960). Inhibering af knogleresorption udtryktes som procent ændring i hydroxyprol inudskil!el se efter 7 dages behandling, idet hver rotte tjente som sin egen kontrol.
Resultaterne opnået ved afprøvning af fire forbindelser med formlen 10 I samt én referenceforbindelse i en dosis på 16 /zmol/kg/dag til grupper å 5 rotter er anført i tabel I.

Som det fremgår af ovenstående tabel, har derivaterne ifølge opfindelsen den fælles nyttige egenskab, at de mere effektivt hæmmer knogleresorption end den nært beslægtede, kendte forbindelse Etidronat.
30 Hæmningen af knogleresorption medfører en nedsat urinudskillelse af hydroxyprolin, som frigøres i forbindelse med knogleresorption.
Evnen til at hæmme knogleresorption gør de omhandlede derivater nyttige ved behandling af knoglesygdomme, der er karakteristiske ved 35 en forøget resorption, f.eks. osteoporose og hypercalcemi som følge af cancer.

Nogle af dichlormethyletherne med formlen II er kendte. Disse kendte såvel som de ukendte kan fremstilles som beskrevet i Chem. Ber. 94, 548 (1961) eller i Recueil Des Travaux Chimiques Des Pays-Bas 90, 556 (1971).

Disse dichlormethylethere omdannes til tetraalkyl estere med formlen IV enten direkte eller gennem mellemprodukter med formlen III.
Mellemprodukterne med formlerne III og IV er ukendte.

Spaltningen af tetraal kyl esterne, f.eks. med kogende saltsyre, fører til forbindelser ifølge opfindelsen, hvori R2 = H.
Tetraalkyl estere kan også spaltes ved en alternativ og mildere 10 metode, der er beskrevet i Jour. f. prakt. Chemie 320. 344 (1978). Behandling med brom-trimethylsilan ved stuetemperatur eller moderat forøget temperatur fører til en tetra-trimethylsilylester, som let spaltes med vand til dannelse af den frie syre med formlen I (R2 = H).

Forbindelserne ifølge opfindelsen, hvori R2 er en al kyl- eller aralkylgruppe, kan som nævnt fremstilles ved alkylering af forbindelser med formlen IV, f.eks. ved behandling med natriumhydrid efterfulgt af et al kyl- eller aral kyl hal id. Reaktionen udføres i et 20 passende opløsningsmiddel, såsom dimethyl formamid eller tetrahydro-furan. En spaltning af tetraalkyl esterne resulterer i forbindelserne ifølge opfindelsen, hvori R2 = al kyl eller aralkyl.
De omhandlede forbindelser er som ovenfor omtalt beregnet til brug i 25 farmaceutiske sammensætninger, der er anvendelige til behandling af osteoporosis, rheumatoid arthritis og andre arthritiske sygdomme, atherosclerose, hypercalcemi som følge af ondartede lidelser eller primær hyperparathyroidisme, Paget's sygdom og andre tilstande med en unormal calciumbalance.

Forbindelserne ifølge opfindelsen kan også anvendes til forebyggelse af tandsten, f.eks. som en bestanddel i tandpasta.
Den nødvendige mængde af en forbindelse med formel I (i det efter-35 følgende betegnet som den aktive bestanddel) til opnåelse af en terapeutisk virkning vil naturligvis afhænge af den givne forbindelse, administrationsmåden og det pattedyr, der er under behandling. En hensigtsmæssig dosis af en forbindelse med formlen I for et pattedyr, som f.eks. befinder sig i en hypercalcemisk tilstand, som DK 167808 B1 7 ovenfor omtalt, er 0,01 til 25 mg pr. kilogram legemsvægt, idet den mest foretrukne dosis er 0,02 til 10 mg/kg legemsvægt, f.eks. 0,05 til 5 mg/kg, hvor den omtalte dosis administreres én eller flere gange om dagen.
5 Når der er tale om profylaktisk behandling af f.eks. postmenopausal osteoporosis, er en hensigtmæssig daglig dosis af en forbindelse med formlen I 0,01 til 10 mg pr. kilogram legemsvægt, idet den mest foretrukne dosis er 0,02 til 5 mg/kg legemsvægt.

Selv om det er muligt at indgive en aktiv forbindelse som sådan, er det undertiden foretrukket at præsentere den som en farmaceutisk formulering. Den aktive bestanddel udgør hensigtsmæssigt fra 0,1 til 99,9 vægtprocent af formuleringen. Dosisenheder af en formulering 15 indeholder hensigtsmæssigt mellem 1 mg og 1 g af den aktive bestanddel. Til brug ved topisk administration udgør den aktive bestanddel fortrinsvis fra 1 til 2 vægt% af formuleringen, men den aktive bestanddel kan udgøre så meget som 10 vægt%.
20 Med udtrykket "dosisenhed" skal der forstås en enhedsdosis, dvs. en enkelt dosis, som kan indgives en patient, og som let kan håndteres og emballeres, idet den derved forbliver som en fysisk stabil enhedsdosis, der omfatter enten det aktive materiale som sådan eller en blanding af dette og faste eller væskeformige farmaceutiske 25 fortyndingsmidler eller bærere.
Sammensætningerne ifølge den foreliggende opfindelse indeholder en effektiv mængde af mindst én forbindelse med formlen I sammen med faste eller væskeformige farmaceutiske bærere og/eller hjælpemidler.
30 Bæreren eller bærerne skal være "acceptable" forstået på den måde, at de skal være forligelige med de øvrige bestanddele i formuleringerne og ikke skadelige for modtageren af disse.
Sammensætningerne omfatter sådanne, som er i en form, der er egnet 35 til oral, rektal, parenteral (herunder subkutan, intramuskulær og intravenøs) eller topisk indgivelse.
Sammensætningerne kan hensigtsmæssigt præsenteres i dosisenhedsform og kan fremstilles ved en hvilken som helst af de fremgangsmåder, DK Ί 67808 ΒΊ 8 som kendes inden for farmacien. Alle fremgangsmåder omfatter det at bringe den aktive bestanddel i forbindelse med bæreren, som udgør én eller flere ekstra bestanddele. Almindeligvis fremstilles formuleringerne ved på ensartet og intim måde at bringe den aktive bestand-5 del i forbindelse med en væskeformig bærer eller, en f i ndelt fast bærer eller begge og derpå om nødvendigt at formgive produktet til opnåelse af den ønskede formulering.
Sammensætninger ifølge den foreliggende opfindelse, som er egnede 10 til oral indgivelse, kan være i form af adskilte enheder, såsom kapsler, poser, tabletter eller pastiller, der hver indeholder en forudbestemt mængde af den aktive bestanddel, i form af et pulver eller granuler, i form af en opløsning eller en suspension i en vandig væske eller en ikke-vandig væske eller i form af en I® olie-i-vand-emulsion eller en vand-i-olie-emulsion. Den aktive bestanddel kan også foreligge i form af en bolus, en grødlignende substans eller en pasta.
Sammensætninger til rektal indgivelse kan foreligge i form af et 20 suppositorium indeholdende den aktive bestanddel og en bærer, såsom kokossmør, eller i form af et lavement.
Sammensætninger, som er egnede til parenteral indgivelse, omfatter hensigtsmæssigt en steril vandig præparation af den aktive bestanddel, som fortrinsvis er isotonisk med modtagerens blod.
Sammensætninger, som er egnede til intra-artikulær indgivelse, kan foreligge i form af en steril vandig præparation af den aktive bestanddel, som kan foreligge i mi krokrystallinsk form, f.eks. i 30 form af en vandig, mikrokrystallinsk suspension. Liposomale formuleringer eller bionedbrydelige polymersystemer kan også anvendes til at præsentere den aktive bestanddel ved intra-artikulær indgivelse. 1
Sammensætninger, som er egnede til topisk indgivelse, omfatter væskeformige eller semivæskeformige præparater, såsom linimenter, lotions, applikationer; olie-i-vand- eller vand-i-olie-emulsioner, såsom cremer, salver eller pastaer, herunder tandpastaer, eller opløsninger eller suspensioner, såsom drops.

Ud over de ovenfor nævnte bestanddele kan sammensætningerne ifølge opfindelsen indeholde én eller flere yderligere bestanddele, såsom fortyndingsmidler, puffere, smagsstoffer, bindemidler, overfladeaktive midler, fortykkelsesmidler, smøremidler, konserveringsmidler, 5 f.eks. methylhydroxybenzoat (herunder antioxidanter), emulgeringsmidler og lignende.
Sammensætningerne kan yderligere indeholde andre terapeutisk aktive forbindelser, som almindeligvis anvendes ved behandlingen af de 10 ovenfor nævnte patologiske tilstande, f.eks. vitamin Dg og D3 og hydroxylerede derivater, f.eks. la-hydroxy-vitamin· D^, lcc-hydroxy-vitamin Dg, la,25-dihydroxy-vitamin D^, la,25-dihydroxy-vitamin Dg, calcitonin (human, svine eller laks), mitramycin, natriumfluorid, østrogener og ikke-steroide antiinflammatoriske lægemidler, f.eks.
15 acetylsalicylsyre, indomethacin, naprosyn og timegadin.
De omhandlede forbindelser kan administreres til en patient, som er i en af de ovenfor nævnte patologiske tilstande, i en daglig dosis (for voksne) på fra 0,7 til 1750 mg, fortrinsvis fra 1,5 - 1000 mg, 20 og ved veterinær brug i daglige doser på fra 0,01 til 25 mg/kg legemsvægt.
Eksempel 1 (ej ifølge opfindelsen) 25 (n-Butoxvmethvlen)-bisphosphonsvre. tris-benzvlaminsalt
En blanding af tetraethyl (n-butoxymethylen)-bisphosphonat (10,0 g) og 6N saltsyre (60 ml) til bagesval edes natten over. Den resulterende blanding inddampedes til tørhed i vakuum. Den resterende saltsyre 50 fjernedes ved gentagen inddampning med absolut ethanol. Det rå produkt opløstes igen i absolut ethanol (100 ml), og benzylamin (12,2 ml) tilsattes under omrøring. Det krystallinske salt isolere-des ved filtrering og tørredes i vakuum.
NMR (CD30D): δ = 0,86 (t, 3H), 1,1-1,8 (m, 4H), 3,70 (t, IH), 3,80 35 (t, 2H), 4,04 (s, 6H) og 7,3-7,7 (m, 15H) ppm.

Tetraethyl -(phenoxvmethvlen)-bi sphosphonat 10
Eksempel 2 5 Diethylphosphit (20,6 ml) tilsattes dråbevis ved 20°C til en omrørt suspension af 55% natriumhydrid (6,24 g) i dimethyl formamid (90 ml).
Efter omrøring i yderligere 15 minutter afkøledes blandingen til 0°C, og dichlormethylphenyl ether (9,27 g) tilsattes dråbevis. Omrøring fortsattes natten over ved 45°C, og blandingen afkøledes i 10 isbad, fortyndedes med vand, neutraliseredes og ekstraheredes med ethyl acetat. Den organiske fase vaskedes gentagne gange med vand, tørredes og inddampedes i vakuum til frembringelse af en olie, som blev oprenset ved kromatografi på silicagel. Elueringsmiddel: ether-acetone 85:15. Den rene, omhandlede forbindelse opnåedes i 15 form af en farveløs olie.
NMR (CDC13): δ = 1,22 (t, 6H), 1,25 (t, 6H), 4,1-4,5 (m, 8H), 4,87 (t, IH) og 6,9-7,5 (m, 5H) ppm.
Eksempel 3 20 (Phenoxvmethvlenl-bisphosohonsvre. tris-benzvlaminsalt
Denne forbindelse fremstilledes som beskrevet i eksempel 1 ved at erstatte tetraethyl-(n-butoxymethylen)-bisphosphonat med tetra-25 ethyl-(phenoxymethylen)-bisphosphonat. Farveløst, krystallinsk salt.
NMR (D20): δ = 4,07 (s, 6H), 4,52 (t, IH), 6,8-7,4 (m, 5H) og 7,40 (s, 15H) ppm. HD0 = 4,66 ppm blev anvendt som intern reference.
Eksempel 4 30 (Phenoxvmethvlen)-bi sphosphonsvre
En blanding af tetraethyl-(phenoxymethylen)-bisphosphonat (20 g) og 6N saltsyre (150 ml) til bagesval edes natten over, og den 55 resulterende opløsning inddampedes til tørhed i vakuum. Den resterende saltsyre fjernedes ved gentagen inddampning med absolut ethanol. Resten optoges i acetone (7 ml), og ether (200 ml) blev tilsat. Det krystallinske produkt filtreredes, vaskedes med ether og tørredes i vakuum.

Di natri um-(2-naohthvloxvmethvlen)-bi sphosphonat
Ved at følge fremgangsmåden beskrevet i. eksempel 10 omdannedes 50 tetraethyl-(2-naphthyloxymethylen)-bisphosphonat til den omhandlede forbindelse i form af den frie syre, som opløstes i absolut ethanol og behandledes med et overskud af benzylamin til dannelse af et krystallinsk tris-benzylaminsalt. Den frie syre frigjordes fra tris-benzylaminsaltet i vand ved behandling med et overskud af 55 "Dowex" 50 W (H+-form). Ionbytningsharpiksen filtreredes, og fil tratet titreredes med 2N natriumhydroxid til pH=5,5. Koncentration i vakuum og tilsætning af acetone gav et farveløst, krystallinsk dinatriumsalt, som isoleredes ved filtrering og tørring i vakuum.
NMR (D20): δ = 4,78 (t, J=16 Hz, IH) 7,2-7,6 (m, 4H) og 7,7-7,9 (m, DK 167808 ΒΊ 14 3H) ppm. HDO = 4,66 ppm anvendtes som intern reference.
