Description

Hüftstiel für ein künstliches Hüftgelenk mit Kragen
Beschreibung
Die Erfindung betrifft einen Hüftstiel für ein künstliches Hüftgelenk mit proximalem Kragen, mit dem sich der Hüftstiel auf dem für die Implantation des Gelenks vorbereiteten, resezierten Femurknochen abstützt.
Der Femurknochen muß darüber hinaus zur Vorbereitung für die Implantation des Hüftstieles mittels einer Raspel bearbeitet werden, mit welcher der Knochenmarkraum für die Aufnahme des Stiels ausgeraspelt wird. Danach kann der Hüftstiel in den ausgeraspelten Raum eingeführt und mit mehreren Schlägen in seinen endgültigen Sitz gebracht werden. Entweder ist zur Dauerfixation ein Zement (PMMA) vorgesehen oder aber das Implantat weist eine spezielle Oberfläche auf, durch welche hindurch Knochentrapekel wachsen können.
Beim Eintreiben des Implantates (mit einem Einschlaginstrument) neigt dieses aufgrund der Form des Knochenmarkraumes dazu, in eine varische Stellung zu kippen. Dies hat selbstverständlich Auswirkungen für die Stellung des Gelenkkopfes des künstlichen Hüftgelenks und damit für die spätere Stellung im Knochen.
Aufgabe der vorliegenden Erfindung ist es, diesem Phänomen durch eine konstruktive Maßnahme entgegenzuwirken, also den Hüftstiel beim Eintreiben in den Femurknochen daran zu hindern, eine Varusstellung einzunehmen.
Gelöst wird diese Aufgabe durch einen Hüftstiel mit den Merkmalen des kennzeichnenden Teils des Anspruchs 1. Weitere vorteilhafte Ausgestaltungen ergeben sich aus den Unteransprüchen.
Demgemäß wird vorgeschlagen, daß der Hüftstiel einen geraden Rücken oder geraden lateralen Verlauf aufweist, daß im oberen proximalen lateralen Bereich ein sattelförmiger Preßhügel und im weiter unteren oder distalen ventralen Bereich ein weiterer Preßhügel angeformt ist.
Durch die beiden Preßhügel wird dem Hüftstiel beim Eintreiben in den Knochenmarksraum des Femur ein Kippmoment verliehen, welches der Neigung, in die Varusstellung zu gehen, entgegenwirkt, so daß diese exakt aufgehoben oder kompensiert wird. Anders ausgedrückt leistet es die konstruktive Ausgestaltung des Hüftstieles, diesen aus einer varischen Vorzugsstellung in eine demgegenüber valgische Stellung zu drücken.
Das Kippmoment wird dadurch erzeugt, daß der Druck der den Hüftstiel umgebenden Spongiosa im Bereich der Preßhügel erhöht ist gegenüber dem Anpreßdruck der Spongiosa im übrigen Bereich.
Die konstruktive Ausgestaltung ist anwendbar auf zementlos zu implantierende Hüftstiele, ebenso wie auf zu zementierende Hüftstiele.
Vorzugsweise ist der laterale Bereich des Hüftstiel, d. h. also sein Rücken, abgeflacht ausgebildet.
Zur Stabilisierung beim Eintreiben in den Knochenmarksraum weist der erfindungsgemäße Hüftstiel gemäß einer vorteilhaften Weiterbildung medial und dorsal gesehen wenigstens jeweils zwei Rillen auf, die im wesentlichen parallel zum geraden lateralen Verlauf orientiert sind. Die Rillen bieten eine Sicherheit gegenüber einer Rotation um die Längsachse des Hüftstiels.
Die Erfindung wird anhand eines Ausführungsbeispiels gemäß der Zeichnungsfiguren näher erläutert. Hierbei zeigt:
Fig. 1 die Ansicht des Hüftstiels,
Fig. 2 eine Schnittansicht entlang der Linie H-II,
Fig. 3 eine Schnittansicht entlang der Linie &Igr;&Pgr;-&Igr;&Pgr;, und
Fig. 4 den Hüftstiel aus Fig. 1, von lateral gesehen.
Die erfindungsgemäßen Merkmale ergeben sich in aller Deutlichkeit bereits aus Fig. 1. Deutlich erkennbar ist hier in Ansicht der Hüftstiel 1 mit einem insgesamt geraden lateralen Verlauf (in Fig. 1 die linke Außenkontur). Vorliegend ist der "Rücken" lateral auch abgeflacht, wie sich aus den Schnittansichten gemäß Fig. 2 und 3 ergibt.
Im proximalen lateralen Bereich ist auf dem "Rücken" ein erster Preßhügel 2 ausgeformt. Im weiter distal gelegenen Bereich ist ventral ein weiterer Preßhügel 3 ausgebildet. Beide Preßhügel 2 und 3 sind vorzugsweise integraler Bestandteil des Grundkörpers des Hüftstiels 1, d. h., daß diese gleich mit ausgegossen worden sind und das Ursprungsmodell (aus Wachs) bereits über diese Preßhügel 2 und 3 verfügte. Idealisiert ist in Fig. 1 eine Achse zwischen den höchsten Punkten der Preßhügel 2 und 3 dargestellt, in dessen Mitte ein Schwenkpunkt M angedeutet ist. Wird nämlich der Hüftstiel 1 in den Markkanal des Femurs eingetrieben, so legt sich der Hüftstiel im Bereich seiner Preßhügel 2 und 3 an die im Femur befindliche Spongiosa an der Wandung des ausgeraspelten Bereiches stärker an als an die übrigen Bereiche, so daß im Bereich der Preßhügel in Richtung der Pfeile P jeweils eine stärkere Kraft ausgeübt wird als dies in anderen Bereichen der Fall ist. Hierdurch
entsteht ein Kippmoment um den Schwenkpunkt M herum, weswegen die ansonsten vorhandene Neigung des Hüftstiels, in eine varische Stellung zu gehen, kompensiert wird durch eine erzwungene Schwenkbewegung, so daß in Fig. 1 der distale Endbereich entgegen dem Uhrzeigersinn schwenkt, also in Richtung einer Valgität. Die Varusstellung wird demnach kompensiert durch eine erzwungene Schwenkbewegung in Richtung einer Valgusstellung.
Aus Fig. 4 ist im übrigen ersichtlich, daß sich der Preßhügel 2 über die gesamte Breite des hier abgeflachten Rückens des Hüftstieles 1 erstreckt.
Im dargestellten Ausführungsbeispiel sind auf der von medial und von dorsal gesehenen Seitenflächen jeweils zwei Rillen 4 und 5 vorgesehen, die parallel zu dem lateralen Verlauf des Rückens orientiert sind. Die Rillen 4 und 5 dienen vorwiegend zu einer Sicherung gegenüber einer Rotation während des Eintreibens des Hüftstieles in den Markraum des Femurs um eine Hauptachse herum, also um jene, die in Fig. 4 angedeutet ist.
Durch die erfindungsgemäße Maßnahme wird leichter ein optimaler Sitz des Hüftstieles 1 im Femurknochen erzielt.

Claims (3)

Schutzansprüche
1. Hüftstiel für ein künstliches Hüftgelenk mit proximalem Kragen, dadurch gekennzeichnet, daß er einen geraden lateralen Verlauf aufweist, daß im proximalen lateralen Bereich ein sattelförmiger Preßhügel (2) und im weiter distalen ventralen Bereich ein
weiterer Preßhügel (3) angeformt ist.
2. Hüftstiel nach Anspruch 1, dadurch gekennzeichnet, daß der
laterale Bereich abgeflacht ist.
3. Hüftstiel nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß er medial und dorsal gesehen wenigstens jeweils zwei Rillen (4, 5) aufweist, die im wesentlichen parallel zum geraden lateralen Verlauf orientiert sind.
