Description
- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die vorliegende Erfindung betrifft eine bifurkationsförmige Basisstruktur zum Bilden eines bifurkationsförmigen Transplantats in Kombination mit einem Paar Gefäßtransplantate sowie eine Baugruppe mit der Basisstruktur und dem Paar röhrenförmige Transplantate, für eine Gefäßanordnung für die Behandlung von abdominalen und anderen Aneurysmen.
- Gefäßaneurysmen sind das Ergebnis einer anomalen Erweiterung eines Blutgefäßes, die sich normalerweise aus einer Krankheit und/oder genetischer Veranlagung ergibt, die die Arterienwand schwächt und ihr erlaubt, sich auszudehnen. Während Aneurysmen in jedem Blutgefäß auftreten können, treten die meisten in der Aorta und in den peripheren Arterien auf, wobei die Mehrzahl der Aortenaneurysmen in der abdominalen Aorta auftreten, normalerweise unter der Nierenarterie beginnen und sich oft distal in eine oder beide Hüftarterien erstrecken.
- Aortenaneurysmen werden am häufigsten in offenen operativen Verfahren behandelt, wobei das kranke Gefäßsegment umgangen und mit einem künstlichen Gefäßtransplantat wiederhergestellt wird. Während dieses als eine effektive operative Technik betrachtet wird, vor allem wenn die Alternative eines normalerweise tödlichen Zerreißens des abdominalen Aortenaneurysmas betrachtet wird, leidet die herkömmliche Chirurgie der Gefäßtransplantate an einer Anzahl von Nachteilen. Das operative Verfahren ist komplex und erfordert erfahrene Chirurgen und gut ausgestattete chirurgische Einrichtungen. Selbst bei den besten Chirurgen und der besten Ausstattung sind die behandelten Patienten jedoch häufig älter und durch Kreislauferkrankungen und andere Krankheiten geschwächt, wodurch die Anzahl der geeigneten Patienten vermindert wird. Selbst für geeignete Patienten hat die herkömmliche Aneurysma-Wiederherstellung vor dem Zerreißen eine relativ hohe Sterblichkeitsrate, normalerweise von 3% bis 10%. Die Morbidität im Zusammenhang mit der herkömmlichen Chirurgie umfaßt den Myokardinfarkt, die Niereninsuffizienz, Impotenz, Lähmung und andere Bedingungen. Außerdem erfordert selbst bei einem erfolgreichen chirurgischen Eingriff die Genesung einige Wochen, wobei sie oft einen längeren Aufenthalt im Krankenhaus erfordert.
- Um einige oder alle dieser Nachteile zu überwinden, ist eine Transplantatanordnung im Gefäßinneren für die Behandlung von Aneurysmen vorgeschlagen worden. Obwohl sie sehr vielversprechend sind, leiden viele der vorgeschlagenen Verfahren und Vorrichtungen an anderen Problemen. Oftmals weisen die vorgeschlagenen Transplantatstrukturen ungeschützte Anker oder Gerüste auf, die thrombogen sein können. Es ist außerdem schwierig, Transplantatstrukturen zu schaffen, die mit dem Blutgefäßlumen abgedichtet bleiben, um das Durchsickern oder das Umleiten des Blutes in das geschwächte Aneurysma zu verhindern, insbesondere wenn sie externen Deformationskräften ausgesetzt werden, die aus der Gefäßausdehnung und -kontraktion entsprechend dem Herzschlag resultieren. Viele Gefäßtransplantatstrukturen haben Schwierigkeiten beim Anpassen an die interne Arterienwand, speziell weil die Wand im Ergebnis der Atherosklerose und der Kalkablagerung eine im hohen Grade ungleichmäßige Oberfläche aufweisen kann, wobei sie sich mit dem Herzschlag und der Blutströmung des Patienten ausdehnt und zusammenzieht. Außerdem sind viele vorhergehende Gefäßtransplantatstrukturen schwierig innerhalb des Zielgebiets des Gefäßes zu positionieren und zu verankern.
- Aus diesen Gründen wäre es wünschenswert, wenn die Transplantatstrukturen leicht im Zielgebiet anzuordnen wären, sie kaum oder nicht thrombogen wären, eine stabile Dichtung mit der Gefäßwand bereitstellten, um ein Durchsickern oder ein Umleiten des Blutes zu verhindern, und sich an die gleichmäßigen und ungleichmäßigen Wände der Blutgefäße anpassen könnten, selbst wenn sich die Wand mit dem Herzschlag des Patienten ausdehnt und zusammenzieht.
- 2. Beschreibung des technischen Hintergrundes
- Gefäßtransplantate und Vorrichtungen für ihre transluminale Anordnung sind in den US-Patenten Nr. 5 219 355, 5 211 658, 5 104 399, 5 078 726, 4 820 298, 4 787 899, 4 617 932, 4 562 596, 4 577 631, und 4 140 126 und in den europäischen Patentveröffentlichungen 508 473, 466 518, und 461 791 beschrieben.
- Expandierbare und selbstexpandierende Gefäßnetze sind in den US-Patenten Nr. 5 147 370, 4 994 071, und 4 776 337, den europäischen Patentveröffentlichungen 575 719, 556 850, 540 290, 536 610, und 481 365 und in der deutschen Patentveröffentlichung
DE 42 19 949 beschrieben. Eine flexible Gefäßnetzstruktur mit entgegengesetzt gewundenen spiralförmigen Elementen, von denen einige an bestimmten Positionen getrennt sind, um die Flexibilität zu ver bessern, ist von Angiomed, Karlsruhe, Deutschland kommerziell verfügbar, wie in einer Broschüre mit dem Titel Memotherm Iliaca Stents beschrieben ist. - Katheter zum Anordnen von Gefäßnetzen sind in den US-Patenten Nr. 5 192 297, 5 092 877, 5 089 005, 5 037 427, 4 969 890, und 4 886 062 beschrieben.
- Gefäßtransplantate, die für offene chirurgische Implantationen vorgesehen sind, sind in den U.S. Patenten Nr. 5 236 447, 5 084 065; 4 842 575; 3 945 052 und 3 657 744 sowie in den PCT Anmeldungen WO 88/00313 und WO 80/02641 und in
SU 1697787 - Nickel-Titan-Legierungen und ihre Verwendung in medizinischen Geräten sind in den U.S. Patenten Nr. 4 665 906 und 4 505 767 beschrieben.
- Aus US-A-4 562 596 ist ein einstückiges bifurkationsförmiges Transplantat mit zwei unterschiedlich dimensionierten Schenkeln bekannt. Eine bifurkationsförmige Verkleidung bildet ein gemeinsames Lumen am proximalen Ende und zwei divergierende Strömungslumen am distalen Ende, eins in jedem der beiden Schenkel. Am proximalen Ende der Verkleidung ist ein radial zusammendrückbarer Ring vorgesehen, der in der eingesetzten Position des Transplantats an der Wand der Arterie anliegt. Umfänglich beabstandete vertikale Streben mit Verankerungshaken an ihren proximalen Enden erstrecken sich axial vom proximalen Ende der Verkleidung über den bifurkationsförmigen Abschnitt in Richtung auf das distale Ende der Schenkel.
- Aus EP-A-0 551 179 ist ein bifurkationsförmiges Transplantat mit zwei getrennten Durchflußlumen bekannt, die am distalen Ende divergieren. Eine Verkleidung erstreckt sich außen an einem perforierten röhrenförmigen Rahmen, der mehrere getrennte röhrenförmige Abschnitte aufweist, die axial durch die Verkleidung oder durch Brückenelemente verkettet sind.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung umfaßt eine bifurkationsförmige Basisstruktur zum Bilden eines bifurkationsförmigen Transplantats in Kombination mit einem Paar Gefäßtransplantate, wobei die Basisstruktur ein Verankerungselement und eine Verkleidung aufweist, wobei das Verankerungselement einen radial komprimierbaren, perforierten, röhrenförmigen Rahmen (Gerüst) umfaßt und die Verkleidung, die in dem Rahmen angeordnet ist, ein durch den Rahmen definiertes Lumen im wesentlichen abdeckt, so daß die Basisstruktur an ihrem proximalen Ende ein gemeinsames Strömungslumen definiert, wobei das proximale Ende der Verkleidung von dem proximalen Ende des Rahmens distal beabstandet ist, wobei sich die Verkleidung distal von dem Rahmen erstreckt und bifurkationsförmig ist, so daß die Basisstruktur an ihrem distalen Ende zwei unterschiedlich dimensionierte, divergierende Strömungslumen definiert, jeweils eines in jedem der zwei Schenkel, wobei der Rahmen mehrere getrennte röhrenförmige Segmente enthält, die axial nur durch die Verkleidung miteinander verbunden sind.
- Die vorliegende Erfindung umfaßt ferner eine Basisstruktur, die ein erstes und ein zweites Gefäßtransplantat umfaßt, die jeweils so dimensioniert sind, daß sie in dem ersten und zweiten Verkleidungsschenkel verankert werden können, um kontinuierliche Verlängerungen der jeweiligen divergierende Strömungslumen zu bilden.
- Die Gefäßtransplantate können einen radial zusammendrückbaren perforierten röhrenförmigen Rahmen (Gerüst) mit einem proximalen Ende, einem distalen Ende und einem axialen Lumen zwischen diesen Enden umfassen. Eine Innenverkleidung, üblicherweise ein Gewebe, eine Kunststoffschicht oder eine Membran oder dergleichen bedeckt die gesamte oder den größten Teil der Oberfläche des Lumens des röhrenförmigen Gerüsts und erstreckt sich von einem Ort in der Nähe des proximalen Endes zu einem Ort in der Nähe des distalen Endes. Die Verkleidung ist an wenigstens einem Ende sowie an mehreren Stellen zwischen den Enden an dem Gerüst befestigt. Optional kann über wenigstens einem Abschnitt der Außenseite des Gerüsts eine zweite Verkleidung vorgesehen sein, um beide Seiten des Gerüsts abzudecken. Eine solche Außenabdeckung sorgt für eine umfängliche Abdichtung gegen die Innenwand des Blutgefäßlumens, um ein Entweichen des Blutflusses zwischen dem Transplantat und der Lumenwand in den behandelten Aneurysmus oder die behandelte Stenose zu vermeiden.
- Die Transplantate der vorliegenden Erfindung finden besondere Anwendung bei der Behandlung vaskulärer Zustände wie etwa abdominaler oder anderer Aneurysmen, Gefäßstenosen und anderer Zustände, die die Schaffung eines künstlichen Gefäßlumens erfordern. Zur Behandlung von Gefäßstenosen kann das Transplantat als Netz (Stent) dienen, um die Gefäßdurchgängigkeit in ähnlicher Weise zu bewahren, wie es in den obenbeschriebenen US-Patenten und ausländischen Patentdokumenten, die sich auf Netze beziehen, beschrieben ist. Weitere intraluminale Anwendungen der vorliegenden Erfindung umfassen das Spannen des Harnleiters, der Harnröhre, des Gallentrakts und dergleichen und können auch zur Schaffung vorübergehender oder langzeitiger Lumen verwendet werden wie etwa die Bildung einer Fistel.
- Solche Transplantatstrukturen besitzen mehrere Vorteile gegenüber den bisher vorgeschlagenen Entwicklungen, insbesondere für vaskuläre Anwendungen. Durch Abdeckung des Lumens des röhrenförmigen Gerüsts wird die Neigung des Transplantats, Thrombosen hervorzurufen, die aus vorstehenden Gerüstelementen resultiert, stark verringert oder beseitigt. Eine solche Verringerung der Neigung zu Thrombosen wird erzielt, obwohl die Vorteile, eine Gerüststruktur zu besitzen, die über das Transplantat hervorsteht, beibehalten werden. Ein solches Außengerüst unterstützt das Verankern des Transplantats an Ort und Stelle und bewahrt die Durchgängigkeit und Gleichmäßigkeit des Transplantatlumens, wobei beides ein Vorteil gegenüber Transplantatstrukturen darstellt, die nur an jedem Ende verankert und gestützt werden. Die bevorzugten Gefäßtransplantate der vorliegenden Erfindung sind außerdem selbstexpandierend und leicht einzusetzen. Die Selbstexpansionseigenschaft eines solchen Gerüsts wirkt außerdem äußeren Verformungskräften entgegen, die aus der ständigen physiologischen Expansion und Kontraktion des Blutgefäßlumens resultieren. Außerdem ermöglicht das Fehlen von Treibkeilen, Zacken und anderen eindringenden Elementen auf dem Transplantat, daß das Transplantat sich enger an die umgebende Gefäßwand anpaßt, und erleichtert das Bergen und/oder das Neupositionieren des Transplantats, wie im folgenden näher beschrieben wird. Zusätzlich ermöglicht eine elastische röhrenförmige Gerüststruktur, daß sich das Transplantat selbst an unregelmäßige Bereiche der Blutgefäßwand anpaßt, wenn sich diese infolge der Pumpwirkung des Herzens des Patienten ausdehnt und zusammenzieht.
- Das röhrenförmige Gerüst der Gefäßtransplantate enthält vorzugsweise mehrere radial zusammendrückbare Band- oder Ringstrukturen, wovon jede einen entspannten (d. h. nicht komprimierten) Durchmesser besitzt, der größer als der Durchmesser des zu behandelnden Blutgefäßes ist. Benachbarte zusammendrückbare Bandelemente sind jeweils voneinander unabhängig oder können an einer Stelle oder mehreren Stellen dazwischen verbunden sein. Wenn sie verbunden sind, sind die Bänder vorzugsweise an lediglich zwei diametral entgegengesetzten Punkten verbunden, um eine Flexibilität des Gerüsts über seine gesamte Länge zu verbessern. Unabhängige Bandelemente halten durch ihre Befestigung an der (den) inneren und/oder äußeren Verkleidung (Verkleidungen) zusammen.
- Alternativ kann das röhrenförmige Gerüst der Gefäßtransplantate mehrere seitlich zusammendrückbare axiale Elemente mit benachbarten axialen Elementen, die vorzugsweise nicht direkt miteinander verbunden sind, enthalten. Die axialen Elemente enthalten gewöhnlich eine Vielzahl sich wiederholender Struktureinheiten, z. B. rhombenförmiger Elemente, die axial verbunden sind. Die axialen Elemente werden entweder durch Einnähen oder durch Einfassen der axialen Elemente in Taschen, die zwischen der Innenverkleidung und einer Außenverkleidung, die über dem Gerüst angeordnet ist, gebildet sind, an der inneren Verkleidung befestigt. Die Taschen können durch Befestigen der Innenverkleidung und der Außenverkleidung aneinander entlang axialer Linien zwischen benachbarten axialen Elementen gebildet sein.

- Die vorliegende Erfindung schafft eine bifurkationsförmige Basisstruktur and eine Baugruppe mit der Basisstruktur und zwei Gefäßtransplantaten, besonders für die transluminale Anordnung innerhalb des Gefäßsystems für die Behandlung von Aneurysmen und anderen Gefäßbedingungen, aber ebenso in anderen Körperlumen, wie z. B. des Harnleiters, der Harnröhre, des Gallentrakts, des Magen-Darm-Trakts und ähnlichen, für die Behandlung anderer Bedingungen, die von der Einführung einer verstärkenden oder schützenden Struktur in das Lumen profitieren. Die Erfindung kann außerdem bei der Erzeugung künstlicher Lumen durch festes Gewebe und feste Strukturen, wie z. B. der Anordnung einer TE-Fistel über ein Endoskop, Anwendung finden. Die Gefäßtransplantate werden endovaskular angeordnet. "Endovaskular" bedeutet hier die Anordnung durch perkutane oder abschneidende transluminale Verfahren unter Verwendung eines Katheters über einen Führungsdraht unter Fluoreszenzführung. Die Katheter und die Führungsdrähte können durch herkömmliche Zugangsstellen zu dem Gefäßsystem eingeführt werden, wie z. B. durch die Arm- und Subclaviusarterien für den Zugriff auf die Aorta und durch die Femoralarterie für den Zugriff auf die Aorta oder auf die peripheren Blutgefäße und die Zweigblutgefäße.
- Ein Gefäßtransplantat zur Benutzung mit der Basisstruktur, die später mit Bezug auf
5 bis12 beschrieben wird, enthält ein radial zusammendrückbares perforiertes röhrenförmiges Gerüst und eine innere oder Innenverkleidung, die in einem zentralen Lumen befestigt ist, das durch das Gerüst definiert ist, und optional eine zweite oder äußere Verkleidung, die über dem Äußeren des Gerüsts ausgebildet ist. Das radial zusammendrückbare Gerüst kann verschiedene Formen annehmen, wobei es gewöhnlich aus mehreren unabhängigen Strukturelementen wie etwa Ringen, Bändern, schraubenlinienförmigen Elementen, serpentinenartigen Elementen, axialen Verstrebungen, parallelen Stangen und dergleichen besteht oder diese umfaßt, die aus einer entspannten größeren Durchmesserkonfiguration in eine Konfiguration mit kleinerem Durchmesser zusammengedrückt werden können, um, wie oben erläutert wurde, die Einführung zu erleichtern. Natürlich muß (müssen) die Verkleidung (Verkleidungen) an dem Gerüst sowohl in seiner radial zusammengedrückten Konfiguration als auch in seiner ausgedehnten, entspannten Konfiguration befestigt bleiben. - Eine bevorzugte Konfiguration für das röhrenförmige Gerüst umfaßt meh rere radial zusammendrückbare Bandelemente, wobei benachbarte Bandelemente miteinander nur an zwei diametral entgegengesetzten Punkten befestigt sind, um die Flexibilität zu erhöhen. Gemäß einem besonders bevorzugten Aspekt sind die diametral entgegengesetzten Befestigungspunkte drehversetzt, um eine Flexibilität in mehr als eine Richtung zu schaffen. Ein geeigneter Weg zum Bilden eines derartigen rohrförmigen Gerüsts wird nachstehend detaillierter beschrieben. In einer anderen bevorzugten Konfiguration sind wenigstens einige der Bänder des Gerüsts unabhängig, das heißt nicht direkt miteinander verbunden. Statt dessen sind die Bänder nur mit der Verkleidung (den Verkleidungen) verbunden, die die axiale Integrität des Transplantats aufrechterhalten. Vorzugsweise sind die unabhängigen Bänder zwischen inneren und äußeren Verkleidungen angenäht oder versiegelt, wie weiter unten detaillierter beschrieben wird. Andere geeignete Gerüststrukturen sind in der Patentliteratur beschrieben.
- In einer alternativen Konfiguration enthält das perforierte röhrenförmige Gerüst mehrere seitlich zusammendrückbare axiale Elemente, die direkt, z. B. durch Annähen, oder indirekt, z. B. durch Laminierung, an der Innenverkleidung befestigt sind. Die axialen Elemente können mehrere sich wiederholende Strukturelemente wie etwa Rhomben sein oder können aus zwei oder mehr sich überlappenden Elementen bestehen. Durch Positionierung der axialen Elemente in Taschen, die zwischen einer Innenverkleidung und einer Außenverkleidung gebildet sind, können die axialen Elemente einzeln nachgeben, wobei die gewünschte radiale Kompressibilität und die gewünschten Selbstexpansionseigenschaften für das Transplantat geschaffen werden.
- Die Abmessungen des röhrenförmigen Transplantats hängen von der beabsichtigten Verwendung ab. Üblicherweise besitzt das Transplantat eine Länge in der Größenordnung von ungefähr 50 mm bis 500 mm, für vaskuläre Anwendungen vorzugsweise im Bereich von 80 mm bis 200 mm. Der entspannte Durchmesser liegt gewöhnlich im Bereich von ungefähr 4 mm bis 45 mm, wobei er für vaskuläre Anwendungen vorzugsweise im Bereich von ungefähr 5 mm bis 25 mm liegt. Das Transplantat ist radial auf einen Durchmesser im Bereich von 3 mm bis 9 mm zusammendrückbar, für vaskuläre Anwendungen vorzugsweise im Bereich von 4 mm bis 6 mm.
- Die Verkleidung (Verkleidungen) setzt (setzen) sich aus herkömmlichen biologischen Transplantatmaterialien wie etwa Polyester, Polytetrafluorethylen (PTFE), Polyurethan und dergleichen zusammen, wobei sie gewöhnlich die Form von gewebten Stoffen, ungewebten Stoffen, Polymerschichten, Membranen und dergleichen besitzen. Ein gegenwärtig bevorzugtes Gewebe-Verkleidungsmaterial ist ein glattes Polyestergewebe wie etwa Dacron®-Garn Typ 56 (Dupont, Wilmington, Delaware) mit einem Gewicht von 40 Denier, bei 27 Fäden, mit 178 Ketten pro Zoll in Umfangsrichtung und 78 Garnen pro Zoll in Füllrichtung gewebt.
- Die Verkleidung wird am Innenlumen des röhrenförmigen Gerüsts angebracht und bedeckt den größten Teil der oder die gesamte Innenfläche des Lumens. Beispielsweise kann die Verkleidung am röhrenförmigen Gerüst entlang mehrerer in Umfangsrichtung voneinander beabstandeter axialer Linien angenäht oder anderweitig befestigt werden. Durch eine solche Anbringung kann sich die Verkleidung beim radialen Zusammendrücken des Gerüsts entlang mehrerer axialer Faltlinien falten. Die Verkleidung kann sich ferner öffnen und der Lumenwand des röhrenförmigen Gerüsts anpassen, wenn sich dieses ausdehnt. Bei Verwendung einer Innenverkleidung und einer Außenverkleidung können die Verkleidungen alternativ vernäht oder durch Wärme oder Ultraschall verschweißt werden, so daß sie das röhrenförmige Gerüst zwischen sich einschließen. In einer beispielhaften Ausführungsform, bei der zwischen der Innenverkleidung und der Außenverkleidung mehrere unabhängige Bandelemente angeordnet sind, sind die Verkleidungen entlang Umfangslinien zwischen benachbarten Bandelementen aneinander befestigt, so daß sie Taschen zum Halten der Bandelemente bilden. In einer zweiten beispielhaften Ausführungsform, bei der zwischen der Innenverkleidung und der Außenverkleidung mehrere axiale unabhängige Bandelemente angeordnet sind, sind die Verkleidungen entlang axialer Linien zwischen benachbarten Bandelementen aneinander befestigt, um Taschen zum Halten der Bandelemente zu bilden.
- Die Verkleidung schließt vorzugsweise an wenigstens einem Ende und im Fall von nicht gabelförmigen, geraden Transplantaten vorzugsweise an beiden Enden umfänglich gegen das röhrenförmige Gerüst ab. Außerdem wird in manchen Fällen vorgezogen, das distale und das proximale Ende der des perforierten röhrenförmigen Gerüst üblicherweise über eine Länge im Bereich von ungefähr 1 mm bis 25 mm vorstehen zu lassen, d. h., nicht mit dem Verkleidungsmaterial zu bedecken. Ein nicht mit der Verkleidung bedecktes Gerüst ermöglicht das Durchströmen von Blut durch die Perforationen in die Nebenarterien wie etwa die renalen Arterien im Fall von Transplantaten für eine abdominale Aorta, wobei für das Verankern des Gerüsts im Blutgefäßlumen eine zusätzliche Fläche geschaf fen wird. In einer exemplarischen Ausführungsform erstreckt sich die Verkleidung durch das Gerüst über die Außenfläche in die Nähe entweder eines oder beiden Enden, so daß ein wirksamerer Abschluß mit der benachbarten Blutgefäßwand geschaffen wird.
- Das radial zusammendrückbare perforierte röhrenförmige Gerüst besteht aus einem elastischen Material, gewöhnlich aus einem Metall, häufig aus einer wärme- und/oder formbeständigen Legierung, wie etwa Nickeltitaniumlegierungen, die unter dem Handelsnamen Nitinol® im Handel erhältlich sind. Die Gerüste können auch aus anderen hochelastischen Metallen wie etwa MP-35 N, Elgiloy, 316 L rostfreiem Stahl 316 L und dergleichen zusammengesetzt sein. Im Fall von Nitinol® und anderen Memory-Legierungen kann der Phasenübergang zwischen dem austenitischen und dem martensischen Zustand zwischen einer Einführungstemperatur, z. B. der Raumtemperatur (ungefähr 22°C), und der Körpertemperatur (37°C), eintreten, um die Belastung des nichtexpandierten Gerüst zu minimieren und die radiale Expansion des Gerüsts aus seinem radial zusammengedrückten Zustand zu verbessern. Außerdem kann aufgrund der hochelastischen Natur der Legierung eher eine Expension, als eine Wiedergewinnung der eigentlichen Form aufgrund einer Phasenänderung erzielt werden.
- In manchen Fällen ist es wünschenswert, ein röhrenförmiges Gerüst zu bilden, das in verschiedenen Regionen entlang seiner Länge verschiedene elastische oder andere mechanische Eigenschaften aufweist. Beispielsweise können verschiedene Regionen des röhrenförmigen Gerüsts wärmebehandelt werden, so daß einige Regionen elastische Eigenschaften besitzen, während andere formbar werden, so daß sie durch äußere Kräfte verformbar werden. Beispielsweise kann das Transplantat durch Schaffung wenigstens eines formbaren Endabschnitts und eines elastischen (radial zusammendrückbaren) Mittelabschnitts durch innere Ballon-Expansionskräfte (um das Ende bzw. die Enden in der Innenwand des Blutgefäßes zu verankern) starr aufgeweitet und eingesetzt werden, wobei das mittlere aufgrund der elastischen Natur des röhrenförmigen Elements offen bleibt.
- Formbare Endabschnitte sind von besonderem Vorteil, da sie mit einer ausreichenden Kraft expandiert werden können und erneut expandiert werden können, falls dies erforderlich ist, um eine gute Abdichtung gegen die Blutgefäßwand sicherzustellen. Alternativ könnten die formbaren Enden aus einem von jenem des Mittelabschnitts des röhrenförmigen Gerüsts verschiedenen Material gebildet sein. Eine Verwendung verschiedener Materialien wäre besonders zweckmäßig, wenn das Gerüst aus mehreren unabhängigen Bändern gebildet ist, wobei eines oder mehrere Bandelemente an einem oder beiden Enden aus einem formbaren Metall geformt sein könnten. Gewöhnlich erstreckt (erstrecken) sich ein solches formbares Ende (solche formbaren Enden) über eine Strecke im Bereich von 5 mm bis 50 mm, vorzugsweise von 5 mm bis 20 mm.
- Formbare Abschnitte oder Segmente können auch in anderen Teilen des röhrenförmigen Gerüsts ausgebildet sein. Beispielsweise könnten einige in Umfangsrichtung beabstandete Segmente des röhrenförmigen Gerüsts formbar sein, während die restlichen Umfangssegmente elastisch wären. Das Gerüst würde somit elastisch bleiben, hätte jedoch eine zusätzliche Formbarkeit, um durch Anlegen einer inneren Expansionskraft die Expansion zu erlauben. Eine solche Konstruktion wäre vorteilhaft, da es möglich wäre, den Durchmesser der Transplantat- oder Netzstruktur zu erweitern, wenn der Innendurchmesser (der insgesamt aus der elastischen Expansion hervorging) aus irgendeinem Grund nicht groß genug wäre. Das Verhältnis von elastischem Material zu formbarem Material in dem röhrenförmigen Gerüst kann so gewählt werden, daß ein gewünschtes Gleichgewicht zwischen dem Maß an ursprünglicher, elastischer Öffnung und der Verfügbarkeit einer zusätzlichen, formbaren Öffnung hergestellt wird. Eine solche Konstruktion kann durch selektive Wärmebehandlung von Abschnitten eines aus einem einzigen Legierungsmaterial, z. B. einer Nickeltitaniumlegierung, zusammengesetzten Gerüsts oder durch Bildung von Umfangssegmenten des Gerüsts aus verschiedenen Materialien mit verschiedenen elastischen/formbaren Eigenschaften erreicht werden. Insbesondere könnten einzelne seitlich zusammendrückbare axiale Elemente
204 (die im Zusammenhang mit1B beschrieben wurden) aus Materialien mit verschiedenen elastischen/formbaren Eigenschaften gebildet werden. - In
1 –4 wird eine Transplantatstruktur10 ,100 ,200 , die nicht unbedingt Teil der vorliegenden Erfindung ist, beschrieben. Die Transplantatstruktur10 enthält eine Gewebeverkleidung12 und ein radial zusammendrückbares, perforiertes, röhrenförmiges Gerüst14 . Zur Vereinfachung ist das Gerüst14 in2 durch sich selbst veranschaulicht. Das Gerüst ist in jeder dieser Figuren in seiner ausgedehnten (entspannten) Konfiguration veranschaulicht, es kann aber radial zusammengedrückt werden, indem eine radial nach innen gerichtete Druckkraft ausgeübt wird, normalerweise, indem das Transplantat10 in einer äußeren Hülle angeordnet wird, was im folgenden ausführlicher beschrieben wird. - Das röhrenförmige Gerüst
14 enthält mehrere radial zusammendrückbare Bandelemente11 , von denen jedes ein im Zickzack verlaufendes oder Z-förmiges Element enthält, das einen ununterbrochenen kreisförmigen Ring bildet. Jedes Bandelement11 besitzt typischerweise eine Breite w im Bereich von 2 mm bis 15 mm, wobei das röhrenförmige Gerüst zwischen 1 und 30 einzelne Bandelemente enthalten wird. Benachbarte Bandelemente11 sind vorzugsweise mit einem kurzen Abstand d voneinander beabstandet und durch Brückenelemente13 verbunden. Durch das Bereitstellen von nur zwei diametral entgegengesetzten Brückenelementen13 zwischen jedem benachbarten Paar aus Bandelementen11 wird die Flexibilität verbessert. Wie unter Bezugnahme auf1A weiter beschrieben wird, kann die Flexibilität weiter verbessert werden, wenn die Bandelemente nur durch die Verkleidung verbunden werden. - Normalerweise wird das perforierte röhrenförmige Gerüst
14 an jedem Ende offen gelassen, z. B. wird wenigstens ein Abschnitt des letzten Bandelements11 durch die Verkleidung12 unbedeckt bleiben. Die Verkleidung12 wird an die Bandelemente11 genäht oder anders befestigt sein, vorzugsweise an den Verbindungen oder den Knoten, wenn das Element die Richtung umkehrt, um das Z-Muster zu bilden (obwohl das Nähen nicht in einer Weise zwischen den Bandelementen hinübergehen soll, die die Flexibilität einschränken würde). Die Verkleidung12 wird normalerweise von dem inneren Lumen des röhrenförmigen Gerüsts14 durch die Lücke zwischen benachbarten Bandelementen zum Äußeren des Gerüsts nach außen verlaufen, wie in1 veranschaulicht ist. Der Abschnitt der Verkleidung12 außerhalb des röhrenförmigen Gerüsts14 hilft, das (die) Ende(n) des Transplantats10 gegen die Wand des Blutgefäßes oder anderer Körperlumen, in denen es angeordnet ist, abzudichten. - Das Verbindungsmuster der benachbarten Bandelemente
11 ist in3A und3B am besten veranschaulicht.3A veranschaulicht das röhrenförmige Gerüst14 , wie es aussehen würde, wenn es auf einer flachen Oberfläche abgewickelt wird.3B ist3A ähnlich, mit der Ausnahme, daß die Bandelemente ausgedehnt sind. Die Ausdehnung ist bei 30° gezeigt, sie wird sich im Gebrauch aber häufig bis zu 60° oder höher erstrecken. - Ein bevorzugter Weg zum Bilden des röhrenförmigen Gerüsts
14 ist unter Bezugnahme auf4 beschrieben. Ein Rohr aus dem gewünschten elastischen Material, wie z. B. einer Nickel-Titan-Legierung, mit einer Phasenübergangstemperatur signifikant unter 37°C, vorzugsweise zwischen 30°C und 32°C, wird erhalten. Das Rohr wird Abmessungen aufweisen, die ungefähr gleich den ge wünschten Abmessungen des Gerüsts sind, wenn es radial zusammengedrückt ist. Das Rohr kann gezogen, gewalzt oder anders behandelt werden, um die gewünschte Wandstärke, den gewünschten Durchmesser und ähnliches zu erreichen. Geeignete Wandstärken liegen in dem Bereich von ungefähr 0,1 mm bis 0,5 mm. Dann wird ein Muster aus axialen Schlitzen in dem Rohr gebildet, wie in4 veranschaulicht ist. Die Schlitze können durch elektro-erosive Bearbeitung (EDM), photochemisches Ätzen, Laserschneiden, spanabhebende Bearbeitung oder andere herkömmliche Techniken gebildet werden. Nachdem die Schlitze gebildet worden sind, wird das Rohr mechanisch auf seinen gewünschten abschließenden (entspannten) Durchmesser ausgedehnt und bei einer geeigneten Temperatur wärmebehandelt, um das Rohr in dem gewünschten ausgedehnten Zustand formzufestigen. Scharte Kanten werden durch herkömmliche Techniken entfernt, wie z. B. Entgraten, abschleifendes Strangpressen oder ähnliches. Das Ergebnis der Ausdehnung ist das in1 und2 veranschaulichte röhrenförmige Gerüst. - Vorzugsweise wird jedes Ende der Verkleidung
12 entlang des Umfangs an den distalen oder proximalen Enden des röhrenförmigen Transplantats oder in der Nähe dieser abgedichtet. Wie in1A veranschaulicht ist, kann dies erreicht werden, indem das Ende der Verkleidung12 über die äußere Oberfläche des Transplantats10 gefaltet wird. Zweckmäßigerweise kann dies durch die Lücken erfolgen, die zwischen benachbarten Bandelementen14 vorhanden sind. Wo die Verbindungen13 bleiben, kann die Verkleidung12 sorgfältig auf die darunterliegende Oberfläche des Gerüsts genäht werden, wie bei18 in1A gezeigt ist. Andere Techniken für das Abdichten der Verkleidung entlang des Umfangs umfassen das Warm- oder das Ultraschall-Schweißen der Verkleidung, das Laminieren einer äußeren Dichtung, das Nähen eines äußeren Verstärkungselements oder ähnliches. - In
1A ist eine beispielhafte Ausführungsform100 eines Gefäßtransplantats, die ein wesentliches Merkmal der vorliegenden Erfindung veranschaulicht, beschrieben. Das Transplantat100 enthält ein perforiertes, röhrenförmiges Gerüst102 , das mehrere unabhängige (unverbundene) Bandelemente104 enthält, die durch die Lücken106 voneinander getrennt sind. Das perforierte, röhrenförmige Gerüst102 ist in seiner Konstruktion dem Gerüst14 des Transplantats10 ähnlich, mit der Ausnahme, daß die benachbarten Bandelemente104 nicht direkt miteinander verbunden sind. Die Bandelemente104 werden nur durch eine innere Verkleidung108 und eine äußere Verkleidung110 verbunden, wobei die innere und die äußere Verkleidung die ansonsten frei schwebenden Bandelemente104 zusammen verkleiden oder mit ihnen eine Mehrschichtstruktur bilden. Um die Bandelemente104 am richtigen Platz zu sichern und die Verkleidungen an dem perforierten, röhrenförmigen Gerüst102 zu sichern, werden die innere und die äußere Verkleidung entlang der Umfangslinien112 miteinander verbunden, die sich vorzugsweise in den Lücken106 zwischen den benachbarten Bandelementen104 befinden. Die Verkleidungen können durch Nähen, Warmschweißen, Ultraschallschweißen oder ähnliches miteinander verbunden werden. In der beispielhaften Ausführungsform sind die Verkleidungen108 und110 aus einem polymeren Schichtmaterial gebildet und durch Ultraschallschweißen miteinander verbunden. Die Bandelemente104 an jedem Ende des Transplantats100 müssen weiter an den Verkleidungen108 und110 gesichert werden. Sie könnten z. B. genäht, geschweißt oder anders mit den Verkleidungen verbunden werden, um sie am richtigen Platz zu halten. Die Abmessungen, Materialien und die anderen Aspekte des Transplantats100 werden im allgemeinen die gleichen wie diejenigen des vorher beschriebenen Transplantats10 sein. - In
1B ist ein alternatives, nicht zu der vorliegenden Erfindung gehörendes, Gefäßtransplantat veranschaulicht. Das Transplantat200 enthält ein perforiertes, röhrenförmiges Gerüst202 , das mehrere seitlich zusammendrückbare axiale Elemente204 enthält. Jedes axiale Element204 enthält mehrere rhombische Konstruktionselemente, die in einer linearen Weise miteinander verbunden sind. Es ist klar, daß jedes rhombische Konstruktionselement seitlich zusammendrückbar ist, so daß das Gerüst202 als Ganzes von einer Konfiguration mit vermindertem Durchmesser zu einer Konfiguration mit ausgedehntem Durchmesser radial zusammengedrückt werden kann. Wie in1B veranschaulicht ist, befindet sich das Gerüst in einer teilweise zusammengedrückten Konfiguration. Die axialen Elemente202 werden zwischen einer inneren Verkleidung206 und einer äußeren Verkleidung208 gefangen. Die innere Verkleidung206 und die äußere Verkleidung208 werden entlang mehrerer axialer Linien210 aneinander gesichert, die zwischen benachbarten axialen Elementen204 angeordnet sind. Auf diese Weise wird jedes axiale Element204 innerhalb einer zwischen der inneren Verkleidung206 und der äußeren Verkleidung208 gebildeten Tasche gefangen. Wie bei den vorausgehenden Ausführungsformen können sich die Enden des Gerüsts über die Verkleidungen erstrecken, um für eine verbesserte Veranke rung und ein verbessertes Durchströmen auf jeder Seite des Transplantats zu sorgen. - In
5 wird eine bifurkationsförmige Basisstruktur zum Bilden eines bifurkationsförmigen Transplantats in Verbindung mit einem Paar der soeben erörterten Gefäßtransplantate10 beschrieben. Die bifurkationsförmige Basisstruktur20 enthält ein Ankersegment22 , das ein radial zusammendrückbares, perforiertes Gerüst ist, das gemäß der Erfindung typischerweise eine Struktur aufweist, die zu der soeben unter Bezugnahme auf1A erörterten ähnlich oder identisch ist und mehrere getrennte rohrförmige Segmente umfaßt, die axial nur durch eine Verkleidung24 verbunden sind. Das Gerüst des Ankers22 wird typischerweise eine Länge in dem Bereich von ungefähr 5 mm bis 50 mm und einen Durchmesser im Bereich von ungefähr 5 mm bis 30 mm aufweisen. Die Verkleidung24 wird innerhalb des Gerüsts22 angeordnet sein, wobei sie typischerweise in der Nähe des oberen Endes des Gerüsts entlang des Umfangs abgedichtet ist, wobei sie z. B. umgefaltet und vernäht ist, wie vorausgehend beschrieben ist. Wie bei dem geraden Transplantat nach1 –4 ist das proximale Ende der Verkleidung24 von dem proximalen Ende des Ankersegments22 distal beabstandet, typischerweise mit einem Abstand im Bereich von 1 mm bis 25 mm. Das Gewebe24 definiert ein gemeinsames Strömungslumen an seinem oberen Ende und ein Paar divergenter Strömungslumen an seinem unteren Ende, eines in jedem Schenkel26 und28 . Die Schenkel26 und28 sind vorzugsweise durch das Gerüst des Ankers22 nicht bedeckt. Die Gewebeschenkel26 und28 werden jeder einen Durchmesser im Bereich von 6 mm bis 18 mm und eine Länge im Bereich von 5 mm bis 30 mm aufweisen. Die Abmessungen jedes Schenkels sind nicht die gleichen. - In
6 wird ein Katheter30 zum Einführen des Gefäßtransplantats10 oder der bifurkationsförmigen Basisstruktur20 beschrieben. Der Katheter30 enthält eine Welle32 , die ein Paar axial beabstandeter Stützen34 und36 aufweist. Ein Zugdraht38 erstreckt sich durch ein Lumen40 der Welle32 und durch Vorsprünge in jeder der Stützen34 und36 . Eine Hülle42 ist verschiebbar über der Welle32 angeordnet, so daß sie über die Stützen34 und36 vorgeschoben werden kann. Der Führungsdraht34 erstreckt sich durch die Welle32 und den Wellenzapfen46 , um die Gefäßeinführung des Katheters30 zu erleichtern. Ein radial zusammendrückbares Transplantat G (wie das Transplantat10 ) ist über dem distalen Ende des Wellenzapfens46 angeordnet, wobei es im allgemeinen zwi schen den Stützen34 und36 ausgerichtet ist. Der Zugdraht38 wird dann durch die Stützen34 und36 vorgeschoben, so daß er durch jedes Ende des Transplantats G geht, um das Transplantat am richtigen Platz zu halten, bis der Zugdraht zurückgezogen wird. Während der Zugdraht38 am Platz beleibt, kann die Hülle42 axial über das Transplantat vorgeschoben werden, um das Transplantat auf seinen gewünschten niedrigen Profildurchmesser radial zusammenzudrücken. Die Hülle42 enthält ein nach außen erweitertes (d. h. nach außen trichterförmiges) distales Ende46 , um das Vorschieben der Hülle über das Transplantat zu erleichtern, insbesondere so, daß das Transplantat wieder ergriffen werden kann, wenn es teilweise entfaltet ist, wie im folgenden beschrieben ist. Der äußere Trichter kann permanent im Körper der Hülle befestigt sein, wird aber vorzugsweise selektiv zwischen der trichterförmigen Konfiguration und einer nicht trichterförmigen oder geraden Konfiguration (die mit der gestrichelten Linie gezeigt ist) entfaltbar sein, um die Einführung der Hülle durch das Gefäßsystem oder andere Körperlumen zu erleichtern. Eine Vielzahl geeigneter Mechanismen zum selektiven Ausdehnen des distalen Endes der Hülle sind in der Technik bekannt, wie z. B. Zugdrähte und ähnliches. Die Hülle42 wird durch das Gefäßsystem durch eine herkömmliche Einführungshülse mit einer proximalen Blutstillungsklappe eingeführt. - Der Katheter
30 kann modifiziert werden, um alternative Zuführungstechniken für das Transplantat G zu schaffen. Der Katheter30 kann z. B. an seinem distalen Ende oder in dessen Nähe einen Ballon für die Verwendung mit Transplantaten enthalten, die formbare Abschnitte aufweisen, die ausgedehnt werden müssen. Der Katheter30 könnte außerdem Puffer oder andere Einrichtungen zum Ausrichten des Transplantats auf der Welle46 enthalten, während die Hülle42 eingezogen wird. Eine Vielzahl anderer Katheterkonstruktionen und anderer Techniken für die Zuführung des radial zusammendrückbaren Transplantats und von Netzstrukturen der vorliegenden Erfindung. - In
7 –12 wird das Anordnen einer bifurkationsförmigen Transplantatstruktur in einem abdominalen Aortenaneurysma AA eines Patienten beschrieben. Zu Anfang wird der Zuführungskatheter30 durch eine Einführungshülse50 über einen fortschreitenden Zugang (z. B. die Subklavia SC) eingeführt, wie in7 veranschaulicht ist. Die bifurkationsförmige Basisstruktur wird anfangs in der Hülle42 gehalten, so daß sie radial zusammengedrückt bleibt. Nachdem die zusammengedrückte Basisstruktur20 richtig angeordnet ist, wird die Hülle42 zurückge zogen, wodurch sich die Basisstruktur20 am richtigen Platz ausdehnen kann, wie in8 veranschaulicht ist. Der Katheter30 kann dann zurückgezogen werden, wobei der Führungsdraht GW am Platz gelassen wird. Ein Gefäßtransplantat10 wird dann an dem Katheter30 befestigt und wieder eingeführt, so daß das zusammengedrückte Gefäßtransplantat innerhalb des Schenkels28 der Gewebeverkleidung liegt, während es mit der Hülle42 bedeckt ist, wie in9 veranschaulicht ist. Die Hülle42 wird dann zurückgezogen, so daß sich das Gefäßtransplantat10 sowohl innerhalb des Schenkels28 als auch innerhalb der linken Hüfte LI ausdehnt, wie in10 veranschaulicht ist. Der Katheter30 wird dann zurückgezogen, während der Führungsdraht von der linken Hüfte LI zur rechten Hüfte RI umgesetzt wird. Alternativ könnte ein separater Führungsdraht eingeführt werden. Der Katheter30 wird dann erneut über den Führungsdraht mit einer Hülle42 eingeführt, die ein zweites Gefäßtransplantat10 bedeckt, und zu der rechten Hüfte vorgeschoben, wie in11 veranschaulicht ist. Dann wird die Hülle42 zurückgezogen, wodurch sich das zweite Gefäßtransplantat10 sowohl innerhalb der rechten Hüfte RI als auch im zweiten Schenkel26 der Gewebeverkleidung ausdehnen kann. Die fertiggestellte bifurkationsförmige Transplantatstruktur befindet sich dann am richtigen Platz, wie in12 veranschaulicht ist, wobei der Führungsdraht GW, der Katheter30 und die Intubatorhülle50 zurückgezogen werden können. - Der femorale Zugang und die rückläufige Anordnung der Transplantatstrukturen der vorliegenden Erfindung entgegen der natürlichen Eingriffsrichtung sind möglich, obwohl ein derartiger Zugang gegenwärtig nicht bevorzugt wird.
- Die Anordnung und die erneute Anordnung der Stenttransplantatstruktur der vorliegenden Erfindung kann durch die Verwendung eines bildgebenden Ultraschallkatheters oder -führungsdrahtes erleichtert werden, wie z. B. durch die Führungsdrähte, die im US-Patent Nr. 5 095 911 und in PCT WO 93/16642 beschrieben sind. Derartige Ultraschall-Führungsdrähte können anstelle des herkömmlichen Führungsdrahtes
30 verwendet werden, der in7 –12 veranschaulicht ist, der typischerweise durch eine Blutstillungsklappe am proximalen Ende des Zuführungskatheters30 abgedichtet wird. Verriegelungseinrichtungen, Klemmen, Markierungen und ähnliches können an dem Zuführungskatheter30 und/oder dem bildgebenden Führungsdraht bereitgestellt werden, um die richtige Anordnung des Ultraschall-Meßwandlers innerhalb der Stenttransplantatstruktur während des Anordnungsverfahrens sicherzustellen. Das behandelte Aneurysma oder die andere behandelte Anomalie können dann vor der Freisetzung des Stenttransplantats10 genau lokalisiert werden. Nach der teilweisen Anordnung kann die richtige Position des Stenttransplantats10 mit der bildgebenden Ultraschallvorrichtung bestätigt werden. Falls die Position nicht richtig ist, kann das Stenttransplantat10 in die Hülle42 zurückgezogen und vor der völligen Freigabe erneut angeordnet werden. Die Verwendung eines bildgebenden Ultraschall-Führungsdrahtes ist vorteilhaft, weil es keine Notwendigkeit gibt, den Führungsdraht gegen ein separates bildgebendes Ultraschallkatheter auszutauschen.
Claims (2)
- Bifurkationsförmige Basisstruktur zum Bilden eines bifurkationsförmigen Transplantats in Kombination mit einem Paar Gefäßtransplantate (
10 ), wobei die Basisstruktur ein Verankerungssegment und eine Verkleidung (24 ) umfasst; wobei das Verankerungssegment einen radial komprimierbaren, perforierten röhrenförmigen Rahmen umfasst und die Verkleidung, die in dem Rahmen angeordnet ist, ein durch den Rahmen definiertes Lumen im Wesentlichen abdeckt, so dass die Basisstruktur an ihrem proximalen Ende ein gemeinsames Strömungslumen definiert; wobei das proximate Ende der Verkleidung von dem proximalen Ende des Rahmens distal beabstandet ist; wobei sich die Verkleidung distal von dem Rahmen (22 ) erstreckt und bifurkationsförmig ist, so dass die Basisstruktur an ihrem distalen Ende zwei unterschiedlich dimensionierte, divergente Strömungslumen definiert, jeweils eines in jedem der zwei Schenkel (26 ,28 ); wobei der Rahmen (22 ) mehrere getrennte röhrenförmige Segmente enthält, die axial nur durch die Verkleidung miteinander verbunden sind. - Baugruppe, die die Basisstruktur nach Anspruch 1 und ein erstes und ein zweites Gefäßtransplantat (
10 ) umfasst, die jeweils so dimensioniert sind, dass sie in dem ersten bzw. in dem zweiten Verkleidungsschenkel verankert werden können, um kontinuierliche Verlängerungen der jeweiligen divergenten Strömungslumen zu bilden.
