Abstract
Description
The invention relates to a stripping aid for on a Spiral implants arranged in guidewire.
For more than 10 years, interventional radiolo Gie metal spirals for vascular occlusion using catheters implan animals. Such spirals are repeated in the literature wrote. W. Castaneda-Zuniga et al. describe in Radiology 136; 230-231 (July 1980) an improved form of the so-called Gianturco spirals using catheters made of polyurethane or Polyethylene can be introduced. The primary spirals are through suitable techniques to a ball or a secondary spi rale deformed. Implant around the spiral in its secondary form to be able to do this by pulling the primary spiral onto one Stretched guidewire. When implanting through a catheter on the guidewire there is still a wide one on the guidewire re slidable spiral arranged as a scraper that serves the implant, d. H. the primary coil out of the catheter push and push off the guide wire so that the implant remains in the organ pathway or the vessel.

From the German utility model G 91 05 004.9 is an Ab Brushing aid for helical spirals arranged on a guide wire Implants for organ pathways, with a scraper guidewire surrounds, known. This stripping aid contains a guide rail ne, at the front end of a connector for the distal End of the guide wire is present, with one on the guide tion rail slidably arranged handle, the central partially engages around the guide rail and on which a connecting element for the distal end of the guide wire and with a slidable on the guide rail, encompassing this, lockable locking element. With this Ab The implant is brushed by pulling back the handle and thus stripped of the guide wire. The scraper remains stationary, and only the guide wire can be moved.
In addition, this stripping aid has the disadvantage that the di the ends of the scraper and the guide wire are constantly inserted into the corresponding connection elements can.
The object of the present invention is therefore a wiper help create both guidewire and wipers fer to be moved independently and relative to each other can and with the simple insertion of the scraper and Füh approximately in the connection elements provided for this purpose becomes.
Diese Aufgabe wird erfüllt durch

This task is accomplished by
a scraper for arranged on a guide wire, spiral implants for organ pathways, wherein a scraper surrounds the guide wire, with a guide tube, at the front end of which there is a cap with a bore for carrying out guide wire and scraper, a sliding sleeve arranged on the guide tube , The verbun with a connecting element for the distal end of the guide wire designed as a nipple, a stop ring and with a connecting element for the distal end designed as a nipple, characterized in that the stop ring is arranged on the front end of the guide tube, the connecting element is connected with a second sliding sleeve, which is arranged between the stop ring and sliding sleeve, the connecting elements for guide wire and wipers are arranged inside the guide tube, while the sliding sleeves enclose the guide tube and each over a Bol zen are connected to the connecting elements, the guide tube is provided with a slot in which the bolts are guided during a longitudinal movement of the sliding sleeve, which has two widenings at the rear end of the guide tube and the sliding sleeve are each provided with a recess .
In a preferred embodiment, each of the two is lines at the rear end of the guide tube again in two Different shaped widenings, each on the right and left are arranged from the slot, divided. The two are there Widenings on one side of the slot are semicircular forms while the two expansions opposite these have the shape of a wide indentation into which the verb a stud between the connecting elements and sliding sleeves grab and the front side flanks beveled more are, which causes the bolts when the sliding sleeves rotate are automatically inserted into the slot of the guide tube.
The connection elements have a cylindrical shape with egg an outer diameter that is slightly smaller than the In diameter of the guide tube, and are with a central Verse slot for guiding the guide wire or wiper hen, which is a widening with a circular cross-section has the nipple. The two sliding sleeves are flat if cylindrical with an inner diameter, the ge is slightly larger than the outer diameter of the guide tube. The recesses of the sliding sleeves preferably extend over a quarter of the sleeve circumference and point to the better hand have preferably two surrounding troughs. Ver Binding of the connection elements with the sliding sleeves indicate the closing elements perpendicular to the central slot a hole that ends just before the slot. The sliding sleeves are flat if provided with a hole in which a connecting bolt can be inserted into the bore of the connection elements intervenes. This ensures that the connection elements are firmly connected to the sliding sleeves and along along of the tube can be moved, the connecting bolts be guided in the slot of the guide tube.
In a preferred embodiment, the two have expansion at the end of the guide tube at a distance from each other that corresponds to the distance between the two nipples when the scraper is completely pushed onto the guide wire.
For inserting guidewire and wiper into the specified see connecting elements are the sliding sleeves and thus also the associated connecting elements to the rear end of the Guide tube moved, so that each of the recesses the sliding sleeves, each with an expansion of the slot in the guide pipe matches. In this case the semicircles are correct shaped widening of the guide slot with the circular Wells of the connection elements match, so that the two nip pel of guide wire and scraper simply into the respective Connection elements can be inserted. The connecting bolts are in this position of the sliding sleeves in the semicircular widenings of the guide slot opposite lying notches. Since the front side flanks of the Notches are more chamfered when turning the sliding sleeves the bolts automatically into the slot of the Guide tube introduced. As a result of this rotational movement agree neither the circular recesses for receiving the nipple in the connecting elements with the recesses of the sliding sleeves the two widenings at the end of the guide tube, so that the nipple of guidewire and wiper against an out are secured. Finally, guide wire and Ab slider passed through the central hole of the cap, which is facilitated by the fact that the cap is cylindrical End piece with a tapered outer diameter, which in the Guide tube is insertable, and the bore within this End piece is flared.
The two sliding sleeves are free along the guide tube movable, with the two nipples of guidewire and wiper within the respective firmly connected to the sliding sleeves Connection elements are secured against slipping out, so that Guide wire and scraper both independently of each other can also be moved relative to each other. The two sliding sleeves preferably have for locking on the guide tube a continuous threaded hole into which a knurled screw can be screwed in.
In a preferred embodiment of the stripping aid, this is Provide the guide tube with a measuring scale. The stop ring with a width of 3-10 mm is in the basic position the guide tube arranged that the position of the rear surface of the stop ring with a value of 3-10 mm corresponding to its Width on the measuring scale. The front surface of the An The impact ring is in the snap-in position with the value 0 on the measuring scale. To strip off the on the Guide wire located implant will slide the two sleeves for guidewire and wiper together along the Guide tube moved to a position where the front Edge of the sliding sleeve of the scraper with a value on the Measuring scale corresponds to that of the length of the extended implant on the guide wire. The front sliding sleeve will now fixed by tightening the knurled screw before the rear one Sliding sleeve with the guide wire is withdrawn so far that the spiral stretched on the guidewire is just wiping fer touched. The rear sliding sleeve is now tightened the knurled screw is locked on the guide tube. For stripping of the implant, the sliding sleeve for the wiper is in Rich tion of the stop ring shifted. While the implant was removed is streaked, its spiral-shaped secondary form develops. Due to the positioning of the The rear ring is located relative to the measuring scale End of the implant still on the guidewire when the shoot sleeve for the scraper touches the stop ring. The position The implantation can now be checked again and changed if necessary. For the implementation of the final ejection of the implant exist according to the Weilem embodiment of the stripping aid different possible keiten.
If the stop ring is firmly connected to the guide tube, can, in order to finally eject the implant, the locking of the rear sliding sleeve for the guide wire can be released, and the sliding sleeve can be moved backwards, causing the Füh guidewire is fully inserted into the scraper and the implant is stripped. For this, however, the front must first right sliding sleeve for the scraper can be locked.
In another embodiment, the stop ring is limited movably arranged on the guide tube. The stop ring is provided with a threaded hole in the for locking a knurled screw on the stop ring on the guide tube is screwable. To limit the mobility of the stop to enable the ring is the slot at the front end of the guide Rungsrohres with a cross section that the thread diameter of Knurled screw of the stop ring corresponds, expanded and on rear end of this widening with a perpendicular to it fenden recess, the cross section of the Ge winch diameter corresponds to the knurled screw. In the basic then the stop ring is positioned so that the knurl screw engages in the recess described above. For Ab When the implant is thrown, the knurled screw is loosened hammer ring twisted so that the thumbscrew in the expanded End of the slot and the stop ring to the front postponed. Now the front sliding sleeve for the Ab The slider is moved forward and the implant is complete be discarded. In principle, this is also the end the discharge by pulling back the slide sleeve for the guide wire easier to carry out manually.
In a further embodiment for electrocoagulation of the Im Both the guide tube and the sliding sleeves are planted with an insulating, autoclavable plastic layer over pulled.
The preferred embodiment of the stripping aid is now based on explained in more detail by figures.

These two expansions are in turn divided into 2 unevenly shaped expansions, which are arranged on the right and left of the slot ( 13 ). The two expansions ( 14 , 15 ) on one side of the slot ( 13 ) are semicircular, while the two opposite expansions ( 16 , 17 ) have the shape of a wide indentation, the front side flanks of which are more chamfered.
In Fig. 2 the stop ring with ( 9 ), the threaded hole for the knurled screw with ( 27 ) and the knurled screw with ( 28 ) be distinguished.
In Fig. 3 the sliding sleeve with ( 7 ), the recess with ( 18 ), the bore for receiving the connecting bolt with ( 24 ) and the threaded bore for the knurled screw with ( 25 ). From the cross section of the sliding sleeve ( 7 ) it is clear that the recess ( 18 ) extends over a quarter of the circumference of the sliding sleeve ( 7 ), and the bore ( 24 ) for receiving the connecting bolt perpendicular to it and for the threaded bore ( 25 ) is arranged. Gripping troughs ( 34 ) are recessed on two opposite sides of the sliding sleeve ( 7 ).
In Fig. 4, the connecting element with ( 8 ), the continuous slot with ( 19 ), the circular recess for receiving the nipple with ( 20 ) and the hole in which the connecting pin engages, with ( 21 ). From this illustration it is clearly Lich that the bore ( 21 ) perpendicular to the slot ( 19 ) and the recess ( 20 ) and ends just before the slot ( 19 ).
From Fig. 5 it can be seen how the sliding sleeve ( 7 ) and the connection element ( 8 ) are arranged concentrically to each other and by inserting the bolt ( 12 ) into the holes provided for this purpose ( 24 and 21 ) in the sliding sleeve ( 7 ) and the connecting element ( 8 ) are connected to one another. The connection of the sliding sleeve ( 7 ) and connecting element ( 8 ) takes place so that the slot ( 19 ) together with the recess ( 20 ) for receiving the nipple are arranged centrally to the recess ( 18 ) in the sliding sleeve ( 7 ). In the installed state, the guide tube ( 4 ) is located between the outer peripheral surface of the connecting element and the inner surface of the sliding sleeve. To lock the assembled sliding element, the knurled screw ( 28 ) can be screwed into the threaded bore ( 25 ), whereby a clamping effect is exerted on the guide tube ( 4 ). The sliding sleeve ( 7 ) can also have a ring ( 35 ) in the form of a U, so that the finger does not slip when being pushed forward.
As can be seen from Fig. 6, the two sliding sleeves ( 7 ) for the guide wire and ( 10 ) for the scraper are moved to the rear end of the guide tube ( 4 ) so that the recesses ( 18 ) of both sliding sleeves ( 7 , 10 ) with the widenings ( 14 , 16 and 15 , 17 ) of the slot ( 13 ) match. This has the consequence that the semicircular widenings ( 14 and 15 ) with the recess ( 20 ) of the connecting elements ( 8 and 11 ) match, so that the nipples ( 22 and 23 ) of the guide wire ( 2 ) and the scraper ( 3 ) can easily be inserted into the recesses of the associated circuit elements ( 11 or 8 ). The bolts ( 12 ), which connect the respective sliding sleeves ( 7 , 10 ) to the terminals ( 8 and 11 ), are guided in the notches ( 16 and 17 ) in this position of the sliding sleeves. By rotating the two sliding sleeves ( 7 and 10 ) by 90 ° in the illustration shown here upwards while simultaneously moving them forward, the two connecting bolts ( 12 ) are automatically inserted into the slot ( 13 ) of the guide tube ( 4 ). Due to this rotation, the recesses ( 18 ) of the two sliding sleeves ( 7 , 10 ) and the recesses ( 20 ) of the two connecting elements ( 8 , 11 ) are no longer congruent with the two semicircular expansions ( 14 , 15 ) of the slot ( 13 ), whereby the two nipples ( 22 , 23 ) are secured against slipping out. Finally, the guide wire ( 2 ) is passed through the central bore ( 6 ) of the cap ( 5 ) together with the scraper.
As can be seen from Fig. 7, the stop ring ( 9 ) is locked in the working state of the insertion aid fe with the aid of the locking screw ( 28 ) on the guide tube ( 4 ) that the rear surface of the stop ring ( 9 ) with a value of 3-10 mm on the measuring scale. The two sliding sleeves ( 7, 10 ), which each have a gripping trough ( 34 ) on their top and bottom, are now moved together, being in contact with one another, to a position on the guide tube ( 4 ) which is determined in that the front edge of the sliding sleeve ( 10 ) for the stripper ( 3 ) corresponds to a value on the measuring scale that speaks ent the length of the implant in the stretched state. Then the rear sliding sleeve ( 7 ) for the guide wire ( 2 ) is locked with the help of the knurled screw ( 26 ) on the guide tube ( 4 ) and the sliding sleeve ( 7 ) of the scraper ( 3 ) forwards in the direction of the stripping of the implant Stop ring ( 9 ) moved. If the sliding sleeve ( 7 ) strikes the ring ( 9 ), the implant is almost completely stripped, there are only the last 8-10 mm still on the guide wire ( 2 ). In this position of the stripping aid, the positioning of the implant can be checked and, if necessary, changed. For final stripping, either the sliding sleeve ( 7 ) of the scraper ( 3 ) is locked, the locking of the sliding sleeve ( 7 ) of the guide wire ( 2 ) is released and the sliding sleeve ( 7 ) is moved backwards, causing the guide wire ( 2 ) to be completely in the wiper ( 3 ) is pulled in and the implant is wiped off. On the other hand, the knurled screw ( 28 ) of the stop ring ( 9 ) can be loosened and then both the stop ring ( 9 ) and the sliding sleeve ( 10 ) can be moved to the front to completely throw the implant.
Optionally, the guide tube ( 4 ) and the sliding sleeves ( 10 ) can be equipped with an insulating plastic layer in order to provide the implant with a thrombus immediately before final ejection by means of electrocoagulation and to fix it in the organ pathway.
