Abstract
Description
The invention relates to a method and means for measurement of mercury (Hg) from the dental material amalgam is released.
The metals mercury, silver, silver, copper and tin contained in the dental material amalgam are known to be released from their solid binding by abrasion during chewing and by electrochemical corrosion in very small amounts. The most toxic of these is mercury (Hg), which is why the public has repeatedly been asked to ban amalgam. On the part of dentistry, the great dental advantages of this material will be countered. The resulting risk-benefit assessment depends crucially on how much mercury from the fillings gets into the organism. The mercury emerges from the fillings both in elemental form as a gas (Hg °) and in ionized form (Hg 2+ ), and abrasion in tactful amalgam particles is also possible. The first two forms ge long z. T. in the bloodstream, so that this mercury contributes to the burden on the organism, while the particles leave the body unchanged.
Aus Berglund, A., Pohl, L., Olsson, S., Bergman, M. (1988) De termination of the rate of release of intra-oral mercury vapor from amalgam. J. Dent. Dies ist aus Berglund, A. (1990) Estimation by a 24-hour study of the daily dose of intra-oral mercury vapor inhaled after release from dental amalgam. J. Dent. Res., 69, 1646-1651 bekannt. From Berglund, A., Pohl, L., Olsson, S., Bergman, M. (1988) De termination of the rate of release of intra-oral mercury vapor from amalgam. J. Dent. Res., 67, 1235-1242 is known to Air with the vaporous mercury from the mouth into a so-called gold vacuum measuring device that shows the amount of mercury. Independently of the air samples, but not simultaneously with them, can They also obtained saliva samples and exposed them to ionized mercury be searched. This is from Berglund, A. (1990) Estimation by a 24-hour study of the daily dose of intra-oral mercury vapor inhaled after release from dental amalgam. J. Dent. Res., 69, 1646-1651 known.
A disadvantage in the extraction of air from the oral cavity is that the suction pipe in the half-open mouth is performed and therefore the Hg ° during suction containing air mixed with inflowing mercury-free outside air. This results in a dilution of the Hg ° concentration in the examined air sample. This procedure also requires the determination of the volume of the air sample (flow meter, Gas collector) to which the amount of mercury found in the sample is based must be gen. When measuring the mercury concentration A disadvantage in saliva is that during a total saliva volume did not produce for a certain period of time it is known why the saliva is released overall set amount of mercury cannot be calculated.
The aim of all methods is to record the time-dependent release of mercury from the dental fillings (mass per time), from which the amount released daily can be calculated. This is the decisive parameter for estimating the toxic hazard potential emanating from the amalgam. The problem with the previous methods is such that the Hg release from amalgam fillings was only measured in successively taken samples of different states, which means different sampling techniques and e.g. T. different analysis devices required. This limits the comparability of the measurement results. In addition, it is not possible to calculate the amount of Hg 2+ released per unit of time using saliva samples.
The object of the invention is methods and means for measurement of mercury released from the dental material amalgan is set to provide, with a sample both Hg forms were recorded.
This problem is solved by the features of the patent claims che 1, 3 and 6. The sub-claims describe advantageous Embodiments of the invention.
An advantage of the method is that the detection of all of the Hg 2+ released in the saliva is made possible by replacing the unknown volume of saliva with a known aqueous NaCl solution. The Cl anions serve to stabilize the Hg 2+ ions. If the mouth of an amalgam user is rinsed out with a mixture of this solution and a non-polar solvent, both forms of mercury can be isolated in one operation.
The process takes advantage of elemen's great fat solubility Tarem gaseous mercury to make them from amalgam to measure the released state of Hg. However, come for use on patients only non-toxic and at room temperature liquid fats into consideration. Except for those for laboratory search for fat solubility of Hg ° used edible oils also liquid paraffin in pharmaceutical purity ver turns. By adding an aqueous solution to the paraffin the ionized liberated from amalgam can be simultaneously Collect mercury, to stabilize it the aqueous one Solution contains C⁻ ions in physiological concentration. The Concentration of both forms of mercury is thus in liquid me dium determined, so that the analysis is uniform in known Way in the wet reduction process in connection with the AAS is carried out. For additional precious metal collectors Adsorption of Hg ° from the air, or on the completely different term gold film measuring device can be omitted. A white Another advantage is the exact dimensioning of both the solution volume mina and the duration of the mouthwash, which is based on the concentrations found in paraffin and aqueous solution released amount of the respective mercury form per unit of time can be calculated easily and reliably. For the patient the new method brings an improvement in that he only goes through the procedure for extracting mercury from his mouth once must undergo space. Since the procedure is also Examination of mercury emissions from amalgam test pieces can be used transferability errors when comparing in vivo minimized with in vitro results.
The invention is illustrated below with the aid of an embodiment explained in more detail:
The patient rinses the mouth well twice with water, where through all the mercury that has previously accumulated from the mouth the cave is removed.
Immediately afterwards, he rinses his mouth with a rinsing liquid liquid from 15 ml paraffin and 15 ml aqueous 0.9% NaCl Solution for two to four minutes. The tongue should often, but not too violently, to the mixture to bring evenly into contact with all amalgam surfaces.
After the rinsing time, all the rinsing liquid in a centrifuge tube (50 ml capacity, art fabric, with screw cap) emptied and centrifu until water and paraffin have separated well.
Analytical determination of the mercury concentration in a paraf fin and aqueous sample using conventional methods (atomic tion spectrometry).

5. Berechnung der Hg-Freisetzungsrate (ng Hg/min):
- a) für Hg° (Paraffin):
ng Hg°/min = (ng Hg/ml)Paraffin-Probe × 15 ml/2 min; - b) für Hg2+ (wäßrige Lösung):
ng Hg2+/min = (ng Hg/ml)wäßr.Probe × 15 ml/2 min.
- a) for Hg ° (paraffin):
ng Hg ° / min = (ng Hg / ml) paraffin sample × 15 ml / 2 min; - b) for Hg 2+ (aqueous solution):
ng Hg 2+ / min = (ng Hg / ml) aqueous sample × 15 ml / 2 min.
- a) für Hg° (Paraffin):
The determination of the mercury release rate after 1.- 5. can both a few hours after the last meal (so-called unstimulated amalgam) as well as immediately after chewing of chewing gum (so-called stimulated amalgam), whereby the higher Hg levy is simulated during meals. With these According to a common scheme, the organism can evaluate Amount of mercury taken in daily from the amalgam fillings be counted.
Nach Ablauf der In kubationszeit erfolgen die weiteren Schritte analog zu 3.-5.A lockable is used for material studies Incubation vessel with a rinsing liquid after 2nd and one Stirring magnet loaded. This is then converted into a thermostat water bath (37 ° C) that is running Magnetic stirrer stands, and preheated. Then the Put the amalgam sample into the incubation vessel and immediately sealed (beginning of the incubation period). When the In the next steps are carried out analogously to 3.-5.
Instead of paraffin, edible oils can also be used.
