Description
The invention relates to a device for whole-body plethysmographic pulmonary function measurements according to the preamble of claim 1.
Medical science uses small animals like guinea pigs, rats etc., and in animal experiments the effect of medication and pollutants on lung function to explore. Pulmonary function measurements are also in diagnosis and Therapy of lung patients (asthma patients, brochure critics, etc.) of crucial importance Importance.
In der tierexperimentellen Lungenfunktionsdiagnostik sind folgende Maßnahmen üblich: - "Meerschweinchenüberlaufmethode" (LESSIN, A. W. und KRAMER, R. L. (1969). The bronchodilator action of analeptics in the guinea-pig. J. Pharm. Die notwendige Luftröhrenoperation läßt wiederholte Untersuchungen beim gleichen Tier nur bedingt zu.In animal experimental lung function diagnostics are the following measures are usual: - "guinea pig overflow method" (LESSIN, A. W. and KRAMER, R. L. (1969). The bronchodilator action of analeptics in the guinea-pig. J. Pharm. Pharmac., 21, 309-313): The deeply anesthetized laboratory animal is Trachea section a tube is implanted in the trachea; The animal's lungs are pumped through this tube ventilated at a fixed frequency and volume; the one there The resulting pressure is measured and is a measure of the Shortness of breath. The disadvantages of such an approach lie on the Hand. It is very unphysiological breathing conditions. Deep anesthesia changes breathing. The necessary tracheal surgery leaves repeated examinations at the same Animal too limited.
- Einkammriges ganzkörperplethysmographisches System (z. B. KOLLER, E. A.: Die Wirkung von Micoren auf Atmung und Blutdruck. Helv. Physiol. Acta 20, 97-113 (1962) und GJURIS, V., B., Heicke u. E. WESTERMANN: Über die Stimulierung der Atmung durch Bradykinin und Kallidin. Naunyn-Schmiedebergs Arch. exp. Path. u. Pharmak. The effect of micores on breathing and blood pressure. Helv. Physiol. Acta 20, 97-113 (1962) and GJURIS, V., B., Heicke and E. WESTERMANN : On the stimulation of breathing by bradykinin and kallidin Naunyn-Schmiedebergs Arch. Exp. Path. And Pharmak. 247, 429-444 (1964): The laboratory animal sits in an airtight chamber. Breathing-dependent pressure fluctuations in the chamber allow conclusions to be drawn on the pressure conditions in the thorax of the animal. They are compared on an XY chart recorder with the respiratory flow rate, which is registered via a tracheal cannula. The device is mainly used for calculations of the respiratory resistance, the disadvantage of which is the need to anesthetize the laboratory animal and to equip it with a tracheostomy tube.
- Zweikammriges ganzkörperplethysmographisches System (JOHANSON, W. G., PIERCE, A. K., A noninvasive method for measurement of airway conductance in small animals, J. appl. Physiology 30; 146-150 (1971), GORDON. T., D. PISTORIUS, Tierexperimentelle Lungenfunktionsanalysen während Inhalation von Zinkoxidstaub, Zbl. Bakt. Hyg., I. Abt. Orig. A 227, 116-120 (1974)). - Two-chamber whole-body plethysmographic system (JOHANSON, W. G., PIERCE, A. K., A noninvasive method for measurement of airway conductance in small animals, J. appl. Physiology 30; 146-150 (1971) GORDON. T., D. PISTORIUS, animal experimentation Pulmonary function analysis during inhalation of zinc oxide dust, Zbl. Bakt. Hyg., I. Dept. Orig. A 227, 116-120 (1974)). In this system, the laboratory animal sits in a two-chamber Whole-body plethysmograph, the two chambers of which are filled with water Rubber sleeve to be separated from each other closes around the animal's head. Volume fluctuations The head and body chambers are continuously separated registered as a function of time. By relatively complicated The specific airway conductivity is calculated. By differentiating the tidal volume against time the respiratory flow is calculated. Furthermore, the breathing rate as well as the ratio of inhalation to exhalation will. The advantage is that on the non-anesthetized animal repeated lung function examinations were carried out gently can be. The complicatedly calculated lung function parameters however show considerable inter- and intra-individual Fluctuations so that for an accurate pulmonary function examination, especially progress checks, only to a limited extent are suitable.
J. H. COMROE, jr.: A new method for measuring airway resistance in man using a body plethysmograph: Values in normal subjects and in patients with respiratory disease, J. clin. Invest. 35, 327/35 (1956), die oszillatorische Methode (GOLDMAN M. et al.: A simplified measurement of respiratory resistance by forced oscillation, J. appl. Physiol. A large number of devices have been developed for recording human respiratory mechanics. Devices for whole-body plethysmography are of practical importance (DUBOIS, A.B., St. Y BOTHELO and J.H. COMROE, jr .: A new method for measuring airway resistance in man using a body plethysmograph: Values in normal subjects and in patients with respiratory disease, J. clin. Invest. 35, 327/35 (1956), the oscillatory method (GOLDMAN M. et al .: A simplified measurement of respiratory resistance by forced oscillation, J. appl. Physiol. 28, No. 1 (1970)) and spirometry (ULMER, W. T., REICHEL, G., NOLTE, D .: Die Lung function Stuttgart: Thieme 1975). For a device for whole body plethysmography it is known (from DE-OS 17 91 124), on the one hand, chamber pressure fluctuations in one cabin intended for receiving the test subject via piezo or mechanical-electrical Detect transducers and, on the other hand, the respiratory current strength (i.e. the time differential of the tidal volume) of the test person using a pneumotachograph and to supply corresponding electrical quantities to a coordinate recorder which Record pressure / respiratory flow rate curves. These curves are then evaluated by in significant points (corresponding to the extreme values of the recorded chamber pressure) Auxiliary lines are created and the reciprocal steepness of these auxiliary lines as a measure of the Pulmonary resistance value is determined.
The invention has for its object a device for whole-body plethysmographic Pulmonary function measurements to create not only a relatively simple evaluation allowed, but in particular also has an improved sensitivity.
Starting from a device of the type mentioned in the introduction, this object is achieved according to the invention solved with the measures of the characterizing part of claim 1.
In the hypothetical case of breathing without resistance, the device according to the invention would show a straight line inclined at 45 ° or -45 ° on the XY registration device, for example a conventional XY recorder. The airway resistance, even at rest, causes a hysteresis-like deformation of this straight line. Breathing disabilities, such as those caused by an asthma attack or by foreign bodies, lead to the widening of this loop, the downward deviation of the curve from the above-mentioned straight line corresponding to the amount of air which is retained in the lungs by an asthmatic reaction ( "tethered air"). By means of the device according to the invention can be on the subject, z. B. the non-anesthetized, spontaneously breathing laboratory animal, continuous lung function measurements are carried out over long periods of time. The measured value used (corresponding to the parameter "bound air") is characterized by high reproducibility and particularly high sensitivity; allows an exact differentiation of pathological from normal conditions. In addition, the shape analysis of the resulting breath loops allows a distinction to be drawn between breathing obstacles that are primarily effective in inhalation or predominantly in exhalation (bulges of the loop up or down). Furthermore, a distinction can be made as to whether respiratory obstructions are more localized in large or small bronchi, since a central respiratory obstruction becomes effective in an early phase of breathing movement, whereas peripheral obstructions only become effective in a later phase of exhalation. The device described has been tested in use in small animals. It can also be used in human medicine without difficulty.
With a particularly suitable for measurements on laboratory animals Embodiments of the invention are preferably a body chamber with associated device for determining the displacement volume and a separate head chamber is provided, to the device for determining the tidal volume connected.
Sorgt man dafür, daß in der Kammer im wesentlichen die BeziehungOne ensures that in the chamber essentially the relationship
Druck × Volumen = KonstantPressure × volume = constant
Entsprechend einer weiteren Alternative lassen sich Durchflußmengenmesser mit nachgeschalteter Integrationsstufe zur Volumenmessung heranziehen.is fulfilled, the device can be constructed in an advantageous further embodiment of the invention in such a way that the body chamber and the head chamber are each equipped with a pressure probe for determining the course values of the displacement volume or the breath volume and the output of the one pressure probe to the vertical input and the output of the another pressure probe is connected to the horizontal input of the XY registration device. The volume measurement is reduced to a pressure measurement based on the relationship P × V = C. Pressure probes are particularly simple and precise. In principle, measuring devices which determine the relevant volumes can of course also be connected directly to one or both chambers. According to a further alternative, flow meters with a downstream integration stage can be used for volume measurement.
A single-chamber device is preferably suitable for human examinations, in which the chamber is designed to accommodate the entire subject and to Determining the displacement volume is equipped with a pressure probe, the Device for determining the course value of the breath volume compared to the Chamber volume isolated can be connected to the test person's airways. For one such a connection can expediently be provided with a mouthpiece provided with a hose while the subject's nose is closed with a nose clip becomes.
A particularly suitable device is used to determine the course value of the tidal volume Spirometer, which is connected to the mouthpiece by a tube which is led out of the chamber connected is. However, a flow meter can also be used for this purpose with a downstream integration level.
In den Figuren zeigt The invention is described in more detail below on the basis of preferred exemplary embodiments explained. In the figures shows

The two-chamber whole-body plethysmograph 10 of FIG. 1 has a head chamber 11 and a body chamber 12 , which are separated from one another by an expandable cuff 13 surrounding the head of the animal. The head chamber 11 and the body chamber 12 are surrounded by a water jacket 14 . The right end of the body chamber 12 in FIG. 1 is closed by means of a displaceable stamp 15 . A chamber 16, 17 is assigned to each of the chambers 11, 12 . The pressure probe 16 to the head chamber 11 is connected to the vertical input 18 of an XY -Registriergeräts 19, while the horizontal input 20 of the recording unit 19 with the body of the chamber 12 is associated pressure sensor 17 is connected.
When using the device according to FIG. 1, the non-anesthetized laboratory animal lies on a carriage indicated at 21 , on the end of which facing the head chamber a lattice construction 22 prevents the animal's head from retreating. The cuff 13 surrounding the animal's head and separating the two chambers 11, 12 from one another is filled with water which is under a predetermined pressure. In both chambers, identical gas conditions are established in that the gases are heated to body temperature by means of the water jacket 14 , identical gas volumes are ensured in the two chambers 11 , 12 by appropriate adjustment of the plunger 15 , and 100% atmospheric humidity is maintained in both chambers . As a result, the relationship P × V = C applies, so that the displacement volume in the body chamber 12 and the tidal volume in the head chamber 11 can be detected as pressure fluctuations in both chambers. Breathing-related pressure fluctuations in the chambers 11, 12 are recorded by means of the pressure probes 16 and 17 and recorded as volume fluctuations by the XY registration device 19 as trend values in their interdependency. The fluctuations in the body chamber volume on the X axis and the fluctuations in the head chamber volume on the Y axis are registered.
FIG. 2 shows two normal, non-pathological breathing loops 26, 27 recorded by means of the device according to FIG. 1. The hysteresis loops are narrow. Bound air is hardly detectable. There is no shortness of breath. Curves 28 to 32 , on the other hand, apply to pathologically changed conditions that were triggered in guinea pigs by the inhalation of an allergen. The hysteresis loops are broadened in a characteristic way. In relation to the volume of animals, very large amounts of "captive air" can be detected.
The whole-body plethysmograph 35 of FIG. 3, which is suitable for use in humans, has an airtight chamber 36 in which the subject sits. The subject's nose is closed with a nose clip. A hose 37 is connected at one end to a mouthpiece. The other end of the hose 37 led out of the chamber 36 is connected to a spirometer 38 for recording the tidal volume. Assuming the validity of the relationship V × P = C, the fluctuations in the displacement volume in the chamber 36 can be detected by means of a pressure probe 39 . The spirometer 38 is connected to the vertical input 18 of the XY registration device 19 , the horizontal input 20 of which is connected to the pressure probe 39 .
Breath loops of the type illustrated in FIG. 2 can also be obtained by means of the whole-body plethysmograph 35 . By analyzing the shape of the resulting breath loops, as in the device according to FIG. 1, important lung function parameters, such as e.g. B. bound air, inspiratory or expiratory respiratory obstruction, central or peripheral bronchial obstruction can be determined.
