Beschreibung
Gebrauchsmusterschutz: "Zementfreie Schraubpfanne für den Gelenkersatz des
Hüftgelenkes" (zementfreie PAC-Schraubpfanne)
Die Anmeldung umfaßt eine Kunstpfanne für den Ersatz der knöchernen Pfanne des Hüftgelenkes.
Die Kunstpfanne setzt sich aus 2 Teilen zusammen:
1. Aus einer Metallschale.
Diese Schale ist im wesentlichen als 4/10. Kugel geformt, welche jedoch im Bereich des beckenwärtigen Domes - abhängig von der konfektioneilen Größe der Schale - abgeflacht ist.

In der Mitte des Domes befindet sich eine kreisförmige Öffnung. Die Schale besitzt zum Pfanneneingang hin ein selbstschneidendes Schraubengewinde, welches konisch geformt ist.
Der Konusanstieg beträgt 13,5 ° (Abb. 2.2 und Abb. 4.1) Die Schneideflächen der Gewindeabschnitte bilden eine Ebene, welche in 15 ° Winkelstellung zum Radius des Pfanneneinganges steht (Abb. 1.2). Der Rand des Pfanneneinganges ist mit 6 halbkreisförmigen Einsenkungen versehen, die zur temporären Aufnahme des Eindrehinstrumtariums dienen (Abb. 3.3).
Die Innenfläche der Metallschale ist - alternativ zur Aufnahme eines Polyäthyleneinsatzes oder eines Einsatzes aus Keramik ausgestaltet. Die verschiebefeste Verklemmung von Einsatz und Metallschale geschieht über eine kreisförmige konisch geformte Innenform (Abb. 2.3, Abb. 3.2 und Abb. 4.2).
2. Aus einem Einsatz - alternativ aus Polyäthylen oder Aluminiumkeramik.
Der Einsatz (auch Inlay) entspricht in seiner Außenseite den korrespondierenden Flächen der Innenseite der Metallschale. Nach dem Einsetzen des Inlay's tritt im konischen Bereich von Einsatz und Metallschale eine feste und sichere Verklemmung ein, die sich bei der üblichen Belastung noch verstärkt.
Die Innenfläche des Einsatzes ist für die Aufnahme eines Standard-Kunstkopfes sowohl aus Metall als auch als Auluminiumkeramik geformt und besitzt eine glatte Oberfläche.
Implantation der Kunstpfanne:
Zunächst wird die krankhaft veränderte Hüftgelenkspfanne entsprechend der Außenform der Metallschale ausgefräst. Danach wird die Metallschale in entsprechender Größe in vorberechneter Position in die vorbereitete knöcherne Pfanne eingedreht. Die halbkreisförmigen Einsenkungen am Eingangsrand der Metallschale dienen dabei zur Aufnahme eines Tellers, auf den eine Eindrehratsche aufgesetzt wird. Die Gewindegänge schneiden sich dabei (selbstschneidend) immer fester in die Innenseite der knöchernen Pfanne ein.
Die konusförmig verlaufenden Gewindegänge sorgen dabei für eine sichere und be-
wegungsstabile Primärverklemmung zwischen Implantat und Knochen.
Die kreisförmige Öffnung im Dom der Metallschale gestattet - wenn notwendig - ein zusätzliches Einbringen von autologen und homologen Knochen.
Der abgeflachte Dom der Metallschale vermeidet ein zu tiefes Eindringen der Metallschale in das knöcherne Becken, insbesondere beim Vorliegen einer flachen
knöchernen Hüftgelenkspfanne (Dysplasiepfanne).
Danach wird der Einsatz aus Polyäthylen - alternativ aus Aluminiumkeramik - eingeschlagen, so daß dieser im konischen Teil des Schaleninneren sicher mit der Metallschale verklemmt.
