Description

  • Implantat als Ersatz für spongiös aufgebaute
  • Knochen Die Erfindung bezieht sich auf ein Implantat als Ersatz für spongiös aufgebaute Knochen des menschlichen Knochengerüstes. Solche Knochen sind z. B. die Hand- und Fußwurzelknochen, die den Wachstumsfügen benachbarten Abschnitte der Röhrenknochen, die Wirbelkörper, das Becken, die Schulterblätter und große Teile des Schädels und des Brustkorbes.
  • Bei Knochendefekten, Brüchen, Stauchungsbrüchen oder erforderlich werdenden Versteifungen in Knochengelenken, wo bisher spongiöser Knochen zu ersetzen war, geschieht dies bisher durch Verpflanzung (Transplantation) von Knochen, wobei aber die Tragfähigkeit des verpflanzten Knochens über lange Zeit nicht erwartet werden kann, denn der entnommene Knochen verliert durch den Verpflanzungsvorgang seine Knochenzellen infolge der Trennung vom Blutkreislauf, und nur der mineralische knöcherne Bestandteil verbleibt am Ort der Einpflanzung als ein körpereigenes Gerüst, welches aber nicht tragfähig ist und erst mit der Zeit durch Einsprossen von Bindegewebe zu einer neuen Knochenbildung führt. Dieser Vorgang, nämlich die Einorganisierung eines Knochentransplantats in sein knöchernes Lager benötigt - bis zum Erreichen eines voll tragfähigen Knochens lange Zeit, und zwar etwa 1 Jahr.
  • Die Aufgabe der Erfindung besteht darin, Implantate für spongiös aufgebaute zu ersetzende Knochen oder defekte Knochenteile des menschlichen Knochengerüstes so auszubilden, daß der Knochengerüstteil schon nach kurzer Zeit wieder voll belastbar bzw. funktionsfähig ist.
  • Diese Aufgabe wird nach der Erfindung durch ein Implantat gelöst, welches sich durch ein aus körperverträglichen Fäden oder Fasern räumlich aufgebautes, formstabiles Netzwerk kennzeichnet, welches vorzugsweise durchgehend offenzellige bzw offenporige Struktur aufweist.
  • Wird nach dieser Lösung ein Spongiosadefekt mit einem Körper aus dem formstabilen Netzwerk ausgefüllt, --welches aus körperverträglichen Fasern oder Fäden, insbesondere aus Metallen oder Metallegierungen aufgebaut ist, so wird damit sofort die primäre Stabilität gewährleistet und daß Einsprossen von Bindegewebe in das Netzwerk des Implantates mit anschließender Umbildung in Knochen ermöglicht.
  • Dieses spongiöse Implantat verbleibt auf Dauer in seinem Knochenlager und wird mit der Zeit von Knochen umwachsen.
  • Neben Fäden oder Fasern aus Metallen oder Metallegierungen können in gegebenen Fällen auch solche aus Keramikmaterial, Kohle und/oder Kunststoffen zur Anwendung kommen.
  • Um nach der Erfindung die Form des Netzwerkkörpers zu stabilisieren bzw. zu versteifen, können die Fäden oder Fasern an den Kreuzungsstellen z.B. durch Verschweißen oder auf chemischem Wege verbunden werden. Das Implantat aus dem Netzwerk kann durch Prägen oder Pressen in die gewünschte und erforderliche Form gebracht werden oder dadurch, daß eìn Zuschneiden oder Zerteilen aus einem größeren Netzwerkkörper erfolgt.
  • Nach der Erfindung ist es auch möglich, neben dem spongiösen, aus dem Netzwerk bestehenden Implantat das Anwachsen des Implantats durch spongiöse Knochen zu fördern, die neben dem Netzwerk-Implantat bei der primären Implantation verpflanzt werden, wobei die mechanische Beanspruchung von dem implantierten Netzwerk sofort übernommen werden kann.
  • Die Erfindung wird nachstehend anhand von Zeichnungsbeispielen des neuen Netzwerk-Implantats erörtert.
  • Das Implantat als Ersatz für spongiöse Knochen besteht aus Fasern, Fäden oder dünnen Drähten, die zu einem Netzwerk verflochten oder verfilmt sind und aus körperverträglichem Metall (Titan), Metallegierungen, Keramikmaterial, Kohle und ioder Kunststoff bestehen.
  • Die vernetzten oder verfilzten Fäden oder Fasern werden durch Prägen oder Pressen in die für den jeweiligen Knochendefekt erforderliche stabile Form gebracht oder zu einem größeren Körper formstabil vernetzt oder verfilmt, wobei aus diesem Körper die erforderliche Form herausgeschnitten werden kann. Dabei können die Fäden oder Fasern an den Kreuzungsstellen durch Wärmezufuhr (Verschweißen) oder chemisch verbunden werden, um das Netzwerk formstabil zu versteifen.
  • Ist z.B. nach Figur 1 und 2 ein Stauchungsbruch am Schienbeinkopf bzw. am Fersenbein (linker Teil der Figuren 1 und 2) aufgetreten, so wird zur Aufrichtung des zusammengebrochenen spongiösen Knochens ein Schnitt durchgeführt, und die Knochenteile werden auseinandergespreizt und sodann ein entsprechend aus dem Netzwerk bestehendes spongiöses Implantat, z.B. in Form eines Keiles 1 eingesetzt, wobei der Keil so eingebracht werden kann, daß er in seinem Lager fest verklemmt wird, womit das Prinzip der Ruhestellung durch Distraktion verwirklicht ist,so daß unter Umständen Gipsverbände oder sonstige Stütz- und Haltemaßnahrnen erspart werden. In gleicher Weise kann bei einem# durch Stauchung zusammengebrochenen Fersenbein nach Figur 2 vorgegangen werden.
  • Nach Figur 3 ist ein durch Stauchung zusammengebrochener Wirbelknochendefekt in ähnlicher Weise wie zu Figur 1 und 2 auseinandergesetzt ist, operativ zu beheben, indem ein spongiöses Implantat 2, z.B. in Form eines Quaders, in den auseinander zu spreizenden Wirbelknochen unter festen Einspannungen eingesetzt wird.
  • Wenn feste Verbindungen zwischen zwei spongiösen Knochen herzustellen sind, werden gemäß Figur 4 die Knochenflächen so hergerichtet, wie es z.B'. bei einer Gelenkversteifung erforderlich ist. Es wird dann in den Defekt ein spongiöses Implantat 3 z.B.
  • in Form eines Quaders eingesetzt, unter Umständen gemeinsam mit zu transplantierendem spongiösen Knochen, um das schnellere Umwachsen des spongiösen Implantats mit Knochen zu fördern.
  • Das formstabile spongiöse Implantat 3 wird die Versteifung des Gelenkes wegen der größeren primären Stabilität und Formbeständigkeit besser und sicherer als bisher gewährleisten und läßt eine schnellere funktionelle Beanspruchung erwarten.
  • Bei einer Fußgelenkversteifung entsprechend Figur 5, ist, wie nach Figur 4, ebenfalls so vorzugehen, daß Gelenkflächen entsprechend herzurichten sind, z.B. zu resizieren sind, worauf scheibenförmige spongiöse Implantate 4 evtl. auch gemeinsam mit spongiösem Knochen eingesetzt werden, so daß dann durch Einsprossen von Bindegewebe in das spongiöse Implantat die Bildung von spongiösem Knochen erfolgt und damit die Fußgelenkknochen versteift zusammenwachsen.
  • Nach Figur 6 ist es auch möglich, nach Resektion einer Falschgelenkbildung, z.B. eines Oberschenkels, den Defekt durch Einsetzen eines spongiösen Implantates 5 zwischen die beiden Schenkelteile zu ersetzen, wobei dann die beiden Oberschenkelknochenteile miteinander verschient werden können.
  • Nach einem weiteren Fall ist es auch entsprechend Figur 7 und 8 möglich, durch ein spongiöses Implantat 6 einen starren Kern, z.B. den Schaft einer Gelenkkugelproth#ese (Fig,7) bzw. die Schäfte einer Kniegelenkendoprothese (Fig. 8), einzuhüllen, um dadurch eine großflächige Verbindung zum Lagerknochen zu gewährleisten Damit werden günstigere Voraussetzungen für die zementfreie Implantation von Gelenkprothesen im Bereich der Schäfte von Röhrenknochen geschaffen.
  • Nach einem weiteren Beispiel der Fig. 9 kann ein durch ein Geschwulst 7 zerstörter Knochenteil durch ein Implantat 8 aus einem formstabilen Netzwerk nach der Erfindung ersetzt werden.
  • In allen Anwendungsfällen, die nicht durch die Zeichnungsbeispiele beschränkt sind, in denen das spongiöse Implantat nach der Erfindung zur Anwendung kommt, bleibt das Implantat ständig an der Stelle des zu behebenden Defekts und läßt durch seine primäre Stabilität und seine Formbeständigkeit sofort funktionelle Beanspruchungen zu. Das spongiöse Implantat begünstigt das Einsprossen von Bindegewebe und damit die schnelle~Bildung spongiösen Knochens.

Claims (6)

  1. Patentansprüche 1.)Implantat als Ersatz für spongiösen Knochen, gekennzeichnet durch ein aus körperverträglichen Fäden oder Fasern räumlich aufgebautes, formstabiles Netzwerk.
  2. 2. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die Fäden bzw. Fasern wahlweise aus Metall, Metallegierungen, Keramik, Kohle und/oder Kunststoffen bestehen.
  3. 3. Implantat nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das Netzwerk in sich durch Verbindung der Fäden an den Kreuzungsstellen versteift ist.
  4. 4. Implantat nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß das Netzwerk in einer offenen Fassung untergebracht ist.
  5. 5. Implantat nach einem der Ansprüche 1 bis 4, gekennzeichnet durch einen aus dem Netzwerk durch Prägen oder Pressen hergestellten Formkörper, der unmittelbar oder nach Beschneiden oder Zerteilung das Implantat bildet.
  6. 6. Implantat nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß das Netzwerk in die Form eines Keiles, Quaders, einer Scheibe oder einer einen Kern umgebundenden Hülle gebracht ist.
